Abstract

Kerusakan tulang alveolar pada kasus periodontitis membutuhkan perawatan bedah regeneratif periodontal dengan bahan bone graft untuk merangsang pertumbuhan tulang alveolar, dikombinasikan dengan Platelet Rich Fibrin (PRF) atau membran alloplastik sebagai Guide Tissue Regeneration (GTR). Studi kasus ini bertujuan untuk menunjukan perawatan koreksi defek tulang alveolar pada kasus periodontitis dengan bedah regeneratif periodontal. Pasien wanita berusia 50 tahun mengeluhkan gigi belakang rahang atas kanan dan kiri terasa goyang dan sakit. Temuan klinis dan radiograf terdapat poket dengan rata-rata kedalaman 7 dan 8 mm pada bagian bukal mesial dan distal pada gigi 16, 15 dan 26, kegoyangan gigi derajat 1, dan kerusakan tulang vertikal mencapai 1/3 tengah akar gigi 16,15 dan 26. Pasien mendapatkan perawatan scaling inisial, penghalusan akar, dan penyesuaian oklusi, dilanjutkan bedah periodontal gigi 16, 15 dan 26. Pada prosedur flap dilakukan debridemen poket dan eliminasi jaringan granulasi dilanjutkan pemberian bone graft dengan PRF pada gigi 16, 15 dan pemberian bone graft dengan membran alloplastik (periosteum) pada gigi 26. Penutupan flap dengan penjahitan interdental interrupted. Bedah periodontal regeneratif dilakukan untuk menghasilkan terbentuknya pertumbuhan jaringan periodontal yang baru dan mengembalikan perlekatan jaringan ikat pada gigi. Evaluasi sesudah bedah regeneratif periodontal terlihat bahwa pada kombinasi bone graft dengan PRF atau membran alloplastik (periosteum) menunjukkan penurunan Clinical Attachment Loss (CAL) dan radiograf menunjukkan bertambahnya ketinggian tulang alveolar. Perawatan bedah regeneratif periodontal kombinasi bone graft dengan PRF atau membran alloplastik (periosteum) dapat meningkatkan perlekatan jaringan periodontal pada kasus periodontitis.

Abstract

Background: Of the various techniques developed for the treatment of gum recession, the current innovation in the use of platelet rich plasma (PRF) has been applied to the treatment of root-end closure procedures. Purpose: This study analyzed the effect of the coronally advanced flap (CAF) in combination with platelet-rich fibrin (PRF) during treatment of gingival recession. Methods: This research constituted an experimental study incorporating a split-mouth design which was conducted on eight participants (with 16 recession defects) who were divided into two groups consisting of the CAF group with PRF and another CAF group without PRF. Clinical parameters consisting of gingival recession (GR), keratinized gingival width (KGW), recession width (RW) and clinical attachment level (CAL) were measured both before and 21 days after surgery). Results: The average differences between clinical parameters in the test group were as follows: GR (2.25 ± 0.27), KGW (1.81 ± 0.75), RW (3.44 ± 1.35), and CAL (2.56 ± 0.50); while in the control group they were as follows: GR (2.00 ± 0.71), KGW (1.94 ± 0.78), RW (3.50 ± 1.91) and CAL (2.00 ± 0.76). There were significant differences in the GR and CAL between the test and control groups (p ≤ 0.05). Conclusion: A combination of the CAF procedure and PRF was proven to be more effective in covering the GR and increasing the CAL.

Abstract

Background: Autism Spectrum Disorder (ASD) or autism is a disorder that occurs during brain development characterized by deficiencies in social behavior and nonverbal interactions such as lack of eye contact, facial expressions, and body movements before a child is three years old. Autism Spectrum Disorder (ASD) is not a single disorder and is generally considered a multi-factorial disorder caused by the interaction of genetic and non-genetic factors. Lack of social behavior includes avoiding eye contact, difficulty controlling emotions or understanding the emotions of others, and limiting the range of activities and interests. Case report: A patient, female, 7 years and 4 months complained that the maxillary front teeth shake and want to be removed. Patients are suspected of having symptoms of autism since the age of 2 years and have started screening at that age until finally diagnosed with autism. Autism in patients, in this case, is suspected of autism from genetic factors. Conclusion: People with autism require special examination and treatment regarding the health of their teeth and mouth. These actions require cooperation between dentists and pediatricians or psychologists or other related disciplines so that the desired end results can be achieved as well as possible.
