Abstract

Background: RSUD dr. H. Kumpulan Pane (RSKP) has not yet successfully implemented real-time incident reporting as stipulated by the Minister of Health Regulation No. 11 of 2017. Objectives: Based on this situation, this study aims to analyze the condition of the patient safety incident reporting system in the inpatient unit at RSKP, identify the inhibiting factors, and propose priority policy alternatives to improve the system. Research method: To achieve these objectives, the Grounded Theory method, emphasizing qualitative data analysis from informants including structural officials, healthcare workers, and the quality and patient safety committee at RSKP, was used to reveal the conditions and inhibiting factors of patient safety incident reporting in the inpatient unit at RSKP. Meanwhile, to obtain priority alternatives for improving the incident reporting conditions, the researcher used the Analytical Hierarchy Process method. Results: The results of the study indicate several weaknesses in the reporting conditions, including very minimal incident reporting actions, incomplete implementation of system-oriented approaches, lack of coordination regarding assurance of immunity from punishment for incident reporting actions, unguaranteed independence from external parties, and partial patterns of patient safety incident resolution.  Conclusion: Patient safety incident reporting in the inpatient unit of RSKP remains suboptimal due to a weak reporting culture, insufficient training, unsupportive leadership, and an underdeveloped system. Strategic actions such as timely disbursement of service incentives, the establishment of a dedicated SPO improvement team, and the implementation of proportional sanctions are essential to enhance the overall quality of incident reporting.

Abstract

Anemia is a condition of low hemoglobin levels in the blood. This condition often occurs in adolescent girls due to menstrual cycles and a tendency to diet. Adolescent girls who suffer from anemia are at risk of giving birth to babies with low birth weight and stunting. The incidence of anemia is very high in North Sumatra, one of which is in Sei Rampah District. Because of this condition, researchers feel the need to make efforts to reduce the incidence of anemia by providing iron tablets. The action research method was carried out to determine whether there were changes in hemoglobin levels in the anemia group of grade XII SMAN 1 Sei Rampah students who were given intervention. The students' hemoglobin levels were measured to group which ones had the potential for anemia, then given iron tablets once a week for 4 weeks and then their hemoglobin levels were measured again. Action research plays a role in achieving a decrease in the incidence of anemia. Researchers concluded that there was an increase hemoglobin levels by 96.7% in the group of female students categorized as anemic after the intervention of anemia concept education and the provision of iron tablets with assistance.

Abstract

Background: The implementation of the Hospital Management Information System (SIMRS) at the Regional Public Hospital of Sidikalang (RSUD Sidikalang) aims to enhance the efficiency and quality of healthcare services. However, technical challenges such as the lack of data integration across units and the readiness of human resources (HR) hinder the success of its implementation. Research Objectives: This study aims to analyze the implementation conditions of SIMRS at RSUD Sidikalang, identify the factors that hinder its success, and provide policy recommendations to improve the system’s efficiency, focusing on HR readiness, technological infrastructure, and hospital management support. Research Methodology: This research uses a quantitative approach with a descriptive correlational design. Data was collected through surveys distributed to 161 employees of RSUD Sidikalang involved in the management of SIMRS. Data analysis was performed using the Chi-Square test to determine the relationship between variables, and multiple logistic regression to analyze the factors most affecting SIMRS implementation. Research Results: The findings indicate that the Human Resources (HR) factor has a significant impact on the success of SIMRS implementation. Respondents who rated HR positively were more likely to assess SIMRS implementation as good. Technology and organizational factors also played a crucial role in enhancing the effectiveness of SIMRS. Conclusion: The success of SIMRS implementation at RSUD Sidikalang heavily depends on HR readiness, the quality of technological infrastructure, and hospital management support. Continuous HR training, adequate technology maintenance, and improved coordination between hospital units are key to maximizing the benefits of SIMRS.

Abstract

Background: The medication service at the Outpatient Pharmacy of RSUD Drs. H. Amri Tambunan experiences long waiting times, especially for BPJS guarantee prescriptions, which contradicts the established service time standards, i.e., dispensing medications ? 30 minutes and preparing compounded medications ? 60 minutes, in accordance with health service regulations. Research Objective: This study aims to analyze the waiting times for medication services at the Outpatient Pharmacy, identify types of waste within the service process flow, and identify factors that hinder service efficiency. Additionally, the study aims to provide recommendations for improvement to reduce waiting times and enhance service quality. Research Method: This qualitative study uses Value Stream Mapping (VSM) to analyze the medication service process and identify waste in each service stage. Additionally, a fishbone diagram is used to identify root causes affecting waiting times. Data is collected through interviews with pharmacy staff, the head of the pharmacy installation, and direct field observations. Research Results: This study found various types of waste in each stage of the medication service, including waiting, motion, defects, and overproduction. Factors hindering service time include patient congestion during peak hours, medication unavailability, and prescription inconsistencies with the formularium. Suboptimal use of information systems and a shortage of pharmacists were also identified as major hindrances. Conclusion: Improvements in process flow, coordination between departments, routine training, and the implementation of an integrated hospital management information system are needed to minimize waste and expedite service times at the Outpatient Pharmacy of RSUD Drs. H. Amri Tambunan.
