Abstract

Abstract

Abstract

Background: Sarcopenia has a notable impact on the prognosis of hepatocellular carcinoma (HCC). The prevalence of sarcopenia in patients with HCC is relatively high, with studies reporting rates between 39% and 41.7%. A substantial proportion of individuals diagnosed with HCC also exhibit muscle wasting and reduced muscle function. This study aims to determine the prevalence of sarcopenia in Indonesian patients with HCC (based on the Japanese Socuety of Hepatology (JSH) JSH criteria) and to examine its correlation with poor performance status. Methods: We performed a cross-sectional study of 85 HCC patients between January and October 2021). The skeletal muscle index at L3 (L3 SMI) was measured on CT scans (applying JSH cut-offs for sarcopenia). Clinical data, including Eastern Cooperative Oncology Group (ECOG) performance status, were collected. Bivariate analyses and logistic regression were performed to identify factors associated with sarcopenia. Results: A total of 85 HCC patients (median age: 52 years) were included in the analysis. Sarcopenia was observed in 49.4% of the cohort. Bivariate analysis revealed a statistically significant association between sarcopenia and an ECOG performance status of 2+ (p = 0.003), as well as a MELD-Na score of ≥15 (p = 0.023). In multivariate analysis, poor ECOG-PS remained independently associated with sarcopenia (adjusted OR 4.17; 95% CI 1.50–11.56, p = 0.006).Conclusion: Sarcopenia is highly prevalent among Indonesian HCC patients, and it is strongly associated with poor ECOG performance status.

Abstract

This study aims to evaluate the management of acute diverticulitis complicated by hernia and the role of electroacupuncture in managing postoperative ileus. The case involved a 51-year-old male patient who presented with left lower abdominal pain, fever, and defecation disorder. Physical examination showed signs of infection with localized peritonitis, while laboratory tests revealed neutrophilia. CT scan showed diverticulitis with abscesses in the descending and sigmoid colon, as well as free air around the colon and perihepatic region, and paraesophageal hernia. The patient underwent left hemicolectomy with Hartmann's procedure. Postoperatively, the patient developed ileus which was treated using electroacupuncture. The results showed that the relationship between diverticulosis and hernia is due to connective tissue changes. Approximately 10-20% of diverticulosis cases progress to acute diverticulitis. CT scan is the gold standard for diagnosis, staging and management of diverticulitis. In cases of Hinchey III diverticulitis, laparoscopic washing does not give better results compared to colonic resection. The Hartmann procedure is recommended for patients with multiple comorbidities. Electroacupuncture proved to be an effective adjunctive therapy in managing postoperative ileus. The conclusion of this study is that a multimodal approach, including surgery and complementary therapies such as electroacupuncture, is essential in the management of diverticulitis and its complications. The implication of this study is the need for a holistic approach in managing patients with acute diverticulitis, especially in cases with multiple comorbidities and risk of postoperative complications.

Abstract

Pemeriksaan magnetic resonance cholangiopancreatography (MRCP) menunjukkan adanya batu duktus koledokus. Intervensi berupa endoscopic retrograde cholangiopancreatography (ERCP) disertai ekstraksi batu dan kolesistektomi laparoskopik memperbaiki keluhan nyeri perut. Kolelitiasis perlu dipertimbangkan sebagai diagnosis banding keluhan dispepsia.   Cholelithiasis is often encountered in daily practice. Besides biliary colic, cholelithiasis may show atypical manifestations as dyspepsia. This may lead to misdiagnosis and inappropriate therapy. This case report reemphasized the importance of careful evaluation of dyspepsia. A 33-year-old woman was admitted with epigastric pain. She was diagnosed with dyspepsia, but proton pump inhibitor (PPI) did not result in any improvement. On physical examination, fever, icteric sclera, and right upper quadrant abdominal tenderness were found. Laboratory examination showed neutrophilia and hyperbilirubinemia. An abdominal ultrasound (USG) examination revealed gallbladder stones, cystic duct stones, and cholecystitis. Magnetic resonance cholangiopancreatography (MRCP) examination revealed the presence of stone in the common bile duct. The patient underwent endoscopic retrograde cholangiopancreatography (ERCP) with stone extraction and laparoscopic cholecystectomy. After the procedure, the abdominal pain improved. Cholelithiasis should be considered a differential diagnosis of dyspepsia.
