Abstract

The drastic increase in the volume of organic waste is an unavoidable consequence of population growth and rapid development, both in rural and urban areas. This condition is exacerbated by urbanization and rising living standards, which are directly correlated with an increase in per capita waste production. The phenomenon of urbanization, particularly in Indonesia, has led to a concentration of population in urban areas, which in turn has resulted in a rapid increase in waste generation. Existing management capacity is often inadequate to handle this ever-increasing volume of waste, leading to serious accumulation and environmental pollution. Islamic boarding schools, as Islamic educational institutions, have great potential for managing the environment based on a circular economy. This institution, with its values of independence and togetherness, has strong social capital to initiate and implement sustainable waste management practices integrated with circular economy principles, as demonstrated in several community-based programs. Various initiatives have proven that pesantren are capable of developing independent business units thru waste utilization, for example, in the innovation of organic fertilizer from livestock manure. This community service program was conducted at the Ulumul Qur'an Islamic Boarding School in Stabat, Langkat Regency, with the aim of optimizing organic waste management thru the cultivation of Black Soldier Fly (BSF) or maggots as an environmentally friendly solution and an entrepreneurial opportunity for students. The methods used included socialization, technical training, and the Participatory Learning and Action (PLA) approach to encourage active student involvement. The results of the activity showed an increase in students' understanding of organic waste management, the utilization of boarding school land for maggot cultivation, and the development of environmentally based entrepreneurial skills. This program not only reduces the volume of organic waste but also produces economically valuable alternative feed that can support agriculture and livestock farming. Thus, this activity contributes to the creation of a sustainable waste management system in pesantren while also supporting the economic empowerment of students.

Abstract

Introduction: Anesthetic management for non-neurosurgical procedures in patients with recent severe traumatic brain injury (TBI) presents a formidable challenge. General anesthesia carries inherent risks of hemodynamic instability and increased intracranial pressure (ICP), which can precipitate devastating secondary brain injury. Regional anesthesia offers a neuroprotective alternative, though its application in this specific high-risk population is not extensively documented. Case presentation: A 24-year-old male, ASA status III-E, required open reduction and internal fixation of a clavicle fracture six days after an emergency decompressive craniectomy for an acute epidural hematoma. To mitigate neurological risk, a definitive anesthetic plan consisting of an ultrasound-guided dual plexus blockade was implemented. This involved a combination of an interscalene brachial plexus block (15 mL of 0.375% levobupivacaine) and a superficial cervical plexus block (10 mL of 0.375% levobupivacaine), supplemented with light, non-opioid sedation using dexmedetomidine. The 150-minute surgery was completed with exceptional hemodynamic stability, no requirement for airway manipulation, and no anesthetic or surgical complications. The patient remained comfortable and neurologically intact throughout. Conclusion: This case demonstrates that an ultrasound-guided dual plexus blockade is a safe, effective, and neurologically protective primary anesthetic technique for clavicle surgery in the post-craniotomy patient. By providing dense surgical anesthesia while preserving stable cerebral perfusion pressure, this approach represents a superior alternative to general anesthesia in this fragile patient population. We advocate for its consideration in similar clinical scenarios.

Abstract

The electronic Samsat application (e-Samsat) at the Regional Revenue Management Technical Implementation Unit (UPTD) Medan Selatan is expected to enhance motor vehicle taxpayers' compliance. This compliance is crucial for optimizing regional tax revenue and supporting regional development. However, low taxpayer awareness and the complexity of payment procedures remain challenges that need to be addressed. This study aims to analyze the impact of the e-Samsat service on taxpayer compliance. The research employs a quantitative approach using a survey method, collecting data through questionnaires distributed to motor vehicle taxpayers in the UPTD Medan Selatan area. The analysis technique used is simple linear regression to determine the influence of the e-Samsat service on taxpayer compliance. The results show that the e-Samsat service positively affects compliance. The application facilitates payment access, reduces waiting times, and increases taxpayer understanding of their tax obligations. However, technical issues and a lack of understanding of how to use the application still pose challenges. Therefore, the government is advised to intensify outreach efforts, improve technological infrastructure, and enhance service quality to maximize the positive impact of e-Samsat.

Abstract

The delay in budget absorption that occurred at the Asahan District Health Service was due to several causes. The research objectives to be achieved from this research are analyzing the Work Plan and Budget with the realization of activities, analyzing factors that influence budget realization at the end of the year, and analyzing the consequences of budget delays that occur at the Asahan District Health Service in the 2023 budget year. Researchers use qualitative research with explanatory methods. The findings that can be conveyed in the results of this research include: adjustments to the Work Plan and Budget with the realization of activities for the 2023 fiscal year at the Asahan District Health Service due more to the budget process. This means a budget policy with the principle of quality and targeted spending. Apart from that, the realization of the budget at the end of the year that occurred at the Asahan District Health Service was more due to technical instructions, human resource capacity, especially knowledge of PMK212 regulations, and budget sources such as PAD that was not reached targets and provisions of transfer funds from the center that were not in accordance with the schedule. Consequences of budget delays at the Asahan District Health Service, which can reduce the budget ceiling in the following year and results in activities not being realized.
