Abstract

Latar Belakang : Rumah sakit merupakan salah satu sarana upaya kesehatan masyarakat yang menyelenggarakan kegiatan pelayanan kesehatan masyarakat. Peran sumber daya manusia rumah sakit sangat penting dalam menentukan kualitas jasa rumah sakit baik (Ilyas, 2011). Penelitian ini bertujuan untuk mengetahui jumlah psikiater di Unit Rawat Jalan Jiwa Rumah Sakit Ernaldi Bahar tahun 2015. Metode : Penelitian ini merupakan penelitian kuantitatif deskriptif dengan desain crosssectional menggunakan metode worksampling.Pengamatan dimulai 6-18 April 2015.Objek penelitian adalah seluruh dokter spesialis jiwa yang bertugas di Unit Rawat Jalan Jiwa sebanyak tiga orang. Data dianalisis dengan menggunakan metode Workload Indicator Staffing Needs (WISN). Hasil Penelitian : Hasil analisis menunjukkan waktu kerja tersedia psikiater adalah 97.920 menit pertahun, Standar beban kerja untuk assesmen pasien sebesar 12.240 ,konsultasi 19.548, diagnosa dan terapi 97.920 dan tandatangan blanko pasien sebesar 979200. Standar kelonggaran menyita waktu 0,199 atau 19,9 %. Berdasarkan metode perhitungan WISN dibutuhkan sebanyak 6 orang psikiater tenaga yang tersedia berjumlah 3 orang (ratio WISN <1) beban kerja tinggi. Kesimpulan : Dalam efisiensi kerja psikiater yang ada pihak manajemen rumah sakit perlu mengatur dan memantau  jadwal psikiater  serta memotongan jasa pelayanan bagi psikiater yang tidak hadir pada waktu kerja.   Kata kunci : beban kerja, psikiater, unit rawat jalan jiwa.

Abstract

The eruption of Mount Sinabung impacted not only people living at the foot of volcano, but also surrounding communities. Although damage was not as severe as that experienced by displaced people, the eruption of Mount Sinabung had an influence on lives of people living in Sinabung area, particularly in agricultural sector. The agricultural production of community has diminished as a result of Mount Sinabung's eruption. Aside from lower agricultural production, lower selling prices are also a result of lower quality. This research will show how Karo Regency Agriculture Office empowers Peasants horticulture who have been impacted by Mount Sinabung's eruption. The descriptive qualitative method was employed in this study. The findings revealed that Karo Regency Agriculture Service's execution of empowerment horticultural Peasants following Mount Sinabung's eruption was still not optimal due to a number of circumstances. These factors include that Mount Sinabung is still erupting, mentality of people who have yet to accept instructor's presence, and fact that quality of extension resources is still inadequate, both in terms of expertise and field demands. The absence of field people, presence of extension workers who are members of structural posts, and a limited budget are all hurdles to horticultural Peasants’ empowerment.

Abstract

This research explores Peasants’ social challenges in Indonesia and UNDROP's response to these challenges. Challenges include limited access to resources and land, agrarian conflict, difficulty accessing adequate seeds, and economic uncertainty. UNDROP provides a vital framework to fight for Peasants’ rights, emphasizing land, seed, and food rights. A qualitative research method was used using the UNDROP perspective. Data was collected through primary and secondary sources and analyzed using data reduction, presentation, and conclusion-drawing methods. The results show the need for more vigorous law enforcement and better protection of Peasants’ rights. Efforts to address Peasants’ social challenges include the implementation of the Peasant Protection and Empowerment Act, providing financial and technical support, and implementing UNDROP by the government and various relevant parties. This study concludes that collaboration between the government, Peasant organizations, NGOs, and the international community is crucial in ensuring the protection and welfare of Peasants. Implementing UNDROP is crucial in ensuring Peasants’ rights are protected. This research contributes significantly to understanding the social challenges Peasants face in Indonesia and the efforts that can be made to address these challenges effectively.

Abstract

Introduction: Accurate evaluation of volume status is essential for appropriate therapy because inadequate assessment of volume status can result in unnecessary administration of therapy, which can increase mortality. This study aimed to describe the use of inferior vena cava ultrasound in assessing fluid overload in severe preeclampsia patients. Case presentation: We report a 23-year-old female patient with a diagnosis of acute pulmonary oedema, and severe preeclampsia at G1P0A0 40 weeks gestation with complaints of shortness of breath. On physical examination, the patient appeared short of breath with RR 32 x/minute, fine wet crackles in both lung fields, SpO2 92% with NRM 15 L/m, HR 160 x/minute, lifting strength, CRT < 2 seconds, blood pressure 160/120 mmHg. This patient underwent emergency termination of pregnancy by C-section under general anesthesia rapid sequence induction followed by intensive care in the ICU. Management in the ICU this patient was given mechanical ventilation, midazolam sedation 0.05 mg/kg, analgesic morphine 10 mcg/kg, fluid restriction with a fluid balance target of (-)1000 ml/24 hours and given furosemide 10 mg/hour to reduce fluid overload so that fluid in the lungs can be reduced. Evaluation of fluid overload by IVC ultrasound. Conclusion: Acute pulmonary edema requires proper management to get a good outcome. Measurement of the diameter of the inferior vena cava (IVC) can also be used to assess fluid volume status. Lack of volume is assessed with an IVC diameter of <1.5 cm, while an IVC diameter of >2.5 cm indicates volume overload.

Abstract

Introduction: Blunt trauma from motor vehicle accidents (MVA) often results in multisystem injuries, including chest, head, and musculoskeletal injuries. Management of these complex injuries requires a multidisciplinary approach and can present unique anesthetic challenges. Case presentation: A 22-year-old man was treated in the emergency department (ER) after experiencing an MVA. The patient experienced chest and shoulder pain and was found to have anemia, increased creatinine, respiratory acidosis, prolonged prothrombin time (PT), and increased serum glutamic oxaloacetic transaminase (SGOT). Radiological examination revealed left tension hydropneumothorax, right hydropneumothorax, bilateral lung contusions, fractures of the ribs, clavicle, and scapula. In addition, the patient also experienced subarachnoid hemorrhage (SAH) in the left parietotemporal region and right cistern (Fisher Scale III), brain edema, right pneumo-orbita, type III septal deviation, bilateral inferior nasal concha hypertrophy, and concha bullosa on the medial nasal concha. The patient's physical status was assessed as ASA III. The patient underwent open reduction and internal fixation (ORIF) with an S-plate on the right clavicle. Anesthesia was provided with an interscalene block using 0.375% levobupivacaine and premedication with Fentanyl and Midazolam. Conclusion: This case highlights the complexity of managing blunt trauma patients with multisystem injuries. A multidisciplinary approach, including careful airway management, hemodynamic monitoring, and selection of appropriate anesthetic techniques, is essential for optimal results.

Abstract

Introduction: Central venous catheterization (CVC) is frequently required in intensive care units (ICUs) for administering medications, fluids, and monitoring central venous pressure. However, CVC insertion can lead to complications such as arterial puncture, hematoma formation, and pneumothorax. Ultrasound guidance has been advocated to reduce these complications, but its effectiveness in the ICU setting remains debated. This study compared the complication rates of anatomical landmark-guided versus ultrasound-guided CVC insertion in ICU patients. Methods: A prospective cohort study was conducted in the ICU of a tertiary care hospital. Patients requiring CVC were divided into two groups: anatomical landmark-guided and ultrasound-guided insertion. The primary outcome was the incidence of complications, including arterial puncture, hematoma, and pneumothorax. Secondary outcomes included cannulation time and the number of cannulation attempts. Results: A total of 39 patients were included in the study. The incidence of complications was significantly lower in the ultrasound-guided group (2 complications) compared to the anatomical landmark group (7 complications) (p=0.017). The most common complication was arterial puncture, occurring in 7 patients in the anatomical landmark group and 2 patients in the ultrasound-guided group. Conclusion: Ultrasound guidance significantly reduces the risk of complications during CVC insertion in the ICU. This technique should be considered the standard of care for CVC insertion in this setting.

Abstract

Public service is any form of service or service provided by a government or public institution to the public in general. Administrative services are intended to facilitate the needs of citizens in administrative matters such as identity cards, business permits and other administrative services. This study aims to analyze the performance of administrative services in Kantor Lurah Sei Putih Tengah and to observe any obstacles that are inhibiting the functional performance in this Lurah Office. It shows that there are still obstacles in the performance in the administrative service in Kantor Lurah Sei Putih Tengah. The research method used is a descriptive method with a qualitative approach aimed at understanding in depth how and why a phenomenon occurs, as well as to dig the meaning contained in it from the point of view of the subject being studied. Data collection is done with interviews, observations, and documentation techniques. The results of the study show that the officers of the Kantor Lurah Sei Putih Tengah have performed their basic duties and functions fairly well. It's seen from the division of work that's not just accumulated in one of the officers. Lurah as head of office and secretary lurah as chief of office has done his job while monitoring and overseeing his employees. In terms of timing, it appears that the Lurah Sei White Office has already tried to complete the letter on time but there are still frequent delays because Lurah is often not in the office and can not sign the letter. In the case of typing also Lurah Sei White Central Office still frequently occur typing errors caused by the number of files that come in with the same deadline so that the lack of concentration resulted in typing mistakes to writing errors on the file. Kantor Lurah Sei Putih Tengah has performed its administrative tasks quite well but not to the maximum.

Abstract

The anesthetic management of patients with multiple severe comorbidities presents a formidable clinical challenge. Pathological fractures secondary to metastatic disease often require procedural intervention in individuals with pre-existing cardiopulmonary, airway, and cerebrovascular compromise. General anesthesia in such patients carries a prohibitively high risk of hemodynamic collapse, airway loss, and perioperative mortality. We present a case where a targeted regional anesthetic technique was pivotal in ensuring patient safety. A 59-year-old woman with a pathological fracture of the right humerus, suspected to be a metastasis from thyroid carcinoma, was scheduled for a core needle biopsy. Her medical history was profoundly complex, including a massive, airway-compressing thyroid goiter, persistent atrial fibrillation, recent ischemic stroke with residual hemiparesis, bilateral pneumonia with pulmonary edema, uncontrolled type 2 diabetes mellitus, and acute-on-chronic kidney disease. Given the extreme risks associated with general anesthesia and airway manipulation, we opted for an ultrasound-guided right supraclavicular brachial plexus block as the sole anesthetic. The block was performed successfully using 20 mL of 1.5% lidocaine with epinephrine, providing dense sensorimotor anesthesia of the upper limb. The patient remained hemodynamically stable, spontaneously breathing, and comfortable throughout the procedure, with no perioperative complications. In conclusion, this case demonstrates that an ultrasound-guided supraclavicular block is a safe, effective, and hemodynamically superior alternative to general anesthesia for upper limb procedures in a patient with a confluence of critical airway, cardiac, pulmonary, and neurological comorbidities. This approach obviates the need for airway instrumentation, preserves spontaneous ventilation, and minimizes systemic physiological trespass, thereby enhancing patient safety in the highest-risk surgical populations.

Abstract

Improving the quality of healthcare services requires the implementation of a patient safety culture as the primary pillar for consistently integrating safety principles policies and procedures. This research is carried out to examine the implementation of patient safety culture management at the Merek Community Health Center, Karo Regency, which still faces various obstacles such as a lack of guidelines, limited resources, and insufficient training. This research used a descriptive qualitative approach through interviews, observations and documentation, and analysis referring to George C. Edwards III's policy implementation theory and the dimensions of patient safety culture. The findings point to the conclusion that implementation has not been carried out comprehensively, especially in aspects of communication, understanding of health workers, and reporting and learning culture. Continuous training, a more flexible reporting system, and institutional support are needed to strengthen the holistic application of patient safety values and practices.

Abstract

Job opportunities are available for prospective workers who are looking for work. The existence of job opportunities in an area will greatly open up opportunities for labor absorption, such as opening job opportunities for local inter-worker (AKL) workers, which is the placement of workers between regencies/cities working within one province. The research method used in this study is a descriptive research method with a qualitative approach. Data collection techniques were carried out through interviews, observations, and documentation at the Pematang Siantar City Manpower Office. All data obtained will be analyzed qualitatively with all data collected using the theory of Fred R. David (2019), namely Strategy Implementation with 3 variables, namely Strategy Development, Human Resource Allocation, and Determining Budget Funds. Based on the research that has been conducted, it can be seen that the Pematang Siantar City Manpower Office has attempted to provide the best solution by designing several programs to provide more job opportunities. Viewed from the development strategy perspective, the Manpower Office has realized several work programs such as job fair events held, competency-based training programs that are already running, and focusing on information about the labor market. The analysis revealed several shortcomings, including the government's inconsistency in implementing the program. Furthermore, the government's human resource allocation has been unable to improve the quality of human resources, particularly in Pematang Siantar City. Furthermore, the government's budget allocation has been ineffective in determining appropriate funding, resulting in limited funding and ineffective program implementation.
