Abstract

The most common cause of hyperthyroidism is Graves' disease. TRH and TSH are hormonal factors that modulate and control thyroid function in Graves' disease. In the immunological aspect, Graves' disease is played by the role of T-reg, IL-4, and anti-TPO. Graves' disease treatment goal is to inhibit thyroid hormone secretion by administering thionamide. The evaluation of this treatment is its hormonal and immunological aspects. To describe the effect of thionamide on serum TRH, TSH, IL-4, T-reg, and anti-TPO levels in Graves' disease. This study is a clinical trial study in 25 study participants. All study participants were given thionamide, namely PTU 300mg for three months and blood samples were taken for laboratory tests. Serum TRH, TSH, IL-4, T-reg FOXP3, and anti-TPO levels were examined by ELISA. The mean levels at the beginning and after three months of therapy are: serum TRH 92.589pg/mL and 115.944pg/mL; serum TSH 0.041mU/L and 0.223mU/L; serum IL-4 19.759pg/mL and 23.040pg/mL; T-reg FOXP3 gene polymorphism 0.621ng/mL and 0.518 ng/mL; serum anti-TPO 2697.539pg/mL and 2604.710pg/mL. Increased levels of serum TRH and TSH levels were statistically significant. The change in serum IL-4, T-reg FOXP3 gene polymorphism, and anti-TPO levels were not statistically significant. The administration of thionamide in Graves' disease for three months will significantly decrease Wayne index and serum FT4 levels, increase serum TRH and TSH levels.

Abstract

Latar Belakang. Ubi jalar ungu mengandung senyawa antioksidan berupa antosianin, asam fenolat, vitamin A, C, dan E. Antioksidan dapat meredam kerusakan oksidatif pada sel beta pankreas sehingga mampu meningkatkan sekresi insulin serta mengurangi stres oksidatif pada DM. Objektif. Penelitian ini bertujuan untuk mengetahui pengaruh pemberian ekstrak ubi jalar ungu terhadap kadar glukosa darah dan MDA hepar tikus hiperglikemia. Metode. Penelitian ini merupakan true experimental dengan post-test only control group design. Sebanyak 24 ekor tikus dibagi menjadi empat kelompok, yaitu kontrol negatif (K-), kontrol positif (K+), perlakuan satu (P1), dan perlakuan dua (P2). Kelompok P1 dan P2 diinduksi aloksan dan diberikan ekstrak ubi jalar ungu dosis 150 mg/kgBB dan 200 mg/kgBB selama 21 hari. Hasil. Terdapat perbedaan signifikan kadar glukosa darah antara kelompok P1 dan P2 dengan kelompok K+ dengan nilai p = 0,000. Terdapat perbedaan signifikan kadar MDA hepar antara kelompok P1 dibandingkan K- dan K+ dengan nilai p = 0,001 dan p = 0,046, kelompok P2 dibandingkan K- dan K+ dengan nilai p = 0,037 dan p = 0,001. Kesimpulan. Ekstrak ubi jalar ungu berpengaruh dalam menurunkan kadar glukosa darah dan MDA hepar tikus hiperglikemia. Kata kunci: ubi jalar ungu, glukosa darah, MDA hepar Background. Purple sweet potato contains antioxidant compounds such as anthocyanin, phenolic acid, vitamin A, C, and E. Antioxidants can reduce oxidative damaged of mitochondria in pancreatic beta cell resulting an increase in insulin secretion and a decrease oxidative stress in DM patients. Objective. This study aimed to determine the effect of giving purple sweet potato extract on blood glucose levels and liver MDA in hyperglycemic rats. Methods. This study was true experimental with post-test only control group design. A total of 24 rats were divided into 4 groups, namely negative control (K-), positive control (K+), treatment one (P1), and treatment two (P2). P1 and P2 group were induced by alloxan and given purple sweet potato extract dose of 150 mg/kgBB and 200 mg/kgBB for 21 days. Result. There were significant differences between P1 and P2 with K+ group with p value = 0,000. There were significant difference between P1 group with the K- and K+ group with p value = 0.001 and 0,046, P2 compared to K- and K+ group with p value = 0.037 and p = 0,001. Conclusion. Purple sweet potato extract can reduce the level of blood glucose level and MDA in liver tissues.

Abstract

Background: A goiter is an enlargement of the thyroid gland caused by abnormalities of the thyroid gland in the form of malfunctioning or disorders of the structure of the gland and its morphology. According to WHO, the incidence of goiter is still quite high with iodine deficiency as the main cause. Objective: This study aims to determine the description of ultrasound examinations in goiter patients in Department of Radiology DR. M. Djamil January-December 2019 period. Method: This research was a retrospective descriptive study using cross sectional method using secondary data on 123 patients with thyroid goiter who meet the inclusion criteria in RSUP DR. M. Djamil Padang period January 2019-December 2019. The sampling method used a total sampling technique. Result: The results showed that 112 patients with thyroid goiter disorders met the inclusion criteria. The most clinical diagnosis results were goiter nodusa as many as 61 people (54.5%). The highest incidence was in the age range 41-50 years with 35 people (31.3%). The most gender was 88 women (78.6%). The results of ultrasonography in most goiter patients were diffuse in 43 people (38%).Conclusion: The conclusion of this study, it was found that the most age group of patients suffering from goitre were 41-50 years old, the majority of goiters occurred in the female compared to the male and the results of the most thyroid ultrasound images were diffuse goiter

Abstract

Background: Gestational diabetes mellitus (GDM) is defined as a condition of glucose intolerance that is found during pregnancy. In pregnant women, urinary tract infection (UTI) is one of the most common bacterial infections that can be found. The infection can be worsened by diabetes as it was shown that the risk of UTI in pregnant women with diabetes (27.6%) is higher than those without diabetes (3%-10.1%). This study aimed to present a case of gestational diabetes mellitus and urinary tract infection. Case presentation: A pregnant woman aged 38 years old with a 15-16 weeks gestational age was admitted with gestational diabetes mellitus and urinary tract infection. The patient was treated with an intravenous insulin drip and antibiotic. The patient was discharged after there was a clinical improvement. Conclusion: There are many risk factors that can contribute to the development of UTI in pregnant women, with one of those being inadequate glycemic control. The treatment of UTI in pregnant women with GDM is generally the same as in those without GDM.

Abstract

Latar Belakang: Diabetes melitus gestasional (DMG) merupakan penyakit metabolik berupa gangguan toleransi karbohidrat dengan onset pertama terjadi saat kehamilan. Interleukin-6 (IL-6) diketahui terlibat dalam inflamasi pada DMG. Kemangi (Ocimum basilicum) dilaporkan sebagai tanaman herbal yang mengandung zat aktif yang bersifat anti-inflamasi Objektif: Penelitian ini bertujuan melihat efek ekstrak kemangi terhadap ekspresi IL-6 pada tikus model DMG. Metode: Sampel RNA leukosit berasal dari tikus DMG yang diinduksi streptozotosin sebelumnya. RNA berasal dari tikus hamil normal (K-; n=6), tikus DMG tanpa perlakuan (K+; n=6), tikus DMG yang diberikan ekstrak kemangi secara oral dengan dosis 100 mg/kg (P1; n=6) dan 200 mg/kg (P2; n=6) selama 14 hari. Ekspresi RNA IL-6 dan GAPDH (kontrol internal) diukur menggunakan PCR secara semi-kuantitatif dan visualisasi elektroforesis menggunakan ImageJ. Analisis data menggunakan uji one-way ANOVA dan post-hoc Bonferroni Hasil: Hasil penelitian menunjukkan terdapat perbedaan signifikan penurunan ekspresi IL-6 antara K+ dengan P2 (p<0,01) dan antara P1 dengan P2 (- p<0,001). Kesimpulan: Kesimpulan yang didapat adalah terdapat pengaruh pemberian ekstrak kemangi terhadap ekspresi IL-6 pada tikus DMG.

Abstract

Seorang perempuan usia 34 tahun datang dengan keluhan lemah pada kedua tangan dan kaki, benjolan di leher, riwayat berdebar-debar, penurunan berat badan dan siklus menstruasi yang tidak teratur. Pasien didiagnosis dengan tiroiditis Hashimoto dan sindrom Gitelman. Pasien diterapi dengan levotiroksin dan koreksi elektrolit. Pasien kemudian pulang dalam keadaan perbaikan klinis.
