Abstract

Background: Uncontrolled asthma leads to patients' poor outcomes and decreases health-related quality of life/HRQoL. Little research has been done to analyze the determinants of  HRQoL in asthmatics patients. Objective: To determine asthma control and  patients' compliance levels, and evaluate the appropriateness of medicine use and determinants of HRQoL. Methods: During this prospective study, adult asthmatic patients who currently experienced asthma attack were evaluated in asthma clinic of a district hospital in Jakarta and administered study questionnaires. Demographic variables and data related to asthma control, patients' compliance level and the appropriate use of medications were summarized using descriptive statistics. The determinants of HRQoL were determined using bivariate and multivariate regression analysis. All data were analyzed using SPSS version 22.0. Results: There were 11 males and 43 females aged approximately 45 years old. More than 50% of the patients had uncontrolled asthma and low level compliance to their medications. Nearly all medicines used were inappropriate according to National Asthma Treatment Guideline. Bivariate correlation test revealed four factors which significantly determined the total score of HRQoL, namely asthma duration (P=0.033), asthma control level (p=0.007), asthma severity level (p=0.001) and the presence of smoke exposure in neighbourhood (p=0.032). Further, multivariate analysis showed only the presence of smoke exposure  significantly affecting patients' quality of life. Conclusion: This study uncovered the majority of patients had uncontrolled asthma status and low level of compliance to their medications. In addition, this study highlighted the exposure of smoke exposure as the solely determinant of HRQoL amongst  asthmatic patients.

Abstract

Salah satu tanaman berkhasiat antifertilitas yang banyak tumbuh adalah daun afrika (Vernonia amygdalina Del). Tanaman ini secara empiris untuk mengobati berbagai penyakit, namun belum ada data ilmiahnya. Penelitian ini bertujuan memperoleh ekstrak etanol 70% daun Vernonia amygdalina Del. yang aman digunakan dalam dosis tertentu terhadap hewan uji mencit DDY. Penapisan fitokimia dilakukan berdasarkan pustaka Farnsworth dan Departemen Kesehatan Republik Indonesia tahun 2000. Penetapan kadar flavonoid total menggunakan metode kolorimetri aluminium klorida dan alkaloid total dengan metode kompleks bromocresol green. Uji keamanan I yang dilakukan adalah uji toksisitas akut (LD₅₀) dengan metode Weil C.S. Hasil penelitian menunjukkan bahwa ekstrak daun Afrika mengandung senyawa alkaloid, flavonoid, saponin, tannin, kuinon, dan steroid/triterpenoid. Kadar flavonoid totalnya adalah 1,27%, sedangkan uji keamanan I ditemukan bahwa ekstrak etanol 70% daun Afrika memiliki LD50 yang lebih  besar dari 16 g/kg BB termasuk dalam kategori praktis tidak toksik.

Abstract

The potential incident of medication error in pharmacy unit might arise due to increased workload of pharmacy staff. Excessive workload triggers exhaustion and lack of concentration of the pharmacy staff in pharmacy services. Increasing the number of pharmacy staff is the solution to reduce workload, therefore analysis of workload of pharmacy staff at Graha Permata Ibu Hospital’s pharmacy unit (IFRS GPI) is required to be used in calculating the ideal and productive needs of staff, which is also the purpose of this study. This study is a combination of qualitative and quantitative research with samples from pharmacy personnel activities. The techniques of data collection were obtained through observation using work sampling and in-depth interviews. Data were analyzed using Workload Indicators of Staffing Need (WISN).The result showed direct productive activities amounted to 59.14%, indirect productive activities amounted to 17.22%, 16.99% of non-productive activities, and 6.65% of personnel private activities. Pharmacy staffing need is 26 people. The ratio comparison between existing personnel and staffing need of the pharmacy installation was 0.7, which indicates the numbers of pharmacy personnel are not in accordance with the workload. It concludes a few necessities such as a need for new hires of pharmacy personnel, a new arrangement of work shifts and scheduling, a modification of service flow for requested prescription, a need to maximize the utilization of hospital information systems, a need to increase the role of pharmacists, and to conduct education and continuous training.
