Abstract

Background: Coronary Heart Disease (CHD) is currently one of main causes and the first death in developed and developing countries, including Indonesia. CHD appeared with the clinical appearance of asymptomatic, stable angina pectoris until acute coronary syndrome. ACS is a progressively CHD and takes parameters that describe the process pathogenesis. MPV and platelet count are two parameters in the complete blood count which reflect the activity of platelets in CHD.Objective: This study is to investigate the differences Mean Platelet Volume and Platelet Count in CHD with ACS compared to non ACS.Methods: A cross-sectional study was conducted with collecting data from the medical records of patients with CHD who underwent treatment in the dr. Kariadi General Hospital Medical Center Semarang, the period of January - December 2019. Eighty subjects of 20 non-ACS patients and 60 ACS patients were included in this study which is Mean Platelet Volume and Platelet Count in hematology laboratory were analyzed. The Independent T-test were used for comparing the group means.Results: MPV was found to be higher significantly among ACS patients as compared to non ACS, 9,43 ± 0,3868 fl vs 9,43 ± 0,3868 fl (p-value<0.01) while Platelet Count was lower significantly among ACS patients as compared to non ACS, 282,63 ± 75,084 109/L  vs 322,30 ± 41,980 109/L.Conclusion: There were significantly differences in Mean Platelet Volume and Platelet count between patients with ACS and non ACS.

Abstract

Chronic heart failure is a major public-health problem with a high prevalence, high mortality and complex treatment. A comprehensive analysis is needed to provide optimal therapy to these patients. Non-invasive imaging plays a central part by offering a complete approach in patients with ischemic heart disease (IHD). Cardiac magnetic resonance imaging (CMR) has emerged as an established advanced multi-parametric imaging modality for the functional and anatomical assessment of cardiovascular disease. This review describes the practical aspects of CMR imaging, and then discusses the role of CMR in the diagnosis and management of chronic IHD, its infarct related complications, such as secondary mitral regurgitation, left ventricular (LV) thrombus, and ventricular tachycardia (VT).

Abstract

Latar Belakang: Gagal jantung merupakansalah satu masalah kesehatan masyarakat global dengan jumlah penderita yang diperkirakan terus mengalami peningkatan. Meskipun terdapat kemajuan dalam tata kelolanya, gagal jantung masih memiliki prognosis yang buruk. Vitamin D merupakan mikronutrien yang berpengaruh terhadap prognosis  pasien gagal jantung kronik. Saat ini belum ada data mengenai kadar vitamin D serum pada pasien gagal jantung kronik yang dirawat pada populasi Indonesia. Tujuan penelitian ini adalah untuk mengetahui kadar vitamin D serum pada pasien gagal jantung kronik yang menjalani rawat inap.Metode: Penelitian observasional pada pasien gagal jantung kronik yang dirawat di RSUP. Dr. Kariadi Semarang mulaibulan November 2017 sampaiJanuar1 2018. Pemeriksaan kadar vitamin D serum [25(OH)D] dilakukan dengan metode chemiluminescence immunoassay (CLIA).Hasil: Sebanyak 102 pasien (median usia 55 (22–65) tahun) yang terlibat, terdapat 69 sampel yang memenuhi kriteria seleksi. Median fraksi ejeksi ventrikel kiri adalah 35 (10-76)%. Median kadar vitamin D serum adalah 47.25 (14.25–143.02) nmol/L.Yang menarik,  88,4% pasien menunjukkan kadar vitamin D lebih rendah dari yang direkomendasikan. Dibandingkan dengan pasien dengan kadar vitamin D normal, pasien dengan kadar rendah tersebut lebih sering disertai denganhipertensi atau diabetes, berjenis kelamin perempuan, dan memiliki riwayat rawat inap ulang yang lebih sering.Kesimpulan: Defisiensi vitamin D serum dialami olehsebagian besar pasien gagal jantung kronik yang dirawat. Data ini menunjukkan perlunya pengukuran kadar vitamin D pada pasien gagal jantung kronik dengan hipertensi, diabetes, jenis kelamin wanita dan rawat inap ulang.Kata Kunci : Gagal jantung, defisiensi vitamin D, rawat inap

Abstract

Before molecular medicine era, the lymphatic system was not getting enough attention in medicine world, especially in cardiovascular medicine. Our knowledge about lymphatic system is far left behind compare to vascular system. Many believes lymphatic function is isolated in regulate interstitial fluid hemostasis, fat transport, and immune transport. In deep investigation in lymphatic function shows that alteration in lymphatic vessel could disrupt normal cardiovascular function. Moreover, optimal cardiac lymphatic condition after post-heart tissue injury could enhance tissue regeneration. That is why in modern medicine era preserving optimal lymphatic condition and treat abnormal lymphatic condition might be beneficial in cardiovascular medicine.
