Abstract

Diabetes mellitus (DM) adalah penyakit metabolik yang ditandai hiperglikemik kronis. HbA1c merupakan hasil pemeriksaan untuk melihat kontrol glikemik. Zinc dan Selenium merupakan faktor metaloenzim, berperan dalam mekanisme regulasi dan sintesis  insulin.  Penelitian ini bertujuan untuk mengeksplorasi hubungan antara  Zinc dan  Selenium  dengan HbA1C pada  pasien DMT2.  Desain  cross sectional  terhadap pasien DMT2.  Sampel berjumlah 108 orang pasien di beberapa Rumah sakit di Bandung dari tahun 2011 s/d 2013. Sampel dilakukan secara purposive sampling. Zinc dan Selenium dikumpulkan dengan Semi Quantitative Food Frequency (SQFF).  HbA1c diukur dengan metode afinitas kromatografi. Data dianalisis dengan  Fisher Exact  dan  uji  korelasi  Spearman  (p<0,05).  Penelitian ini telah mendapat persetujuan Komite Etik.  Penelitian menunjukkan bahwa ada hubungan bermakna antara Zinc, Selenium dan HbA1c (p<0,001). Ada hubungan antara Zinc dan Selenium dengan HbA1c,  sehingga penatalaksanaan diit dengan asupan Zinc dan Selenium sangat diperlukan dalam regulasi pasien DMT2.

Abstract

Background: Depression is a mood disorder that is marked by a persistent feeling of sadness, loss of interest; reduced energy and vitality; feelings of low self-worth or guilt; loss of appetite; disturbed sleep or insomnia; and slowness of thought or action. Vitamin D deficiency has been associated with depression. When the level of Vitamin D and serotonin is low, that leads to depression.Objective: To analyze whether vitamin D deficiency or insufficiency is associated with depression.Method: This is a Systematic Review Report. Samples were gathered from two journals – PubMed and Science Direct with specific search terms used were 'vitamin D' and 'depression', then were adjusted based on the inclusion criteria and research question. After that, the findings are combined in order to reach a conclusion so that the research question is well answered.Results: There are eleven journals that describe a link between low level of Vitamin D and symptoms of depression. The connection between the two is formed due to the neuroprotector and immunomodulatory effects of Vitamin D.Conclusion: Previous studies have shown that Vitamin D deficiency linked to depression. Keywords : Depression, level of vitamin D, vitamin D serum.

Abstract

Background: Coronary Heart Disease (CHD) is currently one of main causes and the first death in developed and developing countries, including Indonesia. CHD appeared with the clinical appearance of asymptomatic, stable angina pectoris until acute coronary syndrome. ACS is a progressively CHD and takes parameters that describe the process pathogenesis. MPV and platelet count are two parameters in the complete blood count which reflect the activity of platelets in CHD.Objective: This study is to investigate the differences Mean Platelet Volume and Platelet Count in CHD with ACS compared to non ACS.Methods: A cross-sectional study was conducted with collecting data from the medical records of patients with CHD who underwent treatment in the dr. Kariadi General Hospital Medical Center Semarang, the period of January - December 2019. Eighty subjects of 20 non-ACS patients and 60 ACS patients were included in this study which is Mean Platelet Volume and Platelet Count in hematology laboratory were analyzed. The Independent T-test were used for comparing the group means.Results: MPV was found to be higher significantly among ACS patients as compared to non ACS, 9,43 ± 0,3868 fl vs 9,43 ± 0,3868 fl (p-value<0.01) while Platelet Count was lower significantly among ACS patients as compared to non ACS, 282,63 ± 75,084 109/L  vs 322,30 ± 41,980 109/L.Conclusion: There were significantly differences in Mean Platelet Volume and Platelet count between patients with ACS and non ACS.

Abstract

BACKGROUND: Inflammation occurs in chronic diseases such as type 2 diabetes mellitus (DM). Insulin resistance and inflammation in type 2 DM with obesity can increase interleukin (IL)-6, causing an increase in hepcidin synthesis in the liver. Increased inflammation can exacerbate the course of type 2 DM. This study aims to prove that there are differences in the levels of hepcidin and IL-6 between obese and non-obese type 2 DM.METHODS: This cross-sectional study was conducted on 61 patients with type 2 DM, consist of 22 male and 39 female with an age of more than 40 years. Type 2 DM subjects were obtained from a doctor's diagnosis and were divided into obese and non-obese groups based on body mass index (BMI). Hepcidin and IL-6 levels were examined using the Enzyme-Linked Immunosorbent Assay (ELISA) principle. The data were analyzed using an independent t-test and Mann-Whitney test.RESULTS: The mean level of hepcidin in the obese with type 2 DM group was 25.32±11.54 ng/mL, and non-obese was 11.94±5.31 ng/mL. The median level of IL-6 in the obese with type 2 DM group was 11.9 (5-61) pg/mL, and non-obese 4.8 (1.5-9.8) pg/mL. There was a significant difference in hepcidin and IL-6 levels between the obese and non-obese groups (p=0.000).CONCLUSION: Hepcidin and IL-6 levels in the obese group with type 2 DM were higher than non-obese group.KEYWORDS: type 2 diabetes mellitus, obesity, hepcidin, interleukin-6

Abstract

ABSTRACT Background: expression of VEGF-C and CA 15-3 may be useful to differentiate between malignant and benign breast tumour because VEGF-C plays a role in promoting angiogenesis and lymphangiogenesis in malignant processes and CA 15-3 is the soluble form of transmembrane protein MUC1, a tumour marker which shows higher expression in breast cancer.Objective: to determine the diagnostic value of VEGF-C and CA 15-3 as tumour markers in patients with breast cancer.Methods: this diagnostic study recruited 76 patients that underwent surgical biopsy procedures at Dr. Kariadi and Pantiwilasa Citarum Hospitals Semarang. The VEGF-C and CA 15-3 levels in blood specimens taken before surgical biopsy procedure was determined using ELISA method. An ROC curve and AUC were used to establish the cut-off points and diagnostic value. Pathology examination results from the biopsy specimens were used as the gold standard.Results: the cut-off value for VEGF-C and CA 15-3 were 989.50 pg/mL and 74.00 U/mL. Sensitivity for VEGF-C, CA 15-3 and VEGF-C+CA 15-3 were 76.6%, 64.1% and 89.1%. Specificity for VEGF-C, CA 15-3 and VEGF-C+CA 15-3 were 75.0%, 75.0% and 50.0%. The AUC for VEGF-C, CA 15-3 and VEGF-C+CA 15-3 was 0.831 (95% CI = 0.727-0.934), 0.742 (95% CI = 0.628-0.856) and 0.840 (95% CI = 0.742-0.938).Conclusion: VEGF-C in combination with CA 15-3 is the best diagnostic parameter for breast cancer and has the best accuracy as a tumour marker for breast cancer.

Abstract

Latar belakang: Infeksi saluran kemih (ISK) adalah adanya mikroorganisme dalam urin. Bakteriuria bermakna bila pertumbuhan mikroorganisme murni lebih dari 105 colony forming units (CFU) pada kultur urin. Pemeriksaan laboratorium untuk deteksi ISK adalah dipstik urin (nitrit dan lekosit esterase) dan flowsitometri (jumlah leukosit dan bakteri). Tujuan dari penelitian ini adalah untuk menganalisis hubungan antara hasil pemeriksaan dipstik dan flowsitometri dengan pemeriksaan kultur urin.Metode: Penelitian belah lintang pada 42 sampel pria yang menderita ISK di RSUP Dr. Kariadi Semarang selama bulanAgustus–September 2017. Metode pemeriksaan disptik Combur adalah kolorimetri, metode flowsitometri menggunakan Sysmex UF1000i, dan kultur menggunakan metode konvensional. Uji data korelasi menggunakan contingency coefficient test.Hasil: Tidak terdapat hubungan bermakna dari dipstrik nitrit terhadap kultur (r=0,190 dan p=0,210), terdapat hubungan positif lemah antara dipstik lekosit esterase dengan kultur (r=0,363 dan p=0,012), terdapat hubungan positif kuat dari flowsitometri lekosit terhadap kultur (r=0,534 dan p=0,000) dan terdapat hubungan positif kuat antara flowsitometri bakteri dengan kultur (r=0,534, p=0,000). Simpulan: Tidak terdapat hubungan bermakna dari dipstik nitrit terhadap kultur. Terdapat hubungan bermakna dari dipstrik lekosit esterase, flowsitometri lekosit, terdapat hubungan yang bermakna antara flowsitometri bakteri dengan kultur. Penelitian lebih lanjut diperlukan untuk menganalisis faktor-faktor yang berpengaruh pada diagnostik ISK.

Abstract

Latar belakang: Hipertensi erat kaitannya dengan gangguan vasodilatasi dan disfungsi endotel.Inflamasi vaskular dapat terjadi baik pada permulaan maupun perkembangan hipertensi, inflamasi menimbulkan pelepasan sitokin inflamasi seperti hsCRP. NO berperan sebagai vasodilator pada pembuluh darah. Penurunan NO disebabkan terjadinya penurunan aktivitas nitric oxide synthase (NOS) yang dihambat sintesisnya oleh peningkatan hsCRP. Hipertensi dan inflamasi kronik akan mengakibatkan disfungsi endotel. Tujuan penelitian ini adalah mengetahuii hubungan hsCRP dan NO pada pasien hipertensi.Metode: Rancangan penelitian belah lintang, 30 subjek pasien hipertensi yang berobat di klinik PROLANIS periode Juli sampai September 2017 di Semarang. hsCRP dilakukan pengukuran dengan metoda ELISA, sedangkan NO dengan metoda reaksi Griess. Analisis data mengunakan uji korelasi Spearman.Hasil: hsCRP laki-laki dan perempuan 3,57 (0,19–19,67) dan 2,72 (0,43–15,88) nilai p=0,707. NO laki-laki dan perempuan 59,67 ± 34,28 dan 69,21 ± 43,29 nilai p=0,572. Berdasarkan lamanya hipertensi ≤ 1 tahun dan >1 tahun hsCRP 5,22 ± 6,71 dan 4,73 ± 4,66 nilai p=0,961 dan hipertensi ≤1 tahun dan >1 tahun NO 52,06 ± 41,38 dan 71,12 ± 39,37 nilai p=0,292. Hasil analisis menunjukkan hubungan yang signifikan antara hsCRP dan NO (r=0,692; p<0,001)Simpulan: Terdapat korelasi positif kuat antara hsCRP dan NO pada pasien hipertensi. Diperlukan penelitian lebih lanjut untuk menilai faktor-faktor yang mempengaruhi hipertensi, seperti IL–6 dan TNF–α sebagai sitokin inflamasi yang lainKata kunci:

Abstract

BACKGROUND: Transthyretin protein experiences misfolding and aggregation in preeclampsia due to placental ischemia and inflammation. Placental endothelial damage occurring in preeclampsia stimulates the production of larger young platelets in the bone marrow and can be reflected by an increase in the immature platelet fraction (IPF) and mean platelet volume (MPV). Since the change in these markers in preeclampsia remains controversial, this study was conducted to analyze the differences in levels of transthyretin, IPF, and MPV as easily accessible markers in normotensive and preeclampsia pregnancies.METHODS: Total subjects included in this study were 32 normotensive and 26 preeclampsia pregnant woman. The measurement of serum transthyretin level was using enzyme-linked immunosorbent assay (ELISA) method. Meanwhile, IPF and MPV levels were measured by using a hematology analyzer with flowcytometry method. Data was statistically analyzed using unpaired T-test and Mann-Whitney, with significancy of p<0.05.RESULTS: Median value of transthyretin in normotensive and preeclampsia pregnancy were 11 (6-30) mg/dL and 9 (5-18) mg/dL. The mean value of IPF in normotensive and preeclampsia pregnancy were 4.56±2.19% and 6.33±2.99%. Mean value of MPV in normotensive and preeclampsia pregnancy were 10.41±0.76 fL and 11.32±1.2 fL. There were significant differences in levels of transthyretin, IPF, and MPV between normotensive and preeclampsia pregnancies (p=0.008, p=0.017, and p=0.002, respectively).CONCLUSION: There are significant differences in transthyretin levels, IPF, and MPV values in normotensive and preeclampsia pregnancies. Therefore, serum transthyretin, IPF, and MPV can be developed as affordable parameters for the diagnosis of preeclampsia.KEYWORDS: preeclampsia, transthyretin, IPF, MPV

Abstract

Background: Double stranded RNA (dsRNA), siRNA, miRNA, RNAi, induce DNA methylation in plants and in mammalian cells, including human. Methylation of CpG island and repeat CGG DNA is high prevalence cases in tropical rain forest, but had been neglected till now. Purpose: Aims are knowing the effect of gene silencing to the environment, whereas RNAi cause hypermethylation. Methods: The manuscript was Quasi Systematic Review with Bayesian Analysis. Results: The result used Science Direct search engine, 935 references are caught, plus 11 references from already recorded in Mendeley library and after screening the abstract or title, 920 are excluded with the not relevant either duplication analyzed with the newest Bayesian network to answer the hypothesis. Screening full text of the 18, plus 10 references from already recorded in Mendeley library, then 16 full text were chosen. The chasing 28 full text were check and recheck with meta-analysis of RNAi-methylation using Science Direct (12 references). Conclusion: All of the studies described above indicated that RNAi and CRISPR/Cas9- mediated editing in plant, fishery, mammals, human, based on methylation and demethylation technique set-up by siRNA, miRNA or CRISPR/Cas9.
