Abstract

Berbeda dengan penyakit lain, penanganan kanker membutuhkan kekhususan. Penanganan awal adalah kunci paling menentukan prognosis selanjutnya. Dengan demikian, kualitas pelayanan, dalam hal ini -diagnostik yang tepat, tindakan akurat, pemilihan teknologi yang tepat- adalah hal yang tak dapat ditawar. Kesalahan tindakan pertama berakibat irreversible. Untuk itu, diperlukan tata laksana yang tepat dan kerja sama tim dokter yang solid. Penderita kanker memerlukan pendekatan yang lebih humanistik karena problem psikisnya berbeda. Biaya pengobatan penyakit kanker tergolong mahal. Kesadaran akan kendali biaya harus selalu ada dalam setiap pengambilan keputusan medik. Pada 20 April 1995, RS Onkologi Surabaya hadir membawa konsep Boutique Hospital. Rumah sakit ramping yang sesuai dengan kebutuhan penanganan penyakit kanker dan kondisi masyarakat pengakses. Tulisan ini adalah laporan ilmiah mengenai bagaimana RS Onkologi Surabaya melakukan kendali mutu dan kendali biaya dalam penanganan kanker payudara.Kata kunci: Boutique hospital, kendali mutu, kendali biaya, kanker payudara

Abstract

Strategi paling efektif dalam menanggulangi kanker payudara adalah pencegahan sekunder, yaitu upaya deteksi dini dan pengobatan segera. Penemuan mammografi adalah terobosan terbesar dalam sejarah penanganan kanker payudara. Pemeriksaan mammografi dapat menemukan kanker payudara sebelum timbul keluhan atau disebut dengan stadium praklinis.Sejak 1995, RS Onkologi melakukan kegiatan skrining payudara jenis opportunity screening atau lebih dikenal dengan istilah case finding. Di sini skrining mammografi dilakukan terhadap masyarakat yang datang tanpa keluhan dan atas kesadaran sendiri melakukan deteksi. Tulisan ini melaporkan kegiatan skrining payudara sejak Januari 2008 hingga Desember 2008. Dari 22.813 kasus yang datang di RSOS, 4.125 di antaranya adalah kasus baru. Kegiatan skrining tersebut menemukan 357 kasus kanker payudara dan hanya 8 kasus (0,9%) yang masih stadium 0 (tidak teraba). Dari 1.388 kasus skrining payudara, dilakukan 9 lokalisasi prosedur dan ditemukan 8 kasus kanker payudara T0 (0,57%). Kemudian dilakukan analisis ilmiah mengenai pengalaman menangani lesi payudara yang tak teraba di RSOS.Kata kunci: skrining payudara, lesi tak teraba, lokalisasi prosedur

Abstract

HIGHLIGHTS1. Ovarian tumors diagnostic value has been determined using the tools on contrast-enhanced abdominal CT Scan.2. The tools including primary finding like mass size, septation, solid component, and additional finding such as ascites, peritoneal implants, lymph node enlargement.3. Most tumors were ovarian malignant tumors according to their histopathological results.4. Contrast-enhanced abdominal CT Scan of ovarian tumors evaluated by the tools gave better and more reliable diagnostic value than without using the tools.ABSTRACTObjectives: The purpose of this study was to determine the diagnostic value of contrast-enhanced abdominal CT scan in ovarian tumors in Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, to provide scientific and clinical benefits.Materials and Methods: Samples were taken retrospectively by contrast-enhanced abdominal CT scan raw data in Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, from January 2017 to December 2018. Using inclusion and exclusion criteria, a total of 88 samples were obtained and reviewed blindly by Female Organ Division of Radiology Department, Faculty of Medicine Universitas Airlangga, and by using "tools" (primary and additional findings of modified ovarian tumors malignancy). The data were correlated with histopathological findings and analyzed by statistical tests and the results with and without "tools" were compared.Results: Samples were grouped by age, distributed with a range of 20 years and the group of 41-60 years had the highest age of ovarian tumor samples (46.6%) with 84.1% being ovarian malignant tumors according to their histopathological results. It was dominated by serous, mucinous and endometroid types with sensitivity of 93.3%, specificity of 64.3%, positive predictive value of 93.3% negative predictive value of 69.2%, and accuracy value of 89.8%. More reliable results were obtained by using “tools”.Conclusion: Contrast-enhanced abdominal CT scan of ovarian tumors in Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, evaluated by “tools” still had a better and more reliable diagnostic value than without tools in determining policy steps in handling ovarian tumors with a note that more in-depth research on pitfalls is needed so it may enrich the characteristic findings in imaging.

Abstract

Introduction: Spiculation in mammography is a typical finding for invading breast cancer and is an important criterion in diagnosis and in predicting prognostic and plays an important role in management. The purpose of this research is to determine the characteristics of malignant spiculating mass in mammography in regards to histopathological grading using The Nottingham Grading System.Methods: Patients whom had spiculation in mammography was reviewed using medical record data by two breast imaging consultants and then combined with The Nottingham Grading System criteria obtained from histopathological examination results of core biopsy and surgery specimen. There was 29 cases that met the inclusion criteria.Results: Of the 29 patients that met the inclusion, the spiculated masses grade I, II has mitotic index of 0-12, grade III has mitotic index of 13-25. Histopathologic grade II, III has the most tubular formation of <10%, grade I has 10-75%. Grade I, II has moderate nuclear pleomorphism, grade III has severe nuclear pleomorphism. Most were grade III (44.8%), followed by grade II (37.9%), and minimally grade I (17.2%). Most patients are in stage 3 breast cancer.Conclusion: Malignant spiculated mass with grade I, II has low proliferation index (mitotic index 0-12), histopathological grade II, III had worse cellular differentiation (tubular formation <10%), histopathological grade II, III has moderate to severe nuclear pleomorphism.

Abstract

Highlights: The highest distribution of breast cancer patients’ age based on the 5-year age interval was 45- 49 years old. There was no significant difference between tumor margin and age of breast cancer patients. There was no significant difference between tumor margin and histopathological grade.   Abstract Introduction: Mammography is an X-ray technique used to take images of the breast. It is the primary diagnostic method for breast tumors. Breast imaging reporting and data system (BI-RADS) classification is needed to determine benign or malignant masses by accessing the mass's shape, margin, density, and other features. However, the tumor margin is the most helpful one. This study aimed to know the difference in the distribution of tumor margin types in each histopathological grading in breast cancer patients. Methods: This was an observational analytic study with a comparative approach using secondary data from medical records of patients with breast cancer at the radio diagnostic and anatomical pathology installation of Dr. Soetomo General Academic Hospital, Surabaya, from January 2017 to December 2021. All statistical data were performed using the International Business Machines Corporation (IBM) Statistical Package for Social Sciences (SPSS) version 27, with a p<0.05 considered statistically significant. Results: Out of 235 cases, the highest distribution of breast cancer patients’ age interval was 45-49 years old (20.9%), the primary tumor margin type was spiculated (64.3%), and the highest distribution of histopathological grading was grade 3 (53.2%). There was no significant difference between tumor margin and age of breast cancer patients (p=0.815), with spiculated tumor margin as the most common type in all age intervals. There was no significant difference (p=0.163) in the distribution of tumor margin types in each histopathological grading, with spiculated tumor margin as the most common type in every grade. Conclusion: There was no significant difference between tumor margin and age of breast cancer patients, and there was no significant difference between tumor margin and histopathological grading.

Abstract

ABSTRACT Introduction: One of the risk factors for ovarian cancer is endometriosis. The most prevalent type of ovarian cancer linked to endometriosis is ovarian clear cell carcinoma. The CT scan examination helps identify the type of treatment for each mass lesion and distinguishes endometriomas from other lesions. Case Presentation: MSCT was done on a 42 years old female with abdominal pain and distention over a year; showed a cystic mass (16HU) with solid component (36HU) partly unclear borders, irregular edges, with dimensions ± 14.3 x 16.8 x 16.3 cm in the pelvic and abdominal cavity, impression originates from the adnexas, when administered contrast shows an increased enhancement in contrast (83HU), also found free fluid density(17HU) in abdominal cavity, suggestive malignant ovarian mass. A pathology anatomy examination was done and confirmed clear cell carcinoma of the left ovary, endometriotic cyst in the right ovary, and external endometriosis in the omentum. Conclusion: Endometriosis rarely develops into clear ovarian cell carcinoma; the preoperative characterization of an ovarian cyst and the CT appearance of ruptured endometriotic cysts are comparatively distinct from those of burst functional cysts.

Abstract

Highlights: Most breast cancer patients were in the 45-49 years old age group. There was no difference in the age interval between BI-RADS C-4 and C-5 in breast cancer patients. There was no difference in histopathological grading between BI-RADS C-4 and C-5 in breast cancer patients.   Abstract Introduction: Breast cancer is the most common cancer worldwide. The diagnosis of breast cancer is established by a triple diagnostic, such as clinical examination, radiology (mammography), and histopathology. This study aimed to compare mammography breast imaging-reporting and data system (BI-RADS) category 4 and 5 with histopathological grading of breast cancer at Dr. Soetomo General Academic Hospital, Surabaya. Methods: This was an observational, descriptive study with a comparative approach, utilizing secondary data from medical records of breast cancer patients at Dr. Soetomo General Hospital, Surabaya, from January 2017 to December 2021. There were 234 samples of patients who met the inclusion criteria. All statistical data were analyzed using the International Business Machines Corporation (IBM) Statistical Package for Social Sciences (SPSS) version 26, with a p<0.05 regarded as statistically significant. Results: The breast cancer patients were most prevalent in the 45-49 years old age group (20.9%). The highest distribution of the BI-RADS category was C-5 (85.9). The highest distribution of histopathological grading was grade III (53%). There was no difference in age intervals between BI-RADS C-4 and BI-RADS C-5 in breast cancer patients (p=0.499). There was no difference in histopathological grading between BI-RADS C-4 and C-5 in breast cancer patients (p=0.592). Conclusion: There was no difference either in age interval or histopathological grading between BI-RADS category 4 and 5 in breast cancer patients.
