Abstract

BACKGROUND: Syphilis and yaws are the treponemal infections which have become serious public health problems, and both are serologically indistinguishable. Developed serological tests for syphilis may also be used to diagnose yaws. In remote area, test modality with minimal requirements were needed. This study investigated the diagnostic value of syphilis rapid diagnostic test (RDT) in diagnosing syphilis and yaws.METHODS: For syphilis diagnostic test, serum samples were obtained from patients of outpatient clinic in Dr. Cipto Mangunkusumo National Central General Hospital who were sent for rapid plasma reagin (RPR) and Treponema pallidum haemagglutination assay (TPHA) to clinical laboratory of the hospital. The serum samples were collected and stored at -80°C until the day of testing for syphilis RDT and fluorescent treponemal antibody-absorption (FTA-Abs). For yaws diagnostic test, serum samples were obtained as a part of surveillance study of yaws among children 1-15 years old in West Halmahera. Venous blood samples were used for syphilis RDT and the sera were separated and were sent to Dr. Cipto Mangunkusumo National Central General Hospital for RPR, TPHA, and FTA-Abs tests.RESULTS: For syphilis diagnostic test, among 156 samples, 39 samples were positive with syphilis RDT. The sensitivity of syphilis RDT was similar to RPR and TPHA (100.0%), the specificity was same as TPHA (77.5%), but lower than RPR (84.8%) when compared to FTA-Abs IgM. The sensitivity of syphilis RDT was 62.5% and the specificity was 96.0% when compare to FTA-Abs IgG. For yaws diagnostic test, among 176 samples, 13 samples were positive with syphilis RDT. By using FTA-Abs IgM as gold standard for diagnosing yaws, the RDT have similar sensitivity (50.0%) with RPR and TPHA and syphilis RDT have similar specificity to TPHA (93.1%). If compared to FTA-Abs IgG, the sensitivity of syphilis RDT was 40.0% and the specificity was 98.0%.CONCLUSION: Syphilis RDT gives similar results with TPHA in syphilis and yaws cases. It may be used as a first line screening test latent or untreated syphilis and yaws because of good sensitivity. For yaws diagnosis Syphilis RDT, RPR, and TPHA have low sensitivity, however all those tests have an excellent agreement.KEYWORDS: FTA-Abs, rapid diagnostic test, syphilis, yaws

Abstract

BACKGROUND: Light transmission aggregometry (LTA) and VerifyNow is commonly used to measure platelet responsiveness to clopidogrel. This study aimed to compare the results of LTA and VerifyNow P2Y12 assay for assessing the clopidogrel resistance in patients undergoing percutaneous coronary intervention and determine factors affecting clopidogrel resistance.METHODS: The subjects were 119 patients who underwent percutaneous coronary intervention (PCI) and had given loading dose of 600 mg clopidogrel. Blood samples were taken at 6 hour after clopidogrel loading dose. Platelet aggregation was measured by LTA and VerifyNow.RESULTS: LTA and VerifyNow assay showed fair agreement with Kappa=0.270, p=0.001. The proportion of resistance to clopidogrel using VerifyNow was 21.8% and LTA was 47.1%. Patients with diabetes melitus were more likely to develop clopidogrel resistance than patients without diabetes (OR of 7.67; 95% CI: 1.87-31.50; p=0.005).CONCLUSION: The ability of LTA and VerifyNow in detecting clopidogrel resistance were not comparable. Multivariate analysis results for VerifyNow shows diabetes mellitus as the greatest predictors of clopidogrel resistance.KEYWORDS: agreement, clopidogrel resistance, LTA, predictor, VerifyNow

Abstract

This research aims to describe the planned change process using Kurt Lewin's theory to optimize effective communication during ward handovers by improving the nursing manager's function. The method used is a case study including assessment activities, problem analysis, preparation of planning of action (POA), implementation, and evaluation. The research results show that in the unfreezing phase, problems were found that the performance of effective communication during the handover was not optimal, so the priority strategy for solving the problem was optimizing the planning function for handover activities. In the movement phase, a POA was prepared with indicators that were successfully achieved, namely the preparation of a revised handover SPO draft and socialization of the revised SPO draft with the nursing sector, Head Nurse, and Clinical Care Manager. In contrast, this could not be done in the refreezing phase due to time constraints, so a follow-up plan was prepared and agreed with the PIC of the activity. In conclusion, the planned change process towards optimizing effective communication during handover requires an increase in the function of nursing managers, especially in the planning function. Managers must provide policies, one of which is SOP handover, and must review and revise policies to remain new and can be applied. Abstract

Abstract

BACKGROUND: Intestinal tuberculosis (ITB) is a type of tuberculosis (TB) that affects the gastrointestinal tract. Cathelicidin, an antimicrobial peptide, contributes to defense against Mycobacterium tuberculosis, which causes TB. Its expression is tightly regulated by vitamin D through vitamin D receptor signaling and autophagy pathways. Despite the increasing evidence on TB, studies exploring the correlation between cathelicidin and vitamin D in the ITB remain poorly characterized and almost exclusively derived from pulmonary TB. Therefore, the present study was conducted to compare serum cathelicidin and vitamin D levels between subjects with ITB and without ITB, and to determine the correlation between these two biomarkers.METHODS: This comparative cross-sectional study utilized stored serum samples obtained from participants enrolled in a previous ITB project. Twenty-two ITB and 22 non-ITB subjects aged >18 years who had undergone colonoscopy, histopathological examinations, and fulfilled predefined clinical and diagnostic criteria were included. Serum cathelicidin and vitamin D levels were measured using sandwich enzyme-linked immunosorbent assay (ELISA) and chemiluminescent microparticle immunoassay (CMIA), respectively.RESULTS: Significantly higher levels of serum cathelicidin were identified in ITB subjects, with a median level of 3.67 (2.27‒5.95) ng/mL, compared with non-ITB subjects with a median level of 2.04 (1.66‒2.46) ng/mL (p<0.0001). Although the difference was not statistically significant (p=0.091), vitamin D level tended to be lower in subjects with ITB, with a median level of 11.05 (6.80‒25.25) ng/mL, compared to non-ITB subjects with a median level of 17.95 (12.13‒23.63) ng/mL. A significant moderate negative correlation was found between cathelicidin and vitamin D (r=-0.485, p=0.001).CONCLUSION: The ITB is associated with elevated cathelicidin levels and a tendency toward lower vitamin D levels. The inverse correlation suggests a complex relationship between vitamin D status and cathelicidin expression, highlighting immunological mechanisms involved in ITB pathogenesis.KEYWORDS: antimicrobial peptide, cathelicidin, innate immunity, intestinal tuberculosis, vitamin D
