Abstract

Bruxism is a condition that involves grinding and clenching which can place a large load on the mandible, so it can change the morphology of the mandible. Based on the Levandoski analysis, this study aimed to determine whether there are differences in the mandibular morphology between bruxism and non-bruxism patients. This study was a cross-sectional analytic study conducted on the panoramic radiographic samples of bruxism and non-bruxism patients with a total sample of 30 patients (n = 30) in each group, obtained from the radiographic archives at the Radiology Installation of RSGM Universitas Padjadjaran which have been confirmed as bruxism and non-bruxism patients. Measurements were carried out using ImageJ software based on the reference lines by Levandoski. The results of the study were analyzed using SPSS software by performing normality and homogeneity tests, followed by a statistically independent t-test or Mann-Whitney test. The results of the independent t-test showed that three lines had a p-value < 0.05, namely the line from the point gonion to the maxillary vertical line on the right side, the distance from the top point of the condyle to the maxillary interincisal point on the right side, and the distance from the top point of the condyle to the mandibular interincisal point on the right side. The Mann-Whitney test results showed that two lines had a p-value < 0.05, namely the maxillary vertical midline and the distance from the condyle to the left maxillary vertical line. The other line had a p-value> 0.05. There were differences in the mandibular morphology between bruxism and non-bruxism patients based on the Levandoski analysis, namely the length of the maxillary vertical midline, the distance from the condyle top point to the maxillary vertical midline, the distance from the point gonion to the maxillary vertical midline, the distance from the top point of the condyle to the maxillary interincisal point on the left side, and the distance from the top point of the condyle to the mandibular interincisal point on the left side. On the other four lines, there was no significant difference between bruxism and non-bruxism patients.

Abstract

ABSTRAKPendahuluan: Temporomandibular Disorder (TMD) adalah gangguan yang terjadi pada neuromuskular dan muskuloskeletal yang terjadi akibat disharmoni antara komponen sistem stomatognatik, gigi, dan jaringan di sekitarnya, sehingga dapat mengakibatkan berbagai gangguan yang sering kali ditandai dengan rasa nyeri pada Temporomandibular Join (TMJ) beserta jaringan kompleks disekitarnya. TMD yang disebabkan oleh ruang edentulous pada usia dewasa muda dapat menimbulkan penurunan kualitas hidup seseorang dalam menjalani kegiatan sehari-hari karena sendi temporomandibula merupakan bagian dari sistem stomatognatik yang artinya berhubungan langsung dengan fungsi kerja tubuh dan meliputi tumbuh kembang rahang dan wajah manusia. Tujuan penelitian mengetahui gangguan sendi temporomandibula akibat ruang edentulous pada usia dewasa muda. Metode: Jenis penelitian deskriptif dengan metode cross-sectional. Sampel penelitian adalah mahasiswa preklinik dan klinik Fakultas Kedokteran Gigi Universitas Padjadjaran dengan jumlah 45 orang yang diperoleh menggunakan teknik total sampling. Pengambilan data dilakukan menggunakan kuesioner Temporomandibular Disorder–Index Diagnostic (TMD-ID) dan kuesioner mengenai ruang edentulous. Hasil: Didapatkan jumlah total 45 sampel yang terdiri dari 13 laki-laki (27,95%) dan 32 perempuan (72,05%). Berdasarkan pembagian kategori gangguan sendi temporomandibula akibat ruang edentulous, menghasilkan dua kategori, yaitu TMD positif yang terdiri dari 33 orang (73,33%) dan TMD negatif yang terdiri dari 12 orang (26,67%). Knowing the description of temporomandibular joint disorders due to edentulous spaces in young adults. Methods: This type of research is descriptive with cross-sectional method. The research sample was preclinical and clinical students of the Faculty of Dentistry, Padjadjaran University with a total of 45 people obtained using a total sampling technique. Data were collected using a Temporomandibular Disorder-Index Diagnostic (TMD-ID) questionnaire and a questionnaire regarding the presence of edentulous spaces.  Results: Obtained a total of 45 samples consisting of 13 males (27.95%) and 32 females (72.05%). Based on the classification of temporomandibular joint disorders due to edentulous space, it resulted in two categories, namely positive TMD consisting of 33 people (73.33%) and negative TMD consisting of 12 people (26.67%). Abstract

Introduction: Odontectomy is a minor surgical procedure that often results in dental anxiety. Dental anxiety can interfere during odontectomy treatment. Dental anxiety can be examined using the Modified Dental Anxiety Scale (MDAS) and through the level of the salivary alpha-amylase enzyme, both of which are dental anxiety biomarkers. The objective of this study was to analyse the correlation between anxiety values with salivary alpha-amylase levels using MDAS as an indicator of anxiety in pre-odontectomy patients. Methods: This study was conducted on 60 research samples of women aged 18-40 years who came to the minor surgery clinic of Department of Oral and Maxillofacial Surgery, Universitas Padjadjaran Dental Hospital, to have odontectomy treatment of maxillary or mandibular third molars impaction, not having systemic and oral diseases, and willing to participate in this study. Patients who met the inclusion criteria then required to sign the research consent statement; afterwards, the MDAS data was taken. The salivary alpha-amylase enzyme data was taken using the Cocoro meter (Nipro, Japan) tip which was kept under the tongue for 30 seconds, then the tip was inserted into the monitor, and the enzyme levels would be counted. The data obtained will be analysed using the Spearman rank correlation test. Results: The results showed that there was a significant relationship (p < 0.01) with a strong positive correlation (r = 0.85). Conclusion: There is a correlation between MDAS value and salivary alpha-amylase enzyme as an indicator of dental anxiety.

Abstract

ABSTRAKPendahuluan: Langkah penting pada rehabilitasi gigi tiruan yang berperan untuk mengembalikan fungsi dan estetik dari pasien yang kehilangan gigi adalah pengukuran dimensi vertikal yang terbagi menjadi Dimensi Vertikal Oklusi (DVO) dan Dimensi Vertikal Fisiologis (DVF). Tujuan dari penelitian ini adalah untuk menganalisis  kesesuaian dimensi vertikal sebelum dan sesudah pemasangan gigi tiruan lengkap lepasan. Metode: Jenis penelitian ini merupakan metode observasional dengan desain cross sectional. Subjek pasien poliklinik Prostodonsia RSGM UNPAD. Data dimensi vertikal oklusi pasien sebelum pemasangan gigi tiruan (diukur dengan rumus Niswonger, DVO= DVF-2 mm freeway space pada posisi istirahat fisiologis) dan sesudah pemasangan gigi tiruan saat oklusi sentrik, diukur dengan mengukur jarak 2 titik subnation dan gnation. Hasil: Hasil nilai probabilitas p=0,000 menggunakan uji paired t-test menunjukan terdapat perbedaan yang signifikan pada pengukuran DVF dan DVO sebelum dan sesudah pemasangan gigi tiruan lengkap lepasan (p-value 0,000 ≤0,05). Simpulan: Terdapat perbedaan hasil pengukuran Dimensi Vertikal Oklusi (DVO) antara sebelum dan sesudah pemasangan gigi tiruan lengkap lepasan, tetapi perbedaan hasil ini masih dalam batas toleransi yang dapat diterima.KATA KUNCI: Gigi Tiruan Lengkap Lepasan, dimensi Vertikal oklusi, dimensi vertikal fisiologis, sebelum dan sesudah pemasangan gigi tiruan    Evaluation of vertical dimension before and after insertion of removable dentures: A cross-sectional studyABSTRACTIntroduction: Dentures play a role in returning both physiological function and esthetics of a patient that experiences tooth loss, one of the steps in denture fabrication is vertical dimension measurements that consists of the Vertical Dimension of Occlusion (VDO) and the Vertical Dimension at Rest (VDR), this step is crucial in ensuring the rehabilitation of function and esthetics are achieved. The purpose of this study is to analyze the suitability of vertical dimension measurements before and after insertion of complete dentures. Methods: This is an observational study using the cross sectional method done on 30 patients indicated for removable dentures treated by coassistents and residency at prosthodontic clinic RSGM UNPAD. Sample size is determined using the purposive sampling method. Measurements are done using the 2 dot method, first measurements are done prior to insertion using the Niswonger method which is VDR subtracted by freeway space to get the first VDO measurement. The second measurement is done after denture insertion which then patients are put in the centric occlusion position, the distance between the 2 dots subnation and gnation are measured to get the second VDO after insertion Results: Probability value of p=0,000 using paired t-test analysis shows that there is significant difference between VDO and DVF before and after insertion off dentures (p-value 0,000 ≤0,05). Conclusion: There is a difference in the measurement of the VDO between before and after the insertion of complete removable dentures, and this difference remains within an acceptable tolerance range.
