Abstract

Introduction: Choledocholithiasis, the presence of stones in the common bile duct (CBD), is a challenge in the field of digestive surgery due to the wide range of cases with varying levels of difficulty. Until now, the options for diagnostic and therapeutic modalities have been increasingly evolving, ranging from non-invasive to invasive procedures. Therefore, the Indonesian Society of Digestive Surgeons provides recommendations for the management of CBD stone cases in Indonesia.Method: The consensus was developed using the Delphi survey method involving digestive surgery experts from various cities in Indonesia. Several issues related to the diagnosis and management of CBD stones were formulated and answered based on current research, while also considering the opinions of the experts.Result: This consensus consists of two parts: 15 recommendations related to the selection of diagnostic modalities and 10 recommendations related to management options. Recommendations regarding diagnosis include the use of ultrasonography, abdominal CT-scan, and magnetic resonance cholangiopancreatography (MRCP). In addition to standard therapeutic procedures, this consensus also provides recommendations regarding contemporary procedures such as endoscopic retrograde cholangiopancreatography (ERCP), laparoscopic common bile duct exploration (LCBDE), and open common bile duct exploration (OCBDE).Conclusion: These recommendations are aimed to assist digestive surgery experts in providing the best management for CBD stone cases.

Abstract

Background: Liver resection has been associated with high morbidity and mortality. Improvements in surgical, anesthetic techniques, and multidisciplinary collaborations, can reduce post-surgery complications and mortality. This study aims to provide an overview of the perioperative conditions and the treatments after liver resection.Method: A retrospective study of liver resection surgery between 2019-2020 at Fatmawati Hospital.Results: Of the 11 patients, mean age was 49.7 years, with 63.6% being female and mean BMI was 22 kg/m2, hypertension and diabetes mellitus were found in 18.2% and 18.2% of patients respectively. HBsAg reactive was detected in 36.3%. Based on pathology, HCC was found in 54.5%, while 18.2% were metastatic adenocarcinoma. Postoperative hyperglycemia was observed in 90.9%. Increase in AST and ALT 3 upper limit normal were found in 90% and 72.7% of patients. Mean AST and ALT were 408.3 U/L and 246.18 U/L. Mean urine production at 8-, 16-, 24-, and 48-hours post-operative were 757, 1624, 1880 and 1930 cc. Urine production ≤ 500 cc in the first 8 hours was detected in 44.4% of patients, and elevated creatinine levels 50% post-operative occurred at 11.1%, 22.2%, 22.2% at 16, 24, and 48 hours post-op. Renal support therapy was given to 5 of the 11 patients. D-Dimer levels were increased in all patients.Conclusion: Adequate fluid monitoring and metabolic disorders control such as glucose levels, acute kidney injury, coagulation disorders, and bleeding are important things that need to be considered in the perioperative management of liver resection.

Abstract

Apendisitis merupakan salah satu kegawatdauratan bedah yang sering ditemukan. Apendisitis akut dan apendisitis perforasi merupakan jenis apendisitis yang sering ditemukan. Penegakan diagnosis awal jenis apendisitis penting sebagai salah satu cara untuk mencegah komplikasi sehingga dapat menurunkan angka kesakitan dan kematian. Pemeriksaan penunjang laboratorium yang umum sederhana yang dapat digunakan sebagai diagnosis lanjutan awal untuk apendisitis adalah pemeriksaan kadar leukosit darah. Perbedaan kadar leukosit dapat digunakan untuk memperkirakan jenis apendisitis. Penelitian ini bertujuan untuk menganalisa hubungan antara usia, jenis kelamin dan kadar leukosit dengan jenis apendisitis. Desain yang digunakan pada penelitian ini adalah potong lintang. Sebanyak 98 rekam medis pasien berusia 18 – 65 tahun dengan apendisitis akut atau apendisitis perforasi post operasi appendektomi di RSUP Fatmawati Jakarta dipilih menggunakan tehnik consecutive non-random sampling. Variabel yang diteliti adalah usia, jenis kelamin, jenis apendisitis dan kadar leukosit. Hasil yang ditemukan adalah 54,1% pasien didiagnosa dengan apendisitis akut, 64,3% berusia 18 – 45 tahun, 52% berjenis kelamin perempuan dan 54,1% mengalami leukositosis ringan. Analisa uji chi-square menunjukkan bahwa terdapat hubungan antara kadar leukosit dengan jenis apendisitis (p<0,001), namun tidak ada hubungan antara usia (p=0,097) dan jenis kelamin (p=0,165) dengan jenis apendisitis. Pada penelitian ini dapat disimpulkan bahwa hasil pengukuran kadar leukosit memiliki potensi digunakan sebagai salah satu kriteria untuk memprediksi jenis apendisitis termasuk apendistis akut atau perforasi.

Abstract

Digestive surgery service including surgical management of gastrointestinal disease and digestive cancers are experiencing the impact of COVID-19 pandemic. Therefore it is necessary to formulate recommendation for digestives surgery service, as guidelines to engage in case-by-case assessment of particular patients with digestive diseases. We are aware that the knowledge and science of COVID-19 are still evolving, with new progression every day. This recommendation reflect actual condition and are subject for future adjustment in the future.
