Abstract

AbstractNatural disasters are one of the natural phenomena that can threaten human lives which could cause material and immaterial losses. According to Antara. News, the National Disaster Management Authority or Badan Nasional Penanggulangan Bencana (BNPB) has registered 372 natural disasters in Indonesia since the beginning of 2021. Airlangga University School of Medicine has a Natural Disaster Assistance Unit that provides volunteers and a variety of facilities as needed, but the competence of human resources is still lacking. The lack of specific roles and skills of volunteers to support disaster victims can be a problem. Faced with these issues, FK Unair's Anesthesiology and Resuscitation Community Services Team has formed a disaster response team consisting of people of different life and educational backgrounds from different locations to conduct a disaster management training program to update the latest knowledge.Keywords: Disaster Response Team, Training, Online, Disaster Risk ReductionAbstrakBencana alam merupakan salah satu fenomena alam yang dapat mengancam keberlangsungan hidup manusia dan dapat menimbulkan kerugian materi maupun non materi. Fakultas Kedokteran Universitas Airlangga memiliki Unit Bantuan Bencana Alam yang memiliki relawan dan berbagai sarana yang siap berangkat kapanpun dibutuhkan namun kesiapan sumber daya manusia dinilai masih kurang. Masih kurangnya spesifik tugas dan kompetensi relawan yang berangkat membantu korban bencana dapat menjadi permasalahan. Dengan adanya permasalahan tersebut, Tim Pengabdian masyarakat Departemen Anestesiologi dan Reanimasi FK Unair bermaksud untuk membentuk tim tanggap bencana yang direkrut dari berbagai tempat dan dari berbagai kalangan serta berbagai latar belakang pendidikan yang akan dilakukan pelatihan tanggap bencana untuk update ilmu dan materi baru yang terkini.Kata Kunci: Tim Tanggap Bencana, Pembekalan, Online, Disaster Risk Reduction

Abstract

Background: The global COVID-19 pandemic has significantly strained the healthcare sector, with overwhelmed health services in affected countries worldwide. Despite the need to sustain vital oncology treatments, particularly for children, radiotherapy practitioners are unsure how to treat during the pandemic. At the same time, several anesthesia procedures may increasingly expose anesthesia providers to COVID-19. The challenging services for handling oncology cases in children at Dr. Soetomo Hospital during the COVID-19 pandemic can be carried out properly with various adjustments. Objective: The purpose of this study was to describe how the radiotherapy center in East Indonesia responds to the challenge in this pandemic era.Method: During 2020, we gathered information by using administrative datasets on the number of patients, the types of anesthesia services provided, and undesired events in the radiotherapy room. We use descriptive statistics to describe what is going on in our data.Result: The total number of patients who underwent radiotherapy was 12, with 188 sessions, and uncooperative children received midazolam and propofol during the procedure. The result showed that there was no untoward incident in 2020.Conclusion: Therefore, it indicates that we provided relatively safe sedation services for children's radiotherapy.

Abstract

Introduction: The use of ketamine and tramadol as postoperative analgesics for pediatric are still relatively rare, especially orally administrated. As an analgesic, ketamine blocks the NMDA receptor, the main excitatory transmitter in CNS; whereas tramadol blocks serotonin and norepinephrine uptake, thus preventing pain transmission on the spinal cord. Objective: The aim of this study is to compare the effectiveness of oral ketamine and oral tramadol as analgesics for postoperative acute pain in children. Method: A double-blind randomized clinical trial was conducted at Dr. Soetomo Hospital. The hospital ethical committee had approved this study. The subject includes thirty children aged 5-10 years old who fulfilled the inclusion criteria. They were divided into either ketamine groups or the tramadol group, in which each group consisting of fifteen patients. The regimen dosage that been given was 2mg/kg tramadol and ketamine as postoperative oral analgesics in the form of simple syrup. The FLACC table was used to evaluate pain score before and after administration of drugs (30-minutes, 1-hour, 2-hours, 3-hours, 4-hours, and at discharge from the recovery room). Result and Discussion: Based on the quantitative parameter of the FLACC (scale 0-10), there was a significant difference (p<0.05) between the first-hour postoperative administration and patient discharge from the recovery room. The patient of ketamine group had far lower FLACC value compared to the tramadol group. Rescue analgesics in the form of intravenous fentanyl were given to one patient (6.7%) in the ketamine group and  four patients (26.7%) in the tramadol group. Conclusion: Ketamine proved to be a better and more effective postoperative oral analgesic compared to tramadol in this study.

Abstract

Pain is an unpleasant sensory and emotional experience associated with actual or potential tissue damage. The lack of up-to-date basic knowledge of pain management, especially among healthcare professionals in Primary Health Care Facilities in Surabaya, often leads to the perception that pain is trivial and consequently inadequately addressed. The objective of this social service activity in pain treatment and basic pain management training is to provide appropriate treatment for the community's pain scale and impart current knowledge on pain management that can be applied in daily practice by healthcare professionals and the general public in Surabaya. This initiative involves two methods: treating pain in non-medical community members and providing basic pain management training for healthcare professionals. Examination and anamnesis are conducted for each patient according to the examination questionnaire, which includes the Pain Questionnaire for Laypersons, the Physical Examination Form for Pain Management Social Service, and the Quality of Life Scoring. The initial examination and anamnesis results will determine whether the patient falls into the acute or chronic pain category, allowing for tailored interventions based on patient grouping. As for the basic pain management training, it is conducted by a pain management consultant, comprising the Socialization of the Pain Management Center, a General Lecture on the New Paradigm of Pain Management in the Community, and a Hands-On Pain Management Workshop for patients in pain.

Abstract

Background Medical science has developed rapidly. One of them is ERAS (Enhanced Recovery After Surgery) which is a multidisciplinary team development pathway for perioperative methods that offer a variety of benefits. Some of the advantages of the ERAS method are speeding up the post-operative recovery period, economic benefits, and shortening the Length of Stay (LOS) by minimizing surgical stress so that the patient quickly returns to a normal physiological state. Objective This study aims to analyze the readiness of the team that will support the ERAS application at General Academic Hospital (RSUD) Dr Soetomo. Methods This is an observational analytic study with a cross-sectional design using a questionnaire. Patient baseline data were taken from the examination at the anesthesiology outpatient clinic. Assessment for patient understanding was carried out directly after a brief explanation of ERAS method by the researcher. The patient's level of understanding was assessed by two investigators and rated on a scale of 1 to 5. Primary data and assessment of understanding of doctors and medical personnel were filled in independently by research subjects on online forms and rated on a scale of 1 to 5. Data was collected from March 23 to April 14 2022, at RSUD Dr. Soetomo Surabaya. The analysis results are considered significant if the p-value is less than 0.05. The analysis was carried out using SPSS 19 software. Results Most patients planned to undergo surgery in the field of urology (82.9%), followed by caesarean section (14.6), and almost all patients or their families agreed to surgery using the ERAS method (95.1%). The medical team consists of 110 doctors from 4 areas of expertise, namely specialist doctors and residents in anesthesiology, urology surgery, obstetrics and gynecology, as well as orthopedics and traumatology. The health team consists of 56 personnel, namely anesthesiologists, surgeons, recovery room nurses, nutritionists and pharmacists. The majority (61%) of health workers had heard about ERAS and stated that ERAS was ready to be implemented at RSUD Dr. Soetomo (89%). Most doctors stated that ERAS was ready to be implemented at Dr. Soetomo and its human resources and facilities are considered qualified. Conclusion The ERAS method at RSUD Dr. Soetomo is considered quite ready to start, especially the Obsgyn study program, which is supposed to have an excellent level of understanding of ERAS and is enthusiastic to begin immediately. The patient group can be said to understand and accept the ERAS method well as a perioperative method for handling their cases.

Abstract

Introduction: The use of essential oils in aromatherapy is widespread. However, few studies have explored the effects of smoke from the evaporation of commonly used essential oils. While essential oils are promoted for various benefits, prolonged exposure to inhaled particles from essential oil smoke may pose potential health risks. This study aimed to examine the effects of essential oil vapors on mice. Methods: This was an experimental study investigating the effects of different treatments on lung immunohistopathology, endothelial nitric oxide synthase (eNOS) expression, serum nitric oxide (NO) levels, and serum surfactant protein-D (SP-D) as an inflammation marker in mice. A total of 40 adult male Mus musculus mice (25–30 g) were randomly divided into four groups. Inflammation models were established by exposing the mice to a gas mixture containing vegetable glycerin, propylene glycol, and vitamin E acetate solution. Following inflammation induction, the mice received seven-day interventions with 0.9% NaCl solution, Eucalyptus globulus essential oil (EgEO), and citronella essential oil (CtEO), alongside an untreated inflammatory group. Results: The CtEO group intervention showed significant increases in eNOS expression (P=0.001) but no significant increase in NO compared to the other groups. The correlation analysis of eNOS expression in lung cells, serum NO, and SP-D levels was not significant, P>0.05 (p=0.160; p=0.115; p=0.234). Conclusion: Gas intervention containing 100% oxygen (O2) and CtEO steam increased eNOS expression on the immunohistochemistry (IHC) examination of mice.
