Abstract

Introduction: ICH (intracerebral hemorrhage) is a medical emergency with high mortality and morbidity that occurs in 15-20% of stroke cases. However, in the Multicenter Surgical Trial in Lobar ICH (STICH) study no benefit was found from the initial operative handler in Lobar ICH patients. Many clinical and radiological factors that affect outcomes of patients.Methods: The design of this study was a cross-sectional study. Clinical and radiological factors were analyzed by chi-square test and Kruskal-Wallis test. Then significant results were sought Relative RiskResults: This research was conducted from January 2017 to September 2018 starting from sampling to processing data. Sampling was carried out at the medical record center of Dr. Hospital. Soetomo Surabaya. Data collection is done by using secondary data in the form of patient medical records. All of the collected intracerebral hemorrhage patients were 302 and the total patients who met the inclusion and exclusion criteria were included in the study as the study subjects. Patients who met the criteria obtained 247 research subjects.Conclusion: There are several factors that influence the outcome of ICH stroke patients. Radiological factors and clinical factors both have an impact on outcomes. Further research is needed to find out which factors can be a diagnosis for choosing treatments that provide better outcomes. Statistically more than 1 variable is needed to determine an action that results in a more significant outcome of living patients. This shows that the choice of treatment for patients with different characteristics requires different treatments. Further analysis is needed to look for indications of the use of Operative or Conservative actions16.

Abstract

Introduction: This study aimed to define the CT scan imaging of young adult intracerebral hemorrhage (ICH) patients in Dr. Soetomo General Hospital, Surabaya.Methods: This was an observational descriptive study. The data were collected retrospectively with medical records of 18 to 45 years old ICH patients who were treated in Dr. Soetomo General Hospital, Surabaya between 2014 and 2017. The collected data were then presented into tables.Results: 207 patients were identified in this study. The locations of the ICH were basal ganglia (45.89%), lobar (41.55%), thalamus (10.63%), brainstem (8.7%), cerebellum (4.83%), and others (3.86%). The volume of ICH varied from less than 30 cc (66.67%), 30 to 59 cc (27.54%), and more than 60 cc (5.8%).  Conclusion: The CT scan imaging of young adults ICH mainly had hematoma volume less than 30 cc and was mostly located in basal ganglia area then followed by lobar area.

Abstract

Background: ICH accounts for 20% of stroke cases, with a mortality rate of 40-50%. Despite available agents such as citicoline and piracetam, neuroprotective therapy for ICH remains unsatisfactory. TQ, the main active compound of Nigella sativa, has antioxidant and anti-inflammatory properties, but its role in ICH has not been studied. Objective: To evaluate the effect of thymoquinone on inhibiting the progression of brain damage in a Wistar rat model of ICH and to explore potential mechanisms Methods: Male Wistar rats (n=55, 200–220 g) were randomly assigned to five groups: control (no treatment), ICH without thymoquinone (terminated immediately), ICH with corn oil (terminated after 7 days), and ICH with thymoquinone at doses of 150 and 250 mg/kg body weight orally for 7 days (terminated after 7 days). ICH was induced by autologous blood injection (0.12 ml) into the brain parenchyma. Brain and serum samples were analyzed for NLRP3, TNF-α, IL-6, IL-1β, MMP-9, SOD, MDA, and necrotic cells using ELISA, immunohistochemistry, and histology. Statistical analyses included univariate analysis, Shapiro–Wilk, Kruskal–Wallis, and Mann–Whitney U tests. Results: TQ administration significantly altered MMP-9 levels (p=0.000), which increased compared with untreated ICH. TQ was associated with reductions in NLRP3, TNF-α, and IL-1β, while increasing SOD activityMDA levels remained elevated. These effects were dose-dependent, with the most consistent changes observed at 250 mg/kg. Conclusion: TQ modulated inflammatory and oxidative pathways in ICH and unexpectedly upregulated MMP-9, suggesting a dual role in early injury and later tissue remodeling. These findings support TQ as a potential natural neuroprotective agent.
