Abstract

Indonesia is one of the countries with the largest number of tuberculosis (TB) cases in the world. Environmental factors play significant roles in infection and disease development in children living with adult active TB cases. The aim of this study was to explore the environmental factors affecting TB risk in children, including humidity and number of people living in the same house with the children.  Using a cross-sectional study design, children living with active TB adults for at least 8 weeks were recruited. The subjects underwent clinical examination, tuberculin skin test (TST) and radiological investigations. Home visits were conducted three times daily, namely in the morning, at midday, and in the evening, to measure humidity by digital hygrometer and to observe home conditions. Of 56 index cases living with active TB adults in an urban low socio-economic setting, 64 contact TB children were recruited. These children were classified as class I with negative TST and no clinical signs, class II with positive TST and no clinical signs, and class III with both positive TST and clinical signs. There were 32 (50%) class I, 10 (15.6%) class II and 22 (34.3%) class III children. However, a sub-sample of 43 showed positive results for Mtb 16S rRNA, indicating that all children were infected. The humidity cut-off point was set at 75%, p=0.04 and EP 2.09 (CI= 1.32-3.29), signifying that children living in houses with a humidity higher than 75%, were twice more likely to be tuberculin positive. Environmental conditions playing a role in disease development were humidity and number of people living in the house.

Abstract

ABSTRACT Breast cancer recurrence occurs because of factors such as surgical problem and clinicopathological features. The present study aimed to assess the relation between clinicopathological factors to breast cancer recurrence. Cross sectional design was used to take medical documents of all patients who visited Out Patient Department Soetomo Hospital from January 2015 to June 2015. A total of 228 patients were identified. The rates of breast cancer recurrence were 30%, most of recurrence occurred in first 5 years and the peak was in the first year. There were significant correlations between breast cancer recurrence and the factors observed below: tumor size [p=0.01,PR(95% CI)=1.621(1.086-2.421)]; nodal status 1-3 and ≥ 4 ≥ 4 [respectively p=0.014,PR(95%CI)=1.281(1.030-2.593) ; p=0.011,PR(95%CI) =1.289(1.038-1.601)]; tumor location [p=0.00,PR(95%CI)=2.422(1.576-3.721)]; hospital where the operation performed [p=0.03,PR(95%CI)=1.207(1.026-1.421)]; adjuvant therapy [p=0.00,PR(95%CI)= 4.371(2.231- 8.566)]. However there were no correlations of breast cancer recurrence and age, clinical stage or type of surgery. Conclusion, there are significant correlations between breast cancer recurrence and clinicopathological factors, such as tumor size, nodal status, tumor location, hospital where the operation performed and adjuvant therapy ABSTRAKKekambuhan kanker payudara terjadi karena beberapa faktor, di antaranya faktor klinis dan histopatologis. Penelitian ini bertujuan untuk mengetahui hubungan faktor klinis dan histopatologi terhadap kekambuhan. Penelitian ini menggunakan desain cross sectional dengan mengambil data dari rekam medis pasien yang berkunjung ke Unit Rawat Jalan RSUD Dr. Soetomo periode Januari- Agustus 2015. Sebanyak 228 pasien diidentifikasi. Tingkat kekambuhan kanker payudarasebsar 30%, sebagian besar kekambuhan terjadi dalam 5 tahun pertama, dan puncaknya adalah pada tahun pertama. Ada hubungan yang signifikan antara kekambuhan kanker payudara dengan faktor-faktor yang diamati: ukuran tumor [p=0,01, PR (95%CI) =1,621 (1.086-2.421)]; status nodal 1-3 and ≥ 4 ≥ 4 [masing-masing p=0,014,,PR(95%CI) = 1.281(1.030-2.593); p=0,011, PR (95%CI) = 1.289(1.038-1.601)]; letak tumor [p=0,00, PR(95%CI) = 2.422(1.576-3.721)]; rumah sakit tempat operasi dilaksanakan[p=0,03, PR,(95%CI)=1.207(1.026-1.421)]; terapi adjuvant [p=0,00, PR(95%CI)= 4.371(2.231-8.566)]. Namun, tidak ada korelasi antara kekambuhan kanker payudara dengan usia, stadium klinis, dan jenis operasi. Kesimpulan, terdapat hubungan yang signifikan antara kekambuhan kanker payudara dengan ukuran tumor, status nodal, lokasi tumor, rumah sakit di mana operasi dilaksanakan, dan terapi adjuvant.

Abstract

ABSTRAK Tujuan penelitian ini adalah untuk mendeskripsikan aktivitas guru dan anak dalam penggunaan media kartu huruf dan untuk mendeskripsikan penggunaan media kartu huruf dalam meningkatkan kemampuan mengenal keaksaraan anak usia 5-6 Tahun di TK Kusuma Putra Surabaya. Penelitian ini menggunakan penelitian tindakan kelas yang dibagi ke dalam dua siklus, setiap siklusnya terdiri atas dua pertemuan, dalam setiap pertemuan terbagi menjadi empat tahap, yaitu perencanaan, pelaksanaan, observasi dan refleksi yang dilanjutkan dengan analisis data dengan statistik deskriptif yaitu analisis refleksi berdasar siklus. Hasil dari penelitian ini pada siklus I aktivitas guru menunjukkan presentase sebesar 56,25% meningkat menjadi 87,5% pada siklus II. Aktivitas anak juga mengalami peningkatan, pada siklus I persentase keberhasilan anak adalah 53,13% sedangkan pada siklus II mengalami peningkatan menjadi 84,38%. Nilai rata-rata kemampuan mengenal keaksaraan juga mengalami peningkatan sebesar 33,14%, pada siklus I sebesar 51,24% sedangkan pada siklus II meningkat menjadi 84,16%. Hal ini berarti terjadi peningkatan dalam kemampuan mengenal keaksaraan melalui media kartu huruf, sehingga penelitian tindakan kelas ini dinyatakan berhasil karena telah mencapai tingkat keberhasilan yang telah ditentukan. Abstract

Objective: To give a visualization and explanation whether the use of ARV during exclusivebreastfeedingperiod is able to reduce the rate of transmission during exclusive breastfeeding.Methods: Systematic review of cross-sectional studies, case control, cohort studies, and prospectivestudies using Pubmed, LWW Journals, and Science Direct for literature search. Literature publishedbetween 2010 and 2020 are researched. Results are recorded using PRISMA, MMAT is used for biasriskevaluation.Results: A total of 2090 breastfed infants were included among the 2 studies. The first journal includedwas by Coovadiaet al.2012 was a 3rd phase clinical regarding the use of extended NVP on HIV exposedinfants were exclusive breastfed, concluded that the transmission rate for the group that receivedextendedNVP was only 1.1%. Rutagweraet al. 2019, focused on the shedding of HIV virus into thebreastmilk which is why this study used breastmilk as their sample4,6. Rutagweraet al. 2019. foundalmost 80% of their subjects shed HIV virus into their breastmilk6. This number is correlated to thetransmission of HIV vertically in breastfeeding mother.Conclusion: To conclude, the use of ARV does affect the rate of transmission of HIV from mother tochild during breastfeeding.

Abstract

Background: The prevalence of human immunodeficiency virus (HIV) mother-to-child infection, or vertical transmission of HIV, in Indonesia is about 90%. Ministry of Health of the Republic of Indonesia Regulation No. 21 of 2013 stated that HIV infection in sexual productive age needs integrative professional help to lower the transmissions, including voluntary counseling and testing for pregnant women as well as all people at risk. This study aimed to determine the readiness of health workers to provide pregnancy planning program service for women with HIV.Methods: This study was a descriptive study involving 30 obstetricians and 30 residents of Obstetrics and Gynecology specialist education in Surabaya, Indonesia. The respondents’ knowledge on pregnancy planning for HIV-infected women based on based on guidance released by the Ministry of Health of Indonesia in 2017 and Regulation of Ministry of Health no. 52 the year 2017, as well as their attitude and readiness to provide pregnancy planning services, was explored. Questionnaires were distributed from August to December 2019 and the data collected were analyzed using Statistical Package for the Social Science (SPSS) version 17.0 and results were described in percentage.Results: Most health workers (80%) had good knowledge on providing pregnancy planning program for HIV positive women; however, only 63.3% had good attitude towards it. The majority of respondents (90%) were ready to help HIV patients in planning their pregnancy. Conclusion: Health workers in Surabaya are ready to provide services related to pregnancy planning for women infected with HIV.

Abstract

Background: Gestational diabetes mellitus, defined as glucose intolerance in variable degree with onset during pregnancy, is a risk factor for the development of fetal macrosomia, a newborn with the birthweight of more than 4000g. The objective of this study was to explore an association between glucose level in lean pregnant diabetes mellitus patients, resulting in babies and the incidence of macrosomia in Dr Soetomo General Hospital in Surabaya.Methods: This was a retroprospective analytic study with a cross sectional design by using medical records at Dr. Soetomo General Hospital Surabaya, collected from January 2016 to December 2017. The correlation between fasting glucose level, 2-hour postprandial glucose and glucose level at the time of birth and weight of newborn was assessed.Results: Controlled and uncontrolled fasting glucose level, 2-hour postprandial glucose level and glucose level at the time of birth of mother were not significantly associated with macrosomia newborns. Macrosomia newborn were born with gestational diabetes mellitus mothers compare with pregestational mellitus mothers.Conclusions: There is no association between macrosomia with glucose level of lean gestational and pregestational diabetes mellitus, however, therapeutic approach such as physical exercise should be implemented to reduce effects of uncontrolled glucose level.

Abstract

Background: In 2018, East Java province becomes the province with the highest number of Human Immunodeficiency Virus (HIV) cases in Indonesia with an incidence of more than 8,000 cases. This province has various groups of Indonesian Migrant Workers (Tenaga Kerja Indonesia, TKI) working in various sectors in different countries, making them prone to HIV infection. This study aimed to determine the level of knowledge and attitude towards HIV and Acquired Immune Deficiency Syndrome (AIDS) among migrant worker candidates in East Java.Methods: This was a cross-sectional descriptive study conducted in August 2018 in the Provincial Manpower and Transmigration Office of East Java, Indonesia. The knowledge and attitude towards HIV disease were assessed using a self-validated questionnaire distributed to 104 migrant worker candidates as the subjects of this study.Results: Eighty-nine percent of the subjects had good knowledge, while 98% of them had good attitudes towards HIV disease. Furthermore, 86% had good attitudes toward HIV transmission. However, no correlation was observed between the subjects’ level of knowledge and their attitude (p=0.334).Conclusions: In general, the level of knowledge and attitudes of migrant worker candidates in East Java towards HIV disease is good. Nevertheless, more knowledge and education on healthy behavior need to be envisaged for this group.

Abstract

Background: Indonesia is one of the heavy burden countries with high pulmonary tuberculosis incidence in the world. The physical condition of houses is one of the environmental factors on how the infection spreads. The objective of this study was to provide supporting facts between the physical environment of the houses and the incidence of pulmonary tuberculosis.Methods: This was an observational analytic study with a case-control design, using checklists among pulmonary tuberculosis patients (n 32) and their healthy controls (n 32). Study subjects were chosen by simple random sampling. The study was performed from March to August 2019 at the Public Health Center (Pusat Kesehatan Masyarakat, Puskesmas) in Sawahan district, Surabaya. The incidence of pulmonary tuberculosis was related to various factors, including window existence, window opening habit, ventilation-to-area ratio, humidity, temperature, lighting, and occupancy density. The collected data were analyzed using the chi-square test to assess statistical significance with a p-value <0.05 was considered significantly related.Results: The ventilation-to-area ratio (p<0.05) and house lighting (p<0.00) were related to pulmonary tuberculosis incidence, whereas other factors such as window existence, window opening habit, temperature, and occupancy density were not.Conclusions: The ventilation-to-area ratio and house lighting have been proven to be related to pulmonary tuberculosis incidence. Government regulation on building permit needs to be encouraged to reduce the spreading and the incidence of pulmonary tuberculosis.

Abstract

Sekitar 15% dari semua penderita diabetes akan mengalami ulkus pada kaki dan berdampak pada amputasi. Peran perawat adalah melakukan upaya pencegahan terhadap ulkus diabetik melalui edukasi perawatan kaki dan senam kaki. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh edukasi perawatan kaki dan senam kaki terhadap upaya pencegahan risiko foot ulcer pada klien diabetes mellitus. Desain penelitian yang digunakan adalah quasy experiment (pre-post test with control design). Penelitian ini terdiri dari 2 kelompok (Intervensi dan kontrol) dengan masing-masing responden sebanyak 47 yang diseleksi dengan teknik consecutive sampling. Instrumen penelitian adalah monofilament 10 g jenis SemmesWeinstein monofilament, Garpu tala 128 Hz, pedoman pengambilan sampel darah, pedoman pemeriksaan TTGO, Tensimeter, pengukuran arteri dorsalis pedis serta format checklist perawatan kaki dan senam kaki. Data dianalisis dengan paired samples t-test dengan hasil ada pengaruh edukasi perawatan kaki dan senam kaki pada kelompok intervensi dilihat dari 4 indikator yang diukur yaitu penilaian neuropati diabetik, penilaian vaskuler, penilaian struktur kulit dan penilaian diabetes self care dengan nilai signifikansi masing-masing variabel yang diukur 0,000. Disimpulkan bahwa edukasi perawatan kaki dan senam kaki efektif mencegah risiko foot ulcer pada diabetes mellitus.

Abstract

Gadgets in the modern era very easy to find, every community has gadgets, the use of gadgets today not only in adults but also in children, the use of gadgets in children depends on how parents react to them, some parents make gadgets to be an entertainment medium for children. Excessive use of gadgets will adversely affect children's health and development. This study aims to look at descriptif the attitude parents aboute use gadget in children. This study was a descriptive exploratory study design with a sample of 131 respondents using cluster random sampling techniques from kindergarten schools in the area Banjar Regency, South Kalimantan. The results showed that the attitude parents most of the gadgets use negative attitudes with the use of gadgets in children at minimum 1 hour and a maximum of 6 hours every day. Negative attitudes of parents and lack of concern for the limitations of the use of gadgets in children make the use of gadgets to children higher and can increase the risk of the impact of using gadgets on children. With the results of this study it is expected that nurses can provide health promotion in the form of screen time usage limits and the impact that can occur on the use of gadgets. Keywords: attitude; parents; children; gadget ABSTRAK Gadget pada masa era modern sangat mudah ditemukan dan setiap kelompok masyarakat memiliki gadget, penggunaan gadget saat ini tidak hanya pada orang dewasa tetapi juga pada anak-anak, penggunaan gadget pada anak tergantung pada bagaimana orang tua menyikapinya, sebagian orangtua menjadikan gadget sebagai media hiburan bagi anak-anak. Penggunaan gadget yang berlebihan akan berdampak buruk bagi kesehatan dan perkembangan anak. Tujuan penelitian ini untuk melihat sikap orang tua tentang pnggunaan gadget pada anak. Penelitian ini merupakan penelitian deskriptif dengan sampel 131 responden dengan tehnik random sampling dari sekolah taman kanak-kanak wilayah kabupaten banjar, Kalimantan selatan. Hasil penelitian menunjukan sikap orang tua terhadap penggunaan gadget sebagian besar memiliki sikap negatif dengan penggunaan gadget pada anak minimal 1 jam dan maksimal 6 jam perhari. Sikap orang tua yang negatif atau kurangnya kepedulian terhadap batasan penggunaan gadget pada anak menjadikan penggunaan gadget pada anak semakin tinggi serta dapat meningkatkan risiko dampak penggunaan gadget pada anak, dengan hasil penelitian ini di harapkan perawat dapat memberikan promosi kesehatan berupa batas penggunaan waktu layar dan dampak yang dapat terjadi terhadap penggunaan gadget.
