Abstract

Tuberculous Meningitis (TBM) is a severely disabling condition that often leads to death. It requires the right intensive treatment to minimize disability, speed up recovery, and reduce the risk of death. Rehabilitation is one of the key components in managing TBM. Rehabilitation is expected to speed up recovery and prevent disabilities. The purpose of this case series was to highlight the effectiveness of a comprehensive rehabilitation program in severe TBM patients at Intensive Care Unit (ICU) settings. We present two cases of severe TBM that showed significant changes after the rehabilitation program started. Both patients were at GCS 111. Rehabilitation program was a therapist-based multimodal sensory stimulation in a comprehensive ICU setting with approximately 2 weeks duration. After 5 days of rehabilitation, the first patient’s consciousness began to improve. The second patient’s consciousness began to improve after 1 week of rehabilitation. After approximately 2 weeks, both of them were already at GCS 456 and then transferred to the High Care Unit (HCU). There were only minor disabilities as they were transferred. The rehabilitation program was adapted and continued at HCU. After 1 month, both of them got discharged to home. These cases highlight the importance of rehabilitation programs to be included in the management of TBM patients.

Abstract

Background : Multiple trauma one of trauma caused by huge traumatic force involves many organs and tissues. AIS and ISS is scoring for multiple trauma diagnosis and classification. In this Case Report discuss about multiple trauma patient with Severe Brain Injury cause by Impression Fracture with open fracture in regio cruris sinistra and close fracture femur sinistra. Case presentation : The following is a case report of a 63-year-old female patient came to the emergency room of Soegiri Lamongan with decreased of consciousness due to a traffic accident. Mechanism of injury the patient's head hit the chair and the driver's side of the car pillar and the patient's legs were pinched by the car seat. Since 25-year-old patient has a history of uncontrolled diabetes mellitus but the patient regularly takes anti-diabetic drugs (OAD). Patient was diagnosed with multiple trauma with severe brain injury with suspected DAI with open fracture grade 3A middle 1/3 tibia and close fracture middle 1/3 distal femur.Conclusion : Multiple Trauma has a bad prognosis. The most important from Multiple trauma management is prehospotality management. Additional Severe Brain Injury make the diagnosis become worse.

Abstract

Background: Meningiomas, common primary brain neoplasms, become more frequent with age. They can affect osseous structures, manifesting as hyperostosis and significantly diminishing the well-being of patients. Although meningiomas' precise etiology and pathogenesis remain ambiguous, hormonal influences and other factors have been implicated in their development. This study seeks to explain surgical management and cranioplasty reconstruction to reduce recurrence and improve the well-being of patients.Objective: This case series encompasses a retrospective analysis of medical records from six patients diagnosed with meningiomas with hyperostosis, treated between December 2022 and January 2024, at Dr. Soegiri General Hospital, Lamongan. The analysis focuses on surgical interventions and cranioplasty reconstruction.Cases Presentation: Six patients had chronic cephalgia and cranial masses persisting for 3-6 years. Computed tomography imaging revealed hyperostotic changes contiguous with the neoplastic lesions, prompting craniotomy. Subsequent acrylic cranioplasty was undertaken to ensure encephalic protection and to achieve cosmetic restoration.Conclusion: For patients presenting with meningiomas and hyperostosis, symptomatic management through surgical excision and resection of the hyperostotic bone is recommended. Early implementation of cranioplasty, preferably acrylic materials, is recommended to shield cerebral structures and restore cranial morphology, given its economic feasibility and reduced infection risk compared to autologous bone grafts

Abstract

Penelitian ini bertujuan untuk mendiskripsikan bagaimana penggunaan Sistem Manajemen Pembelajaran (SMP) schoology pada pembelajaran blended learning kediklatan dan untuk mengetahi seberapa jauh pembelajaran ini dapat meningkatkan kompetensi peserta diklat, baik kompetensi pengetahuan, ketrampilan maupun kompetensi sikap. Penelitian ini dilakukan peserta diklat teknis subtantif guru matematika MTs sebanyak 35 guru, bertempat di Balai Keagamaan Surabaya. Hasil penelitian yang sebagian besar data kompetensi peserta diperoleh secara online dan observasi, menunjukkan bahwa ada peningkatan sebesar 19.39% antara nilai awal dan nilai akhir kompetensi pengetahuan (dari rerata 69.00 menjadi 82,38). Dari Pengetahuan dan keterampilan subtansial, terdapat tiga aspek yang dengan rerata sudah berada di atas level tuntas yaitu Aspek Aljabar, Aritmatika, dan Aspek Statistika (rerata 86.6, 80,1, dan 85.3), namun masih ada satu aspek yang dibawah nilai standar kediklatan, yaitu aspek Geometri (rerata 66.32). Dari kompetensi sikap terdapat dua sikap yang berkembang dengan sangat baik yaitu Sikap Kolaboratif dan Komunikatif serta Sikap Keingintahuan (88.2 % dan 85.6 %), sementara Sikap Kritis dan Kreatif serta Sikap lnisiatif masih perlu dikembangkan lebih lanjut. Demikian halnya dari ketuntasan peserta, juga ada dua sikap yang tuntas dengan menonjol yaitu Sikap Kolaboratif dan Komunikatif serta Sikap Keingintahuan (82.86 dan 80.00). Rasa percaya diri peserta terhadap penggunaan SMP Schoology di madrasah mereka masing - masin juga besar (81%). Hasil diatas menunjukkan bahwa SMP Schoology dapat dipergunakan untuk meningkatkan kompetensi peserta Diklat Guru Matematika.
