Abstract

Background: Sepsis cause death in the worldwide pediatric population. Eestablishing the diagnosis of sepsis in children is challenging because the symptoms are varied and not specific. Serum ferritin is an acute phase protein which can be elevated in sepsis. This study aimed to determine the role of ferritin level as a predictor for sepsis mortality in pediatric patient Subjects and Method: A cross sectional study was conducted in children aged between 1 months and 18 years old diagnosed with sepsis who were treated at Dr. Moewardi General Hospital, Surakarta from November 2021 to April 2022.  The statistical analysis used SPSS 25 with p<0.05 was considered significant. The cut off point of serum ferritin level was determined with  ROC curve. Results: A cross sectional study was conducted in children aged between 1 months and 18 years old diagnosed with sepsis who were treated at Dr. Moewardi General Hospital, Surakarta from November 2021 to April 2022.  The statistical analysis used SPSS 25 with p<0.05 was considered significant. Abstract

Background: The prevalence of mortality among critically ill children worldwide, particularly in Indonesia, remains alarmingly high. Numerous studies have indicated that elevated lactate levels serve as a significant predictor of mortality in this vulnerable population. The primary objective of this study is to elucidate the predictive value of lactate levels in determining mortality risk among critically ill children. Subjects and Method: A cohort prospective study was conducted in children aged between 1 month and 18 years old who met the criteria for critically ill children treated in the Pediatric High care unit (PHCU) of Dr. Moewardi General Hospital, Surakarta from January to June 2023. The dependent variable was the mortality. The independent variable was lactate level serum. The data were collected using questionnaire and observation sheet. The cut off point of lactate level was determined with  ROC curve. Results: Among thirty critically ill pediatric patients, 43.3% died, while 56.7% survived. The deceased predominantly comprised females, with a mean age of 5.62 years (SD= 4.14). The mean lactate levels of those who died in the first hour, 6 hours, and 24 hours (Mean= 2.94; SD= 0.90) with a cut-off value of ≥2.35 mmol/L, (Mean= 3.20; SD= 0.50) with a cut-off value of ≥ 2.35 mmol/L and (Mean= 3.65; SD= 0.53) with a cut-off value of ≥2.95 mmol/L, respectively. The 24th hours lactate levels (persistence hyperlactatemia) had the highest sensitivity (92.3%) and specificity (100%) for predicting mortality (p<0.001). Conclusion: In critically ill children, lactate level of ≥2.95 mmol/L is a good predictor of mortality.

Abstract

Buah Merah (Pandanus conoideus Lam), is an endemic plant from Papua and its surrounding. Oil of buah merah known and claimed for cancer, HIV/AIDS, lung tuberculosis, diabetic and decreased blood cholesterol treatment. Since buah merah oil widlly is used in communities, safety test is needed. There were no toxic effects as abnormal behavior or mortality in rats, even using in high dose of 5 ml/200g body weight or approximately 9.4 times of human dose (1 x HD = 30 ml). Due to this situation, the acute toxicity has been done by Biomedical and Pharmaceutical Research and Development Center, at 2006. The result showed that qualitative test based on chemical group compounds, triterpenoid, phenol and steroid have been found clearly. Basically the oil is not toxic for single dose. Since there ‘s no mortality, the LD;0 will be considered as pseudo LD50, that means that doses of 5 ml/200g body weight of Wistar rat is safe for utilization, or the safety dose for human will bein a very large range.

Abstract

Gloriosa superba L belongs to the Colchiceae family. These vine plants reach 2.5 m in height and can grow at 400 in above sea level. The leaves contain chemical compounds such as alkaloid colchisin, glorisin and triterpen which possess analgesic properties. Many studies have been conducted in the tubers of such plants but scientific information on the eflicacy the leaves as analgesic have not thus far been published. Therefore, in this work a study wa conducted to test behavior and analgesic efiicacy of compounds in the Gloria superba L. leaves extracted with ethanol. The results showed the influence on the central nervous system and analgesic eflecls at doses of extract for 2 mg/100g body weight of mice and it's no significant diflerent with asetosal (p< 0, 05).

Abstract

Eflicacy of a plant can be caused by chemical compounds contain, on the other hand can cause toxic eflects. Toxic ejfects of chemicals can be defined as the potential of chemicals to poison the bodies of people who are exposed kembang sungsang (Gloriosa superba Linn.) including Colchicaceae families. Plants used empirically for treatment of Gout, diuretic, rheumatism, etc. Mutagenicity is a test to determine the possibility of compounds are mutagens. To see if these plants have the effect of mutagens, mutagenicity tests performed 70% ethanol extract of the leaves Kembang sungsang. Mutagenicity studies conducted by the Ames method using five test bacterial strains that have been transferred are.‘ Salmonella typhimurium TA 98, TA 100, TA 1535, TA 1537 and Escherichia colt WP2uvrA with and without metabolic activator S-9. The dose tested was 125, Z50, 500, 1000, and 2000 ul /plate. As a negative control was DMSO solution of 100 ul /plate. The results showed that the mutagenic test 70% ethanol extract of leaves Kembang sztngsang. with the addition aktivator without the addition of S-9 did not have the eflect of mutagens.
