Abstract

AbstrakWHO merekomendasikan penggunaan obat tradisional untuk penyakit degeneratif. Tujuan studi adalah untuk mengevaluasi implementasi praktik dokter komplementer-alternatif menggunakan ramuan jamu untuk penyakit degeneratif di 12 provinsi. Data dikumpulkan melalui wawancara menggunakan kuesioner, secara purposive. Hasil studi menunjukkan bahwa, sebanyak 86% dari 145 responden melakukan praktik komplementer-alternatif, dengan penggunaan obat tradisional/jamu. Separuh dari jumlah dokter tersebut, 49 % berada di Provinsi Jawa Tengah, dan 50 % berpraktik sebagai dokter praktik mandiri. 91,2 % dokter melakukan praktik komplementeralternatif dengan alasan utama atas permintaan masyarakat. Menurut persepsi dan pendapat dokter, alasan terbesar dari pasien berobat ke dokter dan meminta pelayanan jamu, adalah karena percaya, mencapai angka 85,1%. Terdapat upaya untuk memisahkan catatan medik pasien (28,3%) dan meminta pasien untuk menanda tangani informed consent (43,4%). Penggunaan tanaman obat oleh dokter terbanyak berturut-turut adalah jahe (artritis), sambiloto (hiperglikemia), seledri ( hipertensi), jatibelanda (hiperlipidemia dan obesitas), serta sidaguri ( hiperurisemia). Menurut pendapat dokter urutan kecepatan kesembuhan pasien, berurutan mulai dari penyakit artritis, hiperglikemia, hipertensi, hiperlipidemia, hiperurisemia dan terakhir adalah obesitas. Pelayanan dengan jamu/obat tradisional oleh dokter di masyarakat sudah menjadi pilihan pasien. Menurut pendapat dokter obat tradisional/jamu ada manfaatnya.Kata Kunci : Praktik complementary aternative medicine, obat tradisional, penyakit degeneratif , dokterAbstractWHO has recommended the use of traditional medicine for degenerative diseases. The objective of this study is to evaluate the implementation of CAM doctors practice in using traditional medicine for degenerative diseases in 12 provinces. Data were collected through interviews using questionnaire to CAM doctors purposively. This study showed that 86 % of 145 the respondents did CAM practice by using traditional medicine. Almost half (49 %) of the doctors, were in Central Java Province and 50 % were independent doctors. There were 91.2 % doctors did CAM practice by demand from the society as main reason. Based on the doctor’s perception and opinion, the main reason for the patients seeing the doctors and asking for traditional medicine were trust (85.1 %). There were initiatives to differentiate CAM patient medical record from the common one (28.3 %) and ask patients to sign the informed consent (43.4 %). The most common medicinal plant utilized by doctors, consecutively were jahe (arthritis), sambiloto ( hyperglicaemia), seledri ( hypertension), jati belanda ( hyperlipidemia, and obesity), and also sidaguri ( hyperuricemia). Based on the doctors’ opinion, the most rapid disease to heal with traditional medicine consecutively were arthritis, hyperglycemia, hypertension, hyperlipidemia, hyperuricemia, and the last was obesity. Traditional medicine prescribed by the doctors in the community had already been the patient choice. Abstract

Telah dilakukan sebuah penelitian observasi, purposif dan deskriptif terhadap dokter praktik jamusecara komplementer-alternatif dengan menggunakan jamu di 9 dari 12 provinsi wilayah SentraPengembangan, Penerapan dan Pengobatan Tradisional (SP3T) di Indonesia selama 6 bulan penelitian.Didapatkan 63 pasien artritis yang yang masuk ke dalam penelitian, menerima terapi konvensionaldan tradisional. Seluruh pasien berusia ≥16 tahun, dengan persentase terbanyak pada usia 51-70tahun (50,8%). Ditemukan 37% pasien memiliki riwayat penyakit hipertensi sebelumnya, dan 7%riwayat rematoid arthritis. Sebanyak 47% pasien dengan hipertensi pada keluarga dan 16% pasiendengan rematoid arthritis pada keluarganya. Terapi konvensional terbanyak yang digunakan dalamterapi pasien arthritis yaitu golongan NSAID (43%), disusul suplemen (22%), fiioterapi (12%), antipirai(10%), kortikosteroid (4%), lain-lain (4%), dan analgetik narkotik (3%). Komponen jamu yang seringdigunakan yaitu jamu osteoarthritis Tawangmangu (37,5%), sambiloto (11,3%), temulawak (11,2%),jahe (8,1%), habbatussauda/jinten hitam (8,1%), dan murat (4,8%). Adapun keterampilan dengan alatyang digunakan yaitu akupunktur (47%), akupresur (13%), stimulasi listrik (7%), akupunktur & stimulasilistrik (7%). Perubahan pasca terapi yang terjadi adalah perbaikan, berupa hilangnya gejala penyakit.Gejala klinis yang paling banyak menghilang saat follow up yaitu gejala sistem neurologis (33%), sistemmuskuloskeletal (31%), dan tak kalah pentingnya yaitu gejala umum (23%), karena 3 dari 4 gejala umum(tidak nafsu makan, letih, dan penurunan berat badan) merupakan gejala yang paling sering ditemuipada penderita rematoid artritis. Meskipun demikian perbaikan gejala klinis ini belum bisa dipastikansemata-mata karena efek terapi jamu saja, karena selain jamu digunakan pula terapi konvensionallainnya. Ditemukan pula peningkatan Quality of Life (QoL) derajat “baik” sebelum terapi (36%) danmenjadi 79% pada masa sesudah terapi.

Abstract

Teratogenic test has been performed on sweet stevia as product of B2P2TO2T cultivation at  rats (Rattus novergicus) pregnant female  Wistar. Stevia sweet was administered orally at a dose of 360, 120 and 40 mg/kg bw, volume 1 ml/100g bw per day during organogenesis period, on the day of pregnancy to the 6th until 15th. During the test, test animals were observed two times daily with the distance of six hours against the toxicity symptoms such as changes in skin, hair, eyes and mucous membranes, bleeding. Animals that experienced abortion, premature birth or death during the trial period were sacrificed and observed immediately with microscopic technic. At 20th day of pregnancy all of the pregnant rat dissected, and put out the fetuses  from the mother's and observated the health conditions in general and  the whole of mothers reproductive systems of fetuses, the outer fetal malformation and soft tissue system of fetal. The conclusion were  sweet stevia  of B2P2TO2T  no worse effect on the mother rats, fetal body weight and morphology of mother rats and the fetus, does not affect the process of development of fetal soft tissue, and does not affect the development of fetal skeleton. Generally the B2P2TO2T stevia sweet substances safe for used, does not cause teratogenic effects.

Abstract

Background: In the COVID-19 pandemic situation, herbal medicine is quite widely used by people. Objective: to assess the description of the handling of mild cases of COVID-19 clients using herbal ingredients, which was carried out at a Traditional Health Care Center in Jakarta. Method: Descriptive research with retrospective case study design on mild COVID-19 cases using or being treated with traditional Indonesian herbal medicine, using secondary data from medical records from 30 COVID-19 clients who had COVID-19 status based on PCR examination. Results: There were 7 types of herbs given either as a single herb or a combination of them. Most of the ingredients used were sambiloto capsules and Jamu B concoction. After drinking the potions, 21 clients were symptom free on day 14, 2 clients were symptom free on days 5 and 7 and were noted to have a habit of drinking jamu for physical fitness. Cumulatively, all clients were symptom free on day 23 with stable condition in 8 clients (26.7%) and 22 clients (73.3%) stated that they were getting lighter. The most cases of transmission of COVID-19 to people around them were 5 clients who infected 5 people. All infected people have mild/asymptomatic COVID-19. Conclusion: Herbal medicine has the potential to be used for COVID-19 clients with mild/asymptomatic complaints, and the habit of drinking herbal concoctions for fitness can shorten the day of symptom relief compared to those who do not drink herbal medicine before getting sick.

Abstract

Dismenore adalah rasa kram atau nyeri yang terjadi ketika menstruasi. Meskipun efektif dalam pengobatan dismenore, penggunaan AINS jangka panjang dapat menyebabkan efek samping pada sistem gastrointestinal dan terjadi pendarahan. Tujuan penelitian ini adalah untuk mengetahui aktivitas antiinflamasi ramuan jamu dismenore yang terdiri dari ekstrak sambiloto, kunyit, kelor, jahe merah,dan coklat hitam terhadap enzim COX yang ditentukan dengan menggunakan metode TMPD  secara spektrofotometri. Berdasarkan hasil pengujian, diketahui ekstrak sambiloto memiliki aktivitas penghambatan baik pada enzim COX-1 maupun enzim COX-2 dengan persen inhibisi masing-masing sebesar 4.5% (1000 ppm) dan 74.29% (1000 ppm). Berdasarkan hasil analisis metabolomik menggunakan LC-HRMS, salah satu senyawa yang terdeteksi pada sampel ekstrak etanol sambiloto yaitu senyawa turunan andrographolide yaitu 14-Deoxy-11,12-didehydro andrographolide dengan rumus kimia C20H28O4. Senyawa paling melimpah kedua pada ekstrak sambiloto ini terdeteksi oleh ESI positif dengan waktu retensi 9.804 menit dan dengan berat molekul 332,1974.

Abstract

Celery (Apium graveolens L) is a very popular plant as a soup and many other vegetable menus. Celery has benefits as a medicinal plant and the efficacy has been known worlwide. Research on Medicinal Plants and Herbs (Ristoja) conducted in 2012, 2015, and 2017 has collected thousands of herbs and some of them contain celery. This study carried out further analysis on the empirical use of celery as an ingredient of traditional medicine by traditional healers (Hattra) through interviews and observations. Based on Ristoja's data, they found 90 herbs and among them there were 20 local names of herbs which are used by Hattra. The celery by Hattra was empirically identified for 10 types of diseases treatment. It was used mostly for hypertension or high blood pressure treatment and justificatied scientifically through other scientific literatures. It was recommended that celery could be use for self-medication a mild hypertension patient. Celery was safe although there are still contra indications. The celery treatment should be socialized through promotive activities in elderly posyandu activities remain under supervision of health workers

Abstract

Buah Merah (Pandanus conoideus Lam), is an endemic plant from Papua and its surrounding. Oil of buah merah known and claimed for cancer, HIV/AIDS, lung tuberculosis, diabetic and decreased blood cholesterol treatment. Since buah merah oil widlly is used in communities, safety test is needed. There were no toxic effects as abnormal behavior or mortality in rats, even using in high dose of 5 ml/200g body weight or approximately 9.4 times of human dose (1 x HD = 30 ml). Due to this situation, the acute toxicity has been done by Biomedical and Pharmaceutical Research and Development Center, at 2006. The result showed that qualitative test based on chemical group compounds, triterpenoid, phenol and steroid have been found clearly. Basically the oil is not toxic for single dose. Since there ‘s no mortality, the LD;0 will be considered as pseudo LD50, that means that doses of 5 ml/200g body weight of Wistar rat is safe for utilization, or the safety dose for human will bein a very large range.

Abstract

The growth of the number of Traditional Healers at Kutai Kartanegara Regency increases, including the Traditional Healer of remedies, using medicinal plants. The presence of herbs Traditonal Healers indirectly in improving helath and this condition needs to be proved by a survey to Traditional Healers with remedies of medicinal plant, using a questionnaire instrument. Traditoonal Healers profiles obtained from interviews using a questionnaire that describes Traditional Healers characteristics, source of knowledge, permission practices, place / space of practice, patient records etc. It is of concern is that almost all Traditional Healers not yet registered with the local Ministry of Health and have no record of the patient. While in the case of the medicinal plants used by Traditional Healers, problems that arise are very varied although for medicinal plants / complaints / same disease. Rationally number of medicinal plants, how making the right ingredients, dosage of medication use and how long to use these medicinal plants, needs to be studied to assure safety of use and provide the expected properties. The assessment results will be the direction of further development is still traditionally used or will be carried out research into herbal standardized, phytopharmaca,even into the drug with the specific active compounds, so that can be directed to the acquisition of patents. In the framework of guidance covering the above matters, suggested the existence of cooperation and coordination with the Faculty of Medicine Faculty of Pharmacy, or who are in the province of East Kalimantan.

Abstract

Circulation disorder of blood at a person is related to metabolic process, and it can not be as the normal equilibirum, and later can affect to appearance of disseases. Dew water, can be produced from destilation process (Systemized Dew Process), filtered process and disinfection with ozoniation. Dew water is stated can be used for disorder of blood circulation treatment. For the examination effect of dew water for blood circulation disorder, dew water was tested for blood description and biochemistry blood description at the animal model with cholesterol diet.It was the experimental pharmacology method, with the blood description as hamoglobline, erythrocyte, leucocyte, thrombocyte, hematochrite value; biochemistry description as total cholesterol, LDL cholesterol, HDL cholesterol, trigliserid; freeze time of blood; the arteri thickness ; body weight and the volume of intake water. Base on the normally of dosage at human, dew water intake for animal model were all day long admintration and of all of research time, every day for 2 month. The result of research has shown decrease of the body weight; increase need for intake water ; repairing haematology and biochemistry of blood, escpecially decrease of total cholesterol and LDL cholesterol; diluting blood; decreasing ateroschlerosis disorder at arteri vessel

Abstract

From the collection of data (SDKI) in 2002-2003, it is found that the number of exclusive breastfeeding in infants below the age of two months covers only 64% of total infants. The most alarming facts are that 13% of infants under two months have been fed infant milk formula and one of three infants aged 2-3 months has been given additional food. Therefore, using Kelor as herba to facilitating breast milk is needed as extract requirement. In addition, this research considers characterization of Kelor extract as a first step to standardize Kelor extract. Sample is extract of Kelor leaf from East Java. To ensure quality requirements of 70% ethanol extract from Kelor leaf for herbal medicine, it has to meet the quality requirement guidelines established by BPOM. Examination includes non-specific parameter such as water content, total ash, total acid insoluble ash, and extract microscopic examination. In the other side, specific parameter includes content of dilute alkohol, content of dilute water, assay of total alkaloid and chemistry compound test. Characteristic of 70 % ethanol extract from Kelor leaf for non-specific parameter are water content 15,68%, total ash 3,04%, total acid insoluble ash 1,13% and loss on drying 29,70% , whereas characteristic of spesific parameter for content of dilute etanol 33,11%, content of dilute water 47,53%, and assay Trigonellin 15,68 %. The conclusion is non spesific characteristic from kelor leaf etanol extract, that is water content does not meet the quality requirement guidelines
