Abstract

Abstract: The purpose of this study was to identify the value of fall risk assessment using Morse fall scale and STRATIFY fall scale, to identify the sensitivity and specificity of Morse fall scale and STRATIFY. Design of this research is a analytic descriptive cross-sectional. Sampling technique used consecutive sampling with the total sample 100 respondent. The analysis used to determine sensitivity and specificity was 2x2 table. The results showed value of Morse fall risk scale using cut off point 45 were 51% high fall risk, 35% moderate fall risk, and 14% low fall risk. The value of STRATIFY’S fall risk scale were 18% high fall risk, 46% moderate fall risk, and 36% low fall risk.  Using cut off point 45 on Morse Fall Scale, it’s sensitivity was 80% and it’s specificity was 56,3%. The sensitivity of STRATIFY was 31% and specificity was 91,8%. Conclusion: Morse Fall Scale more sensitive than STRATIFY, however specificity of STRATIFY higher than specificity of Morse Fall Scale.

Abstract

The Province of the Special Region of Yogyakarta ranks third with the highest number of cases of Diabetes Mellitus (DM) nationally. One of the complications of DM, a severe health problem, is diabetic ulcers. Diabetic ulcer care and Covid-19 prevention are strictly associated with a high risk of transmission, a poor prognosis, and a high mortality rate when infected with Covid-19 are needed during a pandemic situation, one of which is to increase the role of the family (caregiver). The purpose of the activity is to optimize the role of caregivers in the simple care of diabetic wounds at home (stay at home) during the Covid-19 period. Comprehensive education about DM management in general and diabetic wounds in particular to families (caregivers), which is carried out using a direct method in conjunction with homecare services for simple treatment of diabetic ulcers while still implementing the Covid-19 health protocol. Providing good benefits to the family (caregiver) as evidenced by an increase in the average score of knowledge and also an increase in the average score of self-perception of ability in performing simple diabetic wound care. The development of long-term care for chronic patients can continue to be carried out by involving the active role of the family (caregiver) as the closest support system for patients. Education to the family directly increases the knowledge and confidence of the family in performing simple diabetic wound care. This program can be an alternative to optimizing infection prevention and the healing process of diabetic wounds.

The research method was conducted with a quasi-experiment for six months of intervention on sixteen patients obtained with a random sample for January-September 2022 at the PKU Muhammadiyah Hospital Yogyakarta homecare unit. Short form-36 (SF-36) was used to evaluate the quality of life dimension. Results; Interdisciplinary homecare significantly improves the health of patients with long-term care needs; diabetic ulcers (p=0.001). Age and comorbidities were identified as characteristics of patients with diabetic ulcers, which must be considered in treating diabetic ulcers. Conclusion; Assistance with interdisciplinary home care is recommended in long-term care in community settings and patients to improve the quality of life, especially the mental health of patients.
