Abstract

Background: Dementia patients were reported of having high risk for hip fracture. Rehabilitation programs were still being debated for this vulnerable population and the recovery outcomes in this frail elderly population are varied, thus evidenced-based outcomes for this issue are needed.Objective: To investigate rehabilitation programs, especially on physical function outcomes in patients with dementia following hip fracture surgery.Method: PubMed, Scopus and MEDLINE (EBSCO) databases were searched from January 2000 to June 2020 concerning rehabilitation intervention program in dementia patients who received hip surgical treatment. The studies included have the following criteria, (1) investigating physical outcomes of rehabilitation programs (2) Randomized controlled trials (RCTs) study design, (3) study involving dementia patients following hip fracture surgery (4) English full-text journal.Results: Six studies were included in this review. Most study focused on interdisciplinary or multidisciplinary rehabilitation program. Physical outcomes of rehabilitation programs seem to improve physical function, maintaining physical ability for ambulation, and decrease the risk of falls in mild or moderate dementia patients following hip fracture surgery. However, the physical function of older persons with dementia confirmed to be lower than those without dementia.Conclusion: Rehabilitation may show benefit for physical function in elderlywith mild or moderate dementia following hip fracture surgery, and this population needs to be included in the clinical rehabilitation program. Investigating and establishing the strategies to improve outcomes for dementia population is needed for future study.

Abstract

Hip fracture increased due to the aging phenomenon. Hip fracture injuries are identified as one of the most serious healthcare problems affecting the elderly, including instrument and physical function and quality of life problem.This study aimed to evaluate quality of life between 1st and 3rd months older person with hip fracture who had received surgical treatment. A prospective study design with 66 older people who have hip surgical treatment. Face-to-face Quality of life assessment used SF36 instrument was conducted. In general, a significant difference between 1st and 3rd months post hospital discharge was found (p = 0.001). In addition, the role of disability due to emotional problems and role disability due to physical health problems has the highest in differences than another dimension (M ± SD; 55.0 ± 38.6 and M ± SD; 44.3 ± 38.7; respectively), However, there is no significant difference in fatigue subscale (p = 0.624) between 1st and 3rd months post hospital discharge. In general, there was an increase in the quality of life between 1st  and 3rd months among hip elderly patients without concomitant disease. Comprehensive nursing care is needed in order to maintain and improve all dimensions of the quality of life for elderly with hip surgery. Longitudinal studies with a complexity of variables and a larger number of samples are highly recommended for future study.

Abstract

Background: Knowing about prenatal attachment is crucial because it plays a significant role in pregnant women and their babies' health. Healthcare providers, particularly midwives, need to understand the concept of attachment between mother and unborn baby. However, surrogate terms to apply the concept of maternal-fetal attachment are found, and ambiguity on the definition remains. Objective: To provide a more precise definition of the concept of maternal-fetal attachment Method: The Walker and Avant's concept analysis approach was applied. Results: The attributes of maternal-fetal attachment include (i) having a positive emotion, (ii) paying attention to the physical progress of the fetus and mothers themselves, (iii) having a reciprocal interaction with the baby, (iv) monitoring and imaging the unborn baby, and (v) having a desire to protect her unborn baby from harm and increase her health practices. Maternal-fetal attachment is the affection relationship, desire for protection, building the interaction, and the emotional tie between the mother and her unborn baby during the pregnancy. This personal connection is developed, mainly when the quickening is present during the late stage of pregnancy. Maternal-fetal attachment creates an emotional relationship with the unborn baby and leads the mother to express it with behavior. Conclusion: This concept analysis provides new insight into the maternal-fetal attachment concept used for nurses and midwives in their practice.

Abstract

Hemodialysis procedure was influenced by the patient's quality of life. Comprehensive understanding changes of health-related quality of life dimensions to provide the best nursing care planning is needed. This study aimed to determine longitudinal changes for each dimension of health-related quality of life patient who received hemodialysis. Longitudinal study and Generalized estimating equations Statistic analysis were used. Thirty respondents were included in this study between December 2019 and July 2020. Overall trend each dimension of health-related quality of life in hemodialysis patients were decreased. Interestingly, a social function was showed potential to be maintained and improved. Also, age, co-morbidities and education level were found as predictors some of the changes the health-related quality of life dimensions. Developing nursing included social and family care program intervention to maintenance psychological dimensions is recommended for future study.

Abstract

Latar belakang: Populasi lansia merupakan fenomena global. Posyandu lansia Indonesia dibentuk untuk mempertahankan kesehatan lansia. Budaya dan dukungan keluarga merupakan faktor yang penting dalam kegiatan posyandu lansia. Penelitian ini bertujuan untuk mengatahui hubungan antara dukungan keluarga dan keaktifan lansia dalam mengikuti kegiatan posyandu lansia. Metode: Penelitian ini menggunakan survey korelasi dengan pendekatan crosectional. 114 responden yang telah diambil menggunakan teknik accidental sampling dan mengikuti penelitian ini. Analisys statistic Chi-scuare digunakan untuk mengetahui hubungan dukungan keluarga dengan keaktifan lansia dalam mengikuti program posyandu lansia.: Sebagian besar lansia dalam penelitian ini telah mendapat dukungan dari keluarga yaitu 72 responden (63.2%), namun terdapat sekitar 78 (68,4%) responden yang tidak aktif berpartisipasi dalam kegiatan posyandu lansia. Secara statistic didapatkan hubungan yang significant antara dukungan keluarga dan keaktifan lansia dalam kegiatan posyandu lansia (p=0.001) Kesimpulan: Dukungan keluarga diharapkan terus dilakukan untuk dapat memberikan perhatian, motivasi dan dukungan kepada lansia. Keluarga dapat memberikan dukungan dengan mengingatkan dan memberikan fasilitas untuk berpastisipasi dalam kegiatan setiap bulannya. Memberdayakan keluarga dalam mendukung kegiatan lansia menjadi hal yang menarik untuk peneliatian selanjutnya.

Abstract

Self-efficacy is important to ensure self-care to improve the health-related quality of life for hemodialysis patients. This study aimed to determine the relationship between self-efficacy and self-care and the health-related quality of life in hemodialysis patients. The research method was a quantitative correlation design with a cross-sectional approach. The instrument of the study was a questionnaire. A total sampling technique for 64 hemodialysis patients at RSUD (Regional Public Hospital) Wates Kulonprogo was used. Data analysis was conducted through the Kendall Tau test. It showed that 41 respondents (64.1%) were in the moderate category of self-efficacy, while 50 respondents (78.1%) were in the moderate category for the self-care aspect. The results of Kendall Tau analysis showed that self-efficacy was significantly associated with self-care (p= 0.004) and the health-related quality of life (p=0.041) for hemodialysis patients. It is expected that the nurse will periodically assess the biological and psychological aspects of hemodialysis patients.

Abstract

Maternal-fetal attachment is crucial for a mother to establish a healthy psychological relationship with her unborn baby. This relationship progresses during the week of gestation. Many studies support the evidence of the impact of maternal-fetal attachment on mothers’ mental well-being during both antenatal and postnatal. For a baby, maternal-fetal attachment is essential to determine his development. Family, particularly the husband, plays an essential role in supporting the mother to experience positive maternal-fetal attachment during pregnancy. This study was to investigate the relationship between family support with maternal-fetal attachment among expectant mother. This sample was drawn using the purposive sampling method based on criteria that include mothers in their third trimester, mothers with any age range, mothers with no complications during the current pregnancy, and mothers who stay together with their husbands and or family. Seventy-seven expectant mothers completed the Indonesian version of the Prenatal Attachment Inventory and family support questionnaires. Kendal tau was applied to calculate the statistical relationship between two variables (p0.05). The results showed that more than 50% mother obtained support from their family, and more than 50% mother have high score of maternal-fetal attachment. There were a significant relationship between family support and maternal-fetal attachment (p 0.000). The conclusion of this study was that there were a significant relationship between family support with maternal-fetal attachment. This finding provides evidence of the significant role of family support for maternal mental well-being through maternal-fetal attachment during pregnancy. Therefore, healthcare providers have to invite the husband or family to engage in support for the mother during their pregnancy.

Abstract

This research aimed to show the effectiveness cognitive behavioural educational intervention (CBEI) in post trans urethral resection of the prostate in PKU Muhammadiyah Bantul Hospital. The research design used Quasi-eksperiment; posttest only control group. Sample was taken by nonprobability sampling with accidental sampling method (on February-June 2015). There were 20 respondent have TURP procedure and which were divided into two groups. The t-test independent indicated a significant difference in pain respon in two groups (p=0,000). From this study, CBEI was recommended for pain management in patient with TURP.Keywords: cognitive behavioural educational intervention, pain, TURP

Abstract

The Province of the Special Region of Yogyakarta ranks third with the highest number of cases of Diabetes Mellitus (DM) nationally. One of the complications of DM, a severe health problem, is diabetic ulcers. Diabetic ulcer care and Covid-19 prevention are strictly associated with a high risk of transmission, a poor prognosis, and a high mortality rate when infected with Covid-19 are needed during a pandemic situation, one of which is to increase the role of the family (caregiver). The purpose of the activity is to optimize the role of caregivers in the simple care of diabetic wounds at home (stay at home) during the Covid-19 period. Comprehensive education about DM management in general and diabetic wounds in particular to families (caregivers), which is carried out using a direct method in conjunction with homecare services for simple treatment of diabetic ulcers while still implementing the Covid-19 health protocol. Providing good benefits to the family (caregiver) as evidenced by an increase in the average score of knowledge and also an increase in the average score of self-perception of ability in performing simple diabetic wound care. The development of long-term care for chronic patients can continue to be carried out by involving the active role of the family (caregiver) as the closest support system for patients. Education to the family directly increases the knowledge and confidence of the family in performing simple diabetic wound care. This program can be an alternative to optimizing infection prevention and the healing process of diabetic wounds.

Abstract

The COVID-19 pandemic has caused great panic and anxiety worldwide, including for patients dialysis who reported high mortality and decreased patients quality of life including caregivers. This study aimed to investigate the correlation between the quality of life among patients undergoing hemodialysis and caregiver, especially during COVID-19 in Indonesia. A cross-sectional study design has been adopted. An accidental sampling technique was employed in this study between June and December 2021. Short Form-36 and Hospital Anxiety and Depression Scale were used to measure primary outcomes. Seventy-eight patients receiving hemodialysis and Seventy-eight caregivers were recruited. All of the patients had comorbidities (88.7%) and patients tend to be at a high level of depression and anxiety (53.2%), Middle level of depression and anxiety (30,4%). However, only a few (26.9%) caregivers had comorbidities, and all were without a high level of depression and anxiety. This study shows no correlation between the patients quality of life and the family caregivers quality of life during the COVID-19 pandemic, both from the physical (p 0.43) and mental dimensions (p 0.55). This study concluded that hemodialysis status, being older, a number of comorbidities, and depressing status influence quality of life. Program and specific treatment on depression among end-stage renal disease suggested for future study. Abstrak: Pandemi COVID-19 telah menimbulkan kepanikan dan kecemasan yang luar biasa di seluruh dunia, termasuk bagi pasien cuci darah yang dilaporkan memiliki angka kematian yang tinggi, penurunan kualitas hidup pasien termasuk bagi caregiver. Penelitian ini bertujuan untuk mengetahui hubungan kualitas hidup antara pasien yang menjalani hemodialisis dengan caregiver khususnya selama COVID-19 di Indonesia. Sebuah desain studi cross-sectional telah diadopsi. Teknik pengambilan sampel secara kebetulan digunakan dalam penelitian ini antara bulan Juni dan Desember 2021. Short Form-36 dan Hospital Anxiety and Depression Scale digunakan untuk mengukur hasil primer. Tujuh puluh delapan pasien yang menerima hemodialisis dan Tujuh puluh delapan pengasuh direkrut. Semua pasien memiliki penyakit penyerta (88,7%) dan pasien cenderung berada pada tingkat depresi dan kecemasan tinggi (53,2%), tingkat depresi dan kecemasan sedang (30,4%). Namun, hanya sedikit (26,9%) pengasuh yang memiliki penyakit penyerta, dan semuanya tanpa tingkat depresi dan kecemasan yang tinggi. Penelitian ini menunjukkan tidak ada hubungan antara kualitas hidup pasien dengan kualitas hidup caregiver keluarga selama pandemi COVID-19, baik dari dimensi fisik (p 0,43) maupun mental (p 0,55). Penelitian ini menyimpulkan bahwa status hemodialisis, usia lanjut, jumlah penyakit penyerta, dan status depresi mempengaruhi kualitas hidup. Program dan pengobatan khusus pada depresi di antara penyakit ginjal stadium akhir disarankan untuk studi masa depan.
