Abstract

Background: The prevalence of Diabetes Mellitus (DM) in Indonesia continues to increase, even though DM control policies and programs have been implemented. According to existing policies, the family has not been involved in diabetes control. This study evaluates diabetes mellitus management policies by designing family-based community empowerment model interventions with a systems approach to surge capacity.   Methods: The design of this study used a cross-sectional operational analysis conducted in Cirebon City with 26 participants. Data collection was carried out using Focus Group Discussion (FGD) and in-depth interviews using interview guidelines from the surge capacity component. Inclusion criteria were Non-Communicable Diseases program holders at the Cirebon City Health Office and Community Health Centers with the highest and lowest prevalence, and DM sufferers and their families representing each age and gender category. Data analysis was performed using open code.   Results: The non-communicable disease program has not been integrated between the health office and the hospital; funds for the DM prevention program have not met the needs; there are limited human resources with multiple tasks and an excessive workload, so it is not optimal for DM health services; and there is a lack of family involvement in diabetes control, so the incidence of DM is still not usually controlled.   Conclusion: Policy studies using a system approach in surge capacity have been able to dig up various information on DM control efforts in terms of policy, organizational structure, DM surveillance, information systems, integrated services, case screening, budgeting, and community empowerment.

Abstract

Abstract

Biomedical waste including disposable masks requires proper waste handling so it doesn’t pollute the environment and cause re-transmission of COVID-19 disease. The purpose of this study was to look at the relationship between the knowledge, attitudes and practices of the Public Health Centre in Pangandaran Regency. Methodology: Cross-sectional study approach to study assessed KAP. 91 participants were from 15 different public health centres in Pangandaran. Data analysis used chi-square statistical test. Presented with frequency distribution and the form of a tabulation. Result indicated knowledge of respondents was only 41.40% with high scores and practices with high scores were only 44.40% but attitudes towards disposable mask waste management with high scores reached 98.6%. There is a relationship between age and practice with a p-value of 0.009 and a relationship between knowledge and practice with a p-value below 0.05. Conclusion the participants had insufficient knowledge and practice scores but have good attitude scores. Our results conclude that the HCWs good attitude does not affect good practice but knowledge may lead to a proper practice. Findings of this study suggest that a regular training program for the HCWs on BMW specially on disposable mask management may lead to better practice

Abstract

Diabetes mellitus (DM) poses significant health risks for hajj pilgrims due to physical stress, environmental exposure, and limited access to acute care during pilgrimage. In Indonesia, DM prevalence among prospective pilgrims is rising, yet empirical evidence on its predictors remains limited. This study aimed to identify demographic and clinical predictors of DM among hajj pilgrims from Majalengka District, West Java, using data from the 2024 Integrated Hajj Computerised System for Health Sector. A cross-sectional analysis was conducted on 1,140 pilgrims undergoing istithaah health screening. Variables included age, gender, BMI, abdominal obesity, blood pressure, lipid profiles, and family medical history. Descriptive statistics, Chi-square tests, and multivariate logistic regression were employed to examine associations with DM status. Older age (40–60 and >60 years), abdominal obesity, elevated triglyceride levels, and a family history of diabetes were significantly associated with increased odds of DM. In contrast, BMI and gender were not statistically significant predictors. Pilgrims who met the istithaah criteria had significantly lower prevalence of DM, supporting the effectiveness of current screening measures. However, gaps in consistent implementation remain. This study underscores the need to strengthen metabolic risk assessments in pre-hajj screenings by incorporating abdominal obesity and lipid profile measurements alongside conventional indicators. The findings support policy refinement in the istithaah process, including standardisation across districts and enhanced digital integration through Siskohatkes. These improvements are vital to safeguard pilgrims’ health and advance non-communicable disease prevention in Indonesia.
