Abstract

Spinal anesthesia is given to patients undergoing surgery on the abdominal area. Spinal anesthesia can interfere with temperature thermoregulation, thereby inhibiting the compensatory response to temperature, causing shivering. The aim of the research was to determine the incidence of shivering in post-operative patients with spinal anesthesia at the Wijayakusuma Hospital , Purwokerto 2023. This research used a quantitative descriptive research design with a cross sectional design. The sample in this study was 40 post-spinal anesthesia patients using non-probability sampling techniques. Data was obtained from direct observation in the IBS room at the Wijayakusuma Hospital, Purwokerto in April-May 2023. The results showed that 19 people (47.5%) experienced shivering and 21 people (52%) did not experience shivering. .5%), the results of the analysis show that there is a relationship between BMI factors and shivering after spinal anesthesia (p value 0.002 <0.05) where shivering occurs most often in respondents with a thin body mass index (<18.5 kg/m) as many as 9 people (22.5%). Then the results of the analysis also showed that there was no difference between the duration of the operation and shivering after spinal anesthesia (p value 0.768 > 0.05).

Abstract

Kecemasan sebelum operasi adalah reaksi psikologis umum yang dialami pasien menjelang prosedur bedah, yang bisa berdampak pada kondisi mental dan proses pembiusan. Salah satu cara non-farmakologis untuk meredakan kegelisahan ini adalah melalui terapi dzikir, yakni praktik mengingat Sang Pencipta. Penelitian ini bertujuan menguji efektivitas terapi dzikir dalam mengurangi tingkat kecemasan pre-operasi pada pasien yang akan menjalani anestesi umum di RST WijayaKusuma Purwokerto. Pendekatan yang diterapkan adalah pre-eksperimental dengan desain one grup pre-test post-test, menggunakan kuesioner sebagai alat ukur. Intervensi melibatkan pemutaran audio-visual dzikir selama 10 menit. Analisis data dilakukan dengan uji Wilcoxon. Temuan menunjukkan bahwa sebelum intervensi, sekitar 60,6% pasien mengalami kecemasan berat, yang kemudian turun drastis menjadi kecemasan ringan pada 57,6% subjek. Analisis statistik menghasilkan nilai P-Value 0,001 (P<0,05) menandakan adanya pengaruh signifikan dari terapi dzikir dalam menurunkan kecemasan praoperasi pada pasien dengan anestesi umum. Dengan demikian, terapi dzikir terbukti sebagai metode yang ampuh untuk membantu pasien menghadapi operasi dengan lebih tenang

Abstract

Background: Hemodynamic changes are common during spinal anesthesia, particularly following the administration of bupivacaine, which may affect mean arterial pressure (MAP) and heart rate. Monitoring these parameters is essential to maintain hemodynamic stability throughout surgical procedures. Objective: This study aimed to determine the effect of bupivacaine administration on mean arterial pressure (MAP) and heart rate in patients undergoing spinal anesthesia at PKU Muhammadiyah Gombong Hospital. Methods: This quantitative study employed a pre-experimental one-group pretest-posttest design. A total of 181 respondents were recruited using accidental sampling. Mean arterial pressure and heart rate were measured 5 minutes before and 5 minutes after the administration of spinal anesthesia. Data were analyzed using the Wilcoxon Signed-Rank Test. Results: A significant change in mean arterial pressure was observed following bupivacaine administration (p < 0.001). In contrast, no significant change was found in heart rate (p = 0.823). Conclusion: Bupivacaine administration significantly affected mean arterial pressure but did not significantly influence heart rate in patients undergoing spinal anesthesia.
