Abstract

Ventilator associated pneumonia (VAP) adalah yang sering terjadi di rumah sakit terutaman di ruang intensif. VAP merupakah infeksi saluran pernapasan bawah yang mengenai parenkim paru setelah pemakaian ventilasi mekanik lebih dari 48 jam. Kejadian VAP dapat ditekan dengan pelaksanaan bundle VAP. Tujuan penelitian ini untuk mengetahui tingkat kepatuhan dan faktor-faktor yang mempengaruhinya. Metode penelitian adalah deskriptif analitik dengan desain crosssectional dengan sampel sebanyak 45 perawat ICU. Alat ukur yang digunakan adalah VAP Bundle Checklist dari Institute for healthcare improvement (IHI) 2012 dan adaptasi PRECEDE model. Hasil penelitian ini menunjukkan bahwa tingkat kepatuhan perawat terhadap bundle VAP adalah tinggi (75,9%). Analisa dengan Chi square menunjukkan bahwa tidak ada hubungan antara faktor predisposisi, pemungkin dan penguat terhadap kepatuhan (nilai p 0,473). Rekomendasi dari penelitian ini adalah perawat harus meningkatkan pengetahuan dan motivasi terkait dengan implementasi bundle VAP.Kata kunci: Faktor kepatuhan, Kepatuhan perawat, Bundle VAP, Ventilator associated pneumonia

Abstract

Pruritus merupakan rasa gatal yang menimbulkan gangguan dan ketidaknyamanan dimana dapat muncul sesekali ataupun regular. Masalah keperawatan gangguan rasa nyaman dan risiko perusakan integritas kulit dapat ditemukan baik dari respons subjektif pasien maupun temuan klinis objektif. Gatal dan pruritus dapat ditemui pada pasien gagal ginjal kronis dengan dialysis maupun tanpa dialysis. Prevalensi gejala pruritus yang sering ditemui dengan penyakit ginjal kronik yaitu pruritus uremik. Keluhan gatal dari pruritus ini dapat dikurangi dengan pemberian pelembab yang berfungsi untuk meningkatkan dan mempertahankan hidrasi kulit. Tujuan studi kasus ini untuk menganalisis keefektifan intervensi pemberian petroleum jelly untuk mengatasi masalah gangguan rasa nyaman dan risiko kerusakan integritas kulit pada pasien gagal ginjal kronik dengan pruritus. Metode penulisan yaitu analisis studi kasus pada intervensi keperawatan yang dilakukan di pasien selama perawatan di ruang rawat penyakit dalam. Evaluasi terhadap keefektifan tindakan dinilai dari respon pasien terhadap rasa gatal dengan menggunakan Visual Analogue Scale (VAS) serta pemeriksaaan kondisi kulit. Hasil intervensi didapatkan penurunan nilai VAS dari skala 5 ke skala 1 dan kondisi kulit yang lebih baik. Rekomendasi dari studi kasus ini adalah intervensi penggunaan petroleum jelly dapat dilakukan untuk mengatasi masalah keperawatan gangguan rasa nyaman dan risiko kerusakan integritas kulit pada pasien gagal ginjal kronik dengan pruritus.

Abstract

HIV or Human Immunodeficiency Virus is the virus that causes Acquired Immune Deficiency Syndrome (AIDS) HIV-AIDS is an infectious disease that can be transmitted through sexuaӏ intercourse (vaginaӏ, anaӏ or oraӏ). bӏood transfusions, contaminated needӏes. HIV prevention and intervention can be carried out by conducting Voӏuntary Counseӏing and Testing (VCT), nameӏy prevention and earӏy detection efforts to determine the status of a person who has been infected with HIV or not and is at risk, nameӏy through HIV/AIDS counseӏing and testing. The use of VCT so far is stiӏӏ ӏacking due to severaӏ factors, one of which is faciӏities and access. Therefore, the above conditions attracted the writer to find out the effectiveness of using VCT mHeaӏt in preventing HIV-AIDS. This study uses the ӏiterature review method by anaӏyzing 10 journaӏs reӏated to the use of mHeaӏth VCT. Journaӏ searches used the keywords Mobiӏe Heaӏth VCT and Voӏuntary Counseӏing and testing for HIV, in the journaӏ's onӏine database incӏuding PubMed, PubMed Centraӏ (PMC) and Cӏinicaӏ key. The incӏusion criteria for this study were journaӏs in Engӏish with a ӏimitation of the ӏast 5 years (2018-2022). Excӏusion criteria are ӏanguage other than Engӏish and journaӏ year under 2018. Research Resuӏts: Based on the anaӏysis of 10 journaӏs that have been carried out, the resuӏts show that mHeaӏth VCT has severaӏ advantages, nameӏy ӏow cost, easy access, effective, abӏe to detect earӏy and effectiveӏy prevent HIV. As a heaӏth technoӏogy, mHeaӏth is effective if appӏied in heaӏth services, especiaӏӏy in providing information, counseӏing, and testing or VCT to prevent and earӏy detection of HIV.

Abstract

Penyakit Covid-19 mempengaruhi kesehatan orang secara berbeda, dari gejala ringan hingga kritis. Pasien sakit kritis harus dirawat di ruang perawatan intensif. Infeksi virus SARS COV-2 merusak sisem kekebalan tubuh dan menyebabkan komplikasi hingga syok septik yang memerlukan Continuous Renal Replacement Therapy (CRRT).Laporan telaah kasus ini menjelaskan tentang analisis asuhan keperawatan pasien Covid-19 dengan ARDS berat, syok septik, asidosis metabolik berat dengan Hypertension Heart Disease yang dilakukan CRRT.Analisis kasus pada satu pasien ICU Covid-19 yang menjalani CRRT. Ibu SW, 55 tahun, dirawat di ICU Covid karena ARDS berat. Pasien mengalami syok septik (dengan dukungan vasokontriksi, demam 39,1°C, CRP = 98,4, Prokalsitonin = 0,67 ng/mL) serta perfusi ginjal tidak adekuat ditunjukan oleh hemodinamik yang tidak stabil, fungsi ginjal yang menurun, laju filtrasi glomerulus yang rendah (Ur = 265, Cr= 6,21, eGFR= 42,3) dan edema ekstremitas. Diagnosa keperawatan: Perfusi Jaringan Ginjal Tidak Efektif berhubungan dengan malfungsi glomerulus dan Risiko syok. Pasien dilakukan CRRT pada hari ke enam rawat dengan total waktu selama 33 jam. Perawat ICU memantau hemodinamik, keseimbangan cairan, tingkat kesadaran, airway dan pola pernapasan, skala nyeri dan sedasi, ketidakseimbangan elektrolit dan kebutuhan pasien selama CRRT.Covid-19 dikaitkan dengan penyakit pernapasan, dan cedera ginjal akut (AKI) jarang terjadi; namun, ketika pasien mengalami AKI dengan tingkat keparahan tinggi, CRRT harus dipertimbangkan. Perawat ICU memiliki peran dalam pemantauan CRRT. Pasien dilakukan CRRT sebagai dampak dari gangguan ginjal karena syok septik yang disebabkan oleh infeksi virus SARS Cov-2.

Abstract

This study aims to analyze the effectiveness of telemonitoring on the incidence of readmission in patients with heart failure. The method used in this study is a literature review focusing on articles published in the last five years and English, with the target population being patients aged over 18 years. The article search strategy uses online databases, namely "ScienceDirect," "ProQuest," "Sage," and "Scopus," using keywords and Boolean Operators. The results showed that using a telemonitoring system in most articles proved effective in reducing readmissions in heart failure patients. This difference occurs due to differences in the number of samples and the length of time of the study. In conclusion, telemonitoring is effective in reducing readmission in heart failure patients compared to those who receive regular care. Keywords: Heart failure, Rehospitalization, Telemonitoring

Abstract

This study aims to provide an overview and ideas from the results of a literature review regarding the possibility of implementing neurorehabilitation in post-stroke patients with the android-based exergames method. The method used is a literature review of databases: Scopus, ScienceDirect, ProQuest, Sage Journal, Ebscohost, Sage Publication, Google Scholar, and Semantic Scholar from 2018-2022. The research results show the results of a review of 10 selected journals; exergames are a very flexible tool for neurorehabilitation in post-stroke patients suffering from various neurological disorders. In conclusion, the application of exergames as an android-based neurorehabilitation as an effective method can be carried out in post-stroke patients. Abstract

The incidence of heart failure is a health problem that often occurs in society. Globally it reaches more than 23 million patients worldwide and is expected to grow to 46% by 2030. Patients with heart failure characterized by low activity tolerance, low quality of live, increased mortality and re-hospitalization, and increased treatment cost. Heart failure patients experience shortness of breath, fatigue, anxiety. Where the signs and symptoms of heart failure are chronic and progressive. The writing method uses the PRISMA literature review method, via the remote-lib.ui.ac.id database which facilities a literature search engine and is linked to various scientific publications such as Scopus, ProQuest, Clinical Key for Nursing, PubMed with several keywords, (HFrEF OR (Heart Failure)) AND (physical exercise) AND (Physical capacity). From the first search we get 1512 articles, then selected to become 7 articles with cardiovascular restrictions, last 5 years, nursing, English. Activity management in Heart failure is an important intervention to provide, both while in the treatment room and as self-management and self-care. Non-pharmacological and pharmacological nursing management as a way to improve self-care and self-management in heart failure patients is important to provide because it can increase the achievement of treatment goals.

Abstract

AbstrakPemberian asuhan keperawatan pada pasien post-operative mempunyai komponen penting, yaitu manajemen nyeri. Pasien post-operative dapat munculn nyeri yang disebabkan oleh trauma pada tubuh akibat sayatan benda tajam alat-alat operasi. Kompleksitas nyeri memerlukan penanganan agar penatalaksanaan nyeri dapat diatasi secara komprehensif. Salah satu metode yang dapat dilakukan dalam manajemen nyeri dengan menggabungkan kemajuan teknologi, yaitu dengan penggunaan teknologi virtual reality. Adapun penggunaan teknologi ini agar dapat diaplikasikan saat praktik klinis dalam penanganan nyeri post-operative. Tujuan penelitian ini, yaitu mengeksplorasi efek penggunaan virtual reality dalam penurunan nyeri pada pasien post-operative dari berbagai sumber. Metode yang digunakan dalam proses pencarian kajian literatur review ini, yaitu simplified approach. Database yang digunakan pada pencarian literatur berupa ScienceDirect, ClinicalKey, ClinicalKey Nursing, Sage Journals, Scopus, Google Scholar. Kriteria inklusinya adalah yang melibatkan pasien dewasa post-operative, membahas penurunan nyeri berbasis virtual reality, studi menggunakan uji coba percontohan crossover acak, eksperimental terkontrol acak, randomized controlled trial, praeksperimen one group pre test-post test, quasi experimental non equivalent control group pre test-post test, teks berbahasa Inggris, teks berbahasa Indonesia, tahun penerbitan lima tahun terakhir dari tahun 2017 sampai 2022, topik artikel terkait penggunaan virtual reality menurunkan nyeri post-operative, full text, berbahasa Inggris, berbahasa Indonesia, tahun penerbitan lima tahun terakhir dari tahun 2017 sampai 2022. Kriteria eksklusinya adalah topik artikel terkait topik artikel dengan jenis literature review, hanya berupa abstrak, dan artikel yang merupakan duplikasi. Dalam proses pencarian, peneliti menggunakan beberapa kata kunci yaitu “virtual reality”, “post-operative”, “pain”, “post surgery”, “aching”, and “discomfort”. Saat penelurusan ditemukan tujuh artikel dengan hasil bahwa manajemen nyeri pada pasien post-operative dapat dilakukan dengan penggunaan teknologi virtual reality. Perangkat virtual reality dapat membantu dalam proses penanganan untuk menurunkan nyeri pada pasien post-operative. Perangkat tersebut sudah terintegrasi dengan program software yang telah disesuaikan dengan teknologi dalam menurunkan nyeri pada pasien post-operative. Diharapkan perlu kerja sama dengan pihak rumah sakit agar aplikasi virtual reality ini dapat diaplikasikan secara optimal untuk pasien yang mengalami nyeri post-operative.Kata kunci: Nyeri, post-operative, virtual realityReducing Pain with Education Using Virtual Reality Technology in Postoperative Patients : Literatur Review AbstractProviding nursing care to post-operative patients has an important component, namely pain management. Post-operative patients can experience the appearance of pain caused by trauma to the body due to sharp incisions of surgical tools. The complexity of pain requires treatment so that pain management can be handled comprehensively. One method that can be used in pain management by combining technological advances is the use of virtual reality technology. The use of this technology so that it can be applied during clinical practice in the management of post-operative pain. The purpose of this study is to explore the effect of using virtual reality in reducing pain in post-operative patients from various sources. The method used in the search process for this review of the literature is the simplified approach. The databases used in the literature search are ScienceDirect, ClinicalKey, ClinicalKey Nursing, Sage Journals, Scopus, Google Scholar. The inclusion criteria were those involving post-operative adult patients, discussing virtual reality-based pain reduction, studies using randomized crossover pilot trials, randomized controlled trials, randomized controlled trials, pre-experimental one group pre-test-post test, quasi-experimental non-equivalent control group pre test-post test, English text, Indonesian text, year of publication for the last five years from 2017 to 2022, article topics related to the use of virtual reality to reduce post-operative pain, full text, English, Indonesian, year of publication five years the latest from 2017 to 2022. The exclusion criteria are the topic of the article related to the topic of the article with the type of literature review, only in the form of abstracts, and articles that are duplicates. In the search process, researchers used several keywords, namely "virtual reality", "post-operative", "pain", "post surgery", "aching", and "discomfort". During the search, seven articles were found with the result that pain management in post-operative patients can be carried out using virtual reality technology. Virtual reality devices can assist in the treatment process to reduce pain in post-operative patients. The device has been integrated with a software program that has been adapted to technology in reducing pain in post-operative patients. It is hoped that there will be collaboration with the hospital so that this virtual reality application can be applied optimally for patients experiencing post-operative pain.Keywords: Painful, post operation, virtual reality

Abstract

This study aims to investigate the effects of non-pharmacological interventions in diabetes mellitus patients with sleep disturbances. The research method used was a systematic review with a randomized controlled trial (RCT) and quasi-experimental design. Article searches used five databases, Scopus, Proquest, ScienceDirect, Pubmed, and EbscoHost, published from January 2018 to December 2022. The results showed that eight articles met the criteria. Non-pharmacological interventions given to diabetes mellitus patients with sleep disorders include Cognitive Behavioral Therapy (CBT), Acceptance and Commitment Therapy (ACT), aromatherapy, personalized intervention, and nutritional therapy. Most studies report that non-pharmacological interventions can improve sleep quality in diabetes mellitus patients with sleep disorders. CBT therapy is the recommended therapy that is more effective. In conclusion, non-pharmacological interventions can be used as a companion or substitute therapy to improve sleep quality in patients with diabetes mellitus who experience sleep disturbances.
