Abstract

Background: One of the strategies to improve integrated care focused on patients is the supervision of nursing care and documentation. Aims: this study aims to describe the innovation project to improve supervision. Method: This study used the pilot study design to innovation and improvement with the PDSA approach and management function, designed a draft a guidelines, standard operating procedures, forms and fair guideline, socialization, role play, mentoring and evaluation with monitoring and evaluation to four head nurses on scheduled supervision, unscheduled and group directing. Results: the highest increase was 94.76% on group direction process, 70.83% nursing evaluation supervision, supervision more structured, frequency and duration that increased. The impact of nursing care increased 4.94% and documentation increased 7.18%. Conclusions: The improvement of supervision can be done, with pilot studies using tha PDSA approach and management functions to providing a guidance, standard operational procedures, fair guideline and checklist forms, socialization, mentoring and evaluation, supported by direct manager, motivated head nurse and staff. Recommendations: The results of this pilot study are expected to be considered to improve supervision, motivation to implementation, and monitoring and evaluating of supervision. Keywords: nursing care; head nurse; management; supervision ABSTRAK Latar Belakang: Salah satu strategi meningkatkan asuhan terintegrasi berfokus pada pasien adalah supervisi asuhan keperawatan dan pendokumentasiannnya. Tujuan: studi ini bertujuan mendeskripsikan project inovasi meningkatkan supervisi. Metode: Studi ini menggunakan desain pilot study dengan inovasi dan improvement dengan pendekatan PDSA dan fungsi manajemen, dengan merancang panduan, standar operasional prosedur, formulir supervisi dan petunjuk penilaian, disosialisasikan, pendampingan, role play dan dimonitoring dan evaluasi terhadap empat kepala ruang, baik supervisi terjadwal, tidak terjadwal dan pengarahan berkelompok. Hasil: peningkatan tertinggi adalah proses pengarahan berkelompok 94,76%, supervisi evaluasi keperawatan 70,83%, lebih terstruktur dan meningkat frekuensi dan durasinya. Dampaknya asuhan keperawatan meningkat 4,94% dan pendokumentasiannya meningkat 7,18%. Kesimpulan: peningkatan supervisi dapat dilakukan, dengan pilot study menggunakan pendekatan PDSA dan fungsi manajemen untuk menyediakan panduan, standar operational prosedur, petunjuk penilaian dan formulir berbentuk checklist, disosialisasikan, pendampingan dan dievaluasi, adanya dukungan atasan langsung, motivasi kepala ruang dan staf. Rekomendasi: Hasil pilot study ini diharapkan dapat menjadi bahan pertimbangan meningkatkan supervisi, memotivasi penerapannya, dan memonitoring dan evaluasi pelaksanaannya. Abstract

The role of nurses in health care systems is diverse, included being a health care provider to clients, client advocates, educators for clients and families, and nursing service managers. One strategy that can be applied in managerial nursing is delegation. The goal of this study was to assess the applicability of Kurt Lewin's change management method to analyse the management function in nursing delegation between head nurse and team leader. This study was conducted in one of the Jakarta Military Hospital by conducting a mini project consisting of case reports using a fishbone diagram approach, solving problems using Plan-Do-Check-Action (PDCA) tools starting with Plan Of Action (POA), implementation, evaluation, and follow-up. The results of the identification showed that the head of the room in carrying out the delegation was not optimal, included documentation in the delegation book that was not optimal, delays in filling, non uniformity in the format of delegation books in the room, and not understanding due to lack of standardization and control from superiors. Guidelines are used as a reference in delegating nursing to the head of the hospital room, and the documentation of nursing delegates is done correctly because delegates work better when the reporting structure is clear. In conclusion, application fishbone diagram, lewin's change management model, and PDCA cycle can optimize management function in nursing delegation between head nurse and team leader in Jakarta Military Hospital. Recommendations are given to the nursing department, head of the room, team leader, and executive nurses to increase self-awareness, knowledge, and ability in delegating nursing.

Abstract

Nurse Work Stress During The COVID-19 Pandemic. COVID-19 does not only cause problems in physical conditions but also has the potential to cause problems in mental health, including for nurses who are at the forefront of handling COVID-19. This study aims to see a description of the work stress felt by nurses during the COVID-19 pandemic at a COVID-19 referral hospital. The sample consisted of 133 nurses. The results of the analysis show that the average work stress of nurses is 106.19 (32.2%). The highest perceived work stress was inadequate preparation 7.31 (47.8%), while the lowest perceived work stress was a conflict with nurses 10.02 (22.33%). The description of nurses' work stress in this study was lower than in previous studies. This can be because nurses are able to realize the pandemic conditions, work carefully and make better preparations. Management support is very much needed in making the right rules, especially in preventing and controlling the transmission of the Covid-19 virus.

Abstract

Patient safety is a patient who is free from injuries that do not occur or free from damage and/or potential illness, physical, social, psychological, disability, and death related to health care. Nurse compliance is compliance to an action, procedure or regulation that must be done or obeyed. The purpose of this paper was to know the Nurses’ Compliance towards Patient Safety in improving Drug Safety as an Effort to Reduce Drug Errors. The method of this research was a literature review design. The literature search process used several databases including ProQuest, Ebsco, and the Willey Online Library. The search was done on literatures published between 2013 and 2018. Keywords included were keywords relevant to the material, including “nurse compliance - drug safety - drug error - nurse medication error”. The data analysis used a specific conceptual approach which analyzed an outline to achieve objectives using PRISMA. Sixth articles fullfilled the inclusion criteria. This study found various research articles under discussion that the nurses’ compliance about patient safety in improving drug safety as an effort to reduce medication errors is to report patient safety incidents, near injury, potential incidence of injury, non-injured incident and unexpected events. Provision of trainings and updating knowledge about patient safety in terms of reducing medication errors are needed. In addition, compliance to report if there is a patient safety incident also needs to be improved, because the report can be an early response to patient safety incidents in the future which can prevent the same incident to happen again.

Abstract

This study aims to determine the role of nurse educators in optimizing the provision of education in order to increase patient and family knowledge about health so as to improve quality of life. The method used is a pilot study design using a problem solving approach. The innovation program was implemented on 32 nurse educators who were recruited by purposive sampling. The evaluation stage of the FGD implementation and analysis was carried out descriptively. The results showed that the role of nurse educator is needed in an effort to improve the quality of providing education to patients and families. The innovation program carried out is the creation of a nurse educator team guide. The results of the FGD evaluation on nurse educators are the need for uniform documentation of providing individual and group education, nurse educators completing educational points according to competence, the need for competent nurse educators, mentoring and mentoring for junior nurses. In conclusion, the quality of nursing services can be increased by having a team of nurse educators. Keywords: Patient and Family Education, Nursing Services, Nurse Educator

Abstract

This study aims to encourage and improve the quality of nursing care through an innovation program in optimizing supervision in the documentation of nursing care. The method used is an innovation program with a pilot study through a problem solving approach. The innovation program was carried out on the head of the room and team leader who were recruited through purposive sampling using questionnaires, observation sheets and interviews. The results showed that: 1) the problem identification stage, need improvement in the quality of nursing care documentation; 2) the implementation stage, it is necessary to develop a nursing care guide, a checklist form for the quality of documentation, socialization, mentoring and testing of Tele-Supervision; 3) the evaluation stage, the results of the supervision of the documentation of nursing care have been carried out perfectly and completely by the implementing nurse through a trial of the Tele-Supervision form. In conclusion, improving the quality of nursing care documentation needs to be controlled with the implementation of scheduled supervision by the head of the room. Abstract

This study aims to analyze and develop learning methods to improve nurses' competence in continuing education through e-learning in inpatient rooms. The method used is a pilot project starting from data collection, problem analysis, determination of problem priorities, preparation of a plan of action, implementation and evaluation. The results showed that after e-learning learning, it was found that there was an increase in the knowledge of nurses in the surgical inpatient room with an average increase of 29.59% from pretest to posttest. 55% of patients' satisfaction with the quality of nursing care was satisfied, while 45% felt dissatisfied. The nurses' satisfaction who stated that the nurses were pleased with the nursing profession was 96%, while 4% of the total respondents were not satisfied with the 28 nurses. There were 12 nurses (agreed) and 16 nurses (strongly approved) from 28 respondents who stated that continuous training through e-learning could increase knowledge. In conclusion, the continuing education learning method through e-learning can improve nurses' competence in surgical inpatient rooms. Keywords: CPD, E-Learning, Satisfaction, Nurse

Abstract

This study aims to develop guidelines for the role and function of the head of the nursing field. This study method is a case study with a situation analysis. Data obtained through interviews and document study. The stages start from the assessment, problem identification, problem analysis, determination of problem priorities and making a plan of action. Implementation begins with the preparation of a draft guide and self-assessment, hiring expert, socialization and evaluation. This study is an innovation project that already has a permit from the Jakarta Hospital. The results showed that the role and function of the head of the nursing field has the potential to be improved. Innovations are developed by making guidelines and self-assessment referring to the literature covering three main managerial roles and five management functions. The trial of filling out self-assessment tools can be carried out. The conclusion of the roles and functions of the head of the nursing field is very important in supporting nursing governance and improving performance. Abstract

This study aims to determine the shift patterns used by the hospital during the pandemic. The research design uses systematic reviews through online databases such as Science Direct, EBSCOhost, Scopus, ProQuest, Google Scholar, and other search sources. The article selection process uses the PRISMA method. The number of articles obtained was 795, and then only 11 were used in this review. The results of this study indicate that the shift pattern during the pandemic period includes (1) 12 hours per shift; (2) 6 hours per shift; (3) 4-8 hours divided into shifts; (4) pattern 7-7-10; (5) flexible work schedules, short intervals; (6) three stages of rotation and a 14 day quarantine plan. In conclusion, the 12 or 6 hour shift pattern without a break is not suitable for nurses' health, while other shift patterns tend to pay attention to the health and safety of nurses during a pandemic. Abstract

This study aims to analyze tiered supervision in the nursing service area during the COVID-19 pandemic. The research method uses a case study approach with interviews, document review and observation, and performs problem analysis using fishbone analysis. The results showed that the tiered supervision function was less than optimal in the COVID-19 pandemic conditions at Hospital X so that it had an impact on the quality of nursing care. Nursing managers from the top manager to the first manager at RS X only supervise verbally, without a routine schedule, without supervision tools and no tiered supervision standards and have not developed follow-up steps on supervision results in each nursing service area. In conclusion, the optimization of tiered supervision in the pandemic era can be carried out through the innovation of the tiered supervision method with the G-Spreadsheet digital application, which is carried out by all levels of nursing managers at X Hospital.
