Abstract

CV. Citra Buana Mandiri is a used tire recycling or retreading industry, in one month the company is able to produce 800 retreaded tires. The company's production line indicates the occurrence of waste, where one of the concerns is the level of defects produced. The purpose of this research is to identify and minimize waste to improve efficiency in the production line. The method used in this research will be an integration between Lean and Six Sigma. The results of this study show that defects and motion dominate the waste that occurs with a score of 3.60 and 3.0. In addition, the sigma level in the company's production process is at a fairly good level with a value of 3.29. Thus, the suggestions that can be given are related to the evaluation and supervision of SOP implementation and process improvement as well as material and finished product storage.

Abstract

PT. XYZ is a company engaged in the crude palm oil processing sector, one of which is willarine margarine which is exported to various countries. In maintaining global competitiveness, the company strives to produce high-quality products. However, in practice there are still obstacles that need to be overcome. One of them is during the packing process there are three types of product defects identified, namely leakage in the pouch packaging, lack of margarine volume, and damage to the pouch carton. So this research uses the FMEA (Failure Mode and Effect Analysis) method used to determine and transfer the severity, occurrence, and detection levels, so that it is obtained with the highest total RPN, namely leaky pouch packaging of 904, and FTA (Fault Tree Analysis) as a clue with the root of the problem tree, so that it is known that the root causes are lack of rest time, lack of supervision, no concern for the importance of track setting instructions, and no worker concern for cleanliness. After the results are known, improvement proposals are given, namely providing sufficient rest time and always rechecking, providing track setting instructions, and always doing routine cleaning at the beginning and end of work.

Abstract

CV. NKM is a supplier company that takes manufacturing plant waste in the form of sawdust, especially waste from merbau wood, sent to factories whose main raw material is sawdust such as pallet factories, briquettes, etc. In every process there are defects such as powder mixed with other sizes, smelly and moldy powder, and wet powder. In every process there are defects such as powder mixed with other sizes, smelly and moldy powder, and wet powder.  These defects are due to human factors, tools/machines, storage, etc. So this research was conducted using the FMEA (Failure Mode and Effect) method used to determine the severity, occurrence, and detection levels, so that the highest RPN value of 576 was obtained, namely powder mixed with other sizes and FTA produced basic events from the highest RPN value, namely lack of rest hours, lack of supervision, and lack of routine control. After the results are known, an improvement proposal will be given for the highest RPN value, namely providing sufficient hours of rest and enforcing routine control of the tool/machine.

Abstract

Competition in the Temulawak Unowen MSME industry is getting tighter. Many other producers are also trying to enter the market with various new innovations both in terms of taste and packaging. Therefore, it is important for companies to continue to carry out product development in order to remain competitive and maintain their market share. In this study, the Kano and QFD methods were used. The purpose of the Kano Method is to find out what consumers want about cracker production, and QFD is to offer the most important suggestions for improving quality. The results showed that Kano has 12 one-dimensional attributes, 4 Indifferent attributes. On the other hand, the QFD research results show that the validity testing of all three tiers resulted in a calculated R value that is more than R table 0.55, which indicates that the attributes are valid while the reliability test shows that the alpha value of all 3 tiers is 0.663 and the respondents' answers are considered reliable.

Abstract

PT Ravana Jaya is a general contractor that works on steel fabrication and has several partners from small, medium, and large scale companies.  Observations show that errors often occur during the production process, leading to a number of defective products with a defect percentage of 0.12%. The allowable defect percentage ranges between 0.05% and 0.07%, or approximately 1-2 items per month.  Therefore, research should be conducted to look at the human errors that cause defective products during the production process.  This study utilized the Human Error Assessment and Reduction Technique (HEART) and Systematic Error Reduction and Prediction (SHERPA) methods.  According to this study, the highest predicted human error occurred in the welding task with a HEP value of 10.565, and the lowest human error occurred in the welding task with a HEP value of 10.565. To reduce the number of defective products, it is recommended to make improvements, such as conducting strict and regular evaluation and supervision, continuous training, adding dividers between parts, and changes in design.

Abstract

CV. Tayamum K.A.M is a small micro business that manages crackers made from various  sea fish. During the production process, the company often faces problems. This research was conducted to evaluate the performance of the production process using FMEA and FTA methods. The results of the FMEA calculation used to determine the severity level, occurrence level, detection level which later obtained the highest RPN value which is a cracked product that is not up to cuil with a total RPN of 697, FTA analysis shows the root causes of errors such as not re-checking, improper drying time and poor air circulation. After knowing the results of the root cause of the problem, an improvement proposal is given, namely rechecking the machine before use, paying attention to weather conditions when drying, and adding enough ventilation holes or blowers.

Abstract

There are several aspects that affect the company's top priority, namely the smooth production process, one of which is machine performance. At PT XYZ during the period January - June 2024 had high downtime caused by ineffective maintenance management, resulting in disruption of production flow and reducing overall productivity. The purpose of this study is to determine the value of machine effectiveness using the OEE method and determine the factors that cause obstruction of the production process. From the calculation results, the average OEE value was 47.70%, below the world-class standard of 85%. In addition, this study identified six major losses with the highest value being equipment failure losses with an average of 48.38%, FMEA analysis showed the highest RPN value of 441 on the machine factor with the failure mode occurring, namely the presence of foreign objects in the raw material entering the crusher machine can cause significant damage to machine components. To increase the effectiveness of production machines at PT. XYZ, corrective actions are needed in machine maintenance and labor vigilance.

Abstract

PT. Ravana Jaya is a general contractor working on steel fabrication and has several partners from small, medium, and large scale companies. Observations showed that workers reported complaints related to pain in several parts of the body in the upper neck, lower neck, back, and left and right shoulders with a percentage value of 85% in the location of pain complaints in the upper neck area. This research uses the RULA (Rapid Upper Limb Assessment) and REBA (Rapid Entire Body Assessment) methods. The results of the RULA and REBA scores in the punching process are 7 and 9 with a high risk category so that it is necessary to hold a work posture improvement solution by adjusting the height of the workstation and using an adjustable bench, using ergonomic tools such as drilling machines and material support tools to reduce physical loads, job rotation and regular breaks to prevent muscle fatigue. These improvements are not only beneficial for the health and well-being of workers, but also for the sustainability of company operations.
