Abstract

PT. ASM is a company operating in the steel sector. In production, it is of course important to maintain machine productivity to maintain production quality performance. In the research, problems were found in the form of machine damage which resulted in production stopping. In this research, the Statistical Quality Control (SQC) method is applied as a quality management approach to control process and product quality by integrating Overall Equipment Effectiveness (OEE) and SQC. In research, there are two types of data, namely primary data and secondary data which are used as references to facilitate the problem solving process. Primary data was obtained through interviews related to the production process, machine function, and causes of defects in production, while secondary data was obtained from company data recaps related to defects in production. Based on the results of the Pareto diagram, it was found that 55% of the problems were imprecise, while 45% were caused by iron cracking during the production process. Thus, it can be concluded that product defects are mainly caused by cracks in the iron and lack of precision in the machine

Abstract

PT. ABC adalah perusahaan kemasan anyaman plastik berbagai produk lini FIBC (Flexible Intermediate Bulk Container) yang sering mengalami reject dari produk FIBC. Tujuan penelitian mengidentifikasi jenis dan faktor yang mempengruhi adanya cacat terparah pada produk FIBC di PT. ABC. Metode penelitian menggunakan FTA (Fault Tree Analysis) serta FMEA (Failure Mode and Effect Analysis). Analisis FTA dipengaruhi 3 faktor antara lain ialah faktor man karena operator bekerja dibawah tekanan, faktor machine karena bagian Spool serta jarum tidak terkait, dan faktor material karena tying tape rusak hingga serabut anyaman tidak rapi, sehingga ditemukan hasil analisa FMEA defect jahitan yang mempunyai nilai RPN (Risk Priority Number) tertinggi antara lain ialah bagian bekerja dibawah tekanan dengan hasil nilai sebesar 216. PT. ABC memerlukan pelatihan karyawan baru pada bagian QC (Quality Control) dan karyawan bagian maintenance agar dapat mengurangi resiko penyebab defect pada produk FIBC (Flexible Intermediate Bulk Container).

Abstract

PT. KAS atau yang dikenaI dengan PT. Karunia Alam Segar adaIah saIah satu perusahaan F&B yang menghasiIkan produk berupa mie Instan. PT. KAS memiIiki produk akhir yakni mie Instan yang sudah dikemas dan diberi tambahan bumbu sesuai jenis rasa. Dari banyaknya proses yang dilalui, produk akhir perusahaan ini menimbuIkan adanya hasiI produk cacat seperti gencet bumbu, gencet mie, suhu dan atau sambungan, dan bumbu oil  bocor daIam kemasan. Dari permasaIahan yang didapat, penuIis meIakukan peneIitian dengan menggunakan metode Six Sigma dan FMEA dengan tujuan untuk meminimalisir banyaknya variasi cacat produk. Berdasarkan hasil pengoIahan data pada kecacatan kemasan mie Instan goreng memiIiki niIai six sigma sebesar 4,96. Sedangkan nilai RPN tertinggi pada suhu dan atau sambungan karena faktor manusia (man) sebesar 504, dan nilai RPN tertinggi pada bumbu oil bocor daIam kemasan karena faktor manusia (man) sebesar 567.

Abstract

Perumda Giri Tirta merupakan sebuah perusahaan BUMD milik Pemerintah Kabupaten Gresik yang bergerak dalam bidang jasa pengelolaan air minum. Perumda Giri Tirta membutuhkan bahan tawas untuk memproduksi dengan pemesanan sebanyak 15 kali setahun dengan jumlah pemakaian sebanyak 1.588.215 kg dengan biaya yang dikeluarkan perusahaan  sebesar Rp. 22.182.299. Jika dibandingkan dengan 2 metode pengendalian persediaan yaitu Economic Order Quantity dan Just In Time maka akan dihasilkan perhitung lebih efisien. Hasil perhitungan dengan metode EOQ frekuensi pemesanan sebanyak 7 kali dengan biaya sebesar Rp. 17.352.969 atau lebih hemat 88% dari biaya menurut kebijakan Perumda Giri Tirta. Sedangkan dengan metode JIT pemesanan dilakukan sebanyak 3 kali dengan biaya sebesar Rp. 6.573.094 atau lebih hemat 95% dari biaya menurut kebijakan Perumda Giri Tirta.

Abstract

The competition in the global industry has shifted towards supply chain competition. Therefore, handling and understanding risks in the supply chain has become very important. The objective of this research is to analyze the risks present in the supply chain flow of PT. ABC and design preventive actions for the prioritized risks. This research utilizes the SCOR analysis and the House of Risk method. After processing, five priority Risk Agents were identified, namely A18, A9, A6, A4, and A5. As a result, eight preventive actions were proposed, including (PA1) Updating work SOPs, (PA2) Implementing a reward and punishment system, (PA3) Building a pre-sedimentation unit, (PA4) Coordinating with various relevant parties, (PA5) Organizing seminars on the importance of maintaining raw water sources for stakeholders and the community, (PA6) Establishing agreements at the beginning of a partnership contract with specific actions in case of errors, (PA7) Calculating Safety Stock, (PA8) Adding backup supplier connections/lines in case the main supplier experiences a shortage of raw materials.

Abstract

PT XYZ merupakan perusahaan jasa yang bergerak di bidang pengolahan air. Perusahaan ini mengalami banyak masalah dalam proses produksi dan tidak ada rencana mitigasi yang terstruktur. Tujuan dari penelitian ini adalah untuk mengetahui risiko-risiko yang diperkirakan ada atau sudah ada dan memberikan rencana mitigasi. Penelitian ini menggunakan pendekatan SCOR untuk pemetaan dan metode House Of Risk untuk menganalisis dan memberikan mitigasi risiko. Setelah itu dilakukan analisis risiko pada House Of Risk fase 1 yang menghasilkan 35 Risk event & 30 Risk Agent dengan 5 hasil prioritas risiko yang didapat dari estimasi ARP diantaranya A16, A28, A26, A10, A18, sedangkan untuk preventive action yang didapat dari estimasi AHP sebanyak 8 yaitu PA1 dengan nilai 0, PA2 dengan nilai 26, PA3 dengan nilai 0,20, PA2 dengan nilai 0,17, PA4 dengan nilai 0,13, PA5 dengan nilai 0,12, PA6 dengan nilai 0,08, PA7 dengan nilai 0,03, PA8 dengan nilai 0,02.

Abstract

As the industrial world continues to advance, competition between companies is becoming increasingly fierce. To enhance customer satisfaction, companies must be able to provide the best quality. CV. ABC is a manufacturing company focused on producing *songkok* (traditional caps). However, during the production process, the company faces several challenges, particularly with defective products. The purpose of this study is to conduct quality control using the Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA) methods.The results of the FMEA calculations are as follows: the sewing machine encountered needle breakage issues with an RPN (Risk Priority Number) of 448, dull and poorly maintained cutting tools during the stitching process with an RPN of 576, low-quality raw materials with an RPN of 504, and a substandard screen-printing tool with an RPN of 648. Through the FTA method, the causes of failure include human factors, machine malfunctions, and poor-quality raw materials.

Abstract

MSME IBS is a home industry that produces baby sofas, baby bedding, etc. MSME IBS faces material handling problems due to poor layout management factors, as well as the realization of the amount of production that does not reach the predetermined target. The research aims to solve the problem of poor layout to be better using the ARC and ARD methods. The resulting efficiency is 49% with a distance of 1512,5m resulting in a decrease of 51% from 100% of the initial distance with a distance of 3085m after making changes from the initial layout to the proposed layout. For this reason, it requires facility improvements to reduce the distance traveled and the accumulation of goods that make the production flow narrow and far by applying the proposed layout between production rooms in order to reduce material handling.

Abstract

Human labor plays a vital role, especially in Manual Material Handling (MMH) processes. While these activities are crucial for smooth operations, they often lead to musculoskeletal disorders (MSDs). MSDs are long-term consequences of improper workstation postures. This research aims to identify the working postures of screen printing workers and propose ergonomic desk designs as improvements. The REBA and OWAS methods can be applied. REBA focuses on assessing the entire body posture of workers, while OWAS aims to identify work activities that potentially cause MSDs. The calculated REBA score is 7, and the OWAS score is 2, indicating a need for preventive measures. The proposed solution is an ergonomic desk design that adjusts to the worker's height.

Abstract

PT. ABC is a business that operates in the cement production sector. This company often experiences silica sand raw material inventory below the minimum limit of the safety stock applicable in the company for each month, as a result of which the high frequency of orders causes the cost of ordering to swell. Therefore, an analysis of raw material inventory control must be carried out in order to determine the maximum inventory quantity. EOQ and JIT are techniques in controlling inventory that are relatively easy to minimize the total cost of inventory in the form of total storage and ordering costs. The results obtained with a need for 297,985 tons, the cost charged by the company in one year is Rp. 100,671,298. With the EOQ method, the cost is Rp. 38,236,200 or 62% more efficient. JIT method costs Rp. 15,609,863, or 85% more efficient than the company's policy.
