Abstract

The causes of brand switching are dissatisfaction, the desire of variety and the influence of promotion. This type of research was associative study to analize link between customer dissatisfaction, need to find variations and the influence of promotion on brand Hand&Body Lotion, that causing the brand Switching Hand&Body Lotion. Location of research conducted at the Faculty of Pharmacy UGM, with sample Strata 1 (S1) student, Pharmacist and Master (S2) as many as 200 people. Determination techniques in non-probability sample used was purposive sampling or sampling based on the criteria, the student of Faculty of Pharmacy who using Hand & Body Lotion for 6 months. Data retrieval method used was a questionnaire. The analytical method used was multiple regression method. The result obtained was Y = -1,551 + 0,182X1 + 0,189X2 + 0,215X3 where Y was the displacement of the brand, X1 was the dissatisfaction of consumers, X2 was the need to find variation and X3 the influence of variations in the promotion. Equation can be interpreted that consumer dissatisfaction , needs to find the variety of products and promotional products significantly influenced the decision on Brand Switching Hand & Body Lotion (p <0.05). Promotion variables had the most impact on brand switching by consumers decision between the other independent variables under study that was equal to 0.215. The desire for a variation variable had a constanta 0,189 and 0,82 for advertisment variable.Keyword : Brand switching, dissatisfaction, variation, promotion

Abstract

Currently, Indonesia’s health development began to emphasize on improving the quality of health care, where service quality should be oriented to customer satisfaction. Departement of Pharmacy at X Hospital Jakarta is one of the medical support required todeliver a quality service that oriented to patient satisfaction. This research was descriptive study using primary data that obtained from the questionnaire that distributed to 400 patients at outpatient pharmacy, which was the patient who secured by insurance (A and B) and cash (F). The category of patients at outpatient pharmacy are patient that secured by corporate, corporate pensioner in Jabodetabek, patient that secured by other insurance and cash patient. The sampling technique used is nonprobability sampling, which was the quota sampling. The level of satisfaction of patients seen by SERVQUAL method and the dimension of service quality was mapped in a Cartesian Diagram.In general, patients were not satisfied with the performance of Outpatient Pharmacy at X Hospital Jakarta. The results showed that the gap between patients’ expected service and perceived service of outpatient pharmacy at X Hospitals was negative. The sequence of the biggest negative gap felt in responsiveness (-0.7100), empathy (-0.6550), reliability (-0.5800), assurance (-0.4975) and tangible (-0.4200). The sequence of the greatest dissatisfaction felt by corporate pensioner in Jabodetabek (30.59%), cash patients (29.08%), patient that secured by other insurance (22.22%) and patient that secured by corporate (18.11%). The sequence of the biggest dissatisfaction perceived by the patient for services in outpatient pharmacy F (37.49%), outpatient pharmacy A (32.47%) and outpatient pharmacy B (30.04%). Based on the mapping of Cartesian Diagram, Outpatient Pharmacy at X Hospital Jakarta should pay more attention to the dimensions of responsiveness, especially regarding speed of service and providing more crew at peak hours.Keywords: Quality of Service, Satisfaction, SERVQUAL, Cartesian Diagram

Abstract

There are some factors that influence human resources management, they are the employee’s attribution, working motivation, and organizational commitment. The purpose of this research is to determine the influence of the employee’s attribution toward human resources management policies to employee’s motivation and organizational commitment in PKU Muhammadiyah Yogyakarta Hospital. This research used questionnaire to 51 employees as subjects and had been working at least 1 year in PKU Muhammadiyah Yogyakarta Hospital. The questionnaires that used in this research consists of questionnaire for employee’s attribution toward human resources management policies, working motivation questionnaire, and organizational commitment questionnaire. To test validity and realibility used 25 respondents, then analyzed with regression analysis to determine the influence of the employee’s attribution toward human resources management policies to working motivation and organizational commitment used SPSS 17 for windows program for analyzing data. The results showed that employee’s attribution toward human resources management policies has positive influence 94,9 % to motivation employees with regression equation Y = 6,425 + 0,967X, employee’s attribution toward human resources management policies has positive influence to organizational commitment 92,6% with regression equation Y=10,133 + 0,735X, and working motivation correlates positively with organizational commitment with r 0,980.Keywords : Employee’s attributions, working motivation, organizational commitment

Abstract

The overall good service system is by patient satisfaction-oriented system, besides the hospital facility aspects, the role of doctors, paramedical and non medical is very important as they will determine the perception of performance and perceived performance of the services provided patients. Asking a good service needs motivation supported by awareness on the importance of excellent service. The study was done in order to see the level of employee knowledge about the excellent service, looking at the relationship of employee knowledge level for the excellent service with the awareness level for excellent level, and to see how the influence of awareness of excellent service to the employee working motivation in Supiori Local Public Hospital. The subjects for this research consists of 37 permanent employee of Supiori Local Public Hospital, then analized using visual binning of SPSS 17 for windows program to see the employee knowledge level on the excellent service, the method of correlation analysis to see the correlation of employees knowledge level for the excellent service, and regression analysis method to see how the influence of awareness for excellent service by the employee to the Supiori Local Public Hospital using SPSS 17 for windows. The results showed that the respondents’ knowledge level classified as good one are 59.5% and 40.5% were considered not good. Knowledge of excellent service with awareness level for excellent service correlate with r-count of 0.692. The awareness effect of the excellent service to the working motivation by the employees in Supiori Local Public Hospital is 12.7% with the regression equation Y = 12.943 + 0.324 X.Keywords : Excellent service, work motivation

Abstract

Increase of public interest in using herbal medicine causes increase in the number of herbal medicine producers and the competition will be harder. Marketing concept states that companies have to satisfy their consumers in order to gain profits. The satisfied consumers will be loyal to certain products or companies, and they will buy the products again in the future. The more important things is that they will carry out word of mouth activity about the products to people around them. The aims of this research were to know the influence of consumers’ satisfaction on their loyalty, the influence of their satisfaction on their word of mouth behavior, and the influence of their loyalty on their word of mouth behavior. The subjects of this research were the consumers of An Nuur’s herbal medicine in Yogyakarta. This research was conducted by survey with questionnaire as an instrument. The samples were taken with non probability sampling design with purposive sampling procedure. The sample consisted of 65 respondents and was taken from November 2011 to May 2012. The criteria of the respondents were they who had used An Nuur’s herbal medicine at least three times, bought it from stores in Yogyakarta, aged at least 17, and agreed to be subjects of this research. Then, the data were analyzed using linear regression analysis to know the relation between variables. The hypothesis was tested using t test. The result of this research showed that consumers’ satisfaction had significant positive influence on their loyalty. The highest consumers perception was the easiness in consuming An Nuur’s herbal medicine, while the lowest consumers perception was the easiness in purchasing An Nuur’s herbal medicine. Consumers’ satisfaction had significant positive influence on their word of mouth behavior. Consumers’ loyalty had significant positive influence on their word of mouth behavior. Consumers’ loyalty had greater influence than their satisfaction on their word of mouth behavior.Keywords: Satisfaction, loyalty, word of mouth, An Nuur.

Abstract

Diabetic foot is common foot problems in persons with diabetes mellitus. It is suggested that the prevalence of diabetic food cases increase along with the growing number of patients with diabetes mellitus. Diabetic foot commonly appears with manifestation of ulcers, infection, gangrene and Charcoat atrophy. Infection occurs in fifty percent of diabetic foot with ulcers that could lead to amputation if it is not immediately and appropriately treated. Proper antibiotics treatment prevents diabetic foot patient from amputation. This study aimed to understand the pattern of antibiotics usage amongst diabetic foot patients. The appropriateness of antibiotics therapy against current guideline was studied and compared with culture and sensitivity test results. Effectiveness of therapy and patient’s safety was then evaluated. Prospective cohort approach was used to this study. Data of diabetic foot patients was collected during November 2011 – January 2012 at Endocrinology Clinic of Sardjito Hospital. Collected variables included subject characteristics, antibiotics and other medication treatments, visual sign of diabetic foot infection and laboratory test results. Descriptive analysis was then conducted toward all variables. Majority of subjects obtained single therapy of antibiotic when diagnosed with diabetic foot infection. These antibiotics were ciprofloxacin and cefadroxil. Combination of 2 antibiotics were about 31,1%, ciprofloxacin with clindamycin was the most combination in this therapy (37%). About 28.7% of antibiotics therapy was not appropriate with existing guideline. Culture and sensitivity tests strengthen this inappropriateness. It was found that 84.4% of antibiotics therapy was effective. However, there were 5.7% of antibiotic orders that potentially interact negatively with other medication during therapy.Keywords: diabetic food infection, antibiotic, appropriateness, effectiveness, drug  safety

Abstract

An application named ‘Dosing GAMA’ has developed for drug doses adjustment in patients with renal and hepar impairment. Dosing GAMA is targeted for clinical Pharmacists to calculate and make dose recommendations for patients, based on renal and hepatic conditions. This study aims to identify the appropriateness of drug dosage adjustment by using Dosing GAMA application in hospitalized patients with renal impairment and to determine the risk factors for the drug dose inappropriateness. This study was a retrospective observational descriptive study, cross-sectional design, used a consecutive sampling technique. The source of the data was Medical Record of hospitalized patients with renal impairment (creatinine clearance ≤50 mL/min) from 2018 of February till 2020 of March in the Academic Hospital of UGM. The names and the doses of the drugs were filled to Dosing GAMA application, and it would evaluate the appropriateness of drug doses. There were 570 drugs of 73 medical records included in this study. This study revealed Dosing GAMA could assess 144 drugs (25,6%) need to adjust, and 82 drugs (56,9%) were inappropriate doses. There were significant correlations of the age characteristic (p=0,000) and the creatinine clearance value (p=0,012) to the drugs dose appropriateness. There were inappropriate doses need to adjust in the hospital. So, the use of health-based technology expected for pharmacists to improve the use of drugs rationally.

Abstract

Dalam rangka memenuhi kebutuhan obat dan untuk menjamin aksesibilitas obat di masyarakat, pemerintah mengeluarkan kebijakan penulisan obat generik di fasilitas kesehatan pemerintah dan harga obat. Penelitian ini merupakan penelitian diskriptif. Data kuantitatif dari observasi formularium dan resep bulan Juli sampai September 2010. Data kualitatif dari hasil wawancara. persentase obat generik yang ada di formularium adalah 33,81 %, persentase kesesuaian obat generik di formularium terhadap kepmenkes adalah 75,47 %, persentase item obat generik yang sering diresepkan berdasarkan formularium 74,02%, persentase item obat generik yang terlayani terhadap yang diresepkan 97,57 %, persentase item obat generik di formularium yang ditulis obat bermerk 14,26%, persentase resep obat generik 80,67 %, persentase resep obat generik yang diganti branded 0,30 %, persentase resep obat generik yang tidak terlayani 0,32%, persentase resep obat bermerek yang diganti generik adalah 8,81 %. Kendala dalam pelaksanaan kebijakan pemerintah tentang penggunaan obat generik di rumah sakit umum tipe B se-eks Karesidenan Surakarta adalah ketidakaktifan Panitia Farmasi dan Terapi dalam sosialisasi dan merevisi formularium agar lebih sesuai dengan kebijakan pemerintah tentang penggunaan obat generik.

Abstract

Extended-Spectrum Beta-Lactamase (ESBL) merupakan enzim hasil mutasi β-lactamase yang mampu menghidrolisis penicillin, cephalosporins dan aztreonam. Patogen penghasil ESBL berhubungan dengan perburukan luaran klinik, salah satunya pada infeksi saluran kemih (ISK). Penelitian ini bertujuan untuk mengevaluasi penggunaan antibiotik pada pasien ISK oleh bakteri penghasil ESBL serta mengetahui hubungan kesesuaian penggunaan antibiotik terhadap luaran klinik pasien di RSUP Dr. Sardjito Yogyakarta. Data dikumpulkan secara retrospektif melalui penelusuran rekam medik periode 1 Januari 2019-31 Desember 2019. Kriteria inklusi subjek usia ≥18 tahun didiagnosis ISK oleh bakteri penghasil ESBL. Subjek dengan terapi antibiotik selama <72 jam dieksklusi. Rasionalitas antibiotik dievaluasi mengikuti algoritma Gyssens. Hubungan kesesuaian antibiotik dengan luaran klinik dianalisis dengan uji Chi-Square. Terdapat 69 subjek memenuhi kriteria inklusi dan eksklusi. Pemberian antibiotik rasional ditemukan pada 41 subjek (59,42%) dengan luaran klinik membaik ditemukan pada 33 subjek (80,50%). Sementara sisanya, yaitu 28 pasien (40,58%) menerima antibiotik tidak rasional dengan luaran klinik membaik ditemukan pada 20 subjek (71,40%). Jenis ketidakrasionalan yang ditemukan menurut algoritma Gyssens yaitu kategori IVA (5%), kategori IVB (1,67%), kategori IIB (5%), kategori IIA (11,67%) dan kategori IIB (12,50%). Penelitian ini tidak dapat menentukan hubungan kesesuaian terapi antibiotik pada pasien dengan ISK oleh bakteri penghasil ESBL dengan luaran klinik (p = 0,381).

Abstract

Amikasin merupakan antimikroba golongan aminoglikosida yang efektif terhadap kuman gram negatif, namun memiliki efek samping pada ginjal (nefrotoksik). Pengaturan dosis yang tepat diperlukan untuk meminimalisir efek samping tersebut namun memberikan efek terapi yang maksimum. Penelitian bertujuan mengidentifikasi profil rasionalitas dosis amikasin dan mengetahui hubungan rasionalitas dosis amikasin dengan kejadian nefrotoksik pada pasien di rumah sakit di Yogyakarta. Penelitian ini menggunakan rancangan cohort retrospective dengan penelusuran data rekam medis pasien yang menjalani rawat inap tahun 2017-2018 dari dua rumah sakit di Yogyakarta (RS Bethesda Yogyakarta dan RSUP Dr. Sardjito Yogyakarta) sejumlah 80 subjek. Kriteria subyek penelitian meliputi rekam medis pasien yang menggunakan antibiotik amikasin minimal selama 3 hari, terdapat informasi serum kreatinin minimal 2 kali yaitu sebelum dan selama atau sesudah mendapatkan terapi amikasin. Penentuan rasionalitas dosis amikasin berdasarkan rumus perhitungan Giusty Huyton dan referensi DIH (Drug Information Handbook). Data dianalisis secara statistik menggunakan uji Chi-square atau uji Fisher.  Hasil penelitian mendapatkan 43 (53,75%) pasien mendapatkan antibiotik amikasin dengan dosis rasional dan efek nefrotoksik ditemukan pada 6 pasien. Rasionalitas pendosisan amikasin tidak memiliki hubungan yang bermakna dengan efek nefrotoksik (p=1,000). Berdasarkan hasil penelitian, perlunya peningkatan keterlibatan farmasis dalam penentuan dosis obat secara rasional masih diperlukan untuk meminimalisir kejadian efek samping obat yang tidak diharapkan mellaui pemantauan kadar serum kreatinin pasien selama penggunaan antibiotik aminoglikosida.
