Abstract

Anastomotic leakage occurs in ±10% of patients after surgery for esophageal stricture. Management often requires re-surgery, which increases morbidity and mortality. This case report shows successful conservative management of anastomotic leakage with semi-elemental diet.

Abstract

The prevalence of malnutrition is considerably high among patients with pancreatic cancer and that Whipple procedure is associated with high morbidity and mortality rates. Post-Whipple procedure nutritional therapy can help to preserve nutritional status, accelerate recovery of functional capacity, and improve patient’s quality of life. A 65-year-old malnourished woman underwent Whipple procedure for adenocarcinoma of ampulla of Vater. Oral pre-operative nutrition was given in the form of normal food and oral nutritional supplement (ONS). Eight hours before surgery, the patient received ONS containing 30 g carbohydrate. The patient was given early enteral nutrition at <48 hours post-operatively and was given oral nutrition as soon as she regained consciousness. The patient experienced post-operative pancreatic fistula (POPF) grade A. Post-operative in-hospital and homecare nutritional therapy included supply of energy, macronutrients, micronutrients, and nutritional education adjusted to the patient’s clinical condition and intake tolerance. Energy intake during hospitalization attained 109% of the estimated total energy requirement that consisted of up to 1.9 g/kg/day protein. Homecare nutritional intake reached almost 2 times basal energy requirement with 2.2 g/kg/day protein intake. The patient experienced improvement of clinical symptoms, POPF, intake tolerance, glycemic control, and functional capacity. The patient was allowed to be discharged and treated in the outpatient clinic, without readmission in the next 2 months. Adequate hospital and homecare medical nutritional therapy in post-Whipple procedure may improve nutritional status and functional capacity, improve clinical outcome, and prevent readmission of patients.

Abstract

Type 2 Diabetes Mellitus is a chronic non-communicable disease that is projected to continue increasing globally, thereby placing a burden on healthcare systems in both developed and developing countries. High levels of homocysteine have been associated with insulin resistance and may contribute to the development of Type 2 Diabetes Mellitus. The administration of folate, either alone or in combination with other B vitamins, has been proven effective in reducing homocysteine concentrations. Aim: to determine the effectiveness of folate supplementation as an adjuvant therapy for patients with type 2 Diabetes Mellitus in correcting glycemic control, as measured by glucose homeostasis markers. Methods: Literature search conducted through three main databases, namely PubMed, Cochrane, and Scopus, applying the inclusion and exclusion criteria set by the author, followed by a critical review. Result: There are four articles consisting of two systematic reviews/meta-analyses, one meta-analysis, and one randomized controlled trial (RCT) that meet the eligibility criteria. The validity of the study is assessed as quite good. The results from the four studies indicate that three meta-analyses did not show a significant effect on the improvement of HbA1c, while one RCT study demonstrated a significant effect on the improvement of HbA1c. Conclusion: Current findings do not provide convincing evidence to support folic acid supplementation as a therapeutic approach to reduce HbA1c in type 2 diabetes mellitus. The effect of folic acid supplementation on HbA1c in type 2 diabetes mellitus needs to be further investigated through ongoing randomized controlled clinical trials with a minimum duration of 12 weeks.

Abstract

Psoriasis Pustulosa Generalisata (PPG) merupakan jenis psoriasis yang jarang terjadi. PPG merupakan penyakit autoinflamasi yang terjadi di kulit dan sistemik. Manifestasi kulit ditandai dengan inflamasi di epidermis, pustul multipel dan hiperkeratosis, sedangkan salah satu manifestasi sistemik berupa sepsis. Pemberian nutrisi berperan dalam pencegahan malnutrisi, mendukung proses penyembuhan dan mengatasi inflamasi. Laki-laki berusia 32 tahun masuk rumah sakit dengan keluhan utama bercak kemerahan dengan bintik-bintik bernanah yang semakin meluas sejak 3 hari SMRS. Keluhan lain berupa demam dan bengkak di tungkai bawah. Status gizi pasien adalah berat badan normal. Terdapat anemia, leukositosis, hipoalbuminemia dan peningkatan kadar C-Reactive Protein (CRP). Pemeriksaan histopatologis tidak dilakukan. Pasien didiagnosis dengan PPG (total body surface area 75%)  dan sepsis et causa Skin and Soft Tissue Infection (SSTI). Pasien mendapatkan terapi antibiotik sistemik per oral, kortikosteroid topikal, dan kompres NaCl 0,9%. Selama perawatan, pasien mendapat asupan nutrisi secara adekuat melalui oral berupa makanan padat dan makanan cair tinggi protein.  Pasien mendapatkan transfusi albumin 20% 100 mL sebanyak 1 kali. Mikronutrien diberikan berupa vitamin A, vitamin B kompleks, vitamin C, vitamin D, asam folat dan zinc per oral. Pasien dirawat di rumah sakit selama 16 hari. Pada akhir pemantauan, status gizi normal dapat dipertahankan, terdapat penurunan luas lesi kulit menjadi sebesar 30%, kadar albumin meningkat dari 2,3 menjadi 2,8 g/dL, dan penurunan kadar CRP. Pemberian makro- dan mikronutrien yang adekuat pada PPG dapat mempertahankan status gizi yang baik, menunjang proses penyembuhan lesi kulit, dan perbaikan kadar inflamasi.
