The Prosthodontics Care For Geriatric Patients Nowadays Indriani Oktaria; Robert Shen

Abstract

The global geriatric (elderly) population is growing as a result of increasing life expectancies. One impact of this development is an increase of lifestyle difference. In the scope of health, especially dental health, the patient’s education is important. The decrease in edentulism cases showsed public awareness of the importance in maintaining the health, function and quality of life in the elderly as well as the necessity of preventive attitudes and interventions against disease. Dental services for geriatric patients are affected by the cost of dental services, dental insurance, dental clinics’ access, debt and needs. The current prosthodontic treatment that improve of missing tooth replacements is in demand is an increasing interest in dental implant treatment. Losing tooth has a negative impact on mastication, aesthetic- and oral health-related quality of life. Excessive bone resorption can occur for many reasons, including age, infection, trauma, metabolic disorders and nutritional deficiencies. The resorption of the alveolar bone, especially in medically compromised patients, is caused by focal infections in the mouth and systemic diseases. Periodontitis or other chronic infections resulted from bacterial and host inflammatory cells. The pathology observation can result in a loss of bone support after a long period of extraction procedures is named residual ridge resorption (RRR). The best treatment for RRR is to avoid tooth extraction so that the loss of teeth and their supportive tissues can be prevented. When there is a loss of teeth and RRR, dentures have been made to reduce the occlusal load are necessary. In addition, the importance of periodic control, utilizing vitamins, oral rinse, topical gels or others, and the intake of nutrients greatly affect the good function of dentures and improve the health of the geriatric patient.

Abstract

Introduction: Denture Stomatits (DS) is pathological changes in the oral mucosa that commonly occurs in denture users, which is characterized by an inflammatory reaction amd erythema in the area of the oral mucosa that is in contact with the denture. Review: There are 3 types of Denture Stomatitis, in order: tipe I Hyperemic Pinpoint lessions (Localized Simple Inflammation), type 2 Generalized Simple Inflammation , type 3 Inflammatory Papillary Hyperplasia . In general, DS occurs on the palatal mucosa. Risk factors such as trauma from occlusion from excessive dentures, poor OH and degenerative conditions ( systemic) are not good and had habbits such as smoking increase the occurrence of DS. The appearance of reddish lessions that occur in the palatal mucosa without symptoms of severe pain is one of the clinical features of DS condition. The diagnosis of DS can be made by clinical examination and microscopic tissue culture examination. Conclusion: Repairing dentures and making changes to the condition of oral hygene is one of the keys to success in accelerating the healing of DS. In addition, if there is a fairly severe candidiasis, topical and systematic antifungal drugs can be given to recover the condition.

Abstract

Denture stomatitis (DS) is a pathological change in the oral mucosa that commonly occurs in denture users, which is characterized by an inflammatory reaction and erythema in the oral mucosa that is in contact with the denture. Cli-nical manifestations often include red lesions due to excessive occlusal trauma, frequently occurring in the palatal mucosa region. Loose and unstable prosthesis use can also be a predisposing factor for DS. This paper discusses cases of DS in patients with complete dentures (CD) and partial complex dentures. Chronic irritation due to heavy traumatic occlusion results in reduced stability of dentures. The fabrication of CD with proper vertical dimension set-ting and selection of smaller teeth elements can reduce progressive alveolar bone resorption. The use of topical gel medication like Oxyfresh can be an alternative in the clinical management of DS cases. Control of systemic conditi-ons, especially in the presence of comorbidities, is crucial to minimize the risk of DS occurrence and expedite the healing process if it does occur. Maintaining oral hygiene and denture cleanliness are also key factors for the suc-cess of optimal denture care.

Abstract

Karies atau gigi berlubang merupakan permasalahan rongga mulut yang paling utama di dunia, dengan anak-anak dan remaja sebagai populasi yang paling berisiko terkena karies. Bukti klinis menunjukkan bahwa karies dapat dicegah, salah satunya adalah dengan mempromosikan cara menjaga kebersihan gigi dan mulut. Kegiatan pengabdian masyarakat ini bertujuan untuk mencegah karies sejak dini, dengan memberikan penyuluhan kesehatan gigi dan mulut kepada masyarakat sekolah TK Cikal, Penjaringan. Upaya untuk menilai tingkat pemahaman dan perilaku menyikat gigi anak TK dilakukan dengan metode partisipatif. Para peserta yang berjumlah 106 orang dibagi menjadi beberapa kelompok kecil dan dirotasi ke pos-pos penyuluhan dengan topik mengenai penyebab gigi berlubang, dampak tidak menjaga kesehatan gigi dan mulut, makan makanan yang tidak sehat dan merusak gigi, serta demonstrasi cara menyikat gigi dengan baik dan benar. Peserta dipanggil secara acak untuk menilai tingkat pengetahuan mengenai materi penyuluhan dan keterampilan menyikat gigi. Hasilnya menunjukkan bahwa sebagian besar peserta (96,3%) memahami materi penyuluhan, tetapi hanya sebagian peserta (46,7%) yang dapat mendemonstrasikan seluruh metode penyikatan gigi dengan sempurna. Kesimpulannya adalah peserta memahami materi penyuluhan, tetapi keterampilan menyikat gigi masih belum dikuasai seluruhnya, sehingga perlu diadakan kegiatan penyuluhan secara berkala disertai dengan pemeriksaan indeks kebersihan rongga mulut untuk menilai efektivitas penyuluhan yang sudah dilaksanakan.

Abstract

Background: Dental caries remains a widespread public health issue in pediatric populations that can severely impair oral function, school performance, and overall growth. School-based dental and anthropometric screenings offer a critical pathway for early detection, oral health promotion, and longitudinal monitoring. This study aimed to evaluate dental caries prevalence, tooth-level disease severity, and nutritional status among playgroup (PG), kindergarten (KG), and elementary school (ES) students at Bina Talenta Graha School, Bekasi, Indonesia, and to compare oral health outcomes across a two-year outreach intervention (2023 vs. 2025). Methods: A two-phase repeated cross-sectional study was conducted in December 2023 and December 2025 during community service programs. In the 2025 screening cohort ( ; , , ), student-level caries experience (presence of decayed [D], missing [M], and filled [F] teeth) and tooth-level counts were assessed by calibrated dental professionals. Anthropometric measurements ( ) were evaluated using Weight-for-Age (WFA for PG/KG) and BMI-for-Age (BAZ for ES) Z-scores based on WHO standards. Comparative longitudinal trends in student-level disease burden between 2023 and 2025 were analyzed descriptively. Results: In 2025, overall student-level caries prevalence was 59.1% ( ), missing teeth prevalence was 11.9% ( ), and filled teeth prevalence was 14.0% ( ). Caries prevalence peaked in kindergarten (67.3%), followed by playgroup (62.5%) and elementary school (55.4%). Missing teeth were most frequent in kindergarten (25.5%), whereas playgroup children had no missing or filled teeth. At the tooth level, 402 decayed teeth, 46 missing teeth, and 44 filled teeth were recorded (total 492 affected teeth; mean 2.55 affected teeth/child). Caries occurred across all nutritional categories, with the highest burden observed in normal-weight (70.8%) and overweight/obese (26.7%) children. Between 2023 and 2025, the number of students with untreated active caries decreased from 171 to 114 (a 33.3% reduction), and students with missing teeth dropped from 69 to 23 (a 66.7% reduction). Conclusion: Dental caries remains a major burden among young school children, particularly in kindergarten. Integrated school-based oral health promotion, structured toothbrushing programs, and parental involvement demonstrated notable reductions in active caries and premature tooth loss over two years.
