Advances in information technology have driven digitalization in the healthcare sector, including in medical record management. Medical record keeping at the University of 'Aisyiyah Yogyakarta's independent physiotherapy practice is still done manually, which risks errors, data loss, and administrative delays. This study aims to develop a digital medical record information system to improve the effectiveness and accuracy of patient data management. The system development method used is Rapid Application Development (RAD), as it enables a fast and participatory process. The system was built using PHP with the CodeIgniter 4 framework and a MySQL database. Testing was conducted using the Black Box Testing method to evaluate the system's functionality. The results of this study indicate that the medical record information system for the independent physiotherapy practice was successfully developed using the Rapid Application Development method. Evaluation results show that the system developed using the RAD method functions well based on black box testing, with no bugs found that affect performance. Therefore, the system is considered ready for launch.
