Abstract

A B S T R A C TIntroduction Corona Virus Disease 19 (COVID-19) infection was first reported inWuhan, China, in December 2019. The author wants to know how the OncologySurgery Division elective surgery patient services before and during early pandemicera of COVID-19 at Dr. M Djamil Padang Hospital. Method: This research is acomparative study with a quantitative approach to elective surgery patients in theOncology Surgery Division before and during early pandemic era of COVID-19 at Dr.M Djamil Padang Hospital. The sampling technique in this study was total sampling.Data analysis will be carried out using statistical analysis in accordance with theexisting data scale and using SPSS software. Results: In this study, 137 sampleswere obtained consisting of 80 samples before the COVID-19 pandemic and 57samples during early pandemic era of COVID-19. Of the 57 patients who underwentelective surgery from the Surgical Oncology Division during early pandemic era ofCOVID-19, all patients were not suspicious / confirmed. The time for oncologyservice assessment was 14.26 days before COVID-19 pandemic and 8.93 days duringearly pandemic era of COVID-19. Conclusion: More patients who underwent electivesurgery in the surgical oncology division were female, both before and during earlypandemic era of COVID-19. The mean age of patients before COVID-19 pandemicwas 48.72 years and 49.53 years during early pandemic era of COVID-19. There isa disparity between the length of time the assessment of elective surgery patients inthe surgical oncology division before and during early pandemic era of COVID-19pandemic.

Abstract

AbstrakFraktur penis merupakan kegawatdaruratan urologi yang jarang dan sebagian besarterjadi pada trauma tumpul pada penis yang mengalami ereksi. Fraktur penis sering terjadipada saat hubungan seksual dan jarang terjadi pada trauma tumpul karena benturan. Traumamenyebabkan robekan dan rembesan darah pada tunika albuginea. Intervensi bedah dapatmemberikan hasil penyembuhan yang baik secara fungsional dan kosmetik. Dilaporkankasus pada laki-laki usia 22 tahun yang datang dengan riwayat nyeri dan bengkak pada penissetelah benturan pada stang motor saat kecelakaan lalu lintas satu jam sebelum masuk rumahsakit. Pemeriksaan fisik didapatkan bengkak, hematoma pada batang penis, deformitasberupa angulasi ke kiri disertai nyeri tekan. Pasien masih bisa buang air kecil. Hasil urinalisisdidapatkan eritrosit 60-70/LPB, sedangkan darah rutin dalam batas normal. Kami memutuskanmelakukan eksplorasi, tampak rupture pada corpus cavernosum dan diperbaiki dengan jahitanprimer pada corpus cavernosum yang robek. Pasien dirawat selama 3 hari dan pulang denganfoley-catheter terpasang. Kasus pasien ini adalah salah satu contoh kasus dari fraktur peniskarena trauma tumpul penis secara langsung yang bukan disebabkan karena hubunganseksual.AbstractPenile fracture is a rare urological emergency that occurs almost exclusively due to blunttrauma on erect penis. Penile fracture often occurs during sexual intercourse and is rare in blunttrauma from impact. Trauma caused rupture and hematoma of tunica albuginea. Early surgicalintervention gives good cosmetic and functional outcome. We report the case of 22 -year-oldmale who presented to the emergency department 1 hour following blunt injury of his penisafter traffic accident. In physical examination we found swelling and haematom of penile shaft,angulation to the left and tenderness. Mixturation was normal. There were 60-70 eritrocytes inroutinely urinary examination. There were no abnormality in routinely blood examination. Weperformed an exploration and found rupture at corpus cavernosum. The rupture was repairedwith primary suture. The patient was hospitalized for 3 days and discharged with folley catheterinserted. This patient was an example of penile fracture caused by direct blunt trauma outsidesexual intercourse which is a rare case.

Abstract

Background: Benign prostate hyperplasia (BPH) is the most common pathological condition that contributes to lower urinary tract symptoms (LUTS) in men. Alpha1-blockers are recommended as first-line therapy for LUTS due to BPH (LUTS-BPH). Phosphodiesterase 5 (PDE5-I) inhibitors, which have been used as first-line therapy for erectile dysfunction, have also been found to be effective in treating LUTS-BPH. However, the appropriate combination therapy strategy is still unclear until now. Methods: This study used an experimental design on 17 BPH patients on medical therapy. The patient was given Tamsulosin 0.4 mg monotherapy for 7 days and then continued combination therapy with Tamsulosin 0.4 mg and Tadalafil 5 mg for the next 7 days. Total IPSS score, obstruction, irritation, and quality of life (QoL) were assessed before treatment, after monotherapy, and after combination therapy. Results: The mean age of the research subjects was 62.12 years. Improvements in the total IPSS value, obstruction, irritation, and QoL showed significant changes in both types of therapy compared to the initial value. Combination therapy gave better results than monotherapy, with a mean difference in the decrease in the total IPSS value of 4.41 (p<0.001), obstruction of 3.18 (p<0.001), irritation of 1.41 (p<0.001), and QoL 0.6 (p<0.001). Conclusion: The combination therapy of Tamsulosin 0.4 mg with Tadalafil 5 mg once a day showed clinically and significantly better results than monotherapy in total IPSS, obstruction, irritation, and QoL values in LUTS-BPH patients.

Abstract

Background. Kidney stones are one of the three most common cases in urology. The best standard management for kidney stones with stone burden >20mm is PCNL (percutaneous nephrolithotomy), a minimal invasive method that uses percutaneous access to remove stones. One of the most complications was bleeding which can be caused by several factors , one of them is stone burden.Objective. to look at the relationship of stone burden with hemoglobin drop after PCNL in patients with renal stoneMethods. This study is an analytical study with retrospective approach towards all patients diagnosed with kidney stones and treated with PCNL at RSUP Dr. M. Djamil Padang at based on patient medical record data. Data analysis used Chi square tests where the relationship between variables is said to be significant if p value <0.05Results. Data showed that from 65 kidney stone patients treated with PCNL, the highest prevalence were men and the most age group are 45-64 years. The most types of stones are staghorn and the highest stone burden is 20 - ≤30 mm. Some cases with the involvement of hydronephrosis with an average length of stay <5 days. Hemoglobin drop mean in kidney stones ≤2gr / dL. Statistical analysis showed that there is no relationship between stone burden and hemoglobin drop after PCNL in kidney stone patients (p value = 0,706).Conclusion. The conclusion of this studied is that there is no significant relationship between stone burden with hemoglobin drop after PCNL in patients with kidney stone.

Abstract

Latar Belakang: Adenokarsinoma primer tipe enteric merupakan keganasan yang jarang ditemukan pada kandung kemih. Kasus ini sulit dibedakan secara morfologi dengan adenokarsinoma sekunder yang berasal dari kolorektal, mengenai kandung kemih melalui metastasis atau invasi langsung. Laporan Kasus: Seorang pasien usia 70 tahun dengan keluhan hematuria dan disuria. Tidak ada gejala dari saluran cerna dan riwayat batu pada saluran kemih. Massa tumor pada kandung kemih ditemukan melalui pemeriksaan ultrasonografi, ditambah pemindaian urologi menunjukkan penebalan dinding kandung kemih pada area latero-superior kanan. Pemeriksaan mikroskopik tampak proliferasi kelenjar yang menyerupai kelenjar kolorektal, dilapisi sel epitel kolumnar berlapis dengan inti atipik. Sel tumor menunjukkan ekspresi negatif CK7, positif untuk CK20, CDX-2 dan β-catenin dari pulasan imunohistokimia. Diagnosis akhir adalah adenokarsinoma primer kandung kemih, tipe enteric. Pasien meninggal 10 bulan kemudian. Kesimpulan: Suatu tantangan dalam mendiagnosis adenokarsinoma primer tipe enteric pada kandung kemih.

Abstract

Aims: Kidney transplantation can have a big impact on recipients, so they must be able to adapt to many changes, including psychological changes. Psychological changes are the most complained about by post- kidney transplant recipients. Psychological changes including feelings of anxiety and worry after kidney transplantation, these feelings arise due to post-transplant complications, feelings of guilt and fear of rejection, treatment regimens and uncertainty about the future. Therefore, recipients must be able to deal with psychological changes after kidney transplantation. Objective: To explore the psychological changes of post-kidney transplant recipients. Methods: A qualitative study using a phenomenological approach. This study was conducted on 6 participants who were undergone kidney transplantation. Purosive sampling was performed and data was collected through 35 to 60 min. Data collection in this study was carried out through in-depth interviews, audio recording and field notes.  The data analysis technique uses the Collaizi method (1978). Result: This study obtained results including: psychological adaptation transitions and post-transplant anxiety control strategies. Conclusion: Kidney transplant provide better psychological changes if the recipients can implement strategies to control emotions. Therefore, it is important to have family, social and nurses support in controlling the psychological changes of post-kidney transplant recipients.

Abstract

Background: Invagination is an acute condition in the pediatric abdomen that results in obstruction and constriction of blood vessels (strangulation) in the proximal intestinal mesenterium trapped in the distal intestine, allowing necrosis of surrounding tissue. Intestinal necrosis and perforation in children are causes of abdominal emergencies that require immediate surgery. In supporting examinations, neutrophil, lymphocyte, and biomarker combination ratios have been used for early detection of systemic inflammatory markers. The threshold value of several biomarkers should be investigated to assess the occurrence of strangulation in the pre-operative period in invaginated patients. This study aimed to determine the relationship between neutrophil-lymphocyte ratio and intestinal viability in pediatric patients with invagination. Methods: This study used retrospective analytic data collection from medical records of invagination patients at Dr. M. Djamil General Hospital Padang for the period January 1, 2020, to December 31, 2022. Results: The majority of subjects were male (53.8%) at the age of ≤ 1 year, with as many as 87.2% having a diagnosis of operative invagination (76.9%) and 56.4% having intestinal viability. The mean neutrophil, lymphocyte, and NLR values were 62.15±17.68, 19.03±8.97, and 3.74±1.55, respectively. The pathological NLR status was found to be 59%. The p-value for the association of diagnosis with intestinal viability was 0.002. The p-value for the relationship between NLR and intestinal viability was 0.001, and the r-calculated was -0.528. Conclusion: There is a significant relationship between NLR value and disease diagnosis on intestinal viability.
