Abstract

Background: Case management is an approach used to help patients locate and manage health resources as well as to enhance effective communication among patients, families, and health systems. Nurses’ role as case managers has been proven effective in reducing healthcare costs among patients with chronic diseases. However, little is known about its implementation in improving access to care in community-based settings. Objectives: This scoping review aimed to examine the components of nursing case management in improving access to care within community settings and to identify the issues of community-based nursing case management for future implications. Design: This study was conducted following the framework of scoping review.Data Sources: The authors systematically searched five electronic databases (CINAHL, PubMed, Science Direct, Scopus, and Google Scholar) for relevant studies published from January 2010 to February 2021. Only original studies involving nurses as one of the professions performing case management roles in the community-based settings, providing 'access to care' as the findings, were included. Review Methods: The article screening was guided by a PRISMA flowchart. Extraction was performed on Google Sheet, and synthesis was conducted from the extraction result.Results: A total of 19 studies were included. Five components of nursing case management to improve access to care were identified: 1) Bridging health systems into the community, 2) Providing the process of care, 3) Delivering individually-tailored health promotion and prevention, 4) Providing assistance in decision making, and 5) Providing holistic support. In addition, three issues of nursing case management were also identified: 1) Regulation ambiguity, 2) High caseloads, and 3) Lack of continuing case management training.Conclusion: Care coordination and care planning were the most frequent components of nursing case management associated with access to care. These findings are substantial to improve nurses' ability in performing the nursing process as well as to intensify nurses’ advocacy competence for future implications.

Abstract

Background: Depression is of major causes for disease and disability in adolescents. One of many factors that make teenager more vulnerable to depression is bullying. Identification of depression symptoms among adolescent is important to be investigated, especially due to bullying.Objective: To review the symptoms of depression in adolescents caused by bullying.Method: This is a scoping review research. Databases used were PubMed, EBSCOhost, Cochrane, and PsycNET. Inclusion criteria were: English and Bahasa Indonesia literature, published from 2013 to 2018, quantitative and/or qualitative research adolescent who suffered from depression due to bullying as the research subject, and available free full text. The article screening was based on PRISMA guidance; articles were assessed with the Crowe Critical Appraisal Tool (CCAT), extraction was done on Google form, and synthesis was performed on the extraction resultResults: 19 articles were obtained from the literature screening. Signs and symptoms of depression due to bullying were divided into three groups: psychic symptoms, physical symptoms, and social symptoms. A common symptom of depression in adolescents suffered from bullying was the desire for death, and the desire for death became the most general signs and symptoms of the adolescents who became bullying victims.Conclusions: Being a victim of bullying may cause adolescents to suffer from symptoms of depression in the form of psychic symptoms, physical symptoms, and social symptoms.ABSTRAKLatar belakang: Depresi merupakan pemicu penyakit hingga kecacatan pada usia remaja. Salah satu faktor yang menjadikan usia remaja rentan mengalami depresi adalah bullying. Identifikasi gejala depresi pada remaja penting untuk dipelajari, terutama akibat bullying.Tujuan: Melakukan review gejala-gejala depresi pada remaja korban bullying.Metode: Penelitian ini merupakan scoping review, artikel diperoleh dari proses pencarian pada database PubMed, EBSCOhost, Cochrane, dan PsycNET. Kriteria inklusi artikel yaitu literatur berbahasa Inggris dan bahasa Indonesia, terbit dari tahun 2013-2018, penelitian kuantitatif dan/atau kualitatif dengan populasi penelitian berupa remaja korban bullying, serta artikel penuhnya dapat diakses. Skrining bertahap dilakukan dan didokumentasikan sesuai bagan alir PRISMA. Artikel hasil skrining dinilai dengan Crowe Critical Appraisal Tool (CCAT), ekstraksi dan sintesis artikel dilakukan pada Google Form.Hasil: Dalam proses screening, diperoleh 19 artikel. Gejala depresi pada remaja korban bullying dapat dikelompokkan menjadi tiga yaitu gejala psikis, gejala fisik, dan gejala sosial. Gejala khas depresi pada remaja korban bullying adalah gejala psikis berupa ide bunuh diri. Ide bunuh diri ini menjadi tanda dan gejala depresi yang paling banyak ditemukan pada remaja korban bullying.Kesimpulan: Kejadian bullying selama masa remaja berpengaruh secara potensial untuk menyebabkan gejala depresi baik gejala psikis, gejala fisik, maupun gejala sosial.

Abstract

Maternal death rate is one of the important health development indicators. Indonesian maternal mortality is still high due to both direct and indirect causes that occur during pregnancy and childbirth. High-risk pregnancy can present complications for both the mother and fetus, and demands early detection. Early detection requires involvement of the community, health cadre, medical officers, and government. There is a need to increase the community health cadre competency in the detection of pregnancy complications, especially high-risk pregnancy knowledge. This study used a quasi-experimental design without control group to determine the effect of training regarding pregnancy complications on community health worker’s knowledge in Wijimulyo, Nanggulan, Kulon Progo, Yogyakarta in October 2018. The research subjects were community health cadre workers chosen by purposive random sampling. There were 43 research subjects who were given pretests, training about complications of pregnancy, and posttests. The instrument used was a knowledge questionnaire about complications during pregnancy that consisted of 23 items with reliability of 0.865. There was significant difference between community health cadre’s knowledge about complications of pregnancy at pretest and posttest. These results indicate that training about pregnancy complications increases the knowledge of community health cadre workers concerning complications of pregnancy. Further research is needed to assess community health cadre’s skills in making early detection of pregnancy complications.

Abstract

Background: Mobile application Cavixaution is a health education media to improve knowledge about cervical cancer prevention. End User Computing Satisfaction (EUCS) which consists of content, format, accuracy, ease of use, and timeliness as its dimensions, becomes a measurement to determine the success of the implementation of an information system.Objective: To evaluate the user satisfaction in using mobile application Cancer Cervix Caution (Cavixaution) as health education media for knowledge about cancer cervix prevention.Method: The study was held in November – December 2019 in the Jagalan Village Bantul. The design of this study was descriptive and cross sectional approach. The data were collected using propotional and purposive sampling that involved 76 women aged 30 – 49 years who were using the mobile application Cavixaution and never done Visual Inspection of Acetic Acid (IVA).Result: Overall users were satisfied with the mobile application Cavixaution with an average rate of 3,16. The average rate for each EUCS dimensions were as follow: ease of use dimension 3,24 (satisfied), content dimension 3,14 (satisfied), format dimension 3,15 (satisfied), timeliness dimension 3,19 (satisfied) and accuracy dimension 3,10 (satisfied).Conclusion: Users are satisfied using mobile application Cavixaution. All dimensions have reached satisfaction level, with ease of use dimension had the highest value, while the accuracy dimension had the lowest satisfaction. This study shows that the mobile application Cavixaution can be used as health education media for knowledge about cervical cancer prevention and the application has fulfilled respondent expectation.ABSTRAKLatar belakang: Mobile application Cavixaution merupakan media pendidikan kesehatan dengan tujuan untuk meningkatkan pengetahuan mengenai pencegahan kanker serviks. Kepuasan pengguna menjadi suatu pengukuran untuk mengetahui keberhasilan dalam penerapan suatu sistem informasi. End User Computing Satisfaction (EUCS) yang terdiri dari lima dimensi, yaitu substansi, tampilan, akurasi, kemudahan dalam penggunaan, dan ketepatan waktu, menjadi salah satu metode untuk mengukur kepuasan pengguna terhadap produk yang berhubungan dengan teknologi informasi dan komunikasi (TIK).Tujuan: Mengetahui gambaran kepuasan pengguna dalam menggunakan aplikasi mobile application Cancer Cervix Caution (Cavixaution) sebagai media pendidikan untuk pengetahuan tentang pencegahan kanker serviks.Metode: Penelitian dilaksanakan pada bulan November – Desember 2019 di Kelurahan Jagalan, Bantul. Jenis penelitian kuantitatif deskriptif dengan rancangan cross sectional. Pemilihan sampel dengan teknik proportional dan purposive sampling dengan melibatkan subjek 76 wanita usia subur, berusia 30 – 49 tahun, yang telah menggunakan mobile application Cavixaution, serta belum pernah melakukan tes Inspeksi Visual Asetat (IVA).Hasil: Secara keseluruhan pengguna merasa puas terhadap penggunaan mobile application Cavixaution dengan nilai rata-rata 3,16. Nilai kepuasan untuk setiap dimensi adalah dimensi ease of use 3,24 (puas), dimensi content 3,14 (puas), dimensi format 3,15 (puas), dimensi timeliness 3,19 (puas), dan dimensi accuracy 3,10 (puas).Simpulan: Pengguna merasa puas dalam menggunakan mobile application Cavixaution. Pada dimensi kemudahan dalam penggunaan, memiliki nilai rata-rata kepuasan tertinggi, sedangkan dimensi akurasi mendapatkan nilai kepuasan paling rendah. Penelitian ini menunjukkan bahwa Mobile Application Cavixaution dapat digunakan sebagai media pendidikan kesehatan tentang kanker serviks, dengan rancangan aplikasi yang telah memenuhi harapan atau keinginan para pengguna.

Abstract

Background: Breastmilk is very beneficial for immunity, growth, development, and reducing infant and toddler mortality. One of the pivotal factors that affects breastfeeding success is the support of community health workers, particularly Posyandu cadres. Health education using audiovisual media is expected to improve cadres’ knowledge in giving breastfeeding counseling among mothers with infant.Objective: To identify the effect of health education using audiovisual media on Posyandu cadres’ knowledge of breastfeeding management in Wijimulyo, Nanggulan subdistrict, Kulon Progo.Method: This was a pre-experimental study with one group pretest-posttest design. A total of 40 Posyandu cadres, who were chosen through purposive sampling, participated in this study. Audiovisual media was used as intervention tool in health education, and the cadres’ knowledge was measured by questionnaire. Data was analyzed by using Wilcoxon signed rank test.Results: There was improvement of pretest and posttest mean score from 15,18 to 16,60. The result of statistical analysis showed significant difference in breastfeeding management knowledge among Posyandu cadres before and after health education using audiovisual media (p = 0,000).Conclusion: Health education using audiovisual media is effective to improve cadres’ knowledge of breastfeeding management.ABSTRAKLatar belakang: Air susu ibu (ASI) sangat bermanfaat untuk sistem pertahanan tubuh, pertumbuhan, dan perkembangan bayi, serta berkontribusi menurunkan angka kematian bayi dan balita. Salah satu faktor yang berperan dalam kesuksesan pemberian ASI di antaranya dukungan dari kader Posyandu. Pendidikan kesehatan dengan media audiovisual diharapkan dapat meningkatkan pengetahuan kader Posyandu sehingga mendukung peningkatan keterampilan edukasi kader pada ibu menyusui.Tujuan: Untuk mengetahui pengaruh pendidikan kesehatan dengan media audiovisual terhadap pengetahuan manajemen pemberian ASI kader Posyandu di Desa Wijimulyo, Kecamatan Nanggulan, Kulon Progo.Metode: Penelitian ini merupakan penelitian pra-eksperimen dengan rancangan one group pretest-posttest. Subjek penelitian sebanyak 40 orang kader Posyandu yang dipilih secara purposive sampling. Instrumen penelitian mencakup alat intervensi, yaitu media audiovisual untuk pendidikan kesehatan dan kuesioner untuk mengukur tingkat pengetahuan kader. Data yang diperoleh kemudian dianalisis menggunakan Wilcoxon signed rank test.Hasil: Terdapat peningkatan mean nilai pretest dan posttest dari 15,18 menjadi 16,60. Analisis statistik menunjukkan perbedaan yang bermakna secara statistik pada pengetahuan responden sebelum dan sesudah mendapatkan pendidikan kesehatan melalui media audiovisual (p = 0,000).Kesimpulan: Pendidikan kesehatan dengan media audiovisual efektif untuk meningkatkan pengetahuan kader terkait manajemen pemberian ASI.

Abstract

Background: Langerhans Cell Histiocytosis (LCH) is a condition when the body produces many immature Langerhans cells which attack the host’s organs such as the lungs. This can result in the deterioration of patient's condition. LCH among pediatric patients are rare. But among those rare cases, lung involvement cases are widely reported. Unfortunately, nursing care on pneumonia children with LHC study case is scarcely applied.Objective: To identify the implementation of nursing care in pediatric LCH patient with pneumonia in the pediatric oncology ward of RSUP Dr. Sardjito Yogyakarta.Case report: A 1-year-5-month-old boy with LCH in the 13th week of the chemotherapy protocol came into the Pediatric Hemato-Oncology Clinic with rapid breathing and underwent oxygen desaturation up to 75% with nasal flare. The patient was treated in the pediatric oncology ward based on the main medical diagnosis of pneumonia with the differential diagnosis (dd) was lung involvement of LCH. Patient was provided nursing care based on main complaints with ineffective breathing pattern and hyperthermia as the main nursing diagnoses.Outcome: The interventions included non-pharmacological techniques such as adjusting the semi-fowler position to 30o-45°, administering chest physiotherapy, regulating environment, and monitoring fluid balance. The non-pharmacological techniques were collaborating with pharmacological therapies, such as antibiotic, antipyretic, and oxygen therapy. On the sixth day of nursing care implementation, hyperthermia was fully resolved and ineffective breathing pattern was partially resolved.Conclusion: Nurses play important roles in dealing with hyperthermia and ineffective breathing pattern in patients with pneumonia and lung involvement of LCH.INTISARILatar belakang: Langerhans cell histiocytosis (LCH) merupakan kondisi ketika tubuh seseorang memproduksi banyak sel Langerhans yang belum matang dan dapat menyerang organ tubuh seperti paru-paru sehingga dapat memperparah kondisi pasien. Kejadian LCH pada anak merupakan kasus langka. Namun, dari kasus LCH pada anak yang berhasil diidentifikasi, keterlibatan organ paru termasuk yang paling sering dilaporkan. Studi kasus terkait asuhan keperawatan pneumonia pada anak dengan LCH belum banyak dilakukan.Tujuan: Mengidentifikasi penerapan asuhan keperawatan pneumonia pada anak dengan LCH di bangsal onkologi anak RSUP Dr. Sardjito Yogyakarta.Laporan kasus: Pasien anak laki-laki berusia 1 tahun 5 bulan dengan LCH dalam protokol kemoterapi minggu ke-13 datang ke Poli Hemato-Onkologi dengan napas cepat, sesak, dan desaturasi oksigen hingga 75% disertai nasal flare. Pasien mendapatkan perawatan di bangsal onkologi anak berdasarkan diagnosis medis utama pneumonia ditandai dengan lung involvement of LCH. Pasien diberikan asuhan keperawatan sesuai keluhan utama dengan diagnosis keperawatan utama, yaitu ketidakefektifan pola napas dan hipertermia.Hasil: Intervensi yang diberikan mencakup teknik nonfarmakologi seperti pengaturan posisi semi fowler 30°-45°, fisioterapi dada, pengaturan lingkungan, dan pemantauan keseimbangan cairan. Tindakan non-farmakologi dikolaborasikan bersama terapi farmakologi, seperti terapi antibiotik, antipiretik, dan oksigen. Pada evaluasi keperawatan di hari ke-6, hipertermia teratasi dan ketidakefektifan pola napas teratasi sebagian.Simpulan: Perawat berperan penting dalam menangani masalah keperawatan hipertermia dan ketidakefektifan pola napas pada kondisi pasien dengan pneumonia ditandai dengan lung involvement of LCH.

Abstract

Background: The first 1,000 days are crucial for a child's development. Indonesia has limited research on responsive caregiving and early learning opportunities, especially for children with nutritional challenges. While studies have addressed general child nutrition, few have explored comprehensive nurturing care interventions. This gap underscores the need for further research. Educational videos, particularly animated ones, effectively capture attention and improve understanding, making them valuable for enhancing maternal knowledge and child growth.Purpose: This study aimed to assess the impact of nurturing care educational videos on mothers' knowledge and child growth in children aged 6-23 months with nutritional challenges.Methods: A quasi-experimental design with non-equivalent and untreated control groups was employed. Seventy-three mothers and their children aged 6-23 months, facing nutritional challenges, were recruited through consecutive sampling. The intervention group received nurturing care educational videos. Statistical analyses included dependent t-tests, independent t-tests, and repeated measures ANOVA.Results: The intervention group showed significant improvements in nurturing care knowledge (p = 0.012), responsive caregiving (p = 0.018), and safety and security (p = 0.006). A significant difference in safety and security was observed between groups at posttest 1 (p = 0.020). Both groups exhibited significant increases in children's body weight and height: the intervention group (body weight, p = 0.005; body height, p < 0.001) and the control group (body weight, p < 0.001; body height, p < 0.001).Conclusion: Nurturing care educational videos had a significant positive impact on enhancing mothers' knowledge and promoting child growth. Notable improvements were observed in nurturing care knowledge, responsive caregiving practices, and safety and security measures among caregivers exposed to the intervention.
