Abstract

Background: Pneumonia causes high mortality, hospitalization costs and health services. There is some growing debate regarding the efficacy of different treatment management approaches. The use of certain antibiotic regimens combination and monotherapy have been associated with improved outcomes.Objective: This study aims to determine the effectiveness of monotherapy and combination therapy on length of stay.Method: This retrospective study included patients with Community Acquired Pneumonia (CAP) who received empirical therapy between January - December 2017 at Jakarta Islamic Hospital of Cempaka Putih. Two hundred and ninety four subjects were included in this study.Results: Subjects who received monotherapy and combination therapy were 73.8% and 26.2%. Mean length of stay was 5 days. The most widely used antibiotic in this study was ceftriaxone with levofloxacin (35%) for combination therapy and levofloxacin (38,2%) for monotherapy. There is a relationship between comorbidities and long-term hospitalization (p=0.008).Conclusion: There was no significant difference between combination therapy and monotherapy with length of stay (p=0.277).Keywords: Pneumonia, monotherapy, combination therapy, Jakarta Islamic Hospital of Cempaka PutihIntisariLatar belakang: Pneumonia menyebabkan tingginya angka kematian, biaya rawat inap dan pelayanan kesehatan. Terdapat beberapa perdebatan yang berkembang mengenai efikasi pendekatan manajemen pengobatan yang berbeda. Penggunaan kombinasi rejimen antibiotik tertentu dan monoterapi telah dikaitkan dengan hasil yang lebih baik.Tujuan: untuk menentukan efektivitas monoterapi dan terapi kombinasi terhadap lama rawatinap.Metode: Penelitian retrospektif ini melibatkan pasien dengan community acquired pneumonia yang menerima terapi empiris antara Januari - Desember 2017 di Rumah Sakit Islam Jakarta (RSIJ) Cempaka Putih.Hasil: Dua ratus sembilan puluh empat subjek dilibatkan dalam penelitian ini. Subyek yang menerima monoterapi dan terapi kombinasi sebesar 73,8% dan 26,2%. Rerata lama rawat inap pada kelompok monoterapi dan dualterapi yaitu 5 hari. Antibiotik yang paling banyak digunakan dalam penelitian ini yaitu seftriakson dengan levofloksasin (35%) untuk terapi kombinasi dan levofloksasin (38,2%) untuk monoterapi. Terdapat hubungan antara komorbid dengan lama rawat inap (p=0,008).Kesimpulan: Tidak terdapat perbedaan bermakna antara terapi kombinasi dan monoterapi dengan lama rawat inap (p=0,277)Kata kunci : Pneumonia, monoterapi, terapi kombinasi, RSIJ Cempaka Putih

Abstract

Covid-19 adalah infeksi saluran pernapasan akut yang disebabkan oleh virus sars-cov-2. Pertama kali ditemukan di wuhan, china pada desember 2019. Peningkatan jumlah kasus Covid-19 terjadi secara cepat dalam waktu yang singkat. Dibutuhkan penanganan yang cepat karena virus ini dapat menginfeksi siapapun tanpa pandang usia. Who mengumumkan Covid-19 sebagai pandemi global pada 12 maret 2020. Upaya pengendalian penularan Covid-19 bukan hanya menjadi tanggung jawab pemerintah, namun memerlukan peran setiap elemen masyarakat, salah satunya dengan menerapkan phbs (perilaku hidup bersih dan sehat). Tujuan penelitian ini ialah untuk mengetahui hubungan pengetahuan dengan perilaku hidup bersih dan sehat sebagai pencegahan Covid-19 pada mahasiswa kedokteran angkatan 2019 di universitas muhammadiyah jakarta tahun 2020. Penelitian ini adalah penelitian deskriptif analitik dengan desain cross sectional. Data diambil dengan kuesioner yang dikumpulkan secara online pada mahasiswa program studi kedokteran universitas Muhammadiyah Jakarta angkatan 2019. Mahasiswa FKK UMJ angkatan 2019 memiliki pengetahuan dan perilaku yang baik mengenai COVID-19 dan PHBS sebagai upaya pencegahan COVID-19. Tidak terdapat hubungan antara pengetahuan dengan PHBS dalam pencegahan COVID-19 pada penelitian ini.

Abstract

Sejak diterapkannya Jaminan Kesehatan Nasional, Fasilitas Kesehatan Tingkat Primer (FKTP) dituntut untuk bekerjasama dengan BPJS. Selain itu, FKTP Amal Usaha Kesehatan ‘Aisyiyah (AUKESA) perlu menunjukkan kinerjanya kepada PP ‘Aisyiyah. Sistem Informasi Manajemen (SIM) merupakan salah satu prasarana utama dalam penyelenggaraan pelayanan untuk menunjukkan kinerja disertai bukti yang otentik, valid, dan real time. Instalasi SIM Klinik Doctor Tool dipilih karena memberikan fitur bridging dengan kebutuhan PCare-BPJS sekaligus dapat menyajikan laporan berkala untuk pemantauan performa oleh PP ‘Aisyiyah. Kegiatan ini dilakukan selama 1 tahun oleh inisiasi FKK UMJ dengan kegiatan rapat koordinasi, instalasi SIM Klinik Doctor Tool, dan pelatihan SDM. Modul Doctor Tool yang diinstalasi ini adalah pendaftaran pasien, layanan antrian, pemeriksaan awal, rekam medis dokter, depo farmasi, dan billing. Modul Rekam Medis Dokter sudah mengadopsi Problem Oriented Medical Record mencakup assesmen sesuai ICD10, ICPC2, serta mengakomodir diagnosis holistik, tindakan medis, peresepan, pemeriksaan lab, surat sehat/sakit, rujukan/rujuk balik, dan perencanaan lainnya. Fitur yang dapat dimanfaatkan oleh PP ’Aisyiyah adalah Dashboard yang dirancang untuk melihat performa dan utilisasi klinik. Laporan yang diterbitkan meliputi rekapitulasi peserta terdaftar, statistik kunjungan, demografi, obat, laba-rugi, dan laporan sesuai ketentuan BPJS. Manfaat yang diperoleh bagi FKTP adalah meningkatknya produktifitas, terbukanya kanal pasien baru, efisiensi waktu kerja, integrasi dengan PCare-BPJS, dan kesesuaian dengan standar akreditasi klinik. Sedangkan, manfaat yang diterima oleh PP ‘Aisyiyah adalah pemantauan performa AUKESA dan deteksi dini apabila diperlukan pendampingan teknis lebih lanjut. AUKESA yang baik dapat dimanfaatkan secara mutual bersama institusi pendidikan untuk menjadi wahana pendidikan pelayanan kesehatan tingkat primer.

Abstract

Dengue virus is a virus transmitted by mosquitoes namely Aedes aegypti. We educate people about dengue virus and how to handle it so that people, especially endemic areas, can better understand the pattern of dengue virus spread which can cause dengue hemorrhagic fever (DHF) as one of the most deadly diseases. If it continues it can lead to shock due to bleeding. DKI Jakarta area, especially East Jakarta, is one of the endemic areas for DHF. As many as 265 cases of dengue hemorrhagic fever (DHF) have occurred in East Jakarta since the beginning of 2020. The community needs to know more about how to prevent DHF through knowledge of lifestyles where the dengue virus reproduces and how it is transmitted. The local community was very enthusiastic about listening to the counseling, as seen from the satisfaction questionnaire, more than 50% answered that they were satisfied with this counseling.

Abstract

The emergence of various outbreaks in the midst of the ongoing Covid-19 pandemic is quite worrying. Among them are mysterious hepatitis, Hand Mouth and Foot Disease (HMFD), and monkeypox, also known as monkeypox. This is the main reason for the importance of the public to know valid information about infectious diseases and how to prevent them. One of the efforts to increase public knowledge is to conduct counseling and assistance on a regular basis, with the hope of increasing public awareness so as to reduce transmission rates. Counseling was carried out with material on introduction to outbreaks of acute hepatitis, monkeypox and HMFD accompanied by Clean and Healthy Living Behavior for prevention and taught how to wash hands in the right way, and how to wear the mask correctly. 76.4% of respondents were satisfied with this counseling activity. There was an increase in the mean value of the participants knowledge before and after counseling, with the mean + SD pre and post test values respectively 84.4 ± 19.7 and 85 ± 21. There was no statistically difference knowledge before and after counseling with a p-value of 0.728.

Abstract

Background: The prevalence of pneumonia in developing countries continues to increase, especially in Indonesia. One of the factors that influence the severity of pneumonia is nutritional status. Malnutrition increases susceptibility to infection. Purposes: This study aimed to determine the relationship between nutritional status and pneumonia severity in children under five. Methods: This research was an analytic study with a cross-sectional design. The data were taken from the medical records of pneumonia patients under five years of age who were hospitalized at the Infectious Disease Hospital Prof. Dr. Sulianti Saroso for January 2018 to October 2019. Bivariate analysis was performed using the chi-square test. Results: Out of 68 patients who met the inclusion criteria, the highest age range was 0-11 months (50%), the most gender was female (55.9%), the group with the highest cases was the severe pneumonia group (73.5%), the most nutritional status was abnormal nutrition (55.9%). There are 86.8% of patients with abnormal nutritional status suffer from severe pneumonia. Conclusion: There is a relationship between nutritional status and pneumonia severity.

Abstract

Background: COVID-19 (Coronavirus disease 2019) is an acute respiratory infection caused by SARS-COV-2 (Severe Acute Respiratory Syndrome Coronavirus 2). The spread of COVID-19 took place quite quickly and occurred in various countries around the world. Prevention mechanisms that need to be implemented include vaccination. The purpose of this study was to find out the different proportion of COVID-19 incidence in groups of respondents with different vaccination status. Method: This research is an analytic study with a cross sectional design. Data was taken with a questionnaire that was collected by google form. Result: Out of 112 respondents who met the inclusion criteria, the most gender was female (73.2%), the most age group was 20 years (64.3%), most of respondents had received the COVID-19 vaccine more than or equal to 2 times (75.9%) and most of the respondents had never suffering from COVID-19 (52.7%). There is a different proportion of incidents of COVID-19 from each group that received the COVID-19 vaccine less than or equal to 1 dose and more than or equal to 2 doses with a p-value 0.000. Conclusion: There is a difference proportion of COVID-19 incidence between two groups that received the COVID-19 vaccine less than or equal to 1 dose and more than or equal to 2 doses. The incidence of COVID-19 was less in the fully vaccinated group.

Abstract

Latar Belakang: Ketidakpatuhan dalam mencuci tangan berkontribusi besar dalam infeksi nosokomial. Mencuci tangan menjadi poin penting dalam mecegah terjadinya infeksi nosokomial. Kesadaran tenaga kesehatan dalam menjalankan hand hygiene yang benar menjadi aspek yang paling penting dalam mencegah penularan infeksi. Beberapa penelitian menemukan bahwa kepatuhan hand hygiene pada kalangan tenaga kesehatan masih cukup rendah. Tujuan: Tujuan penelitian ini adalah mengetahui bagaimana analisis kepatuhan hand hygiene pada tenaga Kesehatan. Metode: Penelitian ini adalah survei cross-sectional terhadap tenaga kesehatan di Rumah Sakit Islam Jakarta Cempaka Putih tahun 2023. Pengumpulan data dilakukan dengan observasi hand hygiene tenaga kesehatan dalam five moments hand hygiene menurut WHO. Olah data menggunakan analisis cross tab dan disajikan dalam bentuk deskriptif. Hasil: Hasil observasi kepatuhan cuci tangan pada five moment hand hygiene didapatkan 93.18% tenaga kesehatan di RSIJ Cempaka Putih sudah mematuhi perilaku cuci tangan 6 langkah pada 5 momen yang dianjurkan oleh WHO. 7 orang (3.98%) melakukan cuci tangan, namun tidak sesuai 6 langkah WHO, ketidakpatuhan ditemukan paling banyak pada saat setelah menyentuh lingkungan pasien dan sebelum menyentuh pasien. Simpulan: 93.18% tenaga kesehatan di RSIJ Cempaka Putih sudah mematuhi perilaku cuci tangan 6 langkah pada 5 momen yang dianjurkan oleh WHO.

Abstract

Covid-19 (Corona virus disease 2019) is an acute respiratory infection caused by SARS-COV-2 (Severe Acute Respiratory Syndrome Coronavirus 2). The spread of Covid-19 is quite fast and occurs in various countries throughout the world. The development of the Covid-19 pandemic in Indonesia continues to show progress in a better direction. This is the hard work of all elements of society and the government in implementing health protocols and following the complete dose and booster vaccination program. The aim of this research is to determine the description of knowledge, vaccination status and the incidence of Covid-19 among Nutrition Study Program Students, Faculty of Medicine and Health, Muhammadiyah University, Jakarta. This research is a descriptive study with a cross sectional design. Data was taken using a questionnaire collected online. Of the 81 respondents who met the inclusion criteria, 4 people (5%) were male and 77 people (95%) were female. Respondents' ages varied from 17 (2 people; 2.5%), 18 (10 people; 12.4%), 19 (26 people, 32%), 20 (10 people; 12.4%), 21 (14 people ; 17.3%), 22 (12 people; 14.81%), 23 (6 people; 7.4%), and 27 years (1 person; 1.23%). Each respondent who received 0 doses, 1 dose, 2 doses, 3 doses and 4 doses of vaccination was 1 (1.2%), 1 (1.2%), 32 (39.6%) , 39 (48.14%), and 8 (9.9%) people. The history of exposure to Covid-19 for each respondent was never exposed, exposed once, and exposed twice, totaling 58 (71.6%), 19 (23.5%), and 4 (4.9%) people. 3 (3.7%) respondents had been hospitalized due to exposure to Covid-19, while 78 (96.3) people had never been hospitalized. The level of knowledge of respondents regarding Covid-19 and vaccination was 68 (84%) people with a good level of knowledge, 11 (13.6%) people with a fair level, and 2 (2.4%) people with a poor level of knowledge.

Abstract

Dengue fever is a type of disease with a high and fast transmission rate, especially in tropical and subtropical regions. One of the dengue fever vaccines developed by Sanofi Pasteur (CYD-TDV) and has undergone phase III clinical trials is the tetravalent dengue vaccine. This study aims to conduct a meta-analysis and review analysis of the effectiveness of the dengue vaccine on the level of efficacy at various age levels based on previously published research results. This research uses a quantitative approach through a meta-analysis study. The data in this study were obtained from multiple sources of scientific literature in the form of research articles published in national and international journals. Data was collected online through Google Scholar, PubMed, ProQuest, Cochrane Library, and Scopus—a literature search from various sources using the keyword dengue vaccine. Data analysis was performed with the distinct stages of the study sample, effect size heterogeneity testing, calculating summary effects, calculating p-values, and determining and plotting publication bias/trim-fill analysis. The research results showed that administration of the tetravalent vaccine (CYD-TDV) to children aged 2-16 years had a moderate relationship with a Random Effect value (r ̅_RE)=0.400. The efficacy level of the CYD-TDV dengue vaccine significantly impacts patients who are given the vaccine regularly at certain time intervals (I2=4.46%).
