Abstract

COVID-19 is currently a global pandemic because of its fast transmission in humans. The disease can be symptomatic or asymptomatic. Patients with the disease can transmit the infection to the medical team who are exposed without proper protection. Prevention of transmission is our main focus of attention as well as early recognition of exposed medical teams among obstetrics and gynecology residents and consultants in our hospital. This article aimed to describe the COVID-19 early detection and primary prevention system among staff and residents in the Obstetrics and Gynecology (OBSGYN) Department, Faculty of Medicine, Public Health and Nursing, Universitas Gadjah Mada and Dr. Sardjito General Hospital from April to June 2020. The data were acquired from the reports of the COVID-19 taskforce from the OBSGYN Department. In order to prevent the transmission of COVID-19 infection, actions taken by the OBSGYN Department are as follows: a) arrangement of staff and residents watch schedules, b) usage of PPE (Personal Protective Equipment) according to protocol, c) contact tracing, and d) early detection. Resident watch schedule arrangement is done by withdrawing residents from affiliated hospitals so that the division of daily duty schedules at Dr. Sardjito General Hospital is divided into 2 shifts of 12 hours each. The PPE usage policy is adjusted according to the risk of COVID-19 exposure in different environments and in accordance with established standards. A total of 65 out of 74 residents (88%) had been exposed to suspected COVID-19 cases, and among the exposed residents, 12 residents (18%) were self-isolated because of the occurrence of symptoms, exposure to patients with positive COVID-19 rapid antibody test, or exposure to patients with positive COVID-19 Polymerase Chain Reaction (PCR) test. The department also conducted early detection of COVID-19 infection among all staff members and residents. As many as 13 out of 27 staff (48%) underwent COVID-19 rapid antibody test with 100% negative results, 66 of the 74 residents (89%) also underwent COVID-19 rapid antibody test with 100% negative results. In conclusion, the OBSGYN Department has implemented several measures to prevent transmission of COVID-19 among its staff and residents with adequate satisfactory results.

Abstract

Background: Postpartum pelvic floor dysfunction is pelvic floor disorder, which can be in the form of pelvic organ prolapse, urinary problem, defecation problem or sexual dysfunction. The incidence of postpartum pelvic floor dysfunction occurs in 46% of postpartum women. Until now, there is no standard instrument used to estimate the incidence of postpartum pelvic floor dysfunction. In previous studies, an instrument was obtained, namely KIPPas Jogja, to estimate the incidence of pelvic floor dysfunction after delivery, but it was still not sufficient for the number of research samples needed.Objective: To evaluate the Pangastuti Jogja Predictor Instrument Card (KIPPas Jogja) as a predictor of pelvic floor dysfunction in the form of pelvic organ prolapse after vaginal delivery.Method: This study was a prospective cohort study. Subjects who gave vaginal delivery were examined according to the KIPPas Jogja instrument and then evaluated for the diagnosis of pelvic floor dysfunction in the form of pelvic organ prolapse with POPQ examination and complaints of pelvic floor dysfunction using the PFDI and FSFI instruments at 3 months postpartum.Results and Discussion: From 133 research subjects, the results of the KIPPas Jogja assessment are high risk in 42.9% of subjects and low risk in 57.1% of subjects. The incidence of pelvic floor dysfunction in the form of pelvic organ prolapse was found in 69.17% subjects. Complaints of pelvic floor dysfunction were present in 20.31% subjects and complaints of sexual dysfunction in 11.3% subjects. The sensitivity of KIPPas Jogja is 80% and specificity is 95% with a positive predictive value of 97% and a negative predictive value of 68% to detect pelvic dysfunction in the form of pelvic organ prolapse. Meanwhile, to predict complaints of pelvic floor dysfunction, measured with PFDI-20, the sensitivity was 93% and specificity was 52%, and the positive predictive value was 33% and the negative predictive value was 96%. To predict sexual dysfunction, KIPPas obtained sensitivity of 64%, specificity of 42% with a positive predictive value of 10% and a negative predictive value of 92%.Conclusion: KIPPas Jogja can be used as a predictor of postpartum pelvic floor dysfunction. Keywords: postpartum pelvic floor dysfunction, KIPPas Jogja, POPQ, PFDI-20, FSFI

Abstract

Latar belakang: disfungsi atau terganggunya fungsi dasar panggul terdiri dari beberapa macam gambaran klinis, antara lain prolaps organ panggul, inkontinensia urin atau fekal, maupun gangguan fungsi seksual. Disfungsi dasar panggul pada perempuan sering dikaitkan dengan kejadian persalinan vaginal yang dialami sebelumnya. Pertolongan persalinan vaginal dapat dilakukan oleh berbagai profesi tenaga kesehatan, termasuk para bidan. Meskipun pertolongan persalinan normal telah dilakukan secara rutin, masih dijumpai beberapa permasalahan terkait pencegahan disfungsi dasar panggul pasca persalinan vaginal. Salah satu cara untuk membantu upaya pencegahan tersebut adalah dengan menggunakan KIPPas Jogja (Kartu Instrumen Prediktor Pangastuti Jogja), suatu instrumen atau alat bantu sebagai prediktor terjadinya disfungsi dasar panggul pasca persalinan vaginal.Tujuan: melakukan sosialisasi penggunaan KIPPas Jogja pada kelompok mitra bidan di wilayah Pandowoharjo Sleman, Daerah Istimewa Yogyakarta dalam rangka upaya pencegahan disfungsi dasar panggul pasca persalinan vaginal. Metode: kegiatan dilakukan dalam bentuk pengabdian masyarakat pada bulan Agustus 2022. Bentuk aktifitas berupa pertemuan kelompok, ceramah, diskusi interaksi, praktikum, penyampaian buklet panduan dan buku pengetahuan tentang faktor risiko disfungsi dasar panggul perempuan, serta video panduan penggunaan KIPPas Jogja, kepada mitra bidan di wilayah Pandowoharjo Sleman, Kabupaten Sleman, DIY.Hasil dan diskusi: terdapat beberapa permasalahan bidan dalam pengelolaan kehamilan dan persalinan yang masih perlu mendapat perhatian. KIPPas Jogja merupakan suatu alat bantu atau instrumen dalam bidang kesehatan yang digunakan untuk membantu melakukan prediksi terjadinya disfungsi dasar panggul khususnya prolaps organ panggul pada perempuan pasca persalinan vaginal. KIPPas Jogja memiliki bentuk kartu, program aplikasi android, serta aplikasi berbasis web, yang dapat berperan pada upaya pencegahan disfungsi dasar panggul perempuan pasca persalinan vaginal. Mitra bidan diharapkan dapat mengenali kasus-kasus disfungsi dasar panggul serta melakukan upaya tindak lanjut pencegahan terjadinya atau memberatnya disfungsi dasar panggul, sehingga akan mengurangi risiko penurunan kualitas hidup perempuan.Kesimpulan: terdapat peningkatan pemahaman mitra bidan tentang disfungsi dasar panggul serta kemampuan melakukan deteksi faktor risiko, tercapai pemahaman baru mengenai instrumen KIPPas Jogja. Selanjutnya dengan pemahaman yang telah ada akan bermanfaat dalam upaya pencegahan disfungsi dasar panggul perempuan, khususnya pasca persalinan vaginal. Kata Kunci: KIPPas Jogja; aplikasi; instrumen; disfungsi; panggul

Abstract

Needle stick injury (NSI) is one of the occupational hazards in the healthcare sector. Needle stick injury refers to the accident of a percutaneous piercing wound caused by a potentially contaminated instrument with a person’s body fluid. Medical students are high-risk people who contact with needles and body fluids. This study aims to assess the prevalence, incidence, knowledge, awareness, and level of NSI reporting among medical students. Systematic searches were conducted with specific keywords in 4 databases: PubMed, ScienceDirect, Scopus, and Google Scholar databases from 2018 to 2023. A total of eight articles were selected for analysis. They presented descriptively. The study revealed that the prevalence of NSI was varied, ranging from 2,7% to 8%. The incidence rate of NSI was 1,26% to 9,6%. The majority of the studies showed that the knowledge level of medical students was not satisfactory, but they had good awareness and good practice toward needle stick injury prevention. Contrarily, it was found that medical students had a low level of NSI reporting. Hence, it is important to make the medical students understand and aware of NSI, have good practice, and stick to the standardized protocol for NSI, including how to report it correctly.

Abstract

Background: Due to its complexity in terms of psycho-social factors as well as maternity and maternal outcomes, teenage pregnancy is still a global issue that receives constant attention. The rate of adolescent fertility (AFR) in Indonesia is 26.64 births per 1000 women who are fertile. Objective: This review article aims to explore the issues of teenage pregnancy, particularly as they relate to congenital abnormalities or birth defect as complications of teenage pregnancy. Method: Database searches were conducted on Pubmed and Google Scholar. Results: We gathered 27 articles with a wide variety of research designs in this review article. Conclusion: Anemia, preeclampsia, and eclampsia are the most frequent maternal complications, while low birth weight (LBW) and preterm labor are the most frequent fetal complications in teenage pregnancy. Gastroschisis is the most prevalent birth defect associated with teen pregnancy, but there is little research on the subject and even fewer comprehensive reviews
