Abstract

Colorectal cancer (CRC) is the development of cancer from the colon or rectum. There are two kinds of surgery which are laparotomy and laparoscopy. This course of action had a significant effect on the length of stay, surgical site infection, and intraperitoneal adhesion. The purpose of this study is to find out the comparison between laparotomy and colorectal cancer laparoscopy with the length of stay, surgical site infection and intraperitoneal adhesion. The researcher used a cross sectional design. The sample for this study was taken from the medical record data of colorectal cancer patients in Dr. Kariadi Semarang from 2013 to 2018. The data were collected using purposive sampling technique. There were 57 samples that included inclusion and exclusion criteria with 28 details of laparoscopy and 29 samples of laparotomy. Numeric-scale data were tested for normality of data by Kolmogorov-Smirnov and found abnormal data using the Mann Whitney test. Nominal scale data were processed using the non-parametric Chi-Square test and it significant if p<0.05. There is a difference in length of stay and frequency of occurrence of surgical site infections and intraperitoneal adhesion between laparotomy and laparoscopy.

Abstract

Latar belakang: Penyakit dan kondisi abnormal dapat menimbulkan nyeri yang kuat, misalnya patah tulang. Diperlukan penatalaksanaan yang tepat, dimana opioid merupakan pilihan terbaik. Tetapi opioid memiliki efek ketagihan dan efek sentral merugikan, karenanya digunakan ketorolac tromethamine golongan OAINS, dengan efek analgesik yang cukup kuat. Namun OAINS memiliki efek samping menimbulkan lesi histopatologis pada gaster. Tujuan : Mengetahui pengaruh pemberian injeksi ketorolac tromethamine intraperitoneal terhadap gambaran mikroskopis gaster tikus Wistar dewasa dengan fraktur kruris.Metode : Penelitian eksperimental dengan rancangan post test only control group design. Sampel tikus Wistar 20 ekor dibagi menjadi 2 kelompok, kemudian difrakturisasi pada kruris kanan. Dimana 2 kelompok tersebut yaitu kelompok kontrol yang mendapat makanan dan minuman standar sejumlah 10 ekor, dan kelompok perlakuan yang diberi ketorolac tromethamine 5 mg/KgBB sejumlah 10 ekor. Ketorolac tromethamine diberikan secara injeksi intraperitoneal selama 5 hari awal pelitian. Pada hari ke-8, dilakukan terminasi pada tikus Wistar dengan pemberian choloroform overdose. Dilakukan pengambilan organ gaster dan dilakukan pembuatan preparat histologi menggunakan pengecatan HE. Setiap preparat dibaca pada 5 lapangan pandang dan dinilai integritas epitel mukosanya menggunakan mikroskop cahaya.Hasil : Rerata integritas epitel mukosa gaster kelompok kontrol adalah 0,50 ± 0,170, rerata integritas epitel mukosa gaster kelompok perlakuan adalah 1,240 ± 0,158. Pada uji Mann Whitney didapatkan perbedaan bermakna dengan nilai p=0,00012. Simpulan : Terdapat pengaruh pemberian injeksi ketorolac tromethamine intraperitoneal terhadap gambaran mikroskopis gaster tikus Wistar dewasa dengan fraktur kruris.

Abstract

Poverty is the wide world problem to the present. Various programs arepresented in an effort to reduce and minimize the problem posed. This paperdescribed the approach based on Islamic teachings. It is the combination ofthe social contract, business, and cooperation. It starts from social akad,followed by the business and commercial contract, and the last is the risksolving the problem. A Comprehensive stage that cannot be directly implementedin stages, and it is good preparation. Meaning the poor in the hadith alsoindicates the importance of attention of all the parties is not limited to anymaterial deficiencies. Abolishing riba contracts become compulsory becausethe impact of usury posed for the poor. Pairing the word riba in Quran withthree words will inspire the solutions and replacement of such contract.Permasalahan kemiskinan adalah sebuah realitas yang dihadapi dalam dunia,dari sejak dahulu kala hingga saat ini. Berbagai program disajikan dalam upaya untuk mengurangi dan meminimalisir problem yang ditimbulkan. Dalam tulisan ini dipaparkan pendekatan yang bersumberkan dari ajaran Islam. Perpaduan antara akad sosial, bisnis dan kerjasama. Akad sosial menjadi sebuah permulaan, kemudian dilanjutkan dengan akad bisnis dan komersil serta pada tahap akhir diharapkan dapat menghadapi risiko. Sebuah tahapan yang komperhensif yang tidak bisa langsung dilaksanakan melainkan dengantahapan dan persi apan yang baik. Arti miskin dalam hadis juga mengindikasikan pentingnya perhatian segala pihak tidak hanya terbatas pada kekurangan materi saja. Pengganti dari akad riba adalah sebuah keharusan, karena dampak riba yang ditimbulkan buat sang miskin. Penyandingan kata Riba dalam qur’an dengan tiga kata yang menginspirasi akan solusi dan pengganti dari akad tersebut.

Abstract

Value is something that is believed to be true, trusted and felt useful, and is manifested in its attitude or behavior. Family counseling is an effort to provide assistance to individual family members through improving family communication so that the potential to develop is as optimal as possible and the problem can be overcome on the basis of the willingness to help from all family members based on their willingness and love for the family. Abstract

This article examines reinforcement techniques in memorizing the Al-Quran. Memorizing the AlQuran is a special part that cannot be separated from Islamic boarding school education. Memorizing is not an extracurricular lesson like in public schools. However, it is one of the main lessons at Islamic boarding schools. In Islamic teachings, memorizing the Al-Quran is a command from Allah SWT. The type of research used is descriptive qualitative. This research observes the implementation of guidance using reinforcement techniques carried out by teachers at the Ma'hadul Qur'an Pondok Pesantren Salafiyah Syafi'iyah Sukorejo to help deal with students' memorization problems. The research results showed that the guidance process using reinforcement techniques was successful. Guidance is carried out using behavioral therapy, then continued with reinforcement techniques. The results of this technique can make students aware that what they have been doing so far can be detrimental to themselves and these students realize that successful people are diligent people, not lazy people.

Abstract

Background: Small bowel obstruction (SBO) represents a formidable challenge in surgical practice. Partial SBO (PSBO) in individuals without a history of abdominal surgery, termed "virgin abdomens," introduces distinct diagnostic and therapeutic considerations. While non-operative management (NOM) has become a cornerstone for adhesive SBO, comprehensive data elucidating its impact on PSBO in virgin abdomens, especially concerning the length of hospital stay (LOS), are not extensively available. This study was designed to meticulously compare the LOS between NOM and operative interventions in patients diagnosed with PSBO affecting virgin abdomens, specifically within a tertiary care setting in Indonesia. Methods: A retrospective analytic observational methodology, utilizing a cross-sectional design, was implemented at Dr. Kariadi General Hospital, Semarang, covering patients admitted from January through December 2024. The study included patients over 18 years of age with a confirmed diagnosis of PSBO in a virgin abdomen. Exclusion criteria encompassed total SBO, obstructions located in the colon, or any record of prior abdominal surgical procedures. The principal outcome measure, LOS, was rigorously compared between the cohort managed non-operatively and the cohort that underwent surgical procedures. Statistical analysis was performed using appropriate tests, with a p-value below 0.05 established as the threshold for statistical significance. Results: The study cohort comprised 167 eligible patients. For the 112 patients (67.1%) subjected to surgical intervention, the mean LOS was recorded at 13.2±8.72 days. Conversely, the 55 patients (32.9%) managed conservatively demonstrated a markedly shorter mean LOS of 7.74±6.12 days. This observed difference in LOS was statistically highly significant (p < 0.001). The mean age of the participants was 50.60±14.94 years, with females constituting 52.1% of the group. Conclusion: In clinically stable patients presenting with partial small bowel obstruction in virgin abdomens, non-operative management was associated with a significantly diminished length of hospital stay when compared to surgical intervention. This conservative strategy proves to be an effective and advantageous approach in appropriately selected patient populations, offering the potential to lessen hospital burden and enhance recovery timelines.
