Abstract

Abstract: Thyroid disorders are the second most common endocrine disorders in the world after diabetes. The most common thyroid disorders are goitre, Graves' disease, Hashimoto's thyroiditis, and thyroid neoplasm. This study was aimed to determine the pattern of thyroid disorder at Prof. Dr. R. D. Kandou Hospital Manado from January 2013 to December 2015. This was a descriptive retrospective study. Data were obtained from Medical Rrecord Installation and Departement of Pathology at Prof. Dr. RD Kandou Hospital Manado periods January 2013 - December 2015. The results showed that of total 244 cases of thyroid disorders the highest percentage was in the period January - December 2015 ias many as 132 cases (54.1%) consisted of 196 (80.3%) females and 48 (19.7%) males. Most patients were in the age group 51-60 years (24.2%) amd lived in mountainous area (66.7%). The most common pathological results were benign colloid goitre (71.9%) and papillary carcinoma (62.7%). Since the incidence of thyroid disorders continues to rise each year at Prof. Dr. R. D. Kandou Hospital Manado, prevention and education to patients with thyroid disorders are needed to minimalize the complications of thyroid disorders. Kelainan tiroid yang paling sering terjadi ialah goiter, penyakit Grave, tiroiditis Hashimoto, dan neoplasma tiroid. Penelitian ini bertujuan ntuk mengetahui pola kelainan tiroid di RSUP Prof. Dr. R. D. Kandou periode Januari 2013 – Desember 2015. Jenis penelitian ialah deskriptif retrospektif. Data diperoleh dari Instalasi Rekam Medik dan Bagian Patologi Anatomi RSUP. Prof. Dr. R. D. Kandou Manado periode Januari 2013 – Desember 2015. Hasil penelitian memperlihatkan bahwa dari 244 kasus kelainan tiroid terbanyak pada periode Januari – Desember 2015 yaitu 132 kasus (54,1%), terdiri dari 196 (80,3%) perempuan dan 48 (19,7%) laki-laki. Penderita terbanyak pada kelompok usia 51-60 tahun (24,2%) dan bertempat tinggal di daerah pegunungan (66,7%). Jenis hasil patologi anatomi jinak dan ganas terbanyak ialah struma koloides (71,9%) dan karsinoma papiler (62,7%).Oleh karena angka kejadian kelainan tiroid terus meningkat setiap tahunnya di RSUP Prof. Dr. R. D. Kandou Manado, upaya preventif dan edukasi terhadap penderita kelainan tiroid diperlukan untuk mengurangi komplikasi kelainan tiroid.Kata kunci: kelainan tiroid

Abstract

Abstract: Primary breast lymphoma is rare and it accounts for around 0.5% of malignant breast neoplasms. Small lymphocytic lymphoma (SLL) is a subtype of non-Hodgkin lymphoma originating from a B-cell lineage. This report presents a case of primary SLL of the breast with lung metastasis that had excellent clinical response. We reported a 67-year-old female complained a right breast mass that suddenly appeared after she got massage on her right left arm. The mass was swollen and painless with no systemic complications. The specimen was submitted to histopathology department and it revealed small diffuse lymphocytic lymphoma. Chest X-ray showed a lung metastasis. The patient was then treated with CHOP regimens (Cyclophosphamid, Doxorubicin, Vincristin, and Prednison) and the clinical result was excellent. The side effects of the medication were minimal. In conclusion, primary breast lymphocytic lymphoma is rare and this case highlights the differential diagnosis for a painless breast mass. The treatment must be individualized and multimodal, with chemotherapy being the most accepted treatment. Keywords: breast lymphoma; breast mass; malignancy

Abstract

Abstract: Burn is caused by exposure to high-temperature substances such as hot, solid liquids or gases such as smoke, steam, engines, stoves, radiators and objects that emit heat energy. In severe burns, there is an excessive neutrophil inflammatory response that triggers SIRS, where there is an excessive increase in pro-inflammatory mediators such as TNFα and IL-6. Propolis contains a number of compounds, such as flavonoids, CAPE, phenol compounds, arginine, ferulic acid and albumin which play a good role in healing burns. This study aimed to find out the effect of propolis on TNF-α expression through in-vivo testing in cases of superficial dermal burns. This was an experimental and laboratory study to assess the bioactive compounds contained in Trigona Sp propolis. Descriptive analysis was performed on the bioactive composition of Trigona Sp propolis and experimental effectiveness of propolis on TNF-α expression in superficial dermal burns. The study was conducted at the laboratory of experimental animals "Alike Quality System", Manado. The results showed that the group treated with propolis had smaller burn areas compared to the groups treated with 1% silver sulfadiazine or with 0.9% NaCl. Granulation had been formed throughout all wounds, however, macroscopically, burns with propolis treatment showed reddish wound appearance while treated wounds with 1% silver sulfadiazine and 0.9% NaCl were darker in color and had thicker crust formation. The one-way ANOVA test showed no significant difference between propolis and 1% silver sulfadiazine on TNF-α expression in dermal superficial burns (p=0.666) meanwhile the effect of propolis compared with NaCl 0.9% on TNF-α expression in dermal superficial burns, showed significant differences (p=0.006 and p=0.040). In conclusion, administration of propolis can reduce the expression of TNF-α in superficial dermal burn. Keywords: propolis; Trigona sp; burns; in vivo study

Abstract

Abstract: Lenvatinib, a tyrosine kinase inhibitor (TKI), has shown potential as a neoadjuvant therapy for inoperable thyroid cancer (TC). In this case series, we present three patients with unresectable thyroid tumors who responded favorably to lenvatinib treatment. A 60-year-old male with papillary thyroid cancer (PTC) experienced a 60% tumor volume reduction after five months of therapy. A 61-year-old male with medullary thyroid cancer (MTC) saw a 99% reduction after four months of lenvatinib and decrease of serum calcitonin and no residual disease six months after surgery. Lastly, a 67-year-old female with anaplastic thyroid cancer (ATC) showed a 70% tumor reduction and significant symptomatic relief. These cases highlight lenvatinib’s efficacy in reducing tumor size and stabilizing disease, improving surgical outcomes in patients initially deemed inoperable due to locally advanced tumors. Lenvatinib’s antitumor effects, driven by its antiangiogenic properties, suggest its potential as a valuable neoadjuvant treatment option in advanced thyroid cancer.
