Abstract

Availability of safe blood and blood products is a critical component in improving adequate blood supply and blood products. Blood donation collected from 1% of the population is usu­ally the minimum requirement to meet a country’s most basic need for blood. Blood product declare as a life saving product which treat diseases and maintains health. Safety blood donor plays a vital role for donor and recipient. Recruiting donor should follow criterias during blood selection. This study aimed to analysis of blood donor referral in blood donation unit of Bojonegoro following some characteristics such as hemoglobin levels, blood pressure, and consumption. This study was conducted by reviewing the deferral donor records during June to July 2022 at Blood Donation Units Bojonegoro. Out of the 668 screened prospective donors, 117 (17%) were deferred. Hemoglobin levels was the major (90% from 117) cause of temporary deferral as well as the second cause of overall deferrals. Blood pressure and latest consumption time constituted 9% and 1% of the deferral cases, respectively. From 117 deferred donor, deferral rate among male donors was 53% (62 donor). Among female donors, it was 47% (55 donor). Thus, the prevalence of deferral was significantly higher among female donors than among male donors. Abstract

Ketersediaan darah di Indonesia masih menjadi tantangan, khususnya pada fasilitas pelayanan kesehatan primer. Kondisi ini berdampak pada keterlambatan penanganan kasus kegawatdaruratan akibat terbatasnya stok darah. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk memperkuat ketahanan stok darah di tingkat puskesmas melalui Program Donor Keluarga Terintegrasi berbasis komunitas di wilayah kerja Puskesmas Sumberrejo, Kabupaten Bojonegoro. Metode yang digunakan meliputi edukasi dan literasi donor darah, serta pemetaan dan pendataan pendonor potensial berbasis keluarga melalui pemeriksaan golongan darah. Hasil kegiatan menunjukkan adanya peningkatan pengetahuan dan kesiapan masyarakat untuk berpartisipasi dalam sistem donor keluarga, yang ditunjukkan oleh peningkatan skor rata-rata pretest dari 53,5 menjadi 89,9 pada posttest. Selain itu, pemeriksaan golongan darah terhadap peserta yang belum mengetahui golongan darahnya berhasil membentuk basis data awal calon pendonor, dengan distribusi golongan darah terbanyak adalah O (40%) dan seluruhnya memiliki rhesus positif. Respon masyarakat, perangkat desa, dan kader kesehatan menunjukkan dukungan positif terhadap keberlanjutan program. Kegiatan ini membuktikan bahwa pendekatan berbasis komunitas yang terintegrasi dengan program puskesmas efektif dalam meningkatkan literasi, partisipasi, dan kesiapsiagaan donor darah, serta berpotensi memperkuat kemandirian sistem stok darah di tingkat pelayanan kesehatan primer.

Abstract

Hepatitis B virus (HBV) remains one of the most significant transfusion-transmitted infections (TTIs), posing ongoing challenges to blood safety in Indonesia. Regular monitoring of HBV reactivity among blood donors is crucial for evaluating the effectiveness of screening and guiding public health interventions. This study aimed to assess the prevalence and temporal trends of HBV reactivity among blood donors in Bojonegoro, while evaluating the effectiveness of current screening procedures and supporting public health interventions. A retrospective cross-sectional study was conducted on 51,351 blood donors at UDD PMI Bojonegoro between January 2020 and December 2021. Blood samples were screened using the Enzyme-Linked Immunosorbent Assay (ELISA) to detect HBV infection. Data were analyzed using descriptive statistics and cross-tabulation. A total of 261 donors tested reactive for hepatitis B, comprising 129 cases in 2020 and 132 in 2021. Although the absolute number of reactive cases increased slightly, the reactivity rate declined from 0.61% to 0.44%, coinciding with an overall increase in the number of donors. Monthly analysis revealed fluctuating patterns, with peaks observed in December 2020 and June 2021. The declining proportion of HBV reactivity suggests improvements in donor selection and public awareness. However, the continued presence of reactive cases highlights the need to strengthen screening systems, expand vaccination coverage, and adopt more sensitive detection technologies, such as Nucleic Acid Testing (NAT). Ongoing regional surveillance remains crucial to support national efforts in controlling HBV and ensuring the safety of blood transfusion services in Indonesia.

Abstract

Syphilis is a disease caused by Treponema pallidum. Syphilis could transmit through sexual intercourse, and can also occur in the vertical direction from the mother to the fetus, through blood products and, in some cases, through medical equipment. Among those, blood transfusions are the main way to spread the disease. WHO estimates that more than 1 million people worldwide are diagnosed with 4,444 sexually transmitted diseases daily. This study was using total 51351 blood sample obtained from blood donor in 2020 and 2021 in Blood Donor Unit (UDD PMI) Bojonegoro. The aim in this study was to find the prevalence of syphilis cases occurred in Blood Donor Unit (UDD PMI) Bojonegoro. This finding may have implication to increase people awareness in syphilis. This study was used descriptive study with cross sectional design and conducted by using cross tabulation as statistical analysis. The data showed that there was slight increase with 0.12% and 0.16% cases of confirmed syphilis in 2020 and 2021 respectively (P = 0,174). Total cases from 51351 blood samples, 71 (0,14%) blood samples were positive for syphilis. The highest cases in 2020 was occurred in January (7 cases) and highest cases in 2021 was occurred in October (13 cases). The conclusion of this study showed that the prevalence of syphilis was low in this study. Even though the percentage might be below 1% in this study, it is still a significant transfusion problem in blood donation quality purpose to be overcomed.
