Abstract

Background: Chronic periodontitis is a disease that causes inflammation of the tooth supporting tissue, loss of periodontal attachment, and loss of alveolar bone. Root planning scaling treatment is carried out to remove irritants and microorganisms. The use of additional materials is required when the pocket depth is more than 5 mm. One of the herbal ingredients that has the potential as an adjunct to periodontal therapy is the 4% ocinumsanctum.Method: The sample consisted of 20 periodontal pockets with a depth of 4-6 mm which were given scaling root planning which were divided into two groups, namely 10 pockets given 4% maximum sanctum gel and 10 pockets given Metronidazole gel 25%. evaluations of the papillary bleeding index, TNF-α, and IL-β were performed at baseline, and on day 21. TNF-α and IL-β levels were taken through the gingival sulcus fluid and then tested using ELISA.Result: Papillary bleeding index, TNF-α, and IL-β levels showed a significant reduction in both groups but there was no significant difference between the 4% ocimum sanctum group and Metronidazole gel.Conclusion: ocimum sanctum application 4% post scaling root planning was able to lower papillary bleeding index score, TNF-α levels, and IL-β.

Abstract

Patients with Type 2 Diabetes mellitus (T2DM) were more prone to suffer from periodontitis due to unique alteration in host immune response. Application of curcumin oral gel as adjunctive therapy was expected to improve periodontal condition due to its anti-inflammatory properties. The objective of this study was to evaluate the effect of curcumin oral gel on periodontal inflammation in patients with T2DM. Sixteen periodontal pockets (n=16) from patients with T2DM were administered with curcumin oral gel using blunt cannula after periodontal curettage. Gingival Index (GI) and Periodontal Pocket Probing Depth (PPD) were evaluated a month after application. GI was assessed using visible sign of inflammation parameters including swelling, redness and bleeding upon probing. PPD was assessed using UNC-15 periodontal probe. The results were analyzed by Wilcoxon test (two related samples). The results showed that GI before treatment decreased by 51.61% a month post application. There was statistically significant difference (Sig 0.003) between GI mean before and after curcumin application. PPD decreased by 42.81% and showed statistically significant difference (Sig 0.000) by Wilcoxon Test. It is concluded that application of curcumin oral gel post curettage could decrease periodontal inflammation in patients with T2DM.

Abstract

Backgrounds: Chronic periodontitis is a periodontal tissue disease caused by specific microorganisms, resulting in periodontal tissue destruction. The gold standard for cases of periodontal tissue destruction is regenerative flap surgery combined with bone graft. The aim of this case report is to describe the repair of bone defects caused by chronic periodontitis with open flap debridement treatment combined with bovine-derived xenograft bone graft and pericardium membrane. Case: A 46-year-old female patient came complaining that her right upper back tooth was swollen and hurt while eating. Clinical findings showed that tooth 16 had grade 2 mobility with a 7 mm periodontal pocket on the distal side accompanied by vertical bone loss on radiographic examination.Method: Emergency treatment was carried out for drainage of the abscess on tooth 16, and then antibiotic medication was given. Initial treatment involves scaling and root planing, occlusion adjustment and splinting. Next, open flap debridement was carried out on tooth 16, bone graft application (Ti-OSS®), followed by pericardium membrane application. Discussion: Evaluation of the results was carried out up to 3 months after treatment; it was found that clinical attachment loss (CAL) was reduced, and radiographic images showed an increase in bone in the distal tooth 16.Conclusion: The use of bovine-derived xenograft bone-graft and pericardium membrane is able to increase the regeneration of alveolar tissue and bone in cases of chronic periodontitis.

Abstract

Perikoronitis adalah peradangan yang terjadi pada jaringan gingiva di sekitar mahkota gigi molar. Salah satu faktor yang dapat memperparah perikoronitis adalah trauma mekanis dari gigi antagonis yang dapat menyebabkan ulserasi atau pembengkakan pada operkulum. Operkulektomi dengan electrosurgery dapat menghilangkan operkulum dengan lebih mudah. Pasien wanita berusia 28 tahun dirujuk oleh residen ortodonti dengan keluhan gusi bengkak yang menutupi permukaan gigi molar kiri paling belakang. Kondisi tersebut dirasakan mengganggu saat makan dan keluhan ini telah dirasakan selama sebulan terakhir. Pada kunjungan pertama dilakukan perawatan awal berupa scaling dan root planing serta edukasi kebersihan mulut (dental health education). Selanjutnya, dilakukan operasi operkulektomi yang dimulai dengan anestesi infiltrasi di lipatan mukobukal gigi 37. Eksisi menggunakan electrosurgery dilakukan pada operkulum yang menutupi mahkota gigi 37. Area bedah dibersihkan dengan larutan salin dan kemudian ditekan menggunakan kasa steril. Penggunaan electrosurgery pada operkulektomi dapat meminimalkan perdarahan dan mengurangi durasi operasi. Pasien merasa lebih nyaman selama prosedur. Satu minggu pasca operasi, jaringan gingiva masih menunjukkan kemerahan dan nyeri saat palpasi. Satu bulan pasca operasi, pasien tidak merasakan keluhan, jaringan gingiva dalam kondisi baik, tidak ada kemerahan, pembengkakan, atau nyeri. Penggunaan electrosurgery untuk perawatan perikoronitis kronis dapat menjadi alternatif dari tindakan bedah konvensional dengan perdarahan minimal dan durasi prosedur yang lebih cepat.

Abstract

Background: Interrelated pathological conditions such as periodontitis and Type 2 Diabetes Mellitus (T2DM) still be the mistery to solve. The production of secondary metabolites due to hyperglycemia conditions, namely AGEs, contributes to inflammation that arises in periodontal tissue. Periodontal inflammation is exacerbated by systemic dysregulation resulting in periodontitis being one of the potential complications aggravating the condition of T2DM. Excessive production of pro-inflammatory cytokines damages periodontal tissue and pancreas B cells and thus leads to insulin resistance. Many approach have been conducted to break the relationship between these two diseases, especially from the inflammatory pathway. Herbal therapy is one of the potential treatment to manage the bidirectional relationship.  Objective: The aim of this study is to elaborate the effects of herbal therapy on decreasing IL-1β cytokines as potential targeted therapeutic targets for both periodontitis and type 2 diabetes condition.Methods: Literature research done from 2013 to 2023 to find relevant articles. Result: Several herbal therapies effective in decreased Interleukin-1β and correlated to systemic and periodontal condition. Conclusion:Herbal therapy is effectively decreasing Interleukin-1β as a potential therapeutic target in periodontitis and Diabetes Mellitus Type 2 management.

Abstract

Background: Tongue carcinoma is different from oral cancer in other areas because it has a high amount of cell growth, localized migration, and a higher rate of spreading to cervical lymphatic nodes. Therapeutic alternatives, particularly concerning recurrent malignancies, are notably constrained. Nevertheless, it is imperative to explore novel methodologies for refractory neoplasms, one of which may involve the application of ibuprofen. Purpose: The goal of this study was to look at how well caspase-3 and -9 break down proteins as possible targets for apoptosis in tongue cancer cells that were caused by ibuprofen. Methods: A controlled laboratory experiment employing a post-test-only design was executed. We used a colorimetric test for caspase-3 and -9 to check for the induction of apoptosis. The suppression of cellular invasion was verified through the Boyden chamber assay. Western blot analysis was utilized to identify the presence of caspase-3 and -9 proteins. The administered doses of ibuprofen were calibrated at 0, 5, 10, 25, 50, and 100 µg/mL. Data were subjected to analysis using two-way ANOVA followed by post-hoc Least Significant Different (LSD), with a significance threshold set at 95%. Results: It was established that ibuprofen at dosages ranging from 25 to 100 µg/mL significantly facilitated apoptosis in cells through an augmentation of the proteolytic activity of caspase-3 and -9. Notably, caspase-9 exhibited a superior proteolytic activity (1.85-fold) compared to caspase-3 (1.30-fold) (P = 0.038) at the 100 µg/mL concentration. The upregulation of caspase-3 and -9 proteins was observed in cells treated with ibuprofen. Ultimately, ibuprofen demonstrated an ability to inhibit the invasion of tongue carcinoma cells across various dosages. Conclusion: Ibuprofen has been shown to induce the proteolytic activities of caspase-3 and -9 in tongue carcinoma cells. However, the proteolytic activity of caspase-9 surpassed that of caspase-3, suggesting that targeting this protein may constitute a promising novel therapeutic strategy for this cancer variant.

Abstract

Background: Oligodontia is a genetic disorder in which the individual does not have more than six teeth either permanent, deciduous, or both teeth. This condition causes multiple diastemas and requires orthodontic treatment.  Abnormal attachment of the superior labial frenulum also causes midline diastema, and recurrence of diastema after orthodontic treatment. Z-plasty frenectomy is commonly used for patients with thick frenulum and shallow vestibule. The presence of two transposed flaps in this technique can provide stress distribution in the tissue to minimize the occurrence of scar tissue formation. Case Report: A female patient, 20 years old and systemically healthy, came to RSGM Prof. Soedomo with complaints of a maxillary front tooth gap that disturbed her self-confidence. The patient was referred by an orthodontist colleague.Result: Clinical examination in the form of a blanch test and vestibule depth. From the examination, it was found that the patient had frenulum attachment to the anterior interdental papilla (Kotlow class III) and a shallow vestibule. In this situation, the Z-plasty technique was chosen. Satisfactory results were obtained with no scarring and no residual connective tissue that could cause diastema recurrence. Conclusion: Frenectomy using the Z-plasty technique was succeessful in removing the thick frenulum without the formation of scar tissue, and proved to be more aesthetically pleasing than other conventional methods.

Abstract

Background: The black triangle is a cervical embrasure to the proximal contact that is not filled with gingival tissue. Until, the treatment of black triangle cases has evolved to offer non-surgical procedures such as hyaluronic acid injection, Injectable Platelet Rich Fibrin (i-PRF), and photobiomodulation therapy. Various studies have also reported the use of hyaluronic acid, i-PRF and photobiomodulation therapy to increase the height of the interdental papillae.Method: Literature searches were carried out systematically from various international databases such as PubMed, ScienceDirect, Google Scholar, and Semantic Scholar. The literature selection stages are carried out through the process of identifying articles, screening, and fulfilling inclusion criteria. A total of 52 articles were published in 2012 – 2022 and have met the criteria for data extraction examination of various clinical parameters from the black triangle case.Result: The results of the review showed that the use of hyaluronic acid injection, i-PRF and photobiomodulation therapy showed significant treatment results in cases of black triangle class I and II. Conclusion: It was reported that the non-surgical procedure for the treatment of black triangle cases produce better result and predictable results, an easy technique, more effective time than surgical procedures and reduced discomfort and met the aesthetic demands of patients based on clinical parameters after 6 month injection.
