Abstract

Pendahuluan: Karsinoma lobular invasif payudara merupakan karsinoma invasif payudara terbanyak ke dua dengan insiden mencapai hingga 15% dari seluruh karsinoma invasif payudara. Tumor Infiltrating Lymphocytes (TIL) adalah suatu respon imun tubuh dalam melawan pertumbuhan dan perkembangan tumor. Penilaian TIL stroma adalah suatu faktor prognostik karsinoma invasif payudara dan dihubungkan dengan luaran yang baik pada subtipe tripel negatif dan HER-2. Tujuan penelitian: Untuk mengetahui karakteristik karsinoma lobular invasif dan untuk menganalisis hubungan subtipe molekuler dengan densitas TIL. Metode: Penelitian ini adalah penelitian observasional dengan pendekatan cross sectional. Sampel karsinoma lobular invasif dari 3 laboratorium Patologi Anatomik di Padang sebanyak 30 kasus. Densitas TIL dinilai berdasarkan rekomendasi The International TILs Working Group, kemudian dikelompokkan rendah, intermediet, dan tinggi. Dilakukan pulasan imunohistokimia (IHK) ER, PR, HER-2 dan Ki67 untuk menentukan subtipe molekuler yaitu luminal A, luminal B, HER-2 positif dan tripel negatif. Analisis bivariat menggunakan uji Chi Square dengan nilai p<0,05 dianggap bermakna. Hasil: Sebagian besar karsinoma lobular invasif dengan densitas TIL rendah (70%), dengan subtipe molekuler terbanyak adalah luminal A. Analisis bivariat menunjukan densitas TIL rendah banyak pada luminal A, dan densitas TIL intermediet banyak pada luminal B. Analisis statistik menunjukkan hubungan yang bermakna antara subtipe molekuler dengan densitas TIL dengan nilai p=0,004. Kesimpulan: penelitian ini menunjukkan adanya hubungan bermakna antara subtipe molekuler dengan densitas TIL pada karsinoma lobular invasif.

Abstract

AbstrakTujuan: mengetahui hubungan ekspresi PD-L1 dengan derajat diferensiasi dan staging karsinoma sel skuamosa (KSS) paru di RSUP Dr. M. Djamil Padang. Metode: Penelitian ini merupakan penelitian observasional dengan pendekatan cross sectional study, sampel sebanyak 35 kasus KSS paru yang telah didiagnosis periode Januari 2022 – Mei 2023. Sampel diperoleh secara simple random sampling dari blok paraffin yang berasal dari jaringan tumor. Dilakukan penilaian ulang pada slaid histopatologik untuk menilai derajat diferensiasi dan data staging didapat dari rekam medis pasien. Ekspresi PD-L1 pada sel tumor dilihat dengan pemeriksaan imunohistokimia. Untuk mengetahui korelasi dilakukan uji analisis statistik bivariat menggunakan uji chi square dengan tingkat kemaknaan p<0,05. Hasil: Dari 35 kasus, didapatkan ekspresi PD-L1 tinggi pada derajat diferensiasi buruk (89,5%) dan derajat diferensiasi baik-sedang (50%). Pada staging lanjut, ekspresi PD-L1 rendah-sedang, tinggi secara berturut-turut (75 %, 100 %) dibandingkan ekspresi PD-L1 staging awal yaitu ekpresi rendah-sedang (20%). Secara statistik, terdapat hubungan yang bermakna antara ekspresi PD-L1 dengan derajat diferensiasi dengan nilai p=0,022. Terdapat juga hubungan yang bermakna antara ekspresi PD-L1 dengan staging dengan nilai p=0,035. Kesimpulan: Penelitian ini menyimpulkan bahwa terdapat hubungan yang bermakna antara ekspresi PD-L1 dengan derajat diferensiasi dan staging KSS paru.

Abstract

Invasive breast carcinoma is a malignant neoplasm with metastatic potential influenced by various histopathological parameters, including tumor budding (TB). TB has emerged as a potential indicator of tumor aggressiveness; however, differences in assessment criteria may affect its clinical relevance. This study aimed to evaluate the association between TB grade and lymphovascular invasion (LVI) as well as lymph node involvement in invasive breast carcinoma using three different assessment criteria. A cross-sectional study was conducted on 44 cases of invasive breast carcinoma that met the inclusion criteria. TB was assessed at the invasive front using the International Tumor Budding Consensus Conference (ITBCC) 2016 criteria, as well as the criteria proposed by Usta et al. and Manimaran et al. LVI and lymph node status were determined through histopathological examination. The associations between variables were analyzed using the chi-square test or Fisher’s exact test, as appropriate. Tumor budding grade based on one standardized assessment system demonstrated a significant association with both LVI and lymph node involvement. In contrast, the other two grading approaches did not show statistically significant associations. As conclusion, within the context of this study, the ITBCC 2016 criteria appear to be more representative in reflecting tumor invasive activity in invasive breast carcinoma. These findings support the potential role of tumor budding as an additional indicator of tumor aggressiveness and highlight the importance of standardizing its assessment method in invasive breast carcinoma.Keywords: invasive breast carcinoma; tumor budding; lymphovascular invasion; lymph node involvement

Abstract

Background: Uterine smooth muscle tumor of uncertain malignant potential (STUMP) presents a diagnostic dilemma, especially in Southeast Asian populations, where delayed presentation often leads to advanced tumor burden. This study aimed to characterize the clinicopathological profile of STUMP in Indonesia using the 2021 WHO Classification and modified Stanford parameters. Methods: A descriptive study analyzed 37 STUMP cases diagnosed between 2023 and 2025 at a tertiary referral center in West Sumatra. A panel of pathologists re-evaluated archival slides for atypia, mitotic activity, necrosis, and growth patterns, establishing inter-observer reliability. Fisher’s Exact Test was employed to correlate tumor size with morphological markers of aggression. Results: The cohort demonstrated a distinct large-mass phenotype, with 54.1% of tumors exceeding 10 cm and a median diameter of 14.5 cm. Pathological review classified 56.8% of cases into WHO Group 4 (atypia with ambiguous mitosis). A statistically significant correlation was identified between tumor size >10 cm and the presence of infiltrative growth margins (81.1%; p=0.034). Conclusion: Indonesian STUMP cases are distinctively characterized by massive size and high rates of infiltrative growth, likely driven by prolonged natural history. The prevalence of ambiguous mitotic figures underscores the utility of the WHO Group 4 category in resource-limited settings. These findings advocate for aggressive surgical management and wider resection margins in large-mass variants to mitigate local recurrence risk.

Abstract

Atherosclerosis is an inflammation that occurs in blood vessels, especially arteries, it could be developed by an accumulation of lipids in the body, which is atherosclerosis causes this to become a major factor of cardiovascular disease. The fruit of the andalas contains flavonoid, morasin C and M for anti-inflammatory. The purpose of this experimental is to see the effect of extract andalas fruits to histologial changes in myocardium of rats with aterosclerosis model. This research was an experimental study with a randomized post test only control group design with using 29 mice which have been divided into 5 groups, there is negative control group (K-), positive control group (K+), treatment group 1 (P1), treatment group 2 ( P2), and treatment group 3 (P3). The results measurement of the weight rat's heart after analysis used the One Way ANOVA test were p = 0.000, that’s mean there was a significant difference of weight rat's heart in each other groups. Measuring the thickness of myocardial cell size used the One Way Anova test, the resulted is p = 0.632, that’s mean there was no relationship between the groups. Measuring the blood vessel thickness were used by One Way ANOVA analysis, the resulted is p = 0.301, that’s mean there was no significant difference in arterial blood vessel thickness in all groups. The conclusion of this study is there was changes in rat heart weight in all groups, there was no significant change in the histopathological size of the myocardium in all groups, lipofuscin was found in 1 sample, necrosis wasn’t found, sicatric tissue wasn’t found, and there was no significant change in the histopathological picture of arterial thickness in this experience.

Abstract

Malignant melanoma is a highly aggressive tumor in which immunohistochemical (IHC) evaluation is often hindered by endogenous melanin, whose brown pigment mimics DAB staining and may lead to false‑positive interpretation. An effective melanin bleaching method is therefore essential to ensure accurate IHC assessment. This study aimed to determine the optimal melanin bleaching protocol for melanoma, particularly for IHC analysis. This preliminary laboratory study used eighteen malignant melanoma samples. Initial bleaching with 0.5% H₂O₂ in PBS or Tris-HCl at 80°C for 4–12 minutes caused marked tissue damage. A second protocol using 5% H₂O₂ in PBS at 50°C for 60, 90, and 120 minutes was tested, followed by a 24‑hour room‑temperature protocol. Tissue integrity, bleaching efficacy, and subsequent HMB45 IHC staining were evaluated. High‑temperature bleaching (protocol 1) produced significant morphological deterioration. Bleaching with 5% H₂O₂ at 50°C effectively removed melanin while preserving tissue structure at 60 and 90 minutes, whereas 120 minutes caused tissue detachment. Room‑temperature bleaching for 24 hours also reduced melanin but resulted in partial tissue damage. HMB45 staining remained strong and interpretable after bleaching at 50°C for 60 and 90 minutes. In conclusion, the most favorable balance between melanin removal, tissue preservation, and IHC quality was achieved using 5% H₂O₂ in PBS at 50°C for 60 minutes. This protocol is recommended as the optimal melanin bleaching method for malignant melanoma tissue.Keywords: melanin bleaching; immunohistochemistry; malignant melanoma
