Abstract

Background: Acute Respiratory Infection (ARI) is a major cause of acute diseases and death in infants worldwide. Percentage of ARI (year 2017) in children aged 12-59 months in Padang was 26.5% and Andalas health center was 33.2%. ARI is influenced by many factors such as environmental condition. This study aimed to analyze risk factors for ARI in children under five in Padang, Indonesia.Subjects and Method: This was a case control study conducted at Andalas community health center, Padang, Indonesia. A sample of 90 children aged 12-59 months was selected for this study. The dependent variable was ARI. The independent variables were humidity, house ventilation, dwelling density, indoor smoke cigarette. Data on ARI status were taken from medical record. The other variables were measured by questionnaire and observation sheet. The data were analyzed by a multiple logistic regression.Results: Poor ventilation (OR = 11.73; 95% CI = 2.16 to 63.86; p = 0.004), high dwelling density (OR = 21.99; 95% CI = 3.75 to 129.04; p = 0.001), indoor cigarette smoke (OR = 5.09; 95 % CI = 1.06 to 24.34; p = 0.042), and high air humidity (OR = 5.00; 95% CI = 0.79 to 31.51; p = 0.086) increased the risk of ARI in children under five and they were statistically significant.Conclusions: Poor ventilation, high dwelling density, indoor cigarette smoke, and high air humidity increase the risk of ARI in children under five.Keywords: Acute respiratory infection, dwelling density, air humidity, children under fiveCorrespondence:Rahmi Hidayanti. Abstract

Every hospital has to implement Hospital Management Information System (HMIS) based on Permenkes No. 82 Tahun 2013. Health care delivery can be more effective and efficient through HMIS implementation. RSUD Solok Selatan has planned to implement HMIS since 2019 until recently. The purpose of this study is to know the readiness of HMIS implementation in RSUD Solok Selatan using COBIT 5 Framework, specifically APO02 process (strategic management), APO07 (human resource management), and APO12 (risk management). This study was conducted with qualitative-quantitative approach. Qualitative data was collected with an interview. Meanwhile, quantitative data was collected with questionnaire according to COBIT 5 Framework. Respondents of this study was determined by RACI chart in COBIT 5. Capability level of APO02 achieved level 0, based on the level 1 assessment was 2,48 (not achieved). The capability level of APO07 was level 0 and level 1 assessment was 1,25 (partially achieved). Capability level of APO12 was level 0 and level 1 assessment was 1,0 (not achieved). Level 0 indicated that COBIT 5 was non implemented or the process fails to achieve its outcome. Readiness in strategic management, human resource management, and risk management was not optimally planned. RSUD Solok Selatan needs to increase its capability to plan HMIS implementation for better output.

Abstract

BackgroundPapillary thyroid carcinoma is the most common endocrine gland malignancy that has the highest incidence rate of 60-80%. In papillarythyroid carcinoma BRAF V600E mutation plays an important role in carcinogenesis. BRAF V600E is a RAS-regulated-serine-threoninekinase and activator of the MAPK pathway cascade that functions to regulate cell proliferation, differentiation and survival. BRAFV600E mutations in PTC is associated with a high incidence of recurrence and reduced survival. The purpose of this study is to lookat the relationship of BRAF V600E expression with histopathological variants and lymphocyte infiltration in PTC.MethodsThis research was a retrospective observational cross sectional study with 40 cases of papillary thyroid carcinoma which werediagnosed in 4 Pathology Anatomy Laboratory in West Sumatera. Samples were obtained from paraffin blocks of tissue from surgeryand were reevaluated for histopathological variant and lymphocyte infiltration. Expression of BRAF V600E in tumor cell were analyzedusing immunohistochemistry staining. Bivariate statistical analysis of Chi-Square were used and p value <0,05 were consideredsignificance.ResultsPositive BRAF V600E expression was found in 24 case (60%). Positive BRAF V600E expression was mostly found in classic PTC(50%). Papillary thyroid carcinoma with lymphocyte infiltration mostly found with negative BRAF V600E expression (79.2%). Statisticalanalysis showed no significant correlation between BRAF V600E with histopathological variant (p=0.74) and no significancecorrelation between BRAF V600E expression with lymphocyte infiltration (p=0.12).ConclusionThe conclusion is BRAF V600E expression has no correlation with histological variants in papillary thyroid carcinoma and no significantcorrelation of BRAF V600E expression with lymphocyte infiltration.

Abstract

BackgroundColorectal carcinoma is the third most common malignancy in the world. Colorectal carcinoma is a heterogeneous tumor withdifferent clinical pathologic features and prognostic values. For the same tumor-stage, patients could have difference prognosis andit has been suggested that the angiogenesis might be correlated with the prognosis, especially expression of vascular endothelialgrowth factor (VEGF) as the main pro-angiogenic factor. High VEGF expression in colorectal adenocarcinoma is associated withincreased blood vessels in invasive tumor area, cell proliferation and metastases. However, relation VEGF expression with thedegree of differentiation and lymphovascular invasion is not known.MethodsThis was a retrospective observational study with cross sectional approach. Samples were obtained from 39 paraffin blocks withdiagnosis adenocarcinoma not otherwise specific (NOS) in four Anatomical Pathology Laboratory in West Sumatera 2018 andevaluated for degree of differentiation and lymphovascular invasion. VEGF expressions in tumor cell were analyzed usingimmunohistochemistry staining. Bivariate statistical analysis used Fisher's Exact test and value p<0.05 was considered significant.ResultsColorectal adenocarcinoma with high grade differentiation entirely had high VEGF expression (100%), while low gradedifferantiation with high VEGF expression was 60.7%. Lymphovascular invasion positive was mostly found with high VEGFexpression (80.6%). Statistical analysis showed significant association between VEGF expression with degree of differentiation(p=0.017) and lymphovascular invasion (p=0.028).ConclusionThe conclusion was VEGF expression had significant association with degree of differentiation and lymphovascular invasion ofcolorectal adenocarcinoma.

Abstract

Background Urothelial carcinoma is the most common malignancy in the bladder with an incidence up to 95% and 70-85% of them are bladder infiltrating urothelial carcinoma. To improve the management of bladder urothelial carcinoma, prognostic factors are required. One such factor is angiogenesis, which affects growth, development and metastasis. Vascular endothelial growth factor (VEGF) is the main pro-angiogenic factor to control angiogenesis. Expression of VEGF is correlated with progression of bladder infiltrating urothelial carcinoma, such as grade and lymphovascular invasion. The aim of this study was to determine the correlation of VEGF expression with grade of differentiation and lymphovascular invasion in bladder infiltrating urothelial carcinoma. Methods This research was a retrospective observational cross sectional study with 48 cases of bladder infiltrating urothelial carcinoma in four Anatomical Pathology Laboratory in West Sumatera 2018 and 2019. Samples were reevaluated of HE slide for grade of differentiation and lymphovascular invasion. VEGF expressions in tumor cell s were analyzed using immunohistochemistry staining. Bivariate statistical analysis used Fisher's Exact test and value p<0.05 was considered significant. Results Bladder infiltrating urothelial carcinoma high grade had more positive VEGF expression (91.7%), while low grade had more negative VEGF expression (58.3%). Lymphovascular invasion positive was mostly found with positive VEGF expression (75%). Statistical analysis showed significant correlation between VEGF expression with grade of differentiation (p=0.001) and lymphovascular invasion (p=0.004). Conclusion The conclusion was VEGF expression had significant correlation with grade of differentiation and lymphovascular invasion of bladder infiltrating urothelial carcinoma.

Abstract

Population growth and changes in food consumption patterns in the community have brought social consequences in the form of increasing waste production which is increasingly diverse in terms of volume, type and characteristics of waste. Waste has a negative impact on the environment and health, pollution of river water quality can increase the number of diseases, one of which is diarrheal disease. The way to overcome the impact of waste is to reduce the generation of waste starting from the main source. Pekanbaru City Environment and Hygiene Agency said that the volume of waste generation in Pekanbaru in 2018 is 492.11 tons / day and only 3-5% of waste is managed by the community with the 3R concept (reduce, reuse, recycle). The effort of the city government to overcome the amount of waste generation is by implementing household waste management through a waste bank program. This study aims to see an overview of the management of waste banks in the city of Pekanbaru based on the results of interviews and field observations. ABSTRAK Pertumbuhan populasi dan perubahan pola konsumsi makanan di masyarakat telah membawa konsekuensi sosial dalam bentuk peningkatan produksi sampah yang semakin beragam dalam hal volume, jenis dan karakteristik limbah. Sampah berdampak negatif pada lingkungan dan kesehatan, pencemaran kualitas air sungai dapat menambah jumlah penyakit, salah satunya penyakit diare. Cara mengatasi dampak limbah adalah dengan mengurangi timbulan limbah mulai dari sumber utama. Badan Lingkungan Hidup dan Kebersihan Kota Pekanbaru mengatakan bahwa volume timbulan sampah di Pekanbaru pada tahun 2018 adalah 492.11 ton / hari dan hanya 3-5% limbah dikelola oleh masyarakat dengan konsep 3R (reduce, reuse, recycle). Upaya pemerintah kota untuk mengatasi jumlah timbulan sampah adalah dengan menerapkan pengelolaan limbah rumah tangga melalui program bank sampah. Penelitian ini bertujuan untuk melihat gambaran umum pengelolaan bank sampah di Kota Pekanbaru berdasarkan hasil wawancara dan observasi lapangan Kata kunci: gambaran; pengelolaan sampah rumah tangga; bank sampah.

Abstract

Brain Derived Neurotrhophic Factor (BDNF) is one of the proteins needed for the growth of neurons. During its development period BDNF plays a role in nerve growth, differentiation, repair, and survival of nerve cells. In addition, researchers from certain groups found that BDNF also had an important role during the implantation period, placental development and development of fetal growth. BDNF is known to have an important role in regulating angiogenesis needed for placental development. Because of this role, BDNF deficiency will cause disruption in placental growth which will eventually cause fetal growth disorders or Intrauterine growth restriction (IUGR). In recent years studies have shown that neurotrophins play an important role in the regulation of placental development and fetal growth. BDNF has been found to be expressed in large amounts in blastocysts which indicate the potential role of BDNF in implantation and development of the placenta. BDNF is also produced in the muscle tissue of one of them in the uterus precisely in the endometrium and myometrium. The discovery of the role of BDNF, is expected to be used as an indicator to assess the occurrence of growth disorders in the fetus as well as additional information about the etiology and pathophysiology of IUGR. Keywords: BDNF; IUGR; Neurotrophin

Abstract

Antenatal care or antenatal care is a planned program that is observation, education and medical treatment for pregnant women, to obtain a safe and satisfying pregnancy and childbirth process. The indicator used to assess the access of pregnant women to antenatal care is K1 direction (first visit) is the contact of pregnant women to health workers and K4 (perspective visit) is 4 or more times contact with health workers. From the data of the Padang City Health Office, it was found that the Air Tawar Puskesmas with this low level was caused by various factors. The purpose of this study was to determine factors related to the use of antenatal services. This type of research is a quantitative study with cross sectional study design with 110 respondents of third trimester pregnant women with purposive sampling technique. Data analysis was performed univariate, bivariate with Chi squre test. From the research results obtained from the five factors studied, it was found that the factors related to the use of antenatal services in freshwater health centers were access or distance with pvalue 0.009 = OR 21.676), service availability factors with a p value of 0.001 = OR 9.293, the role factor midwives with a p value of 0.001 = OR 12.302, while the family income factor, disease complaints obtained results have nothing to do with the use of antenatal services. The author's suggestion is that there is a need for good coordination with the Independent Practice Midwife (BPM), revitalize puskesmas to more strategic places and improve better facilities, improve accessibility that can be minimized, strive to reach pregnant women, especially for accessibility that is less affordable.

Abstract

Prevalence of asthma in children increase especially in the industrial city. Cement factory in Padang city is located near from community, cause air pollution, especially to children. We aim to know the prevalence and risk factor of asthma and allergic disease in this area. A cross sectional study to children age 0-15 years old has been done on May-June 2015, around the Cement factory, with distance from factory <5 km (exposed area) and >10 km (un-exposed area). We used International Study of Asthma and Allergy in Childhood (ISAAC) questionnaire to determine asthma and allergic disease. Data was taken from the parent about asthma, allergic rhinitis and atopic dermatitis symptoms and some risk factors. Data was analyzed with chi-square test. This study found 90 children, 43 (47.8%) from exposed area. Asthma prevalence and atopic dermatitis was found higher in un-exposed area than exposed area (19.5% vs 13.9% and 39.1% vs 37.2%), allergic rhinitis was higher in exposed area (60.4% vs 58.6%), though no significance difference. This study concluded that there were no significant differences in asthma and allergies disease in exposed and un-exposed area of semen factory emissions.

Abstract

Keselamatan rumah sakit saat ini telah menjadi isu global. Terdapat lima komponen penting yang terkait dengan keselamatan rumah sakit yang salah satunya adalah keselamatan pasien. Keselamatan pasien dipengaruhi oleh bagaimana budaya individu dan sistem yang berjalan di dalam organisasi tersebut. Tujuan penelitian ini adalah menganalisis budaya keselamatan pasien di RSUD dr Rasidin Padang tahun 2018. Penelitian ini menggunakan desain penelitian mix method. Pendekatan kuantitatif menggunakan kuesioner AHRQ (Agency Health Research and Quality) tahun 2004 sedangkan penelitian kualitatif menggunakan pendekatan instrumental case study. Subjek penelitian adalah seluruh petugas kesehatan yang berhubungan langsung dengan pasien sebanyak 250 sampel. Data kualitatif didapatkan melalui indepth interview kepada informan untuk menggali secara mendalam hambatan penerapan budaya keselamatan pasien. Hasil penelitian menunjukkan dimensi yang paling tinggi respon positifnya adalah dimensi supervisi (78%) dan dimensi yang paling rendah adalah frekuensi pelaporan (31%). Hambatan yang ditemui dalam pelaksanaan budaya keselamatan pasien adalah perilaku dari petugas kesehatan dan dukungan dari manajemen yang belum maksimal. Rekomendasi yang diberikan adalah pembentukan tim KPRS, edukasi dan monitoring evaluasi.
