Abstract

Patient safety is a top priority for health care facilities because it is related to the quality and good image of the hospital. The impact that occurs when health workers ignore patient safety is that patient care is prolonged, patients can experience physical and psychological trauma, disability and even death. Proper drug administration is influenced by several factors. One of these factors is the nurse behavior factor, where nurses do not apply the six principles of treatment in accordance with the Standard Operating Procedures (SOP) set by the hospital. The research aims to see the role of the Nurse in Charge in identifying the 6 correct drug administration. This research is of the analytical description type using observational methods. The population in the research were all nurses in the Anthurium 3 room with a sample of 14 respondents selected using total sampling. This study looked at the compliance of nurses in identifying the six correct principles as measured by a questionnaire on the implementation of the six correct principles of drug administration. The results of the study showed the accuracy of identifying 6 correct drugs, namely the correct drug with an accuracy of 74.1%, the correct dose of 70%, the correct time of 73.2%, the correct patient 64.2%, the correct route 87.5% and the correct documentation 65%. The implementation of the identification of the 6 correct drug administration can be carried out optimally if all nurses can carry out the protocol according to procedures based on the right drug, right time, right patient, right route, and right documentation.

New patients frequently experience fear or anxiety due to the unfamiliarity of the hospital environment, a phenomenon particularly evident among those with LABC. The introduction of welcome book serves as a potential intervention to enhance patient knowledge and alleviate anxiety. This study evaluates the effectiveness of a welcome book in enhancing patient knowledge and reducing anxiety levels. A case study at a hospital involved a 42-year-old patient with LABC. The study examined the patient's orientation using a welcome book that covered various aspects such as orientation of persons, places, facilities, rules, and educational resources. The patient's knowledge was assessed with a knowledge evaluation sheet, and anxiety levels were measured using the Hamilton Anxiety Rating Scale. The welcome book provided comprehensive information to enhance patient understanding and reduce anxiety. The patient’s pretest knowledge score was 20, which increased to 86.67 after using the welcome book, indicating a substantial improvement. Initially, the patient exhibited mild anxiety upon admission, but after four days of utilizing the welcome book, there was a significant reduction in anxiety levels. The implementation of welcome book for a new patient orientation significantly enhanced the knowledge and reduced the anxiety levels of patient with LABC. This study underscores the importance of comprehensive orientation materials, such as welcome books, in improving patient outcome and suggest hospitals to adopt similar strategies to enhance patient education adn reduce anxiety.

This causes patients to require long-term care, so they must be given discharge planning. This research aims to analyze the effectiveness of implementing discharge planning in diabetes mellitus patients to increase patient readiness to go home in the Cempaka Room at Bali Mandara Hospital. The research method used univariate descriptive analysis, and the sample used was in accordance with predetermined inclusion and exclusion criteria and then one respondent was selected. The research results showed that a 61-year-old patient with a diagnosis of diabetes mellitus who met the criteria was given discharge planning and measured using the adult Indonesian version of the patient discharge readiness scale instrument. The pre-test results showed that the patient's readiness level for discharge was 49.52% with a total score of 104 (level II). After implementing discharge planning, the level of patient readiness for discharge increased to 64.28% with a total score of 135 (level III). The implementation of patient discharge planning has proven to be effective in increasing patient discharge readiness, as evidenced by an increase in patient discharge readiness from level II to level III. Therefore, in providing nursing care, it is necessary to pay attention to the patient's detailed needs and fulfill them by implementing discharge planning so that patients have a good level of readiness to go home to reduce recurrence, complications, and patient readmissions.
