Abstract

Penatalaksanaan nyeri dan kecemasan yang dialami oleh pasien dapat dilakukan dengan pendekatan farmakologis dan non-farmakologis. Pemberian analgetik dan sedatif tidak bisa langsung menghilangkan nyeri dan kecemasan pasien bahkan jika berlebihan bisa menimbulkan depresi pernapasan dan ketidakstabilan kardiovaskuler sehingga perlu dilengkapi dengan pendekatan non-farmakologis seperti penggunaan terapi musik relaksasi dan suara alam. Tujuan penelitian ini adalah untuk mengetahui sejauh mana efektifitas musik relaksasi, suara alam dan kombinasi keduanya terhadap nyeri dan kecemasan pasien. Metode yang digunakan adalah literature review dengan studi kepustakaan dan pencarian elektronik yang menggunakan search engine EBSCOhost (MEDLINE), GALE (infotract.galegroup) dan google dengan kata kunci yang digunakan yaitu patients, anxiety, pain, relaxation, music, dan nature. Kriteria pemilihan sumber antara lain artikel, karya ilmiah atau buku yang membahas tentang teknik relaksasi, terapi musik, dan suara alam yang difokuskan pada nyeri dan kecemasan serta dipublikasikan tahun 2002-2012 dengan bahasa Indonesia atau bahasa Inggris. Hasil dari pencarian didapatkan 20 artikel penelitian (dua di antaranya systematic review) yang memenuhi kriteria. Hasil review dari beberapa artikel penelitian tersebut menunjukkan bahwa 76% perawatan standar ruangan yang dikombinasikan dengan terapi musik lebih efektif menurunkan tingkat kecemasan dan 76,2% efektif menurunkan tingkat nyeri pada pasien dibandingkan tanpa terapi musik. Tujuh piluh lima persen (75%) perawatan standar yang dikombinasikan dengan terapi suara alam lebih efektif menurunkan kecemasan dan 100% efektif menurunkan tingkat nyeri pasien dibandingkan tanpa terapi suara alam. Perawatan standar yang dikombinasikan dengan gabungan antara terapi musik relaksasi dan suara alam menunjukkan bahwa 100% efektif menurunkan nyeri dan kecemasan pasien. Intervensi keperawatan non-farmakologis seperti terapi musik, suara alam dan kombinasi keduanya bukan bersifat menggantikan fungsi manajemen nyeri dan kecemasan pasien namun sebagai pelengkap intervensi farmakologi dalam tatanan klinik.Kata Kunci: kecemasan, musik relaksasi, nyeri, suara alam

Abstract

Background:Preceptors play an important role in clinical guidance, namely guiding andeducating preceptees to become professional nurses. Preceptors have duties as educators andclinical officers who are often constrained by time and workload. When teaching, preceptorsneed methods or strategies so that learning objectives can be achieved and are effective.Objective: The aim is to analyze the competency gap for Clinical Nurses 3 in clinical guidancecapacity at Cibabat Cimahi Regional Hospital. Method: The design of this research is actionresearch. Taking total sampling, the research reached stage 2 of the 4 stages of actionresearch.Results: the results of Clinical Nurses 3 , Nurse Competency in implementing clinicalguidance for students are still low. Part A regarding student evaluation, the average score is31.319 with 22 people with high competency and 25 people with low competency. Part B isabout determining individual goals and teaching, the average score is 42.1 with 17 people withhigh competency and 30 people with low competency. Part C is about determining teachingstrategies, the average score is 60.8 with 18 people with high competency and 29 people withlow competency. Part D regarding knowledge demonstrations held, the average score was 17with 20 people with high competency and 27 people with low competency. Abstract

Background: Early Warning Score System (EWSS) is a system to assess changes in patient conditions in order to respond quickly when deterioration occurs and has been implemented since 2018. At the beginning of implementation, regulations have been prepared, education has been carried out supported by the use of the Hospital Management Information System (SIMRS). The results of the investigation, Early Warning Score (EWS) is carried out routinely and handover is not optimal. Doctor interviews, stated that EWS was reported only with a red grading. The achievement of EWS monitoring in 235 medical records, 85% were not carried out and EWS follow-up was yellow grading 0%. Improvement efforts have been made through coordination meetings, but have not been evaluated.Objective: The study aims to analyze the implementation of EWSS and compile improvement efforts.Method: The research method is wide, participatory and responsive action research model Kurt Lewin with stages of planning, action and fact finding. The study is limited to the planning stage. The sample uses total sampling of EWS assessment in medical records based on the number of beds and the number of minimum actions for EWS assessment in the first 24 hours of inpatient admission. Data collection was carried out by reviewing medical records followed by Focus Group Discussion (FGD) to strengthen the data. The medical record review was analyzed using Quantitative analysis (descriptive statistics) while the FGD used qualitative analysis followed by the preparation of the Plan of Action (POA).Results: The findings of the medical record review were grouped based on the EWS assessment process flow in inpatient care, while the FGD was grouped according to its theme.Optimal nurse understanding was not related to the implementation of EWS where 57.5% of patients' EWS assessments, 7.9% were not assessed for 24 hours and the EWS assessment time span since being admitted to inpatient care was 11 hours 22 minutes. The data is related to the results of the FGD related to factors that influence its implementation, namely vital signs regulation, supervision, critical thinking, and SIMRS is not yet user friendly. The problems that emerged and were compiled in the POA were TTV measurement regulations, EWS assessments were not optimal, EWSS supervision was still weak and SIMRS was not yet user friendly. Conclusion: The results of the review of medical records of new patients were mostly admitted in the afternoon shift (49.8%), EWS assessment of new patients 57.5%, since the first 24 hours of new patients being hospitalized 7.9% were not given an EWS assessment, EWS grading yellow which was re-assessed (2x/shift) 10.8%), the range of EWS assessments since being admitted to hospital was 11 hours 22 minutes. Four themes of results: EWS as early detection, technical understanding of EWS assessment, EWSW implementation in inpatients and factors influencing EWS implementation. The problems that emerged and the POA drafted for TTV measurement regulations were not yet in sync with EWS assessment regulations, EWSS supervision was still weak, EWS assessment was not optimal, SIMRS was not user friendly.Suggestion: Research recommendations are to improve critical thinking, increase medical team involvement, improve supervision, review EWS regulations and implementation of POA that has been drafted.
