Abstract

TB Paru merupakan penyakit menular yang menjadi masalah kesehatan serius di dunia. Agar tidak terjadi penularan yang terus-menerut pasien TB Paru harus patuh menjalani pengobatan sampai tuntas. Salah satu intervensi yang dapat dilakukan agar pasien TB Paru dapat patuh menjalani pengobatan yaitu dengan mobilephone reminder. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh penerapan mobilephone reminder terhadap kepatuhan pengobatan pasien TB Paru. Jenis penelitian yang digunakan dalam penelitian ini adalah quasy experimental dengan pretest and posttest with control group design. Pada penelitian ini sampel yang digunakan sebanyak 60 responden, sebanyak 30 responden kelompok kontrol dan 30 responden kelompok intervensi. Pengambilan sampel menggunakan teknik purposive sampling. Instrumen yang digunakan dalam mengukur kepatuhan pengobatan pasien TB Paru yaitu The 8-item Morisky Medication Scale (MMAS-8). Analisis data menggunakan uji wilcoxon test dan mann whitney test. Hasil uji wilcoxon test pada penelitian ini menunjukkan bahwa kelompok eksperimen memiliki nilai p 0,000 dan kelompok kontrol nilai p 0,105 kemudian kedua kelompok diuji beda menggunakan mann whitney test menunjukkan hasil nilai p sebesar 0,000 dimana hasil tersebut bermakna bahwa ada pengaruh yang signifikan mobilephone reminder terhadap kepatuhan pengobatan pasien TB Paru. Sehingga dengan adanya mobilephone reminder kepatuhan pengobatan pasien TB Paru dapat semakin meningkat dan mencegah terjadinya penularan yang terus-menerus di dalam keluarga maupun di masyarakat.

Abstract

Gout artritis merupakan salah satu bentuk penyakit sendi yang umum dialami oleh lansia, ditandai dengan nyeri hebat, pembengkakan, dan keterbatasan aktivitas. Penatalaksanaan non-farmakologis menjadi salah satu pendekatan alternatif yang dapat digunakan untuk mengurangi keluhan nyeri. Penelitian ini bertujuan untuk mengetahui efektivitas terapi kompres jahe hangat kombinasi akupresur di titik Hegu dan Taixi terhadap penurunan tingkat nyeri pada lansia dengan gout artritis, melalui pendekatan One Group Pretest-Posttest Design. Intervensi yang diterapkan dalam penelitian ini dikenal dengan istilah OMJAHATKU (Olah Mandiri Jahe Hangat Akupresur Taixi Hegu untuk Kesehatan Urat). Sampel terdiri dari 14 lansia yang mengalami gout artritis di Kelurahan Rawasari, Jakarta Pusat, yang dipilih secara purposive. Intervensi dilakukan selama tujuh hari berturut-turut. Pengukuran tingkat nyeri dilakukan sebelum dan sesudah intervensi menggunakan skala numerik. Hasil uji paired t-test menunjukkan adanya penurunan signifikan tingkat nyeri setelah dilakukan intervensi (p < 0,05). Terapi kompres jahe hangat kombinasi akupresur terbukti efektif dalam mengurangi nyeri pada lansia dengan gout artritis. Pendekatan OMJAHATKU dapat menjadi alternatif terapi mandiri yang mudah diterapkan dalam kehidupan sehari-hari.

Abstract

Hipertensi merupakan penyakit tidak menular dengan prevalensi tinggi yang dipengaruhi oleh faktor gaya hidup, termasuk aktivitas fisik dan status berat badan. Pendekatan non-farmakologis berbasis bukti seperti latihan fisik Tai Chi dan program penurunan berat badan dinilai efektif dalam mengendalikan tekanan darah. Penelitian ini menggunakan desain quasi-experimental dengan pendekatan pre-post one group design. Intervensi dilakukan melalui program STDASH (Senam Tai Chi dan Diet DASH) selama 12 minggu kepada 10 responden dewasa dengan hipertensi di wilayah Kelurahan Rawasari, Jakarta Pusat. Pendekatan Evidence-Based Nursing Practice (EBNP) digunakan untuk mendukung pelaksanaan intervensi. Data dianalisis menggunakan uji normalitas, Levene Test, dan Paired T-Test dengan tingkat signifikansi p < 0,05. Karakteristik mayoritas responden adalah perempuan (90%) dengan usia dewasa tengah dan status IMT overweight (80%). Sebagian besar memiliki riwayat hipertensi <5 tahun dan aktivitas fisik sedang. Setelah intervensi, terjadi penurunan rerata tekanan darah sistolik dan diastolik secara signifikan, namun tidak ditemukan perubahan bermakna pada berat badan responden. Hal ini menunjukkan bahwa intervensi kombinasi Tai Chi dan diet DASH efektif menurunkan tekanan darah, tetapi belum cukup untuk menurunkan berat badan dalam durasi intervensi yang sama. Program STDASH berbasis EBNP efektif dalam menurunkan tekanan darah pada penderita hipertensi, namun diperlukan durasi intervensi yang lebih lama atau strategi tambahan untuk mencapai penurunan berat badan yang signifikan.

Abstract

Acute exacerbation in chronic obstructive pulmonary disease (COPD) is characterized by increased production and accumulation of mucus which aggravates clinical symptoms. One of interventions well-known effectively overcoming airway clearance problem is active cycle of breathing technique. The aim of implementing evidence-based nursing practice was to determine the effectiveness of the active cycle of breathing technique on sputum production in COPD patients. The search for evidence was carried out by PICO (problem, intervention, comparison, and outcome) analysis. 18 COPD inpatients and outpatients were selected based on inclusion and exclusion criteria and were given the intervention. Sputum production was measured before and after it. The data analysis was carried out using Wilcoxon with a p value of 0.014 which showed that there was a difference in the amount of sputum production before and after the intervention. The researchers concluded that active cycle of breathing technique is an effective intervention in overcoming airway clearance problems in COPD patients.

Abstract

Diabetes Mellitus is a chronic condition characterized by impaired insulin production, leading to hyperglycemia. Physically and psychologically, Diabetes Mellitus can cause stress and fluctuations in blood sugar levels. This application aims to determine the effect of Progressive Muscle Relaxation (PMR) on stress and blood sugar levels in Diabetes Mellitus patients through the evidenced based nursing practice. This study used a quasi-experimental method with one group pretest posttest design. Data collection was carried out using the Diabetes Distress Scale (DDS-17) and blood sugar level measurements. Determination of the sample using purposive sampling of 25 Diabetes Mellitus patients. This study showed that there was a decrease in stress scores of 0.162 points and a decrease in blood sugar levels of 8.12 mg/dl after the implementation of Progressive Muscle Relaxation for 3 days. Progressive Muscle Relaxation has been proven to be effective in reducing stress and blood sugar levels in patients with Diabetes Mellitus and can be used as an evidence-based intervention in nursing care for patients with Diabetes Mellitus.

Abstract

Diabetes mellitus is a metabolic disorder characterized by increased blood sugar levels (hyperglycemia) caused by lack of insulin, inability of insulin to work or both. Lack of education or information from health workers can have an impact on patient compliance with treatment, which will result in repeated treatment. This innovation program aims to reduce the risk of readmission in Diabetes Mellitus patients by providing integrated education using the DiaTOHC method, patients who are screened using the DERRI application if the score is > 27%, patients are at risk of readmission, then they will be given education using the DiaTOHC method, which is integrated education. When patients are treated, they will be given education on the 5 pillars, after the patient goes home during the control, they will be reviewed whether there are any obstacles or obstacles in the treatment, then in weeks 2, 3 and 4 the patient will be contacted by telephone for re-education and evaluation of treatment. The results of the trial of this program from 17 samples were successful, all of which were readmissions, it was concluded that there was effectiveness in providing education using the DiaTOHC method in Diabetes Mellitus patients to reduce the risk of readmission. Data is collected by interview, observation and filling out questionnaires, after the data is collected, data analysis is carried out by inputting all data found in excel software and then assessing the percentage of patients who are readmitted.

Abstract

Diabetic Foot Care is primary prevention in diabetic foot management to prevent injury to the occurrence of wounds on the feet of people with diabetes mellitus. Objective: To prove that educational interventions can prevent chronic complications, namely diabetic neuropathy (motor, sensory and autonomic) or death of the nerves in the feet, often exacerbated by infection. Method: This study used the Quasi Experiment method with the One Group pretest and posttest design method . Data collection was carried out using NAFF (Nottingham assessment of Functional Footcare) and the Attitude instrument modified by the Knowledge, Attitude and Practice (KAP) questionnaire. Determination of the sample using the accidental sample method of 24 people in Type 2 Diabetes Mellitus patients. Results: This study showed an increase in value of 11.12 points on NAFF and an increase of 1.41 points on attitudes after educational interventions related to diabetic foot care were carried out from May 30, 2024 to June 7, 2024. Conclusion: Diabetic Foot Care Education Intervention has been proven effective in reducing complications in diabetic feet and can be used as an evidence-based intervention in preventive care of diabetic feet in patients with Diabetes Mellitus.

Abstract

Urinary incontinence is a common complication following Transurethral Resection of the Prostate (TURP) for benign prostatic tumors, which significantly impacts the quality of life of patients. Kegel exercises have been proven effective as a non-pharmacological intervention to improve bladder control and strengthen the pelvic floor muscles. This study aims to evaluate the effectiveness of Kegel exercises in reducing urinary incontinence levels in post-TURP patients. Objective to assess the effectiveness of Kegel exercises in reducing the severity of urinary incontinence in post-TURP patients. This study used a quasi-experimental design with a pre-test and post-test approach on 15 post-TURP patients at RSUD Labuang Baji Makassar. Respondents performed Kegel exercises for 7 days with a frequency of 6 sessions per day. Urinary incontinence levels were measured using the ICIQ-UI Short Form questionnaire before and after the intervention. Data analysis was conducted using the dependent t-test. Before the intervention, the average urinary incontinence score was 24.27 (±2.712), which decreased to 21.73 (±3.217) after the intervention, with a p-value of < 0.001. The effect size of 0.915 indicates a significant effect. Kegel exercises are effective in reducing the severity of urinary incontinence in post-TURP patients. This intervention is recommended as a non-pharmacological therapy to improve patients' quality of life.

Abstract

Fatigue is one of the most common symptoms experienced by patients undergoing hemodialysis therapy. This condition can affect the quality of life of chronic patients, because reduced physical activity, such as sedentary habits, and emotional stress can also be the cause of fatigue. Intradialitic exercise has various benefits in reducing fatigue experienced by patients. This study aims to evaluate the effect of exercise training on the level of fatigue in patients undergoing hemodialysis therapy. The study used a design Quasi-experimental research using the pre-test and post test group design. The research population was all patients with end-stage renal failure with the sampling technique being accidental sampling, with amount sample of 22 patients. Intradialytic exercises were performed for six weeks at a frequency of twice a week, with each session lasting a maximum of 20 minutes. Fatigue levels were measured using the Functional Assessment of Chronic Illness Therapy (FACIT) questionnaire. Data analysis was performed using the dependent t-test. The results of the study showed that the average level of fatigue before the intervention is 26.36 (SD ± 3.580 ), while the average score is the fatigue level after intervention is 32.09 (SD ±4.116 ) with a p value of 0.001, the effect size is -3.378, then including in category effect tall based on Cohen'. There is significant difference between fatigue levels before and after interdialytic exercise in patients with chronic renal failure undergoing hemodialytic therapy.

Abstract

Type 2 Diabetes Mellitus (T2DM) is a chronic metabolic disease whose prevalence continues to increase both globally and nationally, and is commonly experienced by women with obesity. Community-based non-pharmacological interventions that focus on education and physical activity are needed to support the management of this disease. One approach used is the implementation of Evidence-Based Nursing Practice (EBNP) through the Education and Aerobic Movement to Burn Blood Sugar (Gerobak Balada) program. This study aims to analyze the effect of the Gerobak Balada intervention on reducing blood glucose levels in clients with diabetes mellitus and obesity. This is a quasi-experimental pre-post study with an intervention group. The intervention was delivered over 12 sessions between October and December 2024 to 31 DM clients in Rawasari Subdistrict using purposive sampling technique. Data were collected using instruments including random blood glucose (RBG) measurements with Codefree test strips, questionnaires on knowledge, behavior, and frequency of treatment visits. Data were analyzed using paired t-tests. A significant reduction was observed in RBG levels from 304.39 ± 68.10 mg/dL to 236.45 ± 60.72 mg/dL (p < 0.001). There were also significant increases in knowledge scores (from 69.90 to 85.68; p < 0.001), behavior scores (from 66.81 to 77.03; p < 0.001), and treatment visit frequency (from 1.68 to 2.16 times; p = 0.023). The implementation of Gerobak Balada proved effective in lowering blood glucose levels and improving knowledge, healthy behavior, and medication adherence in clients with diabetes and obesity. This intervention supports the EBNP approach and has the potential to be integrated into community-based DM prevention programs.
