Abstract

Abstrak: Gangguan kesehatan jiwa adalah penyebab utama kecacatan pada generasi muda diseluruh dunia. Prevalensi permasalahan kesehatan jiwa dikalangan remaja meningkat seiring bertambahnya usia. Masa transisi perubahan remaja yang membingungkan, membuat remaja terpapar dengan berbagai stressor. Kegiatan ini bertujuan untuk memberikan pelatihan keterampilan deteksi dini masalah kesehatan jiwa remaja dan keterampilan manajemen stress remaja kepada kader remaja sehat jiwa di sekolah menengah pertama negeri (SMPN) 4 VII Koto Sungai Sariak Padang Pariaman guna pencegahan masalah kesehatan jiwa pada remaja. Sasaran utama adalah siswa SMPN 4 VII Koto Sungai Sariak berjumlah 30 orang. Metode kegiatan dengan memberikan pelatihan tentang deteksi dini masalah kesehatan jiwa menggunakan intrumen pelaporan mandiri kesehatan remaja IDASS-Y dan workshop manajemen stress pada remaja. Evaluasi kegiatan menggunakan kuisioner pre dan post-test terkait keterampilan siswa dalam melakukan deteksi dini masalah kesehatan jiwa remaja dan manajemen stress. Setelah pelatihan sebagian besar keterampilan kader remaja sehat jiwa mengalami peningkatan. Keterampilan deteksi dini masalah kesehatan jiwa remaja meningkat menjadi 73,3%, Keterampilan teknik nafas dalam meningkat menjadi 100%, keterampilan teknik distrasi meningkat menjadi 86,7%, keterampilan teknik hypnosis 5 jari meningkat menjadi 93,3%, dan keterampilan teknik spiritual meningkat menjadi 83,3%. Kesimpulan: kader remaja sehat jiwa yang telah dibentuk memiliki keterampilan dalam kategori baik.Abstract: Mental health disorders are the main cause of disability in young people throughout the world. The prevalence of mental health problems among adolescents increases with age. The transition period of adolescent change is confusing, exposing teenagers to various stressors. This activity aims to provide training in early detection skills for adolescent mental health problems and adolescent stress management skills to mentally healthy adolescent cadres at state junior high school (SMPN) 4 VII Koto Sungai Sariak Padang Pariaman in order to prevent mental health problems in adolescents. The main target is 30 students of SMPN 4 VII Koto Sungai Sariak. The activity method is to provide training on early detection of mental health problems using the IDASS-Y adolescent health self-reporting instrument and stress management workshops for adolescents. Evaluation of activities using pre and post-test questionnaires regarding students' skills in early detection of adolescent mental health problems and stress management. After training, most of the skills of mentally healthy youth cadres have improved. Early detection skills for adolescent mental health problems increased to 73.3%, deep breathing technique skills increased to 100%, distraction technique skills increased to 86.7%, 5 finger hypnosis technique skills increased to 93.3%, and spiritual technique skills increased to 83.3%. Conclusion: the mentally healthy youth cadre that has been formed has skills in the good category.

Abstract

Stroke adalah kondisi yang terjadi ketika pasokan darah ke otak mengalami gangguan atau berkurang akibat penyumbatan atau pecahnya pembuluh darah. Berdasarkan data dari Puskesmas Gunung Kota Padang Panjang didapatkan Pasien yang mengalami stroke pada tahun 2024 adalah sebanyak 40 orang. Tujuan penulisan ini adalah untuk menerapkan asuhan keperawatan pada Tn.D dengan Stroke di Sago yang meliputi pengkajian, intervensi, implementasi, evaluasi, dan dokumentasi keperawatan dengan memfokuskan pada penerapan strategi pelaksanaan terapi fisik yaitu tindakan Range Of Motion (ROM). Asuhan Keperawatan dilakukan pada Tn.D dengan menggunakan metode wawancara dan observasi. Setelah dilakukan Asuhan keperawatan diagnosa keperawatan yang muncul pada Tn.D adalah gangguan mobilitas fisik berhubungan dengan gangguan neuromuskular, gangguan komunikasi verbal berhubungan dengan penurunan sirkulasi serebral, defisit perawatan diri berhubungan dengan gangguan muskuloskeletal, defisit pengetahuan berhubungan dengan kurang terpapar informasi. Berdasarkan hasil pengkajian yang telah dilakukan pada Tn.D setelah diberikan tindakan Range Of Motion (ROM) selama 4 hari didapatkan hasil evaluasi yang disimpulkan bahwa pada Tn. D masalah yang timbul sudah mulai teratasi. Diharapkan Tn.D atau keluarga agar lebih disiplin lagi dalam menerapkan intervensi yang telah penulis edukasikan sehingga dapat meminimalkan berkembangnya angka kejadian dan resiko yang mungkin terjadi dan mempertimbangkan berbagai masalah fisik maupun psikososial yang akan dihadapi oleh pasien.Kata kunci           : Asuhan Keperawatan, Stroke, Range Of Motion (ROM)

Abstract

Risk of violent behavior is a condition in which individuals have the potential to harm themselves, others, or the environment physically and psychologically. This condition is commonly found in patients with mental disorders, especially schizophrenia, and requires appropriate psychiatric nursing care. This study aimed to implement psychiatric nursing care for patients at risk of violent behavior at Wisma Nuri Prof. HB. Saanin Psychiatric Hospital Padang. The research method used was a case study with a nursing process approach including assessment, diagnosis, intervention, implementation, and evaluation. The assessment results showed that the patient experienced labile emotions, irritability, sharp gaze, high-tone speech, and aggressive behavior. The main nursing diagnosis established was risk of violent behavior with a secondary diagnosis of self-care deficit. Interventions were carried out through implementation strategies including deep breathing exercises and hitting pillows, medication adherence, adaptive verbal communication, and spiritual approaches. After nursing care was implemented, the patient was able to control emotions and reduce aggressive behavior independently. The implementation strategy proved effective in helping patients control the risk of violent behavior and improve self-care abilities.

Abstract

The risk of violent behavior is a psychiatric nursing problem in patients with severe mental disorders due to the inability to control emotions and aggressive impulses that can harm themselves, others, and the environment. This study aims to implement psychiatric nursing care for patients at risk of violent behavior at Wisma Cendrawasih, Prof. Dr. HB Saanin Mental Hospital, Padang, in 2026. The research method used a descriptive case study with a nursing process approach through assessment, nursing diagnosis, intervention, implementation, and evaluation. Data collection was conducted through interviews, observation, and documentation on one patient. The assessment results indicated that the patient experienced emotional instability, irritability, aggressive behavior, and auditory hallucinations. Nursing interventions provided included deep breathing exercises and pillow hitting, adaptive verbal communication, medication education, and a spiritual approach. The results showed an increase in the patient's ability to control emotions, decreased aggressive behavior, and improved communication skills. It was concluded that comprehensive psychiatric nursing care was effective in helping reduce signs and symptoms of the risk of violent behavior and increasing the patient's self-control.

Abstract

World Health Organization (WHO) data for 2023 shows that the number of people with mental disorders worldwide continues to increase, reaching 970 million people in 2023 and predicted to exceed 1.1 billion by 2024. One of the symptoms that frequently emerges is hallucinations, where sufferers experience false sensations without any real external stimulus. This case study aims to describe psychiatric nursing care for patients with a diagnosis of auditory hallucinations at RSJ Prof. Dr. HB Sa’anin Padang in 2026. The research method used a case study through a nursing process approach consisting of assessment, diagnosis, intervention, implementation, and nursing evaluation. The results of the assessment showed that the patient experienced auditory hallucinations, social isolation, and self-care deficits triggered by social stressors such as divorce trauma and economic problems. Interventions were carried out through the application of Implementation Strategy (SP) 1-4 which included rebuking (menghardik), medication adherence, conversation, and scheduled activities. Evaluation results after five days of action showed an increase in the client's ability to control hallucinations independently and improvements in self-care independence. In conclusion, the consistent application of implementation strategies is effective in reducing hallucination symptoms in clients. This study is expected to serve as a reference for nursing staff in optimizing comprehensive psychiatric nursing care for clients with sensory perception

Abstract

The risk of violent behavior is one of the angry responses expressed by making threats, injuring oneself or others and can damage the surrounding environment. Based on data from medical records at Prof. HB. Saanin Mental Hospital, Padang, the nursing problem in the last three years is the risk of violent behavior with a total of 573 clients. The purpose of this case study is to be able to apply psychiatric nursing care to patients with a risk of violent behavior at Prof. HB Saanin Mental Hospital, Padang. The application method used is a case study conducted on the client by approaching using the nursing process, namely assessment, diagnosis, intervention, implementation and evaluation. The results of the assessment showed that the client experienced signs and symptoms of the risk of violent behavior in the form of damaging household furniture, breaking glass, sharp eyes, and being easily offended. The main diagnosis that emerged was the risk of violent behavior with a companion diagnosis of self-care deficit. After nursing care is carried out, the client can do it independently through the Implementation Strategy, namely, SP 1 trains how to control anger by doing physical exercises 1 and 2 (Deep breathing and hitting a pillow), SP 2 by taking medication correctly, SP 3 is speaking verbally (expressing, asking and refusing correctly) and SP 4 is training spiritually/doing 2 selected activities (reciting dhikr and istighfar). It is hoped that through this research, everyone can realize the importance of maintaining mental health.

Abstract

Hallucinations are a condition in which a person loses the ability to distinguish between internal and external stimuli, resulting in the perception of the environment without any real objects or stimuli. This condition is often found in patients with mental disorders and can affect social functioning and quality of life. Based on medical records at Prof. DR. HB Sa’anin Mental Hospital, Padang, the most common nursing problem in the past three years has been hallucinations, with 17,591 patients. The purpose of this case study is to apply psychiatric nursing care to patients with auditory hallucinations at Prof. DR. HB Saanin Mental Hospital. HB Saanin Padang. The method used in this scientific paper is a case study with a nursing process approach, including assessment, diagnosis, planning, implementation, and evaluation. The assessment results indicated that the patient experienced signs and symptoms of hallucinations, including hearing whispers. The primary nursing diagnosis was hallucinations, with a co-diagnosis of self-care deficit (SDC). After nursing care, the patient was able to address self-care issues and control hallucinations independently through the implementation of Implementation Strategies (SP): SP 1: practicing reprimands, SP 2: taking medication regularly, SP 3: having conversations, and SP 4: carrying out scheduled daily activities. These results indicate that the implementation of implementation strategies is effective in helping the patient control hallucinations and improving self-care skills. It is hoped that through this research, everyone can understand the importance of psychiatric nursing care, thereby increasing awareness in maintaining and preserving mental health, and adopting behaviors that support optimal mental well-being in daily life.
