Abstract

In type 2 diabetes mellitus, there are changes in hemostasis components, including overexpression of PAI-1. By these facts, the authors are interested in conducting the study of PAI-1 level in diabetic patients with and without foot ulcer. Methods: The sample of this research was collected cross-sectionally on 20 type 2 diabetic patients with foot ulcer and 20 without foot ulcer. Blood samples were taken to measure serum PAI-1 level, complete blood count and hemostasis screening test. Results: Statistical analysis showed there are significant differences in hemoglobin, trombosit, Fibrinogen and D-dimer levels in the ulcer and non-ulcer groups but no significant difference in PAI-1 level. Conclusions: No higher PAI – 1 level were found in diabetic foot ulcer group than those without diabetic foot ulcer. There was no significant difference between PAI-1 level and grade of diabetic foot ulcer.

Abstract

BACKGROUND: Elevated blood glucose level is a major factor in development of diabetic complications due to unfavorable hyperglycemic induced biochemical as well as hematological indices changes. The aim of this study was to evaluate the correlation between glycemic characteristic and erythrocyte indices in obese subjects with different glycemic status.METHODS: Cross cross-sectional study was designed, and 80 obese subjects were enrolled. The correlations between glycemic characteristic (fasting plasma glucose (FPG), postprandial plasma glucose (PPG), hemoglobin A1c (HbA1c) and homeostasis model assessment of insulin resistance (HOMA-IR)) and erythrocyte indices (Hb, red blood count (RBC), mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH) and mean corpuscular hemoglobin concentration (MCHC)) were evaluated.RESULTS: Of 80 obese subjects with different glycemic status, there were 48 patients with only obesity (HbA1c <5.7%), 19 patients with pre-diabetes (HbA1c 5.7-6.4%) and 13 patients with diabetes (HbA1c >6.4%). Glycemic characteristic and profile lipid (high-density lipoprotein cholesterol (HDL-C), triglycerides (TG)) were differ significantly in the different HbA1c level. Erythrocyte indices were not differ significantly in the different HbA1c level. Partial Spearman's correlation analysis showed that only MCV was significantly correlated with glycemic characteristic of FPG, PPG, HbA1c and HOMA-IR (r=-0.36, p=0.001; r=-0.29, p=0.007; r=-0.27, p=0.014 and r=-0.236, p=0.035; respectively).CONCLUSION: MCV was significantly correlated with glycemic characteristic (FPG, PPG, HbA1C and HOMAIR). Abstract

BACKGROUND Mean platelet volume (MPV) shows the average size of platelets in the circulation and can be used to assess the activity of platelets. Increased of MPV can be sign of atherotrombosis and can be used to assess the risk of vascular complications such as retinopathy diabetic (RD). The aim of this study was to determine correlation between glycated haemoglobin (HbA1c), MPV with degree of retinopathy in diabetes mellitus (DM) patients. METHODS A cross sectional study from June until July 2013 was conducted involving 77 DM subjects. Anamnesis, laboratory examination and funduscopy had been done in all subjects. One way ANOVA was used to assess the differences between MPV with other parameters to the degree of retinopathy and continued with Bonferroni test to assess the differences between the degree of retinopathy with the parameters that significantly different in one way ANOVA. RESULTS One way ANOVA showed significance difference of mean MPV between normal funduscopy group compared than proliferative diabetic retinopathy (PDR) group (9.57 ± 0.63 fl vs 10.45 ± 0.51 fl, p=0.044). PDR group were older (p=0.001), longer suffered from DM (p=0.001) and hypertension (p=0.011). Bonferroni test showed no significance difference of mean MPV between normal funduscopy versus non-proliferative diabetic retinopathy (NPDR) group (p=0.290) and NPDR versus PDR (p=0.409). CONCLUSION There was a significance differences between MPV with the degree of retinopathy. Platelets may play a role in the pathogenesis of vascular complications and that MPV can be used as simple parameter to assess the vascular events in DM.

Abstract

Background: Thrombosis is one of causes increasing morbidity, disablement,and mortality in ischemic stroke’s patient. The basic mechanism of thrombosis is hypercoagulation state, so by finding it earlier can prevent and reduce thrombosis that cause ischemic stroke.Objective: This study to asses the event of hypercoagulation between ischemic stroke’s patient and non stroke. Methods: A total of 21 subjects with ischemic stroke’s patient and 21 subjects with non stroke entered into the study. All participants were examined for hemostatic parameters included prothrombin time (PT), INR, activated partial thromboplastin Time (aPTT), Fibrinogen, D-dimer. To compare the result of hemostatic both of groups were used t independent test if the distribution was normal or used Mann Whitney test if the distribution wasn’t normal. The result would be significant if P <0.05. Result: According to characteristic of population, it was found some variables significantly, included systolic blood pressure, diastolic blood pressure, INR, aPTT ratio and D-dimer. Hypercoagulation in ischemic stroke group was found 47.6% (n=10) and non hypercoagulation group was 52.4% (n=11) , but in non stroke group, hypercoagulation was found about 23.8% (n=5) and non hypercoagulation was 76.2% (n=16). According to statistical analysis there wasn’t significant hypercoagulation between ischemic stroke’s patient and non stroke (P >0.05).Conclusion: There isn’t significant hypercoagulation between ischemic stroke group with non stroke group. Abstract

Introuction:In hemodialysis patients conditionsinflammation can play a main role in causing hiporesponsif erythropoietin therapy. Interleukin - 6 (IL-6) was a predictor of mortality that is more powerful than other inflammatory markers.To determine correlation between Interleukin-6 with respon Erythropoirtin Stimulating Agent (ESA) which assessed based on changes of hemoglobin level before and after ESA treatment to regular hemodialysis patients.Methods: The cohort study of experimental methods of measurement data pre and post test from December 2014 to Mei 2016with 50 hemodialysis patients do a complete blood count, interleukin - 6 and iron profiles before and 1 month after the intervention. Patients were given injections of Recombinant Human Erythropoietin(rHuEPO) for 1 month subcutaneously at a dose of 2,000-4,000 each time the hemodialysis procedure. Kolmogorov-Smirnov test was used to determine the distribution of the data. Spearman assess the association between interleukin - 6 with an increase hemoglobin before and after be given erythropoietin therapy.Results : The mean levels of IL - 6 was 20.24 pg/mL and the mean increase in hemoglobin after erythropoietin administration of 0.56 mg/dL. Found a significant negative correlation (r = -0.287 ; p = 0.043) between IL - 6 and increased hemoglobin level, the higher the levels of IL-6, then the increase of hemoglobin level after erythropoietin administration will be lower than expected. Conclusion : In our trial, we found a negative correlation between Interleukin-6 with hiporesponsifity erythropoietin in reguler hemodialysis patientsKeywords : erythropoietin, inflammation, IL-6, hemodialysis, hemoglobin

Abstract

Introduction : Health-related quality of life’s (HRQOL) assessment has become a vital tool. In this study, we analyze two generic instruments ECOG Scale and SF-36 in NHL patients receiving CHOP chemotherapy. This study intends to find out the the correlation between SF-36 and ECOG scale in NHL patients receiving CHOP chemotherapy.Method : Cross sectional research was conducted from October 2012 up to December 2013. Samples were selected consecutively to NHL patients. SF-36 and ECOG scale was assessed after CHOP chemotherapy cycle 6.Results : There were 21 patients of NHL, 12 male, 9 female, age 20 – 64 years old, mean 47.95 (SD 10.85). The range score of SF-36: 23.9 – 95.9, mean 75,3 (SD 17.93). The range scale of ECOG 0 – 4, mean 0.81 (SD 1.12). By using Spearman correlation, we found a significant correlation between SF-36 and ECOG scale (r = - 0.744, p = 0.0001). SF-36 showed good diagnostic performance to predict good ECOG scale with cut-off •'3d 54.66, with sensitivity 94.7%, specificity 100%, PPV 100%, NPV 66.67%, LR (+) ~, LR (-) 0.05, accuracy 95.23%, and AUC 98.7% (95% CI: 0% - 100%; p = 0.027).Conclusion : SF-36 correlates with ECOG scale and has a very good diagnostic in predicting ECOG scale.Key word : SF-36; ECOG scale; Quality of life; Non Hodgkin Lymphoma; CHOP chemotherapy

Abstract

Introduction : Renal failure is condition where kidney cannot fitter blood waste and toxins adequately. One of complication is bleeding diathesis causes insufficiency of platelet function, coagulation cascade or fibrinolysis system activation. Method :Analytic observaiional research from September-Dece mber 2014 to 31 pre-dialyzed subject.Conclusion :Nocorrelation among high serum creatinine and urea with platelet count. bleeding lime,and platelet aggregation. Nocorrelation between platelet count and function (bleeding time and platelet aggregation).Result : Of 31 subject, mean of c;ge 50.71(S0=10.91) years old; Platelet 250,548 (SD=98,268)fmm3 with thrombocytopenia 19.4% case; bleeding time 4.77 (SD=3.03) minutes;with prolong bleeding time 19.4% case and 3.2% of it is thrombocytooe nia; hypoaggregation, 19.4%case. No significant correlation between platelet count and platelet function;among serum creatinine with platelet count and function;and urea with platelet count and functions. Abstract

Blood transfusion is one of the methods to treat anemia and packed red blood cells (PRC) are the most widely used blood component. Post transfusion results showed that increment of hemoglobin (Hb) does not always match the amount of PRC given. This is thought to be due to the recipient's immune reaction to the PRC component. Leukoreduction is an action to reduce leukocytes from the PRC component thereby reducing the risk of an immune reaction. Alternatively, another method called washed PRC can also be used. In this study, the researcher studied the effectivity of leukoreduction and washed PRC on Hb increase in post-transfusion anemia patients compared with the usual PRC. The researcher used a pre-test post-test group design method of 60 people who were divided into 3 groups (ordinary PRC, washed PRC, leukofiltered PRC). The results showed all three groups reached the transfusion target (a 75% increase in Hb) and there were no differences among the groups both in terms of achievement of the Hb target and the incidence of side effects. There were no differences in the success of transfusion and incidence of side effects among the three groups.

Abstract

Background: Disseminated Intravascular Coagulation (DIC) is a serious complication frequently found in patients with acute leukemia and is associated with increased early mortality. The ISTH-DIC score is widely used to assess the severity of DIC. However, data on the association between ISTH-DIC score and early mortality in acute leukemia patients in Indonesia remains limited. Objective: To evaluate the association between ISTH-DIC score and early mortality (≤60 days) in acute leukemia patients at H. Adam Malik General Hospital, Medan. Methods: This was a retrospective cohort study involving 68 adult patients diagnosed with acute leukemia admitted from January 2022 to June 2024. Medical records were reviewed to collect laboratory parameters (platelet count, prothrombin time [PT], fibrinogen, and D-dimer) to calculate the ISTH score. Statistical analysis was performed using bivariate and multivariate logistic regression. Results: The early mortality rate was 58.8%. A substantial proportion of patients had ISTH-DIC scores ≥5, but no statistically significant association was found between ISTH score and early mortality (p = 0.704). However, PT >1.2 seconds was significantly associated with early mortality (p < 0.001; OR 18.00). Platelet count, D-dimer, and fibrinogen levels showed no significant association. Conclusion: The ISTH-DIC score was not significantly associated with early mortality in acute leukemia patients, but individual components such as prolonged PT showed a strong correlation. Early coagulation assessment remains essential in identifying patients at risk of life-threatening complications.
