Abstract

During 4 years periode from 2011 to 2014 work-related accidents always increased. In 2014 there were 105,383 work-related accidents. The work accident data is dominated by the construction sector. The aim of this study was to analyze the factors associated with work related accidents in construction. This study was conducted by qualitative study design with a case study approach using Fault Tree Analysis (FTA). The population of this study were all construction workers as many as 140 people. The main informants in this study were determined by purposive sampling, workers who had experienced work accidents in May-June 2014. Data collection techniques used in-depth interviews, observation and document review. The results showed that the factors associated with work related accidents in construction were caused by human factors, environmental factors and occupational factors. Human factors that cause work accidents were working not according to procedures, perceptions of potential hazards and low safety, lack of concentration and fatigue. Environmental factors that cause work accidents are limited safety information, unsafe work environment, lack of safety signs and limited safety facilities. Occupational factors that cause work accidents include lack of supervision. Based on this research, it can be concluded that the causes of work accidents are very complex, so there needs to be coordination in project management to optimize supervision of work safety. ABSTRAK Selama kurun waktu 4 tahun dari tahun 2011 sampai tahun 2014 kasus kecelakaan akibat kerja selalu mengalami peningkatan. Pada tahun 2014 tercatat kasus kecelakaan kerja sebanyak 105.383 kasus kecelakaan akibat kerja. Data kecelakaan kerja tersebut didominasi sector konstruksi. Tujuan dari penelitian ini adalah untuk menganalisis faktor penyebab kecelakaan kerja konstruksi. Metode penelitian ini menggunakan desain studi kualitatif dengan pendekatan studi kasus. Metode analisis kecelakaan kerja yang digunakan adalah Fault Tree Analysis (FTA). Populasi dari penelitian ini adalah seluruh pekerja konstruksi sebanyak 140 orang. Informan utama dalam penelitianini ditentukan dengan cara Purposive Sampling yaitu pekerja yang pernah mengalami kecelakaan kerja pada bulan Mei-Juni 2014. Teknik pengumpulan data menggunakan indeph interview, observasi dan studi dokumen. Hasil penelitian menunjukkan faktor penyebab kecelakaan kerja kontruksi disebabkan faktor manusia, faktor lingkungan dan faktor pekerjaan. Faktor manusia penyebab Kecelakaan kerja antara lain bekerja tidak sesuai prosedur, persepsi potensi bahaya dan keamanan rendah, kurang konsentrasi dan kelelahan. Faktor Lingkungan penyebab kecelakaan kerja yaitu terbatasnya informasi keselamatan, lingkungan kerja tidak aman, kurang rambu keselamatan serta terbatasnya fasilitas keselamatan. Faktor Pekerjaan penyebab kecelakaan kerja antara lain kurangnya pengawasan. Berdasarkan penelitian ini dapat disimpulkan bahwa penyebab kecelakaan kerja sangat kompleks, sehingga perlu adanya koordinasi dalam manajemen proyek untuk mengoptimalkan pengawasan terhadap keselamatan kerja.

Abstract

Background: Needlestick injury is a dangerous work accident that causes blood contact with pa-tients and is at risk of being infected with blood borne diseases such as hepatitis B virus (HBV), hepatitis C (HCV), and human immunodeficiency virus (HIV). The purpose of this study was to analyze the influence of unsafe behavior factors on the incidence of accidents due to needlestick injury to health workers in hospitals. Subjects and Method: This research was conducted using a systematic review and meta-analysis study design using PICO, population: health workers, intervention: unsafe action and lack of work experience. Comparison: safe action and good work experience, Outcome: Needlestick Injury (NSI) incident. The articles used in this study came from 4 databases, namely Pubmed, ScienceDirect, ProQuest and Google scholar. The keywords of the article are ("Needle Stick Injury" or "Sharp Injury") AND aOR. The articles included in this study are full paper articles, observational study designs, 2011-2021 and the size of the relationship between Adj Odds Ratio Results: A total of 19 observational studies (case-control and cross-sectional) with 28,487 health workers from the African Continent (Egypt and Ethiopia), the Asian Continent (Thailand, Taiwan, Malaysia and Indonesia) and the European Continent (Bosnia and Herzegovina) were analyzed. From data processing, it was found that unsafe action increased the incidence of Needlestick Injury (NSI) in health workers in hospitals by 2.79 times higher than health workers who behaved safely (safe action) and statistically significant (aOR= 2.79; 95% CI= 2.18 to 3.67; p<0.001). Work Experience increases the incidence of Needlestick Injury (NSI) in health workers in hospitals by 1.97 times higher than health workers who have good work experience and is statistically significant (aOR= 1.97; 95% CI= 1.16 to 3.34; p = 0.010) . Abstract

Background: Today, the world not only has to face a higher number of deaths from TB every year, but other challenges arise in the form of drug-resistant TB with risk factors that can affect the incidence of MDR TB, namely treatment-related factors (direct) and other factors (not direct). direct) such as biological, social, economic, and health system factors. The aim of this study was to estimate the risk of HIV infection, smoking and previous medication history on the incidence of MDR TB, with a meta-analysis of primary studies conducted by previous authors.Subjects and Method: This study was conducted using a systematic review and meta-analysis study design using PICO, population: Tuberculosis Patients. Intervention: Risk Factors for HIV Infection, Smoking, and History of Medication. Comparison: no HIV infection, no smoking and no history of medication, Outcome: MDR-TB incidence. The articles used in this study came from 4 databases, namely Pubmed, ScienceDirect, ProQuest and Google scholar. The article keywords are ("Risk factor" or "associated") AND ("Multidrug resistant Tuberculosis" or "MDR TB") AND “Case Control” AND (HIV or Smoke or treatment). The articles included in this study are full paper articles, case-control study designs, 2011-2021 and the size of the relationship between Adj Odds Ratio. Article selection is done using PRISMA. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 20 case-control studies with 18,790 TB patients with details of 4,220 cases and 14,570 controls from 3 America, Africa, and Asia. HIV infection had a risk of 2.05 times in TB patients becoming MDR-TB (aOR = 2.05; 95% CI = 1.03 to 4.07; p= 0.040). Smoking had a 2.2 times risk in TB patients for MDR-TB (aOR= 2.20; 95% CI= 1.87 to 2.59; p<0.001). History of TB treatment had a 9.08 times risk in TB patients to become MDR-TB (aOR=9.08; 95% CI= 5.49 to 15.03; p<0.001).Conclusion: Risk factors for HIV infection, smoking and history of TB treatment increase the incidence of TB to MDR-TB.Keywords: Multidrugs Resistant Tuberculosis, HIV, smoking, TB treatment historyCorrespondence:Nurul Dwi Andriani. Abstract

Abstract Completeness of medical record documentation was an essential component of healthcare quality that supported patient safety, continuity of care, and hospital legal accountability. Individual factors such as knowledge, attitude, and work experience were assumed to influence healthcare workers’ compliance with medical documentation standards. This study aimed to analyze the effect of individual factors on the completeness of medical record documentation among healthcare workers at Hospital X. A cross-sectional design was applied, involving 82 healthcare workers selected through stratified random sampling. Data were collected using a structured questionnaire and observation sheet that had been tested for validity and reliability. Data were analyzed using Chi-square and multiple logistic regression with a significance level of 0.05. The results showed that most respondents had high knowledge (85.4%) and positive attitudes (75.6%). Bivariate analysis revealed significant relationships between knowledge and attitude with the completeness of medical record documentation (p=0.000). Multivariate analysis revealed that only knowledge had a significant effect on medical record completeness (OR=0.068; p=0.001), while attitude was not statistically significant. This study concludes that knowledge is the main determinant of complete medical record documentation. Strengthening knowledge through continuous training, medical record audits, and structured feedback is necessary to improve the quality of medical documentation in healthcare facilities. Keyword: medical record, knowledge, attitude, healthcare workers, documentation completeness Abstrak Kelengkapan pengisian rekam medis merupakan bagian penting dari mutu pelayanan kesehatan yang mendukung keselamatan pasien, kesinambungan asuhan, dan akuntabilitas hukum rumah sakit. Faktor individu seperti pengetahuan, sikap, dan pengalaman kerja diduga memengaruhi tingkat kepatuhan tenaga kesehatan terhadap standar dokumentasi medis. Penelitian ini bertujuan untuk menganalisis pengaruh faktor individu terhadap kelengkapan pengisian rekam medis oleh tenaga kesehatan di Rumah Sakit X. Penelitian ini menggunakan studi cross-sectional dengan jumlah responden sebanyak 82 tenaga kesehatan yang dipilih melalui stratified random sampling. Data dikumpulkan menggunakan kuesioner terstruktur dan lembar observasi yang telah diuji validitas dan reliabilitasnya. Analisis dilakukan menggunakan uji chi-square dan regresi logistik berganda dengan tingkat signifikansi 0,05. Hasil penelitian menunjukkan bahwa sebagian besar responden memiliki pengetahuan tinggi (85,4%) dan sikap positif (75,6%). Uji bivariat menunjukkan hubungan yang signifikan antara pengetahuan dan sikap dengan kelengkapan pengisian rekam medis (p=0,000). Hasil analisis multivariat menunjukkan bahwa hanya pengetahuan yang berpengaruh signifikan terhadap kelengkapan pengisian rekam medis (OR=0,068; p=0,001), sedangkan sikap tidak menunjukkan pengaruh yang signifikan. Penelitian ini menyimpulkan bahwa pengetahuan merupakan faktor determinan utama dalam kelengkapan pengisian rekam medis. Penguatan pengetahuan melalui pelatihan berkelanjutan, audit rekam medis, dan umpan balik terstruktur diperlukan untuk meningkatkan mutu dokumentasi medis di fasilitas pelayanan kesehatan.
