Abstract

Background: Based on the strategic plan of the Ministry of Health of the Republic of Indonesia for 20202024, all hospitals can gradually implement electronic medical records. However, there are still many obstacles experienced by hospitals in implementing Electronic Medical Record (EMR), readiness assessments can be used to determine readiness, indications and requirements in facilitating the implementation of EMR so as to avoid various obstacles during the implementation process.Subjects and Method: This was a qualitative descriptive study with a case study design. The study was conducted at Bethesda Lempuyangwangi Hospital, Yogyakarta, from May to June 2022. The data collected was the readiness to implement EMR in terms of organizational alignment and management capacity, by conducting indepth interviews with 7 people. The study subjects were determined by purposive sampling who were considered to have the capacity to make decisions related to EMR.Results: Bethesda Lempuyangwangi Hospital in implementing EMR from organizational alignment is said to be ready, the board of directors has known and understood that the implementation of RME has many benefits, the organization has been planned and the unit leadership understands the implementation risk in case of failure, to anticipate this, a longterm vision has been prepared. Readiness to implement EMR from management capacity can be improved because the IT committee has not yet been formed consisting of stakeholder representatives and hospitals, organization is still within the scope of the hospital, where ICT has been included in the organization's strategic plan. The roles and responsibilities for EMR implementation are defined with clear accountability to vendors and have been documented, the board of directors has considered planning the EMR implementation budget, funding for ongoing clinical IT adoption and the required standards are included in future planning and will be part of the IT budget.Conclusion: Bethesda Lempuyangwangi Hospital is ready to implement electronic medical records that will be applied to inpatient units, this can be seen from the alignment of the organization and management capacity where the culture, organization, leadership, strategy, information management, clinical staff, accountability and budget finance have been planned and have started held.Keywords: Readiness, Electronic Medical RecordCorrespondence: I Gusti Agung Ngurah Putra Pradnyantara. Abstract

Background: Cervical cancer is the fourth common cancer worldwide and the second most common cancer cases in women. Cervical cancer screening allows detection of abnormal cervical cells. Having knowledge about cervical cancer and its screening can increase the absorption of services for cervical cancer screening. This study was aimed to determine the comparison of participation in cervical cancer screening in adult women with high education with adult women with low education. Subjects and Method: This study was a systematic review and meta-analysis with the following PICO, population: adult women. Intervention: high knowledge. Comparison: low knowledge. Results: cervical cancer screening. The articles used in this study were obtained from two databases, namely Google Scholar and Pubmed. Keywords to search for "Knowledge," "screening" "cervical cancer" "adult women". The articles included are full-text articles with a cross-sectional study design from 2016 to 2022 and report multivariate analysis with Adjusted Odds Ratio (aOR). Articles were analyzed using the Review Manager 5.3 application. Results: Systematic study and meta-analysis of this study A total of 16 articles from 2 African continents (Ethiopia, Nigeria, Southern Ethiopia, South west Ethiopia, Ivory Coast, South Africa, Northwest Ethiopia, Cameroon, eastern Ethiopia, Ghana), and Asia (Thailand and Malaysia). Showed that women with high knowledge had an attitude to cervical cancer screening 3.63 times compared to women with low knowledge and the results were statistically significant (aOR = 3.63; 95% CI = 2.11 to 6.24; p<0.001). Conclusion: A high level of knowledge increases attitudes to cervical cancer screening in adult women. Keywords: screening, cervical cancer, knowledge. Abstract

Dalam statistik rumah sakit terdapat beberapa indikator pelayanan rawat inap, yaitu BOR, AvLOS, TOI, BTO, GDR dan NDR. Pemanfaatan data pelaporan berhubungan dengan mekanisme alur prosedur pelayanan untuk kepentingan internal dan eksternal rumah sakit. Berdasarkan studi pendahuluan di Rumah Sakit Dkt Dr. Soetarto Yogyakarta, data 3 tahun terakhir terdapat peningkatan indikator pelayanan rawat inap tetapi masih belum masuk kategori efisien. Penelitian ini bertujuan untuk menganalisis tren indikator pelayanan rawat inap selama 3 tahun terakhir di RS DKT Dr. Soetarto Yogyakarta dan mengidentifikasi faktor-faktor yang mempengaruhi efisiensi pelayanan. Kebaruan penelitian ini terletak pada pendekatan analisis longitudinal yang dikombinasikan dengan wawancara mendalam untuk memahami konteks dan upaya perbaikan yang dilakukan rumah sakit. Jenis penelitian ini adalah deskriptif dengan pendekatan kualitatif dengan jenis data sekunder yaitu rekapitulasi SHRI pertahun dalam kurung waktu 3 tahun terakhir serta wawancara kepada petugas pelaporan, dari hasil analisis perbandingan indikator pelayanan rawat inap selama 3 tahun terakhir diketahui hanya nilai LOS yang memenuhi standar efisiensi, untuk indikator BOR, TOI, dan BTO belum memenuhi standar barber johnson karena jumlah kunjungan dirumah sakit khususnya rawat inap masih rendah.   In hospital statistics there are several indicators of inpatient services, namely BOR, AvLOS, TOI, BTO, GDR and NDR. The use of reporting data is related to the mechanism for the flow of service procedures for internal and external purposes of the hospital. Based on a preliminary study at Dr. DKT Hospital. Soetarto Yogyakarta, data for the last 3 years shows an increase in inpatient service indicators but it is still not in the efficient category. The aim of this research is to analyze the benefits of inpatient service indicators over the last 3 years. This type of research is descriptive with a qualitative approach with secondary data, namely recapitulation of SHRI per year within the last 3 years as well as interviews with reporting officers. From the results of the comparative analysis of inpatient service indicators over the last 3 years, it is known that only the LOS value meets efficiency standards, for BOR, TOI and BTO indicators do not meet Barber Johnson standards because the number of hospital visits, especially inpatient stays, is still low.

Abstract

Implementasi Rekam Medis Elektronik (RME) kini diwajibkan bagi semua fasilitas kesehatan di Indonesia sesuai dengan Peraturan Menteri Kesehatan No. 24 Tahun 2022, yang bertujuan untuk menstandarisasi pengelolaan rekam medis dan meningkatkan kualitas layanan kesehatan. Penelitian sebelumnya menunjukkan berbagai tantangan dalam penerapan RME, namun hambatan spesifik di bawah regulasi baru, terutama di rumah sakit daerah, belum banyak diteliti. Penelitian ini bertujuan untuk mengidentifikasi tantangan spesifik yang dihadapi Rumah Sakit Umum Dharma Yadnya Kota Denpasar dalam mengimplementasikan RME sesuai dengan regulasi baru tersebut. Penelitian ini menggunakan desain deskriptif kualitatif, dengan data dikumpulkan melalui wawancara dengan lima tenaga kesehatan yang dipilih secara purposive sampling. Fokus utama adalah mengidentifikasi tantangan yang berkaitan dengan kesiapan staf, kepatuhan hukum, manajemen rumah sakit, dan infrastruktur teknologi. Hasil penelitian menunjukkan adanya hambatan signifikan seperti pelatihan internal yang terbatas untuk staf, pemahaman yang kurang terhadap regulasi RME, dan gangguan teknis yang sering mengganggu pelayanan pasien. Temuan juga mengidentifikasi kesenjangan dalam kesiapan staf dan dukungan teknologi yang tidak memadai sebagai hambatan utama dalam implementasi RME yang efektif. Simpulan dari penelitian ini menunjukkan perlunya peningkatan di beberapa area kritis, termasuk memperbaiki program pelatihan staf, meningkatkan kesadaran hukum, memperkuat praktik manajemen rumah sakit, dan memperbarui infrastruktur teknologi untuk mendukung implementasi RME. Peningkatan ini sangat penting untuk mengoptimalkan layanan pasien dan mencapai manfaat yang diinginkan dari adopsi RME.

Abstract

Evaluasi merupakan suatu upaya untuk menilai kondisi penyelenggaraan sistem informasi kesehatan di rumah sakit sehingga target capaian dapat diketahui dan langkah perbaikan dapat direncanakan. Rekam Medis Elektronik (RME) menjadi salah satu bentuk transformasi digital, namun penerapannya di Indonesia masih menghadapi tantangan. Berdasarkan data Persatuan Rumah Sakit Seluruh Indonesia (PERSI), dari sekitar 3.000 rumah sakit hanya 50% yang telah menggunakan RME, dan hanya 16% yang berfungsi optimal. Kondisi ini menunjukkan bahwa pemanfaatan RME masih jauh dari harapan. Penelitian ini bertujuan untuk mengevaluasi penerapan RME pada unit Instalasi Gawat Darurat (IGD) di Rumah Sakit Umum Dharma Yadnya dengan meninjau tiga aspek (HOT-Fit), yaitu sumber daya manusia, organisasi, dan teknologi. Penelitian menggunakan metode kualitatif dengan desain deskriptif yang dilaksanakan pada Januari-Maret 2024. Subjek penelitian adalah enam orang petugas yang dipilih melalui purposive sampling. Data dikumpulkan melalui wawancara mendalam, observasi terhadap SOP serta sarana dan prasarana, serta studi dokumentasi berupa bukti data ceklis RME. Hasil penelitian menunjukkan bahwa penerapan RME di Rumah Sakit Umum Dharma Yadnya telah memperoleh dukungan dari sisi organisasi, namun masih terdapat hambatan pada aspek SDM dan teknologi. Dengan demikian, dari pihak manajemen harus memberikan dukungan terkait jalannya penerapan rekam medis elektonik serta mempersiapkan fasilitas sarana dan prasarana yang lebih menunjang jalannya RME agar dapat mengoptimalkan kinerja petugas dan mempermudah jalannya penerapan RME.
