Abstract

Indonesia memberlakukan Jaminan Kesehatan Nasional (JKN) awal tahun 2014 yang dikelola oleh Badan Penyelenggara Jaminan Sosial Kesehatan (BPJS Kesehatan). Salah satu program JKN adalah Program Rujuk Balik (PRB), yang diperuntukkan bagi pasien penyakit kronis dengan keadaan stabil yang masih memerlukan pengobatan jangka panjang. Semua peserta PRB berhak mendapatkan pengobatan untuk 30 hari penuh dari Apotek yang telah bekerjasama dengan BPJS Kesehatan, namun ternyata masih ada pasien yang tidak mendapatkan obat secara penuh. Penelitian ini bertujuan untuk melihat jumlah pemenuhan dan pola penggunaan obat PRB di Apotek wilayah Gedebage. Penelitian kuantitatif ini dilakukan di Wilayah Gedebage. Dua Apotek telah dipilih sebagai objek penelitian. Data berasal dari form resep obat bulan Oktober 2015, yaitu nama obat, indikasi (sesuai Formularium Nasional), jumlah obat, dan frekuensi penggunaan obat dari kedua Apotek. Lebih dari 95% semua pelayanan obat PRB telah dilakukan oleh pihak Apotek yang bekerjasama dengan BPJS Kesehatan dengan pola penggunaan obat terbanyak yaitu untuk hipertensi sebanyak 84,09%, disusul diabetes melitus (37,52%) dan penyakit jantung (12,72%). Secara umum, jumlah pemenuhan obat PRB untuk kebutuhan selama 30 hari dapat terpenuhi di kedua apotek dengan penggunaan obat terbanyak untuk penyakit hipertensi.Kata Kunci: Apotek, BPJS Kesehatan, Obat PRB

Abstract

Tahun 2014 Pemerintah Indonesia melakukan perubahan sistem pembiayaan kesehatan yang disebut dengan Jaminan Kesehatan Nasional. Salah satu faktor penting yang harus diperhatikan untuk mencapai tujuan Jaminan Kesehatan Nasional adalah ketersediaan obat. Penelitian ini bertujuan untuk mengidentifikasi dan mengeksplorasi dampak perubahan sistem pembiayaan kesehatan terhadap ketersediaan obat Jaminan Kesehatan Nasional di apotek rujukan. Metode Penelitian Penelitian ini menggunakan metode kualitatif dengan pendekatan grounded theory. Pengambilan data menggunakan wawancara mendalam terhadap 8 informan yang dianggap memiliki pengalaman dalam perubahan sistem pembiayaan kesehatan. Penelitian ini dilakukan pada bulan Maret-Desember tahun 2015. Tempat yang dijadikan objek penelitian adalah 2 apotek di Wilayah Cibeunying Kotamadya Bandung. Hasil Perubahan pembiayaan kesehatan berdampak pada 4 aspek, yaitu: (1) regulasi terkait obat; (2) manajemen apotek dan BPJS Kesehatan; (3) penyediaan obat; dan (4) permintaan obat. Keempat hal tersebut kemudian berdampak kepada ketersediaan obat, yang akan mempengaruhi:(1) kepuasan peserta Jaminan Kesehatan Nasional dan apotek; dan (2) keuntungan apotek.Perubahan sistem pembiayaan kesehatan dapat berdampak pada apotek yang belum disiapkan untuk berubah. Apotek akan mampu beradaptasi apabila memiliki komitmen untuk terus belajar dan memiliki sumber daya yang memadai. Kesimpulan: Dampak dari perubahan sistem pembiayaan kesehatan perlu disertai dengan kesiapan sektor pemerintah maupun swasta.Kata kunci: apotek, Jaminan Kesehatan Nasional, obat, sistem kesehatan

Abstract

Reformasi dalam bentuk sistem pembiayaan kesehatan di Indonesia diresmikan pada 1 Januari 2014. Asuransi kesehatan sosial berbentuk Jaminan Kesehatan Nasional (JKN) saat ini dianggap masih memiliki kekurangan, salah satunya adalah ketersediaan obat dalam sektor swasta, yaitu apotek. Sementara itu belum ada data mengenai ketersediaan obat JKN di apotek. Penelitian ini bertujuan untuk mengetahui dampak perubahan sistem pembiayaan kesehatan terhadap ketersediaan obat, serta faktor-faktor yang memengaruhinya di apotek rujukan. Penelitian menggunakan desain kualitatif yang diolah dengan proses pengkodean, kategorisasi, penyusunan tema, dan sintesis teori untuk mendapatkan informasi secara menyeluruh dari apotek dan pasien yang terkait dengan JKN. Subjek penelitian adalah 10 orang yang merupakan manajer, petugas pencatatan, asisten apoteker, dan pasien. Penelitian dilakukan di apotek rujukan JKN di wilayah Bojonegara Kotamadya Bandung pada tahun 2015. Hasil penelitian menunjukkan adanya dampak dari perubahan kebijakan sistem pembiayaan berupa ketidaksesuaian atau penyimpangan antara lain berupa ketidaktersediaan obat dan kerugian yang ditanggung apotek yang dipengaruhi oleh aspek regulasi, pengadaan obat, sosialisasi, dan manajemen. Ketidaktersediaan obat dalam penelitian ini memberikan gambaran mengenai respon dalam reformasi. Aspek penting dalam perubahan adalah persiapan, serta respon dari pelaksana perubahan tersebut. Keterbatasan penelitian ini adalah jumlah apotek dan subjek yang terbatas.Kata Kunci : Apotek, Jaminan Kesehatan Nasional, Ketersediaan Obat

Abstract

  Background: Hepatitis is an infection or inflammation disease of the liver which is caused by virus, toxic substance, and immunological abnormalities. Soursop plant as a medicinal plants is known to have an antioxidant effect and nowadays is used as an alternative drug for hepatitis. One of the methods to assess liver function is to measure the serum Glutamate Piruvate Transaminase (SGPT) level. The purpose of this study was to find the effect of Soursop Leaf  in inhibiting the SGPT elevation .Methods: An experimental study was conducted on 25 white male rats of wistar strain in the pharmacology laboratory of Faculty of Medicine, Universitas Padjadjaran during the period of September to October 2012. The rats were divided into 5 groups (group 1 and 2 as control; group 3, 4, and 5 as treatment groups treated by 200, 400, 600 mg/kgbw soursop extract, respectively). The soursop extract was administered to the treatment groups for 8 days. On the 8th day, group 2, 3, 4, and 5 received 1.6 ml CCl410% intraperitoneally. After 18 hours, the mean SGPT levels from all groups were measured. ANOVA test was used to analyze the result.Results : The mean SGPT levels were lower in the 3rd, 4th, and 5th group compared to group 2. There was a significant difference among treatment groups. Abstract

Background: Joint pain is frequently found in daily life activities. The prevalence of joint pain increases within the age. One of the medicine used for joint pain is non-steroidal anti-inflammatory drug (NSAID). In connection with inappropriate usage and their side effects, this study aimed to seek the extent ofhousewives’ knowledge on the use of NSAID for joint pain in Hegarmanah village, Jatinangor subdistrict.Methods: This cross-sectional descriptive study was conducted in October 2013 to the housewives resided in Hegarmanah village, Jatinangor subdistrict, West Java. Questionaire sheet was distributed to each of 110 housewives that had been stratifiedly with randomized sample. The questionaire contained identity, age, education level, and knowledge of NSAID in related to joint pain.Results: Based on the data collected, 73 subjects had adequate level of the knowledge and 37 subjects were in a poor level of the knowledge. The proportion of respondents who knew that joint pain was the pain occurs in the joint was 99.1%, the proportion of respondents who knew that the pain relieving drugs are called NSAID group was 40.9%, the proportion of respondents who knew that NSAID had a side-effect was 73.6%, and the proportion of respondents who knew that the side-effect of NSAID is abdominal pain was 61.8%.Conclusions: Most of the housewives in Hegarmanah Subdistrict have adequate knowledge in the use ofNSAID for joint pain relief. Abstract

Background: Rheumatoid arthritis (RA) is an autoimmune disease that causes significant morbidity in most patients and also premature mortality in some. To prevent this, therapeutic approaches call for the early treatment of RA by using disease-modifying antirheumatic drugs (DMARDSs), either as single therapy or combination therapy. Although in the previous decade DMARDs were rarely used as combination, however now they are used widely to treat RA. The objective of this study was to compare the effectiveness of methotrexate (MTX) alone and the combination of MTX & hydroxychloroquine (HCQ) in RA patients.Methods: An analytic retrospective cohort study was conducted from May 2014 until October 2014, on 46 patients with RA in the rheumatology clinic of Dr. Hasan Sadikin General Hospital Bandung from January 2009−October 2014 who were taking MTX or MTX & HCQ for at least 1 year. The secondary data obtained from these patients’ medical record were then analyzed using the independent t-test and Mann-Whitney test.Results: The study showed that 93.48% of the patients were females. The mean change in disease activity measures were not significant for any of the parameter (p-value for SJC = 0.337; TJC = 0.676; ESR = 0.780). Also the comparisons of the disease activity score (DAS 28) before and after therapy was not significant (p-value = 0.584).Conclusion: There is no difference in the effectiveness of DMARD monotherapy with MTX and combination therapy with MTX & HCQ in RA patients.DOI: 10.15850/amj.v3n3.474

Abstract

Background: Dyslipidemia occurs in 60% of hypertensive patients. Effective treatment of hypertensive patients with dyslipidemia can reduce the risk of cardiovascular disease by more than 50%. However, compliance with dyslipidemia medication in dyslipidemia patients is still low. Low compliance can be attributed to poor knowledge. This study aimed to analyze the relationship between knowledge and compliance using antidyslipidemic drugs in hypertensive patients with dyslipidemia.Methods:  A cross-sectional analytical study was conducted from August to September 2022 at the Pasirkaliki Public Health Center (Pusat Kesehatan Masyarakat, Puskesmas), Bandung, Indonesia. The consecutive sampling method was used to collect 64 hypertensive patients with dyslipidemia who were taking anti-dyslipidemic drugs. Primary data on knowledge and compliance were obtained through valid and reliable questionnaires. The Chi-square test was used to determine the relationship between the two variables.Results: The study results showed that 95.3% of respondents had good knowledge. Respondent compliance showed obedience of 78.1%. Respondents with good knowledge had an obedience rate of 78.1%. There was a relationship between knowledge and compliance to using anti-dyslipidemic drugs in hypertensive patients with dyslipidemia (p=0.001).Conclusions: There is a relationship between knowledge and compliance with the use of anti-dyslipidemia drugs in hypertensive patients with dyslipidemia. Almost all patients have good knowledge.  Continuous maintenance and improvement of patient knowledge is required, which can be achieved by conducting health education to better understand the relationship between diseases and the importance of taking medication.

Abstract

Background: Adherence to medication is an important aspect in type 2 diabetes mellitus management. One of the factors that determine medication adherence is family support. This study aimed to analyze the relationship between family support and adherence to oral anti-diabetic medications in patients with type 2 diabetes mellitus.Methods: This study was a quantitative, non-experimental correlational study with a cross-sectional method conducted in December 2022–January 2023 on patients with type 2 diabetes mellitus at the Pasirkaliki Public Health Center, Bandung City, Indonesia.  The study used a minimal sampling method. Data collection analysis was performed using the SPSS program with the Chi-square test.Results: In total, 50 respondents were included, of whom the majority (n30; 60%) had good family support, whereas 17 (34%) had sufficient family support, and only 3 (6%) had poor family support. A good level of medication adherence was indicated by 39 (78%). There was a significant relationship between family support and adherence to oral anti-diabetic medications in type 2 diabetes mellitus (p=0.003).Conclusions: A good relationship between family support and adherence to oral anti-diabetic medications is crucial in enhancing quality of life and wellness among patients with type 2 diabetes mellitus. Research related to other factors that affect adherence to oral anti-diabetic medications warrant further studies.

Abstract

Pemakaian kerudung dalam aktivitas sehari – hari dan proses pencucian yang terus menerus serta penambahan zat penyempurnaan anti kusut seperti resin, katalis, dan softener pada kain kerudung tentunya akan berpengaruh terhadap performa kain kerudung yaitu smoothness appearance, kekuatan tarik kain, dan kelangsaian kain. Penelitian ini bertujuan untuk mengetahui pengaruh pencucian berulang terhadap performa kain kerudung anti kusut serta mengetahui jumlah pencucian yang dapat menurunkan mutu kerudung anti kusut. Proses penyempurnaan anti kusut dilakukan secara exhaust menggunakan resin DMDHEU. Proses pencucian berulang dan pengujian performa kain kerudung seperti pengujian smoothness appearance, kekuatan tarik kain, dan kelangsaian dilakukan berdasarkan ketentuan yang ada di SNI. Adanya pencucian berulang terbukti menurunkan performa kerudung anti kusut dan berpengaruh terhadap pemenuhan syarat mutu yang ada dalam SNI 8098:2017.
