Abstract

Terapi konseling gizi oleh tenaga puskesmas sangat diperlukan untuk pasien DM tipe 2, berfungsi mengontrol diet pasien agar tidak timbul masalah yang lebih kompleks. Pelayanan konseling gizi di puskesmas saat ini, diduga masih belum dilakukan secara optimal. Penelitian dilakukan untuk mengetahui gambaran pelayanan konseling gizi pada pasien DM tipe 2 di Puskesmas Jatinangor. Desain penelitian adalah deskriptif kualitatif dengan strategi studi kasus, pengambilan data dilakukan melalui wawancara mendalam dan observasi. Jumlah responden  terdiri dari 1 nutrisionis, 1 dokter dan 3 pasien. Penelitian dilakukan pada bulan Agustus-November 2015. Analisis data dilakukan secara konten analisis. Tahapan proses konseling gizi berjalan sesuai dengan standar yang ditetapkan oleh Kemenkes. Konseling yang dilakukan masih menggunakan metode yang konvensional. Hambatan yang dialami petugas Puskesmas adalah dalam melakukan monitoring dan evaluasi hasil konseling. Hal ini disebabkan  karena pasien yang melakukan kunjungan ulang masih rendah, selain itu fasilitas. puskesmas terbatas seperti ruang konseling gizi dan food model. Pelayanan konseling gizi di puskesmas belum berjalan dengan baik, metode konseling yang digunakan masih konvensional serta belum didukung fasilitas yang memadai. Perlu peningkatan kapasitas tenaga dalam melakukan konseling dengan pendekatan yang tepat serta pengadaan ruangan konseling dan food model.Kata Kunci: Diabetes Mellitus tipe 2, konseling gizi, puskesmas.

Abstract

Background: Type 2 diabetes mellitus (T2DM) patients require continuous lifelong attention to pharmacotherapy to achieve and maintain optimal glucose control, which can be monitored through measuring HbA1c. This study aimed to see the comparrison of HbA1c levels before and after a minimum of two months use of antihyperglycemic agents in patients with T2DM.Methods: This analytical cross-sectional  study used medical records of patients with T2DM in the center of outpatient medical records at the Department of Clinical Pathology, Dr. Hasan Sadikin General Hospital, in  2013. Sampling was conducted using consecutive sampling technique. The data analysis used the paired t-test and Wilcoxon test for not normal data distribution.Results: Before and after a minimum of two months use  of antihyperglycemic agents, HbA1c concentration was 7.15% (6.1–12.2) and 7.3% (5.9–11.9) respectively. Statistically, the difference of HbA1c concentration of T2DM patients between those period  was not significant (0.15%; p=0,812). Mean HbA1c concentration was found with greatest decrease in patients treated with sulfonylurea (2.25%), followed by insulin (0.1%). There were increases in mean HbA1c concentration in patients treated with metformin (0.05%), metformin+insulin (1.1%), sulfonylurea +metformin (0.6%), sulfonylurea+alpha-glucosidase inhibitor (1.1%), and sulfonylurea + metformin + alpha-glucosidase inhibitor (0.05%).Conclusions: There is no significant difference of HbA1c levels before and after two months to two years use of antihyperglycemic agents in T2DM patients. This result showed the importance of comprehensiveness in T2DM therapy. The therapy should not merely consist of antihyperglycemic medication alone, but also physical exercise, lifestyle modification, and diet control.

Abstract

Elderly is a problem that may disrupt the balance of various molecules in the body, especially fat molecules, which may result in the emergence of various diseases so that blood fat and adipose level examinations need to be done regularly and periodically as an effort of early detection. This study aimed to find correlation between Low Density Lipoprotein (LDL), High Density Lipoprotein (HDL), and triglyceride with body fat percentage among professors in Universitas Padjadjaran (Unpad) who were mostly in elderly age. This study was a linear regression analytical study conducted on 52 Unpad professors (♂ 75%, ♀ 25%) based on data of LDL, HDL, and triglyceride levels obtained through laboratory examination, as well as body fat percentage data by using Bio Impedance Analysis (BIA). The correlation between low density lipoprotein (LDL), High Density Lipoprotein (HDL), and triglyceride with body fat percentage in male professors group was R2=0.072; P>0.05 and in female was R2=0.028; P>0.05. As a conclusion, there was no correlation between Low Density Lipoprotein (LDL), High Density Lipoprotein (HDL), and triglyceride with body fat percentage in Unpad professors.
